Sonopill: minimally invasive gastrointestinal diagnosis and therapy

Abstract

Capsule endoscopy for medical diagnosis in the gastrointestinal (GI) tract has emerged only in the past 10 years. Now established in "pillcams", which have benefitted more than 1 m patients worldwide, it is a clear candidate for further innovation.

Most capsule endoscopy devices record and transmit video data representing the visual appearance of the inside of the gut, but work has begun on other diagnostic techniques, such as the measurement of pH, and there has been some research into the use of capsules for treatment as well.

Medical ultrasound imaging is a safe, inexpensive technique which can be applied in real-time at the point of care. Ultrasound is also capable of treatment through focused ultrasound surgery and, in research, for targeted drug delivery.

The core of the Sonopill programme is the exploration of ultrasound imaging and therapeutic capabilities deployed in capsule format. This will be supported by extensive pre-clinical work to demonstrate the complementary nature of ultrasound and visual imaging, along with studies of multimodal diagnosis and therapy, and of mechanisms to control the motion of the Sonopill as it travels through the GI tract.

This brings research challenges and opportunities in areas including ultrasound device and systems design, microengineering and microelectronic packaging, autonomous capsule positioning, sensor suites for diagnosis and intervention, and routes to translation into clinical practice.

Our carefully structured but open-ended approach maximises the possibility to meet these research challenges while delivering for the UK a sustainable international lead in multimodality capsule endoscopy, to provide greater capabilities for the clinician, more acceptable practice for the patient population, and lower costs for economic wellbeing.

Planned Impact

The gastrointestinal (GI) tract is subject to many different diseases. Some of these are described collectively as inflammatory bowel disease (IBD), notably including Crohn's disease and ulcerative colitis. Others relate to the reaction of the bowel to food, such as Coeliac disease, or to difficult physiological issues such as small bowel motility. Finally, cancer is a major issue, with oesophageal, stomach and colorectal cancers collectively forming more than 20% of all cases of cancer worldwide, and even cancer of the small bowel, a less common form, affecting more than 1 in 500 people in North America. These diseases are the target for Sonopill and the GI tract is its domain.

Sonopill will impact GI tract disease in three ways. First, it will provide a new means of diagnosis, including for early tissue dysplasia. Endoscopic ultrasound has been highlighted as a major innovation in endoscopy. Through the application of high resolution ultrasound (HRUS), the Sonopill will represent just as large a step forward in diagnosis from the present optical pillcams, themselves a revolutionary development in difficult diagnoses. Second, it will provide a new means of treatment. Using high intensity focused ultrasound (HIFU) for localised focused ultrasound surgery (FUS) and ultrasound-mediated targeted drug delivery (TDD), and with the potential for other means of hyperthermia such as microwave ablation, it will offer the GI consultant new therapeutic possibilities. Third, it will provide a means to further research in the GI tract, allowing repeated observation in a way that is presently impossible because of the invasive nature of conventional endoscopy and colonoscopy, and difficulty in passing enteroscopes through the entirety of the GI tract.

To realise clinical impact, Sonopill must reach commercial reality. To that end, we have partnered with a dozen companies covering the complete spectrum of Sonopill research and production, from virtual prototyping (Weidlinger Associates) through MEMS fabrication (Semefab) to testing (Precision Acoustics) and access to clinical practice (Karl Storz Endoskope). These companies will join an Industrial Impact Group (IIG) to ensure we keep in mind commercial constraints. Highly experienced senior figures from industry and universities have also agreed to join our International Advisory Committee (IAC), such as Dr Dalmolin (SORIN), Prof. Rau (Aachen), Dr McLaughlin (Zonare Medical Systems) and Prof. ter Haar (Institute of Cancer Research), and these will be complemented by a Local Clinical Committee (LCC).

In reaching commercial reality, Sonopill will also have economic impact, as the technology emerging from the research is productionised, then realised in mass production. There will also be a significant economic impact in the NHS and other healthcare systems from the reduced costs of diagnosis and treatment. For example, whilst the present £500 cost of an existing pillcam is not dissimilar to the cost of a straightforward gastroscopy, even this relatively simple device leads to much quicker diagnosis of difficult cases, saving the cost of multiple gastroscopies and visits to the clinic. This will, in turn, have an impact on society, as diagnosis and treatment become easier and therefore intrude less into the everyday life of the patient. Sonopill will also impact people and skills, directly by providing challenging training and scientific and engineering research for its cohort of early stage researchers (ESRs), and indirectly through advocacy for science and engineering which will contribute to engagement of the general population with subjects within the science, technology, engineering and maths (STEM) subjects.

Description Capsule endoscopy for medical diagnosis in the gut, in which a pill with sensing capabilities is swallowed, has emerged only in the past 15 - 20 years. Now well established, it has benefited more than 4M patients worldwide and is a clear candidate for continuous innovation.

Most capsule endoscopy devices record video data representing the visual appearance of the inside of the gut, but work has begun on other diagnostic techniques, such as the measurement of pH, and there has been some research into the use of capsules for treatment as well.

The Sonopill programme combined medical ultrasound, both diagnostic and therapeutic, with capsule endoscopy and robotic manipulation for the first time.

The programme team identified relevant clinical targets, progressed development of early versions of capsule endoscopes, designed and tested special silicon chips to fit into the tiny volume of the pill and explored other possible functions for the pill, to complement ultrasound.
Exploitation Route Collaboration took place with Dr Weibao Qiu and other researchers at Shenzhen Institute of Advanced Technology, China, on development of a simplifed Pathfinder Pill and also in possible industrial applications of the Sonopill concept, to complement the healthcare applications that were and remain of immediate interest.

Discussion also took place with Dr Maya Thanou, Institute of Pharmacological Sciences, Kings College London, on drug delivery with the Sonopill, and collaborative work in this area gained studentship funding; a student is now in post and carrying out research actively, in partnership with the SME Curileum.

Description The Sonopill programme research team was in place for 5 1/2 years, according to staff recruitment dates. Many findings were realised, with significant engagement with the scientific and clinical communities and the general public. The Programme had 15 industrial partners with assigned points of contact with the programme team. Many of the companies provided advice and support in kind and the Sonopill team presented challenges to them and supported their horizon scanning and future product research. A strategic link was established between the Sonopill team and Dundee Science Centre (DSC), through a Wellcome Trust funded grant with the Institute for Medical Science and Technology, the host for the programme. This resulted in a permanent exhibition and educational provision. Further related links were also made with Glasgow Science Centre, the largest such centre in the UK. Results from the programme were used to support a successful proposal for the EU H2020 FET OPEN project "AUTOCAPSULE", led by G.Iannaccone from University of Pisa, with original Sonopill partners S.Cochran, University of Glasgow, and P.Valdastri, University of Leeds. This project will explore several themes already considered in the Sonopill Programme, including ultrasound imaging, robotic manipulation and wireless power delivery.

Title Development of Small Bowel Tissue Phantom for Microultrasound (µUS) Investigation 
Description To investigate µUS for transmural imaging, a phantom which is biologically similar to the human GI tract which allows for experiments in which the physiological environment and acoustic properties of the tissue can be manipulated to mimic disease. A phantom utilizing porcine small bowel tissue, which largely mimics human GI histology, has been developed for experimentation with µUS. The phantom was scanned using a single element µUS scanner mounted on a stage motor able to travel in both the x and y-axes to provide biaxial scans for analysis. To simulate physiological conditions, the tissue samples were perfused with degassed phosphate buffer saline (dPBS) at 200 ml/hour via a syringe perfusor to establish sufficient mucosal characterization. Per-fusion was established via a cannula inserted in the mesenteric vessel of the tissue.

Results and Impact As part of the Explorathon event, which is part of the European Researchers' Night, we talked to over 70 students. Many of whom were disadvantaged areas as part of the widening access program of HWU. Student numbers were assessed from the number of stickers given away. We kept the stickers so they can be used as a similar metric in future.
The students were engaged with the Sonopill project, asking a lot of questions regarding ultrasonic imaging, the future of medicine and microengineering. The event was hosted by Dr. H. Lay, Dr. B. Cox and Dr. G. Cummins

This was followed by a Q&A session with a panel of engineers from across the University covering topics from 'what is engineering' to the required schooling, to ethical debates.

Results and Impact Sonopill was invited to take part in Robotastic at Dynamic Earth, this was a free event in one of Edinburgh's most popular tourist attractions. It was held at the same time as a "Living Machines, 2016" a biomimetics conference. We had a lot of interest in our exhibit from conference attendees, tourists and children. The conference attendees were interested in the more technical aspects about this project and the tourists and children were interested in the general technical topic of miniaturised medical devices as well as the application of the device as a clinical diagnostic tool. Several people attending asked for more information on the project, some of whom spoke of their own experiences of conventional endoscopy were directed to the project website to learn more of our work. Two conference attendees also asked for the website address to learn more about the technical aspects of the project.
