Mobility, Mood and Place: a user-centred approach to design of built environments to make mobility easy, enjoyable and meaningful for older people

Lead Research Organisation: University of Edinburgh
Department Name: Edinburgh College of Art

Abstract

Recent research shows that remaining active is a vital component in healthy ageing and that exercise provides protection against mental decline in old age. People are more mobile if they live in an appropriate environment, one that is safe, accessible and has good services. To date, much guidance has focused on overcoming barriers in the environment, such as steps without handrails or poor quality lighting. Removing such barriers is important but this approach alone will not encourage people to be more active. We need to understand the positive qualities that encourage people to go out, remain mobile, and give them pleasure into very old age. Our proposal builds on growing evidence that mood and emotion influence people's willingness to be active, which is in turn influenced by the experience of different environments - the 'mood' of one place versus another.

Places need to be attractive, in order to support positive moods, and to draw people into them. Some places offer peace and quiet, others offer sociability, excitement or a sense of fun. The environment needs to offer different opportunities according to how people feel at the time. Well-designed places don't automatically put people in a good mood, but we think places that match the emotional needs of the moment contribute to a self-reinforcing virtuous cycle. If the mood of a place is right then people will get out and about, feel better, take better care of their environment, and think of their environment more positively.

Positive emotions broaden people's awareness and encourage them to think and act in novel ways, and to be more curious and exploratory, both mentally and physically. Environments that encourage positive moods affect how competent people are at carrying out everyday tasks, such as preparing meals, going to the shops or planning an outing. People who feel competent are more likely to focus on the positive, to feel well, to make healthy choices, and to be more mobile.

We will draw on our studies of the patterns, causes, and effects of health conditions among groups of people (epidemiology), combined with techniques in neuroscience involving brain imaging. We will also work in equal partnership with older people, including stroke survivors and people with dementia, to design together better environments. This innovative combination of approaches will help us to deliver new ideas about the design of places that support positive emotions, reduce anxiety, and encourage people to be more active and mobile, long into old age. Well-designed and maintained environments should make mobility an easy, enjoyable and meaningful choice.

Our research is structured around four Work Packages involving: (1) designing together with older people, (2) examining data from recordings of neural signals (EEG) while people are moving through different environments, (3) studying information from a large group of older people born in the 1920s and 30s, to understand patterns of environment, activity and health over their life course, and (4) working with partners to evaluate and share our results and develop illustrated, user-friendly guidance on how to provide better environments in future.

Our team of researchers has expertise in environment, health, wellbeing, social policy and collaborative design, with an excellent track record of past Research Council and other major grants. We have experience in providing design guidance for the built environment, innovative approaches to measuring people's responses to their environment, working with them to understand their preferences, and analysing the implications of results. We also have experience in mapping important aspects of the environment.

To maximise the impact of our research, we will mobilise our ongoing partnerships with policy-makers at national and local government level, with professional bodies, and with third sector organisations supporting older people and age-friendly environmental design.

Planned Impact

Who will benefit from this research?

For the successful application of our research findings to everyday challenges and opportunities, we will take a cross-cutting approach to the types of stakeholders we engage with and to the ways in which we engage with them. The research beneficiaries will be:

- Local, central and European government, including elected members and officials; Thinktanks and non-departmental public bodies.
- Public sector practitioners in health, housing and social care; Professional organisations spanning these sectors.
- Private sector practitioners in planning, urban design, product design, architecture, landscape architecture, transport consultancy and highways engineering; Professional member organisations spanning these sectors; Mobility and telecare industry.
- Not-for-profit organisations and charities spanning sectors outlined above, plus those dedicated to older people's issues; Older people, their families and carers.
- Academic networks promoting interdisciplinary work in sectors outlined above.

How will they benefit from this research?

Mobility, Mood and Place is rooted in co-creative principles, with projected outputs tailored to the advancement, not only of academic expertise but also of the UK's wider knowledge base, its society, people and economy. As an interdisciplinary team, we have excellent existing connections within different spheres of influence and sectors and will achieve maximum socio-economic impact by strengthening the networks between them.

We have a clear perspective on where ageing and mobility issues sit within the focus of the four UK governments, with whom we have established good contacts. Our team has input into health, quality of life and social inclusion strategies at the critical early stages, including the new Five Year Ageing Well programme in Wales, which will evolve as our project does. By the end of Mobility, Mood and Place we will have produced a series of policy papers. They will offer sustainable, long-term options to those tasked with reducing health inequalities among older populations.

We have strong links with Local Government bodies and various individual councils and health boards. Liaising with them will assist in identifying live research sites and facilitate direct engagement with older stakeholders for research recruitment and communication. We will include design principles and exemplars among our outputs, developed collaboratively by trainee/early career designers, researchers and older people. This will enhance designers' skills before they enter professional practice and refine a model for workshops to be more widely used in architectural education.

We know that age-friendly services can give manufacturers a competitive advantage in the marketplace. Through business-led position papers based on academic evidence, we will provide feedback to industry from an older population with an increasing economic profile and influence. Our work will aid understanding of underlying reasons for different physical behaviours and emotional responses to place, and therefore how people might respond to different interventions in the built environment. We will facilitate training events that bring together professional organisations, so that each develops an understanding of how the others assess, interpret and meet user needs.

Many older people are themselves active in lobbying for more age-friendly design. We aim to better enable them to do so, e.g. through social networking. To reach those who are not technologically enabled, as well as other user groups, we will secure the partnership of charities and not-for-profit organisations. We will work with stakeholders to promote the positive effects of care home residents remaining active in the community and to assist care providers in making more informed choices with respect to the mobility of older service users, including the significant proportion (~50%) with some form of dementia.

Impact This was the first showing of the MMP exhibition beyond the UK, where it toured three Scottish cities as part of Scotland's Year of Innovation, Architecture and Design 2016 and to the House of Lords in London in January 2017. It took place at Scotland House (home of Scotland in Europe) in Brussels in October 2017 as part of 'Health at the Heart of the Urban Development Strategy', a European Week of Regions and Cities fringe event organised by the City of Utrecht. At this same event, another MMP output - The A-Z of Co-Design - was launched as a desktop calendar in English and French (Spanish also available), and given to all delegates together with an illustrated, 32-page guide to the 13 key things uncovered through MMP's co-design process about older people's needs and preferences for age-friendly environments. Together with the participation of Professor Catharine Ward Thompson (MMP PI) in the event's panel discussion, and associated activities with the Covenant on Demographic Change and the City of Utrecht, these activities have led to findings from MMP being fed into the European Union Committee of the Regions' emerging EU Urban Agenda.

Description MMP - Key findings and their implications.
Being able to get outdoors is essential for maintaining health and wellbeing into later life, but many older people find it becomes less easy, enjoyable and meaningful as they age. Meanwhile, our society is growing older: across Europe, the number of people aged 60+ is expected to increase by around two million, annually, for the foreseeable future.
To help ensure that living longer is a positive experience for everyone, we need evidence-based solutions to known challenges. The Mobility, Mood and Place (MMP) research project has been exploring how places can be designed collaboratively to support outdoor activity, health, wellbeing and participation decisions about the community as people age.
Drawing on the participation of over 900 people, most of whom were aged 60+, our findings have implications for the way we design for people of all ages, backgrounds and abilities. These are our key findings and their implications:
• Four qualities of places really make a difference to quality of life: access for all, access to nature, access to others and access to light.
• The mundane matters and the commonplace counts! Everyday things, such as pavement quality, benches and street lighting, can make all the difference as we get older.
• Busy urban environments demand and direct our attention; too much of this makes us cognitively fatigued, less effective and more stressed. Older people walking between different types of urban environments show changes in their emotional response to place based on brain activity patterns. Green spaces seem to be restorative, offering a respite from the demands that busy urban places make on our attention.
• Even a short walk can lift the mood if the environment is sufficiently varied. Things like colour and wildlife, opportunities for social contact, and familiar places, especially those linked to key memories, are highly valued.
• The design and layout of the home can help older people to be more active.
• Well-designed environments can support older adults who have experienced a stroke to engage in, and return to, the activities they value.
• Healthy ageing is influenced by local environments throughout our lives, including right back to childhood. Features of the environment - such as nearby access to public parks - can have lifelong positive associations with healthy ageing, and in particular with better cognitive health in later life.

What are the messages for the future?
• Innovative neuroscience methods offer new opportunities to understand older people's response to place
• We need to take very long term views on fostering resilience and healthy ageing.
• the quality of the outdoor environment we provide for children is important for them then, and throughout the rest of their lives.
• Plan well, design well, maintain well - these are vitally important for everyday, health-enhancing environments throughout our lives.
Exploitation Route This project has already had impact at European, UK and local levels in supporting policies for age-friendly built environments and training practitioners and third sector workers for their implementation in practice. It engaged with the European Union's Committee of the Regions via a roundtable discussion and exhibition in Brussels, and with European Local Authorities via a conference presentation in The Hague. It delivered training in London on effective co-design with older people to professional practitioners and not-for-profit groups, and policy and practice recommendations to health and design professionals in the specialist area of stroke survival and rehabilitation. It has been taken up by local authorities (e.g. Moray Council) who want to develop their age-friendly policies and services, and have used our animation as a way of engaging their electorate with the issues and potential solutions or approaches.
These outcomes and impacts illustrate how our findings may be taken forward more widely in future.

Description By 2050, 89% of the UK's population will live in cities and 24% will be aged over 65. This is the basis on which, in early 2016, the Parliamentary Office of Science & Technology (POST) decided that UK parliamentarians urgently required briefing on creating age-friendly cities. Having read the first academic outputs from Mobility, Mood and Place (MMP), POST contacted the research team to ask for assistance with reviewing the scope of its work. Over the next six months, continued liaison led to the researchers identifying themes for inclusion, peer reviewing drafts and, finally, to MMP (and earlier research by the team) being cited in POSTnote 539 published in October 2016. During the development phase of POSTnote 539, MMP researchers made two key recommendations, the first being that the Note should highlight the importance of urban greenspace for people's health and wellbeing. In the published Note, there is a whole section given over to greenspace and its association with multiple health benefits. With MMP researchers' involvement, this aspect of age-friendly urban placemaking has also been gaining traction in international policy arenas, with the World Health Organization (WHO) leading the way. For example, in November 2015 - on the basis of her "invaluable contribution to our work on green space and health" - MMP PI, Professor Ward Thompson, was invited to speak on the subject at a WHO European Environment and Health Task Force (EHTF) meeting in Skopje, Macedonia. The meeting was deemed to be of "particular strategic importance" to the subsequent Ministerial Conference and Declaration on Environment and Health, signed in Ostrava, Czech Republic, in June 2017. Among other objectives, this committed Member States to "supporting the efforts of European cities and regions to become healthier and more inclusive, safe, resilient and sustainable through an integrated, smart and health-promoting approach to urban and spatial planning". Other outputs arising from the Macedonia meeting include a WHO European Region report on 'Urban Green Space and Health' (Copenhagen, 2016) citing research papers from our work, particularly as it relates to mobile EEG measures of neural activity. Partnered by the Covenant on Demographic Change as part of an Impact Acceleration Award, MMP has also been working with European cities and regions on age-friendly policies and strategies, most notably the City of Utrecht, convenors of the Health at the Heart of the Urban Development Strategy discussion during the European Week of Regions and Cities 2017 (with Professor Ward Thompson as a panellist, alongside peers from Utrecht, Métropole de Lyon, the WHO and DG Santé). In February 2018, following a gathering of 43 mayors and 85 high-level political representatives in Copenhagen on the same topic, Deputy Mayor Victor Everhardt of Utrecht (a panellist at the Brussels event), announced the creation of a new WHO Europe Healthy Cities Network working group on environment and health. The second key recommendation made by the MMP team to POSTnote 539 is reflected in its headline point on "the lack of robust evidence about the effectiveness of proposed interventions". This is a theme carried over from evidence given by Professor Ward Thompson to the House of Lords Design Commission Inquiry on Built Environment and Behaviour (Neighbourhoods and Housing) in November 2015. The Commission's invitation to give evidence in person was made on the quality of MMP's earlier written response to the Call for Evidence which cited, among other outputs, the team's journal paper on the home as an enabler of active lifestyles (Dr Brookfield as lead author). In January 2017, over seventy guests from industry, policy and the not-for-profit sector joined the researchers for an exhibition and reception at the House of Lords at which Baroness Greengross talked about the importance of the project. To effect the step-changes in age-friendly placemaking envisioned in policy, industry requires guidance on appropriate methods and techniques. Since late 2015, MMP has been working with the Centre for Accessible Environments (CAE) to promote co-design as an effective way of involving end users in the design process. Having contributed to the development of The A-Z of Co-Design, MMP's practical guide to the participatory design process, CAE included the publication as a centre pull-out within Access by Design, a journal distributed to 700 industry subscribers. Writing in the foreword to the A-Z, the Centre's Director, Jean Hewitt, describes it as "nothing short of inspirational", "a catalyst to fresh thinking", and an "engaging tool which will help us all to use participatory design as the norm". Originally published as a folding wallchart, the A-Z was reprinted in English, Spanish and French as a desktop calendar for 2018. Through partners, including the Design for All Foundation and the Covenant on Demographic Change, 500 copies were distributed to professionals, local and national government, and third sector organisations throughout Europe and Francophone Canada, leading to multiple new/renewed connections, including with the Laboratory for Inclusive Mobility (France) and the Francophone Network of Age-Friendly Cities and Communities. In February 2017, MMP entered into a Memorandum of Agreement with CAE to develop a professional training course based on the A-Z, with the first session delivered in June 2017 to participants from a range of disciplines, including planners, urban designers and strategists from the Royal Borough of Kingston, Future for London, Royal Town Planning Institute and Urban Design London. Comments on the usefulness of the training included "I'm running a project on Ageing London - Housing and Connectivity so I will use the research design recommendations" and "I'll definitely use some of the literature". In addition to the A-Z, key non-academic outputs from MMP include a three minute animation on findings and recommendations, an illustrated, 32-page guide to co-designing age-friendly environments, and a touring exhibition. Promoted through social media, the animation has been widely admired and shared, with comments including "a great animation and a very important message" (Age Scotland), and "excellent video about developing places to support healthy ageing" (Greenspace Scotland). Diarmaid Lawlor, Head of Urbanism at Architecture and Design Scotland (A&DS), has called MMP "a hugely important project giving older people a voice in making urban and rural environments better places for lifelong living" and the exhibition in particular "an opportunity to bring world class thinking and engagement in touch with local initiatives to inform debate and influence stakeholder thinking". Having promoted the exhibition in three Scottish venues as part of its nationwide Say Hello to Architecture programme, the Festival of Architecture, and Year of Innovation, Architecture and Design 2016, A&DS also promoted and hosted the launch of the 32 page guide to co-design in November 2017, leading to ongoing engagement with Moray Council on a project to better align land use planning with healthcare objectives, the council having already incorporated aspects of MMP into its new draft policy on placemaking.

PI Contribution Building on an increasingly close and supportive relationship over the research phase of Mobility, Mood and Place (MMP), the MMP team entered into a Memorandum of Agreement with CAE in February 2017 to develop a professional training course based on the 'A-Z of Co-Design', a non-academic output from MMP (published in July 2016 as a folding wallchart, and reissued in January 2018 as a desktop calendar in three languages: English; Spanish; and French). The course content was co-created between February and June 2017 as part of an ESRC Impact Acceleration Award. The content draws on both the methods and findings of Mobility, Mood and Place (MMP), specifically MMP's exploration of the use of co-design techniques by early career designers and older people. The one-day session is divided into two parts: a morning of theory; and an afternoon of co-design activities based in and around CAE's training suite in central London. A pilot version was delivered on 26th June 2017 by Jean Hewitt (CAE) and Iain Scott (MMP), with assistance from Dr Katherine Brookfield and Máire Cox (both MMP).
Collaborator Contribution CAE is marketing the course, and providing the training suite. The morning (theory) session includes CAE's material on inclusive design, including the legislative framework. Overall, the course has been co-designed such that CAE can continue to deliver it, subject to market demand, beyond the duration of MMP's Impact Acceleration Award.
Impact Eight participants attended the pilot session of the course in June 2016, drawn from a target group of stakeholders (commercial and local authority planners, urban designers and architects, plus access consultants and members of Future for London and Urban Design London). Each filled in an evaluation form. Feedback was overwhelmingly positive and, between June and December 2017, was fed into the development of the finalised version of the course. This finalised version was delivered - in the first instance - at the CAE training suite on 28th March 2018, with four participants and, again, very positive feedback. The course may run again in the future.

UPDATE (March 2017): The relationship with the Centre for Accessible Environments (CAE) has gone from strength to strength. CAE wrote the foreword for our A-Z of Co-Design, distributed it free to over 600 subscribers of industry journal, Access by Design, and exhibited at the MMP conference. The Centre is now partnering our ESRC IAA to develop a training programme (based on the A-Z) for professional practitioners. Staff and Board members attended our reception at the House of Lords; as did Alastair Somerville and Nicola Mathers.

Results and Impact Bill Turnbull, star of BBC's Breakfast News TV, Strictly Come Dancing, Classic FM etc., came to the Psychology department on 23rd January 2017 with a BBC film crew to find out about the Lothian Birth Cohort 1936 (LBC1936)/Disconnected Mind study.

BBC1 are planning a series in the Spring called 'Holding back the years'. Each programme will feature a celebrity revisiting a place that means a lot to them, and the programmes will have a focus on the processes of human ageing. Mr Turnbull was at the University of Edinburgh in the 1970's (he shared a flat with Gordon Brown for a term!), and so his programme is based here. It was natural that, to find out more about ageing, and with an Edinburgh locus, he and the BBC filming team visited the LBC1936. A busy afternoon's filming was filled with his interviewing LBC1936 Director Ian Deary, speaking with about a dozen of the LBC1936 participants, and seeing and taking part in some cognitive testing. Look out for the series, especially the Edinburgh-based Bill Turnbull programme in a few months' time.

Results and Impact The new Edinburgh Speakers Festival 2016 took place from 9th-11th September and our own CCACE Director was invited to be part of this year's prestigious line-up. Speakers covered a large variety of topics including cognitive ageing, devolution, politics, history, literature, fashion, Islam, capitalism, cycling, RBS, the Lockerbie bombing, whisky and women's equality, but all with a focus on Scotland. Ian's talk was entitled "Understanding Healthy Cognitive Ageing: Scotland's Unique Contribution" and took place on Saturday 10th September.

This is what Ian said about the event:

"Well" said Disconnected Mind Director Ian Deary, "That's the first time I have been on the same bill as Ken Livingstone and Ruth Davisdson! What an ecclectic programme." Ian spoke on healthy cognitive ageing to an audience at the Edinburgh Speakers Festival. Of course, the LBC1936's results were highlighted, with a focus on the likely protective factors for cognitive ageing. "There was a middle-aged finalist from this year's Brain of Britain Radio 4 show in the audience", said Ian, "so that was a good basis from which to explain how crystallised ability ages pretty well." As usual, the photos of Mr Scott and his brain images went down well with the enthusiastic attendees.

Most of the materials we exhibited were related to our work on ageing, including Mobility, Mood and Place and Inclusive Design for Getting Outdoors. The A-Z of Co-Design proved particularly popular with visitors to our stand, as many were seeking ways to work more collaboratively with partners and communities. Therefore, even if our research questions were not of particular relevance to them, our methods were. For example, the Scottish Fire and Rescue Service, who are currently looking at ways to make the service more inclusive. A number of the following asked for business cards so that they could get in touch to arrange a longer / more targeted meeting: Alison Clyde (Generations Working Together); Anne Connor (Outside the Box); Heather Edwards (Care Inspectorate); Karen Windle (University of Stirling); Peggy Winford (Scottish Government); Scott Neill (City of Edinburgh Council).

Active Design: Building Healthier Cities. It was advertised as follows:

"We shape our buildings; thereafter they shape us," said Sir Winston Churchill in his speech to the meeting in the House of Lords, October 28, 1943.
People have the power to design our built environment. But how we design our homes and cities can also have power over us, from our health to how we socialise.

A University of Cambridge study found that about 676,000 deaths each year were down to inactivity, compared with 337,000 from carrying too much weight. He said eliminating inactivity in Europe would cut mortality rates by nearly 7.5%, or 676,000 deaths, but eliminating obesity would cut rates by just 3.6%.

So with that in mind, how can we design a healthier, active built environment?

Come join us as we find out! On Thursday the 3rd of February Newham Fabians will be joined by Kris Sangani, LCC Trustee and former coordinator for Redbridge LCC, Councillor Clyde Loakes, Deputy Leader of Waltham Forest Council, and Prof Catherine Thompson, Professor of Landscape Architecture.

Results and Impact Penicuik U3A discuss healthy cognitive ageing
"It's always a good sign when they have to put out extra chairs," said Ian Deary, after his talk on 16th February to Penicuik's branch of the University of the Third Age. "They said they usually get 30-50 people, but I lost count after 60. It's great to see such enthusiasm for hearing about healthy cognitive ageing and the Lothian Birth Cohorts' contribution to that. I encouraged people in the audience to ask any questions as we went along, and they were good at that. However, there were lots more excellent questions and discussions at the end of the talk. They wanted to know the evidence about many things, including genetics, dementia, physical exercise, smoking, sleep and cognitive engagement. I was especially pleased, when I was having my tea and custard cream afterwards, when an audience member congratulated my team on demonstrating reverse causation for some potential factors in cognitive ageing!" We note that Ian cycled to and from Penicuik to give the talk, which we hope contributed a little towards keeping him sharp.

Results and Impact This event focused on Improving Care for Older People in Northern Ireland. Convened by Claire Sugden MLA, the Chair of the Northern Ireland Assembly's All-Party Group on Ageing and Older People, it took the form of keynote presentations by politicians, senior civil servants and senior representatives of third sector organisations, followed by questions and comments from the floor.

For us, as well as hearing about policy and practice in Northern Ireland (and how it differs from the rest of the UK), it was a chance to reconnect with one of the keynote speakers, Roger O'Sullivan; formerly of CARDI, now Director of the Ageing Research & Development Division at The It was also an opportunity to introduce the second keynote speaker, Chris Matthews (Department of Health, Social Services and Public Safety) to our work. He was keen that we keep in touch and provided me with an update on a potential future stream of influence: an emerging Department of Communities in Northern Ireland.

I subsequently submitted a 600-word briefing paper on MMP for inclusion in the official event publication. It has been accepted and will go out to all the event delegates, plus wider contacts in government, the Northern Ireland Assembly and stakeholders more widely.Institute of Public Health in Ireland, who has since emailed me with a number of potential contacts for further dissemination of our research.

In addition to the presentation, I distributed printed materials which outlined our project's work on co-design.
The MMP presentation sparked discussion around ideas of public spaces and how they are used by different generations, the effects of the economic divide between poor and rich older people in terms of access to services and the experience of older migrants in European urban centres. Our research generated a lot of interest with many delegates noting our website address in order to access more information. Results and Impact 'Sensing through Impairments' was the fifth in the Feeling Good in Public Spaces series co-hosted by the ARCC Network and the Feeling Good Foundation. The event focused on research and case studies that tackle how to design an environment to empower people who have lost a sense. Held at the central London offices of the IBI Group, where two of the presenters were based, it attracted a capacity crowd of practitioners, early career and established researchers, policymakers and representatives of third sector organisations. For MMP, it was a chance not only to share methods and emerging findings, but to network with some recently-made contacts, including a Parliamentary Office of Science and Technology Research Fellow and the Director of the Centre for Accessible Environments, both of whom we also met (separately) for a longer period the following day. Subsequent to the event, the co-hosts blogged about it on LinkedIn and on the ARCC Network website and will be developing their feature into a longer paper to be published in Access by Design (the journal of the Centre for Accessible Environments).

UPDATE (March 2017): Katherine Brookfield and Sara Tilley have since participated in the final Feeling Good in Public Spaces event and ARCC's Knowledge Exchange Manager attended our reception in the House of Lords. We have been invited to participate in an ARCC initiative to synthesise Design for Wellbeing findings and present them to stakeholders and are attending both events (20th March 2017 in York; 4th April 2017 in London).

Results and Impact The Lothian Birth Cohort 1936 (LBC1936) got a good airing on BBC Radio Scotland on Sunday 26th February 2017. LBC1936 participant Margaret Macintosh and Study Director Ian Deary were interviewed by presenter Cathy MacDonald on the programme Sunday Morning With... "Cathy MacDonald was clearly very taken with the uniqueness and long-term nature of the LBC1936 sample and the importance and range of the findings", said Ian, "and so a piece that was planned to be 5-10 minutes long ran for about 20 minutes. It was great to hear Margaret's enthusiasm for the project, her reflections on what we do, and what we might add to the study." The radio piece was stimulated by the front-page article in The Times on 21st February 2017 which discussed the study on personality stability that was conducted on the LBC1936's sister study, the 6-Day Sample.
