REVERSE - Prevention of Seizures by Closed-loop wearable in Epilepsy

Lead Participant: BRAINPATCH LTD

Abstract

REVERSE is an innovation project developing the world's first **non-invasive, closed-loop system designed to predict and prevent epileptic seizures before they occur**. The project addresses one of the most urgent unmet needs in neurology: **drug-resistant epilepsy**, which affects around 15 million people worldwide who do not respond to existing medications.

Today, most epilepsy technologies are reactive. Wearable devices typically detect seizures after they have started, while invasive implants require surgery and are suitable for only a small fraction of patients. REVERSE takes a fundamentally different approach by combining **artificial intelligence, wearable brain monitoring, and non-invasive neurostimulation** into a single, automated therapy.

At the core of REVERSE is a closed-loop system that continuously monitors brain activity using a wearable EEG patch. Advanced AI algorithms analyse this data in real time to **predict seizures up to 10--30 minutes before onset**. When a high-risk state is detected, the system automatically triggers **non-invasive neurostimulation**, aiming to disrupt the seizure before it develops. This predictive and preventive approach has the potential to restore safety, independence, and quality of life for people living with unpredictable seizures.

The project brings together a strong international consortium. Alpha Brain Technologies (Netherlands) leads system integration and AI-based seizure prediction. BrainPatch Ltd. (United Kingdom) contributes its expertise in non-invasive neurostimulation and device manufacturing. Clinical validation is led by the Centre Hospitalier Universitaire Vaudois (CHUV) in Switzerland, one of Europe's leading epilepsy centres. Together, the partners are developing and clinically validating a prototype that will be ready for large-scale clinical trials within one year of project completion.

REVERSE is funded under the Eurostars programme and runs for 36 months. Beyond its clinical impact, the project establishes an entirely new therapeutic category: **predictive, preventive, non-invasive neuromodulation**. By reducing seizures, hospital admissions, and long-term disability, REVERSE has the potential to deliver significant benefits not only to patients and families, but also to healthcare systems and society at large.

With REVERSE, Europe is positioning itself at the forefront of next-generation epilepsy care---moving from reacting to seizures to **preventing them altogether**.
