ESPRIT with Pervasive Sensing (Programme Grant)

Abstract

Elite athletes walk a fine line between performance success and failure. Although regarded by the public as examples of ultimate fitness, in reality they often exhibit vital signs bordering on clinical pathology. Their physiological parameters challenge our notions of what we consider clinically normal, for, as individuals, athletes represent a unique model of human stress adaptation and often, sadly, mal-adaptation. Understanding this human variance may assist ultimately in understanding aspects of well being in the population at large, in the work place and during healthy exercise, as well as when undergoing lifestyle changes to overcome disease, age-related changes and chronic stress.To maximise the potential of GB athletes and support the quest for gold at future World Championships, Summer and Winter Olympic and Paralympic Games, the UK's sports governing bodies and the UK sports governing bodies and research councils have identified the opportunity for engineering and physical science disciplines to support and interact with the sports community during training. Not only will this secure competitive advantage for UK athletes, it will also, of more general application, contribute understanding of the biology of athletic performance to gain insights which will improve the health and wellbeing of the population at large.The vision of ESPRIT is to position UK at the forefront of pervasive sensing in elite sports and promote its wider application in public life-long health, wellbeing and healthcare, whilst also addressing the EPSRC's key criteria for UK science and engineering research. The proposed programme represents a unique synergy of leading UK research in body sensor networks (BSN), biosensor design, sports performance monitoring and equipment design. The provision of ubiquitous and pervasive monitoring of physical, physiological, and biochemical parameters in any environment and without activity restriction and behaviour modification is the primary motivation of BSN research. This has become a reality with the recent advances in sensor design, MEMS integration, and ultra-low power micro-processor and wireless technologies. Since its inception, BSN has advanced very rapidly internationally. The proposing team has already contributed to a range of novel, low cost, miniaturised wireless devices and prototypes for sports and healthcare.

Description The ESPRIT programme has generated a range of outputs. The results have been exploited for elite sport training and London Olympics; its clinical results have been exploited in a range of acute/chronic conditions (e.g. orthopedics, diabetes, trauma, minimally invasive surgery). The consortium has also exploited many of the results commercially involving partnership with established companies as well as university spin-outs.

Title Batteryless, wireless, implantable pH and lactate sensor 
Description We have developed a micro chemical sensing system that employs simultaneous detection of pH and lactate. The system is powered using inductive telemetry, which has been used to demonstrate effective, batteryless operation. Sensor readings can be carried out with smartphones and tablets equipped with Near Field Communication (NFC) hardware, supporting remote powering of the sensor interfacing electronics and data readout. This indicates the potential of this device for implantable use. The pH and lactate sensors were electrochemically stable throughout our experiments and demonstrate good linearity in the biologically relevant ranges.

Title Flexible/stretchable graphene-based sensors for strain, pressure, electrochemical, optical and impedance measurements, and acoustic signal generation 
Description Graphitic carbon-based sensors fabricated on flexible and stretchable polymeric substrates for a wide variety of sensing applications. These include: high sensitivity strain/pressure sensors for both wearable/implantable applications and electronic skin applications for robotics; electrochemical (potentiometric and amperometric) sensors for the detection of a wide range of clinically relevant analytes such as pH, Na+, K+, Lactate and glucose; electrical bioimpedance sensors optimised via finite element modelling; electrode structures for recording of electro-physiological signals (such as ECG, EEG and EMG) and delivery of stimulus pulses; optical sensors; and devices suitable for the generation of acoustic signals.

Description An implantable optical sensor that provides real-time measurements of tissue/blood oxygen saturation as well as pulse was developed. This device has a very small platform and is wirelessly powered using inductive telemetry making it suitable for use in an implantable or wearable manner in a wide range of diseases.

Title Methods for the production of graphitic carbon-based flexible/stretchable electronic structures on polymeric substrates 
Description Methods for the production of conductive structures on flexible/stretchable polymeric substrates for the fabrication of sensors and electronics. The approach is based on post-processing of graphitic carbon structures. It is low cost, simple, and provides high accuracy in the definition of geometries with a reduced number of steps (relative to alternative techniques) and without the use of additional chemical or photolithographic stages. This approach has a resolution limit of approx. 150 microns.

Title Microfluidic sweat sensor 
Description We report the development of a flexible microfluidic platform with fully integrated sensing for on-body testing of human sweat. The system can simultaneously and selectively measure metabolites (e.g. lactate) and electrolytes (e.g. pH, sodium) together with temperature for internal calibration. The platform is designed such that a continuous flow of sweat can pass through an array of flexible microneedle type sensors (50 µm diameter) incorporated into a microfluidic channel.

Title Ultrasonic implantable sensor actuated by an external scanning probe 
Description Small factory form sensor employed for ultrasonic power harvesting inside soft tissue with the capability to measure physiological parameters such as pH, temperature, electrolyte concentration and biopotential. The sensor is actuated by an out-of-body scanning probe that is responsible for the generation of the acoustic pressure field around the implantable sensor and retrieving back the physiological data sent by the latter using a backscattering detection mechanism.

PI Contribution The ESPRIT team investigated the use of pervasive sensing technologies in assessing the performance on young talented athletes in bob skeleton. In addition, the team also assessed the relevance of biochemical indices with performance and effect of warming up on sport outcomes.
Collaborator Contribution The British Bobsleigh & Skeleton Association provided access to young talented and elite athletes in blobskeleton for the joint studies.
Impact The multidisciplinary collaboration provided a unique opportunity for the biochemist and engineers of the ESPRIT team to work with the sport scientists and athletes of the Bobskeleton. It had led to new findings and helped the development on wearable devices for other sports applications. The warming up approach was adopted by the elite team, and British Bobskeleton got the first gold medal in the Winter Olympics in Vancouver 2010.

Collaborator Contribution The talent squad team of British Bobskeleton worked closely with the Imperial team in identifying the relevant parameters and also helped run a pilot study on the feasibility of using wearable sensing technology for young talent athlete training.
Impact A first ever pilot study on using pervasive sensing technology with Bobskeleton was conducted jointly in the result of this collaboration and interesting results were obtained highlighted that the technology could potentially help quantifying the skill of the athletes.

PI Contribution The ESPRIT team worked with the British Cycling on a few different aspect of applying pervasive technologies in performance training: * Performance focussed ankle and foot modelling in elite cycling - the research team identified key elements for optimising the design of foot wear which could enhance the overall performance of an elite cyclist. * Sensing socks - the ESPRIT team developed a new sensing socks which partly facilitated the foot modelling and new foot wear design. • Cycle ergometer - a new cycling ergometer was designed and developed by the team which enable simulation of different road conditions and capture of performance indices. * Preparation and training approaches - the ESPRIT team identified key preparation and training approaches which could enhance the performance
Collaborator Contribution The sport scientists of British Cycling was working closely together with the ESPRIT team in designing and developing the technologies, and they also helped linked with the coaches and provided access to elite athletes and data.
Impact This collaboration provided an opportunity of the ESPRIT engineers to work with the top sport scientists and coaches of British Cycling in developing novel technologies. The findings of the preparation was adopted by the team prior to the Games. The team had fantastic results in major international competitions that year - World champs and Olympic medal outcomes - 8 track medals, 3 road medals Olympics

PI Contribution The research team worked closely with the BOA in developing pervasive sensing technologies for short-track ice-skating. A few prototypes were developed and trialed with the elite squads prior to the Vancouver 2012 Winter Olympic.
Collaborator Contribution The Sport scientists of the BOA team worked closely with the research team in identifying the relevant parameters and provided advises for the development of the prototypes. In addition, the sport scientists also helped in liaising with coaches and athletes in arranging trials on the developed prototypes. Furthermore, Dr. Marco Cardinale, the head of the BOA team, was also served as the member of the Research Steering Committee of the ESPRIT programme, and whom has provided invaluable advises on the programme and helped establish links with the sports
Impact Promising results were obtained from the short-track skating trials as the result of this collaboration and it has also demonstrated the feasibility of using pervasive sensing technologies for elite sports training. Although the collaboration was ended as Dr. Cardinale and the team left BOA after London 2012, this collaboration has led to the new collaboration with Qatar ASPIRE where Dr. Cardinale is now the Head of Sports Physiology of Aspire Academy in Doha.

Collaborator Contribution British Rowing provided detailed requirements and advises on the regulations for using technologies for training and competitions. In addition, they also helped participating in a couple of trials of the sensing technologies.
Impact The result of the collaboration has led to the development of the blackbox for rowing and the associated software, and the blackbox has later taken up by Sensixa Ltd (the imperial spun-off company) for commercializing the technology.

Collaborator Contribution Ken Van Someren - the Director of Sport Sciences at the English Institute of Sport was on the research steering group of the ESPRIT programme from the beginning until 2012. Throughout the programme, the EIS facilitated collaborative research with different sport teams, such as British Skeleton. In addition, the sport scientists of the EIS team worked closely with the ESPRIT team to help identifying challenges and opportunities in applying pervasive sensing technologies in different sports.

Title A SEWING DEVICE 
Description The patent discloses a surgical apparatus for the intracorporal suturing procedure. The device is a suturing probe which can manipulate a semicircle double pointed needle to perform suturing. The suturing thread is attached to the middle of the needle. The device has two movable jaws which can alternatively hold one end of the needle. One of the jaws holds the needle and drives it to pierce through the tissue. When the needle tip comes out from the tissue, the needle enters the channel of the second jaw. The locking mechanism in the first jaw releases the needle and the other jaw will grasp and retrieve the needle. Then the thread is left inside the tissue and one stitch is finished. The procedure is repeated until the wound is completely closed. During the piercing process, the needle is rotated around its geometrical centre by the jaw; this feature can guarantee a minimal tearing force on the tissue, avoiding the damage caused by the lateral movement of needle tip. When the probe is inserted into a surgical port, its jaws are closed with needle locked to one jaw. Later, the jaws are open to perform stitching. This feature enables it to go through a relative small surgical port without damage to the port. The suturing probe only requires simple driving mechanism at proximal end, so an adaptor can be designed to connect the suturing tool and its driving unit, making the tool disposable. The suturing probe has two versions. They can be mounted on the tip of a long rigid shaft and flexible joints respectively. Although the two versions have similar needle driving design based on slider-crank mechanism, their needle locking mechanisms are different. For the rigid one, the needle locking and driving mechanism are actuated by using rigid linages and nitinol flat springs, which makes it work accurately and robustly. For the flexible one, the needle locking and driving mechanism is actuated by using a tendon driven mechanism, making it possible to adapt to a flexible joint structure.

Description A bendable portion, also called a flexible joint, which is arranged at the distal end of a flexible surgical device is disclosed. Its application includes flexible endoscope or other flexible operative instruments for minimally invasive surgery (MIS). This design is a combination of rigid and compliant mechanisms. It has the advantages of accurately controllable, monolithically manufactured and easily assembled. A basic embodiment of the flexible joint section resembles a helical spring bended by pulling tendons routed through the channels passing axially of the spring. Since the helical spring represents an assembly of infinite number of flexural points, it is difficult to control reliably. To limit the number of DOF and increase axial stiffness, rolling contact joints are added at each pitch segment of the spring. Two antagonized tendons are routed 90o apart from the rolling contact joint in radically direction. Therefore, the flexible joint is constrained to only planar movement without unwanted axial compression. The aforementioned design could have many variations, e.g. the spring structure could either be single or double helix. Multiple flexible joint could be combined to achieve spatial bending and S-shape bending.

Description This medical device has been designed and developed by The Hamlyn Centre at Imperial College London. It measures different biological parameters like oxygen level in the skin. The ICL StO2 uses optical sensors to capture the small skin colour changes with blood flow. This technique is already been used in laboratories. The ICL StO2 is a much smaller and less costly alternative to current devices used in hospitals. It was developed to meet the needs of both patients and the clinical team for continuous follow up of tissue health. Results and Impact The ESPTIT team organised and presented at the ESPRIT Annual Conference 2010, Henry Ford College, Loughborough University, Loughborough. The team presented the latest developments of the project and demonstrated the prototypes to the sports scientists, coaches, academics, industrial leaders, and clinicians who attended the conference. In addition to the conference, a few focus workshops were also organised in parallel to discuss topics on paralympic sports, sports medicine, talent trainability challenge, and workload and technologies challenge. The conference and workshops initiated and facilitated discussions on the multidisciplinary research topics on sensing and sports.

Apart from identifying and clarifying the research directions among the research teams in the ESPRIT programme, the conference helped initiated many discussions and triggered a number of new collaborations. One example was the ESPRIT team's collaboration with British Sailing on new approach on accurate boat position training.

Very positive comments were received from the attendees after the event, and many of them expressed their interests in this new field and intended to introduce on the HPC approach their learnt during the school into their research. Results and Impact The ESPRIT team organised the ESPRIT Rowing Workshop at British Rowing, London, Jan 31, 2011, and invited all the leading experts in technologies for rowing. Apart from presenting and discussing the state of the art technologies for elite rowing athlete training, the workshop was organised with a joint discussion session inviting the rowing coaches and sports scientists to discuss with the research teams on how technologies could potentially lead to performance improvements, and outlining some key directions of research.

This workshop discussed the potential research directions to apply pervasive sensing technologies for rowing. The workshop had led to collaborative research and studies between the ESPRIT team and the coaches of the GB Rowing teams.

Results and Impact The ESPRIT team organised the ESPRIT Tennis and Wheelchair Sports Workshop invited all the sports scientists, technology developers and leading expert in the field to discuss the state of the art pervasive sensing technologies and how the technologies could be applied to assist elite athlete performance training. With the strong interests in the ESPRIT technologies, the sport scientists raised a many interesting questions in the workshop and led to fruitful discussions.

This workshop raised the research questions and potential research directions for application of pervasive technologies in tennis and wheelchair sports. A number of new collaborative projects were directly results from this workshop. For example, the workshop led to a joint pilot study between the ESPRIT team and the Lawn Tennis Association on using pervasive sensors for elite athletes training.

Results and Impact The research team hosted the ESPRIT Annual Conference 2011 and invited all the stakeholders, from coaches, sports scientists, clinicians, to professional sports industry representatives, and other academic researchers. In addition, presenting and demonstrating the ESPRIT research outcomes, the Annual Conference invited a few collaborators to present and demonstrate their recent works. The talks and demonstrations triggered many discussions and questions after each session.

The event provided a unique opportunity to gather all the stakeholders, researchers and engineers together and which had led to a number of collaborative projects and joint research. For instances, a new collaborative project between the ESPRIT team and the British Cycling sports science team was the result of a detail discussion between the teams during the conference.

Results and Impact Presented at the "New multimedia and remote sensing for the enhancement of sports performance" - LTN Sports Technologies - Evening Networking Event, Queen Mary Bioscience Innovation Centre, London, Oct 13, 2010. The event was well attended with local company representatives, researchers and stakeholders in the field of sensing and sports performance. The talk triggered some discussion on the potential impact of pervasive sensing technologies in the sports performance industry.

The talk raised the industries awareness of pervasive sensing technologies and their potential application in professional sports training. Shortly after the talk, a representative of a leading motion capture technology companies approached us and expressed their interests of the technologies and discussed the potential collaboration.

Results and Impact Professor Daniel Elson, our Hamlyn Centre Director of Studies, talked through a career that has mirrored the typical innovation translation pathway - from a PhD in laser physics, and time spent in the Faculty of Engineering developing imaging tools for key hole surgery, a move to the Division of Surgery saw him work directly with surgeons to begin translating those technologies into image guidance tools. He discussed progress made developing diagnostic methods and technologies, and the value of adopting a clinical, rather than purely technological driven, approach to innovation and problem solving, working with surgeons to identify challenges today and tomorrow.

Results and Impact The ESPRIT team has been selected to exhibit at this year's Royal Society Summer Science Exhibition, and the exhibition has not started yet. It is envisaged that there will be over 12000 people (from general public, primary school students, to renowned Royal Society Fellows, Nobel Laureates and Politicians) will visit the exhibition.

Although the exhibition is yet to start in July, we have already received enquiries from, other institutions, companies and media. Given the wide audience and media coverage on the event, we anticipate that the exhibition will draw a lots of interests from school pupils, general public, to fellow researchers, scientists, and experts from the industries.

Results and Impact ESPRIT research was featured in one of the leading Science Magazines, The Analytical Scientist. The article featured wearable biosensors for monitoring our personal health and the external environment that improve performance in sport and fitness. It argued that the sensors are easy to use, will cost little and provide useful, real-time information. Professor Vadgama from Queen Mary University provided insight on chemical sensing that offer almost limitless range of potential uses, including the tracking of personal health metrics while Dr Lo from Imperial College London emphasizes the role of sensors in sports and athletes' performance.

The article has led to many queries received by sport scientists, sports enthusiast and other researchers, and result in follow up discussions and potential collaborative works.

A number of speakers were invited to give a talk in the area of Body Sensor Networks. We were also invited to give a talk on "Bio-inspired Body Sensor Networks"

The symposia was well attended, and also facilitated fruitful discussions on this new research topic. Shortly after the sympoisa, we were invited to join a consortium bidding for a new EU project.
