Musical impact: A study of the effects of music making on musicians' health and wellbeing

Abstract

Few pursuits are as dynamic and enjoyable as making music. Each day, thousands of people in the UK engage in some sort of musical activity. Central to these endeavours is good health. Physical and mental wellbeing can shape how musicians, from beginners to professionals, pursue their art and the pleasure they take from it. The results of recent research, however, suggest that injury and ill health are widespread among musicians and that healthy approaches to training and working in music are far from uniform. This project investigates the health and wellbeing of musicians. In doing so, it generates new knowledge of the physical and mental demands of music making and sheds light on the ways in which musicians at all levels meet those demands, both constructively and destructively. While musicians typically have a long history of self-sufficiency in managing the challenges of performing, this project aims to complement musicians' own ingenuity by providing comprehensive, evidence-led resources to help maximise educational and professional opportunities. Musical Impact is a project of Conservatoires UK (CUK), in association with Cardiff University, Cardiff Metropolitan University, Association of British Orchestras (ABO), British Association for Performing Arts Medicine (BAPAM), the Musicians' Union, and the International Health Humanities Network (IHHN).

Musical Impact operates through three interrelated work packages (WPs). WP1 comprises a four-year study of musicians' physical and mental fitness for performance. While previous research has largely looked at cross-sections of musicians, WP1 takes the form of a large-scale longitudinal study with conservatoire students, staff and alumni, seeking to understand the incidence, extent and development of injuries and ill-health among British musicians. WP1 employs standardized measures of health promotion, anxiety, perfectionism, cardiovascular fitness and physical strength and flexibility, complemented by qualitative exploration of the cultures of musicians' health.

WP2 places practice and performance under investigation, documenting their physical and mental demands and the characteristics of musicians who successfully meet those demands. Physiological and biomechanical assessments will be made during the practice and performance of musicians, including the recording and analysis of muscle activity using electromyography and the monitoring of energy expenditure with portable gas analysers.

WP3 builds upon WPs 1 and 2, exploring health promotion in music education and the profession and exploring practical applications to enhance training and support services. WP3 includes the development of two programmes of health promotion: one for junior conservatoire students and one for senior conservatoire students. These programmes will be piloted and evaluated through case studies, a questionnaire survey and interviews, before being delivered in full at junior- and senior-programmes across all CUK conservatoires.

Musical Impact is the largest research initiative of its kind worldwide. It moves beyond existing research to contribute needed insight into chronic and acute health problems and their impact over time (WP1), the physical and mental demands of music making (WP2), and effective strategies for health promotion (WP3). The project's outcomes will be delivered through a combination of scholarly outputs (including an edited book), six workshops held for music teachers and students, and two freely-available resource packs.

Planned Impact

Who will benefit from this research?

Beneficiaries of Musical Impact are musicians young and old, amateur and professional, students, teachers and performers. The work will build on existing links with three major organisations representing users: Association of British Orchestras (ABO), British Association for Performing Arts Medicine (BAPAM), and the Musicians' Union. All three will provide access to professional musicians as participants in health promotion initiatives as well as research. Musical Impact is also likely to be of interest to the general public, since music is central to the lives of so many people, and it is envisaged that public relations staff at each CUK institution will contribute to the dissemination of the project's methods and findings through the media - print news, radio, TV, online - over the course of the project.

How will they benefit from this research?

Music making is widespread in the UK - among young and old, by amateurs and professionals, in community settings and concert halls. While we often think of music making as enjoyable and health-promoting, research shows that injuries among musicians are rife. Surprisingly, little has been done to address this problem, either in terms of music-relevant research or the application of research to educational and professional contexts. With recent regulations on noise in the workplace and several grass-roots calls to promote health more effectively among musicians (from teachers, schools, conservatoires and professional bodies), Musical Impact will fill a much needed gap, generating new knowledge, engaging students, teachers and employers, and building bridges between research and its application in the real world. Indeed, the findings of our work have the potential to extend performing careers, open up new types of job opportunities within the music and creative industries, improve the chances of performing artists' positive health extending into older age, and impact on future training methods for musicians of all ages. Our team is well placed to realise a lasting impact, given our placement within the sector and proven track records of engaging key stakeholders and specialist and non-specialist audiences.

Specifically, impact will be made through three pathways: (1) three conference strands aimed at the research community, including musicians involved in practice-based and artistic research, (2) six project workshops, reaching 400-600 practitioners - students, teachers and performers - and (3) freely available resource packs designed for music teachers and secondary/higher education institutions, made available via the project website. To facilitate this, collaborative agreements are in place with ABO, BAPAM and the Musicians' Union (see Letters of Support). The findings of Musical Impact will reach those in conservatoires with responsibility for planning and delivering curricula and therefore future training methods for musicians of all ages via scholarly outputs and workshops. They will reach those who manage employment within the creative industries (such as orchestras and professional choruses) and performers and teachers via workshops and resource packs. They will reduce the financial burden on individuals and charities such as the Musicians Benevolent Fund, which has contributed £80,000 over the past five years to the cost of treatment for student musicians. They will improve the chances of performing artists' positive health extending into older age and thus extend their careers. They will also open up new types of job opportunities in health promotion within the music and creative industries.

Description Findings from Musical Impact cluster into three groups:

1. Music students' perceptions, attitudes, and behaviours toward health

Our findings show that music students have higher levels of wellbeing and lower fatigue than comparable samples outside of music. However, they also reveal some concerning perceptions, attitudes, and behaviours towards health. Specifically, engagement in health responsibility and stress management are lower than expected which, along with high perfectionistic tendencies, limited use of coping strategies, poor sleep quality, and low self-rated health, paints a troubling picture both for music students and for those who support their training. The findings also show moderate levels of general health-related fitness, and this raises the question of whether 'moderate' fitness is enough for people undertaking physically and mentally demanding music making. By deliberately including learning and support services related to health and wellbeing - physical, as well as psychological and emotional - in students' timetables and by expanding health-related provision more generally, we can increase knowledge, active participation, and responsibility for health matters across the sector.

2. Performance stress

Our findings show that, across high-stress public performances, there is a significant increase in both cortisol and cortisone levels, indicating a heightened fight or flight response. However, for a low-stress performance, the results show the opposite pattern: a significant decrease in both cortisol and cortisone, as well as a reduced cortisol-cortisone ratio, indicative of a relaxation response. This latter finding suggests that making music, in itself, can be a stress-reducing - and potentially health-promoting - activity. In relation to performance auditions, our findings show that such high stress performance situations elicit greater cardiovascular reactivity, as compared with low stress situations. When looking within single performances, we observe the greatest cardiovascular reactivity in the pre-performance period, before musicians begin playing.

3. Musicians' pain

Our findings show that nearly 80% of music students report pain in the upper body. This is higher than that cited in the majority of studies with professional musicians and suggests that advanced students are at particular risk of developing musculoskeletal health problems. The overall prevalence remains high regardless of specialism, with no differences between musicians whose performance requires symmetrical or asymmetrical body alignment. Across the study, pain frequency maps show that pain is most commonly felt in the neck, shoulders, lower back, and right arm.

Description Musical Impact has impacted on musicians, conservatoires and sector partners. FOR MUSICIANS: Research from the project has directly engaged with over 1500 musicians, ranging from Conservatoires UK (CUK) junior and senior students to professional musicians around the UK, including: • 90-minute fitness screenings for over 600 musicians, • 3-hour vocal health screenings for over 150 musicians, • 4-hour biomechanical assessments for 20 musicians, • 1-hour physiological assessments for 30 musicians, • 20 hours of in-depth interviews with over 30 CUK students and professional musicians. FOR CONSERVATOIRES: All CUK institutions and other European conservatoires have engaged with the project, including: • data collection, nationally and internationally, • presentations, lectures, seminars and workshops, reaching a total audience of over 5000 people. FOR SECTOR PARTNERS: The project has generated interest within and beyond our immediate sector partners, including: • data collection, within specific orchestras and across the sector, • mainstream reports of research findings - e.g. on YouTube, BBC Radio 3 and BBC Radio 4, • presentations, lectures, seminars and workshops, reaching a total audience of over 7000 people. Drawing together the project's impact is the constitution of the HEALTHY CONSERVATOIRES NETWORK (HCN). The HCN brings together key stakeholders from across the performing arts sector to assist in supporting aspects of health promotion and occupational wellbeing for performing artists, including: • biannual meetings since 2015, • in-depth case studies of good practice, as well as a growing resource of Healthy Performer films, • over 250 members from among CUK staff and students, plus members from the UK performing arts sector, international conservatoires and UK universities. All members are actively engaged with the network to work towards a shared vision: the creation of healthy conservatoires. The HCN aims to inspire and support environments that promote and enhance the health and wellbeing of performing artists, in order for performers to achieve their full potential and to build healthy, sustainable careers. It provides a forum where members can come together to: • receive news and updates on the health needs identified as priorities for performing artists, • communicate and discuss 'hot topics' in conservatoires and provide access to peer support, • share good practice, • engage with research, innovation and evidence-informed practice, • obtain guidance on creating and maintaining a healthy conservatoire. Collectively, members of the HCN aim to: (1) Lead organisational and cultural change: by helping to embed Healthy Conservatoire principles and creating change in organisational ethos, culture, policy and planning processes. (2) Generate high visibility innovative action: through research evidence and case studies of existing practice the HCN aims to showcase CUK's commitment to creating world-leading support for the promotion of health and wellbeing. (3) Provide leadership and advocacy: by communicating the needs of the sector, HCN members aim to advise senior decision-makers on the sustainable development of health and wellbeing. Title Fit to Perform Protocol 
Description The Fit to Perform screening protocol was developed as a physical and mental health assessment package for musicians, first compiled in 2006 and then expanded and refined in 2013. It comprises four stages: 1. Stage 1 (5 min): introductory briefing for the participant, collecting signed informed consent from the participant, and a preliminary screening using the Physical Activity Readiness-Questionnaire (PAR-Q) to assess participants' suitability for taking part in a sub-maximal cardiovascular fitness test (ACSM, 2014). 2. Stage 2 (40 min): survey, delivered online using Surveymonkey®, including self-report measures of health-promoting behaviors, perceptions, and attitudes to health and wellbeing based on existing questionnaires (see section 'Stage 2 Measures' below). Participants also completed a pain drawing on a digital interface (Apple iPad 2) using a stylus pen designed for tablets (CS100B, Wacom, Vancouver, WA, United States) and a commercially available sketching software (SketchBook Pro) (Barbero et al., 2015; Cruder et al., 2017). 3. Stage 3 (35 min): assessment of body composition (i.e., height, weight), resting blood pressure, lung function, strength and endurance (i.e., hand grip, plank and press-up), flexibility and range of motion, and cardiovascular fitness. Participants completed a version of the Nordic Musculoskeletal Questionnaire-Extended (Dawson et al., 2009). Where pain in the arm, shoulder, and hand was reported, they also completed the Quick Dash (Germann et al., 1999; Beaton et al., 2005). 4. Stage 4 (10 min): debriefing and summary of a selection of Stage 3 fitness results (i.e., blood pressure, cardiovascular fitness, grip strength, sit and reach, and press up) where data were immediately processed and could be compared against published norms, as well as providing relevant health-promotion leaflets and health education information.

Results and Impact The Healthy Performer project has resulted in the creation of the "Resources" area within the Healthy Conservatoires website. This section contains (1) films for public consumption to summarise, disseminate, and promote research and good practice in performers' health; (2) case studies of innovative approaches to performers' health from UK performing arts institutions, and (3) a hub to collect resources for performers' health from across UK institutions and organisations. These resources will be hosted indefinitely, and continue to be populated and expanded as further resources are created as a direct result of the Healthy Performer project and via collaborations with performing arts institutions.

The launch of this section has had a notable impact on engagement with the Healthy Conservatoires website. Prior to the section's launch at the Healthy Conservatoires Network meeting on 18 November 2021 the website received a consistent 250-500 views per month. Following this event, traffic immediately increased to a consistent 800-1000 views per month.
