What's Up With Alex (WUWA)? Animated Storytelling for Mental Health Literacy Among Young People

Abstract

This collaborative study will examine whether animated stories produced by Aardman Animations can assist young people in building awareness of and responding to mental health challenges. Typically, young people face multiple and significant barriers regarding their mental health. They may be unaware that they need help in the first place or where to access support. While such an audience may be emotionally intelligent, they can lack both the mental health literacy and finances to access timely help - something highly valued when facing very stressful periods, for example, around examination times. It is at such points that young people struggle to allocate time to seek support.
Our central question is: Can co-created, animated stories and companion app increase young people's mental health literacy?
This study adds a new dimension to the portfolio of health humanities projects supported by the AHRC, aligning strongly with and bridging two streams in its new Strategic Development Plan: a) in supporting research on mental health as an interdisciplinary contemporary challenge; and b) addressing how arts and humanities research can support creativity and the creative economy. In so doing, it brings together expertise in world-beating animation, health and the arts and humanities alongside a scientific, social and behavioural evaluative layer that seeks to advance transformative impacts in policy, provision and practice relevant to statutory and third sector organisations supporting mental health literacy among young people.
Our chief partner, Aardman, have a stellar history in film animation, winning numerous major awards and nominations including Academy Awards, Golden Globes, BAFTA Awards, Annie Awards and many others. Aardman has prided itself on creating short films on mental health to advance literacy, not least 'Share the Orange' (Alzheimers Research UK), 'Bristol Ageing Better' (Isolation and Loneliness) and, most recently, 'New Mindset', narrated by Stephen Fry. This proposed programme of work will strengthen a grand ambition at Aardman for advancing mental health literacy through animated storytelling.
Telling stories are as much part of us as breathing air. Indeed we are storytelling animals. Stories are key in helping us understand our environment, our experiences and relationships. They help us build and convey our identities, the building blocks of our selves. Furthermore, in a complex, uncertain, changing and seemingly contradictory world, stories are uniquely suited for helping us to navigate our way through it. There is growing interest in how stories and metaphor are core to life and can help people negotiate mental health. Importantly, stories can also interrogate, challenge and transform existing power relationships, including those that predominate within healthcare contexts.
Creative practices in the arts and humanities have the potential for transforming people's mental health. In fact, we can view creative resources from the arts and humanities as constituting a kind of additional or shadow health service. However, there is little in the way of research evidence for animated stories and mental health literacy. We need much clearer and robust evidence of potential impact on young people. With a growing burden of mental distress on young people combined with rising costs for the delivery of services, it is timely to investigate how animated stories could play their part in advancing mental health literacy among young people and helping them find ways to deal with life's challenges.

Planned Impact

This project has a potentially wide impact on public engagement and academic research. The principal beneficiaries of these impacts may be divided into three key groups:
1. Policy-makers: The project is designed to bring together a major animation company (Aardman), arts and humanities scholars, young people, service user/ PPI representatives, health, social care and education personnel, to respond to contemporary anxieties about mental health literacy among young people in the UK. As such, it is anticipated that research findings generated by the project may stimulate and inform mental health debate, particularly as it relates to the development of policies advancing less-centralised resources for enhancing mental health literacy and the maintenance and recovery of mental health and well-being. The inquiry will contribute to debates about broader questions concerning the role of creative approaches to public mental health.
2. Health, social care and education personnel: The project will also be of interest to health, social care, education, community arts personnel involved in mental health work with young people and the promotion of mental health literacy. The interdisciplinary subject matter and methodology of the project aligns it with the emergent field of health humanities, in this case through a combination of film animation, storytelling/narrative, psychology, language and public health. In particular, it will afford knowledge dissemination benefits with regard to the ways in which these fields inter-animate one another. It is hoped that the research findings may ultimately inform mental health literacy practices for young people relevant to Public Health England, NHS, private health, social care, education and community-based initiatives and, potentially, suggest increased effectiveness of public health approaches to reducing mental health difficulties and advancing peer education and support for young people adjusting to life's challenges.
3. The public: We envisage potential social benefits for young people with mental health difficulties most of whom will not be receiving statutory help or support during the period of funding, these emanating primarily from its impacts on public awareness and understanding of issues pertaining to mental health literacy. In particular, it is envisaged that the project will stimulate and inform wider societal discussions of topics including the respective responsibilities of different communities for the preservation of health and treatment of mental health; the benefits of shared creative activity among young people working on the project; generate public contribution to debate about ways to tackle the challenges faced by young people; informing attitudes to and/or beliefs about the feasibility of creative public health as a possible solution to the failure of biomedical and psychiatric approaches to date to address the mental health of young people; and empowering young people individually and collectively to deploy greater agency in their own mental health and well-being.
In addition, by engaging arts and humanities scholars and the creative personnel of Aardman with issues of contemporary mental health literacy among young people the project will stimulate discussion and debate about new kinds of creative public health enterprise in the future. In so doing it is hoped that the project will produce important social benefits in enhancing public understanding of new approaches to young people and their mental health and well-being needs, effecting positive changes in policy, organisational, practitioner, public behaviour.

Description The research phase to the campaign is still active and further findings and final publication outputs will be updated on the forthcoming digital showcase hosted on the Mental Health Foundation website (June 2022 onwards).

There are five sets of discoveries or findings from this project as well as additional books and a peer-reviewed high profile commentary respectively on the value of storytelling to mental health and wellbeing and mental health literacy and young people.

Findings 1: A scoping review on digital video interventions
We conducted a scoping review to map existing research in the area of digital video interventions for mental health literacy among young people. Digital video interventions represent effective tools for enhancing mental health literacy. However, there is a need for active involvement of end-users in co-creation and to attend to the production quality so that the digital video intervention is as relevant, informed, and effective as possible.

Findings 2: A collection of Digital Stories created by young people
As part of the creative research process, we conducted sets of Digital Storytelling workshops both before and after the release of the short animations. Reflections on workshop participation suggest that the storytelling/creative process enabled young people to engage fully in mental health issues, reflect more deeply on their own levels of mental health literacy and to learn from each other's mental health stories to improve their own literacy. Furthermore, the Digital Storytelling approach had currency with the young people as a tool for engagement and could be deployed in other contexts to promote mental health and other health literacies more widely.

Findings 3: A quantitative study of the effectiveness of the campaign animations
This study examines the impact of the campaign animations on mental health literacy. Seventy-one (19 males, 51 females, M age = 19.20 years, SD = 1.66, range = 17-22) young people completed a one-sample, pre-post experiment to measure changes in knowledge, attitudes, confidence, and stigma of mental health struggles, as well as help-seeking for mental ill-health before and following exposure to animations. Results revealed that knowledge, attitudes, confidence, and willingness to seek support improved at post-test. There were also significant reductions in mental health stigma following the animations. Continued long-term investment in campaigns such as 'What's up With Everyone?' seems warranted given the impact on mental health awareness, help-seeking, and stigma.

Findings 4: A qualitative study on young people's perceptions of a series of co-created youth mental health animations
We conducted seven Youth Juries to explore young people's perceptions of co-created youth mental health animations. In general, the films were favourably received by participants who provided constructive comments (both positive and negative) about them. The data suggests that the animations contributed to their understanding of mental health issues and reducing the stigma.

Findings 5: A qualitative study of trust in the campaign's online resources
We conducted six TrustScapes focus groups to explore young people's trust-related concerns about and recommendations for co-created youth online resources to promote mental health literacy. The website contains elements perceived to be both trustworthy (e.g., relatable, high-quality design) and untrustworthy (e.g., privacy policy, cookies setting).

Co creation At Aardman we have always prided ourselves in being able to understand and connect with our audiences We are very used to working closely with our audiences in developing our content, user testing is always an essential part of any of our projects but co creation took this one step further. We found that engaging young people in the creative process had the power to unlock greater understanding of the issues they face and the help they feel is needed letting them be a part of a solution rather than telling them what they need. The project has shown us how vital it is to closely involve audiences in the discussion and creation around resources to support them and not to make any assumptions about how what they need, like and feel. It has highlighted that by doing this we can achieve greater connect ion and therefore greater impact in the content we create. Collaboration with the medical and academic sectors We found it very enlightening to work with such a wide and multidisciplinary group of partners on the project. Having access to the wealth of expertise around mental health meant that we could validate the suggestions of the young people who were only really experts in their own mental health to ensure we were considering the bigger picture of young people's mental health and that the messages an d content we were providing was backed up by accurate research. Through the project we have established a valuable network of contacts in the health and academic sectors which we hope will continue to be mutually beneficial for future endeavours to support public health. Contribution creative industries can have to public health The project has demonstrated to us the power our creative industries can have in translating and communicating important messages around public health into engaging content which genuinely connects with people and has real positive impact. Mental health understanding And last but no means least, the WUWE project has given the whole team here a much better understanding of the public mental health challenges we face and the current infrastructure that supports it, as well as more specifically the needs and concerns of young people in this area. So we hope armed with this knowledge we cannot only provide better resources for our own staff and audiences, but also be alert to future opportunities to support initiatives around this. On 10th March 2023, we received the following impact assessment from Gene Beresin, MD, MA, Executive Director and Sara Rattigan, MS, Communications Director, The Clay Center for Young Healthy Minds, Massachusetts General Hospital, US: Dear Paul, It is with great pleasure that we are sending you an update on the performance, metrics, and impact of What's Up With Everyone? (WUWE) based on our posting your material on the website of the Clay Center for Young Healthy Minds at Massachusetts General Hospital. As you will see below, the impact has been impressive and synergizes beautifully with our multi-media posts. We are delighted that we can collaborate with your wonderful work! Please note our commentary is followed by two attachments demonstrating webpage traffic and google search results. In July of 2021, The Clay Center for Young Healthy Minds in the United States joined numerous organizations and groups to support the promotion of the What's Up With Everyone? (WUWE) campaign aimed at increasing mental health literacy among young people. The mission of The Clay Center, which sits within the Department of Psychiatry's Division of Public and Professional Education at Massachusetts General Hospital, is to prevent, reduce, and de-stigmatize youth mental health conditions and illness through education. Further, the Clay Center has a mission to promote the social, emotional, cognitive, and behavioral well-being of children and young people. Given The Clay Center's narrative, multi-media approach to delivering its high-quality educational content, embracing WUWE as part of its outreach seemed a natural fit to support each the campaign and its own audience. It should be noted that the domains addressed by WUWE: Perfectionism, Loneliness and Isolation, Independence, Social Media, Competitiveness, as well as seeking help, are brilliantly done, highly accessible to youth as well as their caregivers, are educational and entertaining, and evidence based. These features synchronize well with many of the same issues and challenges we see as very important for the healthy development of young people. The use of animation, capturing cognitive distortions, emotional dysregulation, and providing a means of coping - of taking these issues on as well as building resilience are all done with great care and, in our view, are highly appealing to the audience. We think the brevity of the animations, the quality of the characters, and the seminal issues young people face are executed with great care and clarity. In short, we applaud the extraordinary quality of WUWE. Working closely with WUWE campaign lead Professor Paul Crawford of the University of Nottingham, The Clay Center published a blog post on its website guest authored by Professor Crawford highlighting key elements of the campaign. The post embeds the feature WUWE campaign video, feeding in from Aardman Animations official YouTube page, and includes a call to action to engage with the campaign through social media, and learn more at the WUWE official website. Following publication of the blog post, The Clay Center promoted WUWE through its own social media channels, linking back to each The Clay Center blog post, and various WUWE videos and resources. While we cannot measure the user traffic our marketing efforts directed to official WUWE channels and platforms, we can gauge the engagement of the campaign through our website. And the ongoing impact is noticeable. The WUWE Clay Center webpage has seen continually increasing traffic since first launching July 2021, with the most prominent increase taking place between July 2022-February 2023 (see Figure 1, attached). This 8-month period has seen more than 4x the web traffic to the post when compared to the previous 12-month period (July 2021-June 2022). Some highlights include: • Despite social media marketing of the WUWE Clay Center post early on, most visits to the webpage (86%) are from organic search traffic, followed by traffic from third-party sites (12%), including education platforms like Google Classroom and UMass Medical School. • Webpage visitors from organic search traffic spent an average of 1 minute 54 seconds on the page, compared to a 55 second average across industries, in the United States; even more striking, visitors from third-party sites spent an average of 3 minutes 58 seconds on the page. • The page has had visitors from more than 30 countries worldwide, including high engagement with the page from more than half of those countries - particularly the United States, the United Kingdom, Australia, the Philippines, Canada, and Ireland. • As of March 2023, in the United States, The Clay Center's WUWE post appears as the number three result when entering the search term "what's up with everyone campaign" into Google's search engine - preceding the Mental Health Foundation's WUWE post of a similar nature. Further, when entering the more generic search term "youth mental health campaign", The Clay Center's WUWE post appears at the top of the search results list (see Figure 2, attached). All of this says to us that - similar to the Clay Center's most popular educational web content: 1) Individuals have found the WUWE Clay Center webpage, by searching, due to a need for this kind of information. 2) Individuals have found value in this information once they arrived at the page, demonstrated by the amount of time spent on the page, and the inclusion of the page on third party websites, including education-based websites for student communities. In addition, The Clay Center's WUWE post continues to help raise awareness for the campaign overall as a top-ranking source of information through Google Search for those seeking to learn more about the specific WUWE campaign, and more notably those seeking to learn more about youth mental health campaigns, in general. The Clay Center plans keep the WUWE post up on our website indefinitely. The goals and approach of the campaign align with our own, and the information provided continues to serve those in need. We look forward to watching engagement increase over the coming months and years. We would be delighted to collaborate with you in future episodes of WUWE!
