Abstract

Recent advances in surgery have made a significant impact on the management of major acute diseases, prolonging life and continuously pushing the boundaries of survival. Despite increasing sophistication of surgical intervention, complications remain common and poorly understood, contributing significantly to mortality and morbidity. Surgical site infections, catheter related sepsis, wound dehiscence and gastrointestinal anastomotic leakage are recognised complications following surgical interventions or invasive monitoring of critically ill surgical patients. Current methods for detecting these complications rely on episodic clinical examination with 'snap shot' laboratory testing. There is therefore a pressing need to develop new sensing technologies that can be seamlessly integrated with existing surgical appliances to provide continuous sensing and early detection of these adverse events, thus minimising post-operative infection, complication, and readmission. All these will also have a direct impact on healthcare economics, and more importantly the prognosis and quality-of-life of patients after surgery.

The proposed project is organised into three research themes: 1) Multimodal Sensing and Miniaturised Embodiment; 2) Active Sensing with Low Power Microelectronics; and 3) Data Inferencing and Stratified Patient Management. These research themes address key technical issues related to sensor design, miniaturisation, and self-calibration, as well as low-power on-node processing, inferencing, and clinical decision support. These research themes are connected by three clinical exemplars in surgical sensing with increasing levels of technical complexity. The vision is to develop smart sensors integrated with surgical appliances and to be inserted in close proximity to the surgical site, encased within surgical drains/catheters, or placed in locations to more seamlessly monitor the systemic inflammatory response. The devices will be implanted during elective surgery or at biopsy, interrogated wirelessly, and eliminated by natural processes, or routine removal of 'hosts' such as the drains or catheters. The research programme is underpinned by extensive experience of the team in body sensor networks and bio-photonics in healthcare. Through an integrated programme of engineering research and development of a novel real-time active sensing paradigm, the project aims to transform the care pathways for surgery with greater consideration on personalised treatment, system level impact, real-time response to complications, patient concordance and quality of life. We expect that the outcome of the research will help improve surgical workflows, support safe discharge and home/community-based recovery, reduce unplanned readmissions, and influence the future of healthcare policy.

Planned Impact

The proposed project addresses some of the pressing, as well as long-term challenges faced by healthcare systems around the world including the NHS. Surgical site infection accounts for over 15% of all hospital acquired infections; anastomotic leakage is associated with morbidity, poor functional outcome, increased risk of death and local recurrence in cancer patients. They both challenge the model of early discharge of patients following new therapeutic regimes such as minimally invasive surgery. For cancer, greater precision in response detection through sensing would improve patient confidence in treatment efficacy and would augment clinical decision making through a more thorough appreciation of disease progression. The proposed project covers the entire development cycle from fundamental theory to clinical application, with new devices/platforms that will establish tangible clinical impact within the next 10 years, as well as concepts that will reshape the landscape of sensing technologies for years to come.

People - To stimulate creative and adventurous approaches that underpin the objectives of the project, we will actively encourage young researchers to spearhead and take ownership of blue-sky idea generation with direct clinical interaction. Secondment to industrial collaborators and across consortium partners will be arranged for all research staff. The team will also be encouraged to lead public dissemination activities and develop the necessary managerial, vocational and entrepreneurial skills for career progression.

Knowledge and Economy - Through technological innovation and effective collaboration with all project stakeholders, we will ensure effective knowledge transfer, and clinical translation with measurable clinical impact.

Commercial - The potential IPs to be generated range from licensing, platform development, and commercial spin-offs, to mixed independent exploitation of the basic technology developed. We already have strong support from the industrial partners of the project and will work with them to identify potential opportunities for commercial exploitation throughout the duration of the project. To this end, an IP advisory group will be formed to provide critical review and management/technical recommendations to the research team and to establish effective dissemination routes to both academic and industrial communities. Key project team members have already extensive experience in commercial exploitation of academic research. Such expertise is essential to the success of our exploitation strategies to ensure the long-term sustainability of the Centre.

Clinical - The impact of the proposed project to develop smart sensing for surgery will be demonstrated through certified devices with extensive in vivo validation. Uptake by the NHS and clinical partners will provide clinicians with data that provides the basis for changing clinical practice to improve patient outcomes. In partnership with Imperial's Centre for Health Policy, cost-effectiveness analysis will be conducted to demonstrate cost savings to the NHS.

Communications, Engagement and Society - Through clinical evidence, the project team will actively promote public awareness of our research and develop a shared vision for future healthcare. There will be significant patient and public engagement through conferences, science fairs and open days. In partnership with the Institute of Global Health Innovation (IGHI) at Imperial, we have an on-going programme of such events and the project team will work closely with the IGHI media team to use public lectures/exhibitions to maximise the potential impact of the project and its public engagement and to ensure interaction with and exposure of our work to young school children, who may wish to pursue a career in bioengineering.

Description Surgical site infection (SSI) accounts for over 15% of all hospital-acquired infections and burdens approximately 20% of high-risk surgical patients. SSIs include superficial/deep incisional or organ/space infections and affects 2-5% of surgical patients, giving an attributable mortality of 0.9%. For example, deep incisional SSI is common following small bowel (33.9%) and large bowel surgery (26.6%), extending the post-operative length-of-stay to over three weeks. Similarly, urinary and intravascular catheters represent conduits for infection and despite preventative strategies such as maximum sterile precautions, 'no touch' technique, and antibiotic impregnation, catheter related sepsis remains a major cause of morbidity and mortality. Clinically, catheter associated blood stream infection is responsible for more than two-thirds of deaths associated with Hospital Acquired Infections (HAIs) and are five times as deadly as other HAIs. Similarly, about 95% of urinary tract infections are catheter related, the presence of which leads to high-level bacteriuria through residual urine in the bladder, chemical or ischaemic damage to the bladder mucosa, through over distension, mechanical irritation from the presence of the catheter, and the presence of a foreign body to support biofilm formation. Early detection of SSIs, catheter related sepsis, wound dehiscence and anastomotic failure are critical to patient management to ensure prompt instigation of appropriate therapy and to avoid the associated mortality. The vision of the project is to harness existing sensing technologies and initiate new research opportunities that will underpin the development of smart sensing platforms for surgery to ensure early detection of surgical site infection, continuous monitoring of tissue viability, and quantitative assessment of functional restoration.

We are in the 3rd year of the project development and the project team are developing three platforms involving a novel sensor enabled bio-patch for tissue viability, smart drainage and catheters for postoperative care and miniaturised intra-luminal implant that is self-powered and can provide continuous sensing of the local tissue environment.
Exploitation Route For all the embodiment planned for the project, we have completed the design and development of functional prototypes that can be validated via preclinical studies. Future exploitation included mixed independent exploitation (licensing and spin-off), as well as academic dissemination and large scale clinical studies. The IP generated by the project will be exploited with strategies ranging from licensing, platform development, to mixed independent exploitation of the basic technology developed. Some of the technologies developed have already shown promise in initial clinical studies and we expect that tangible platforms that are of commercial value will be developed for the project.

Title A novel approach for stress condition monitoring using disposable flexible sensors 
Description By integrating flexible amplifiers with a commercially available flexible polyvinylidene difluoride (PVDF) mechanical deformation sensor and a pH-type chemical sensor, this proposed system proposed by our research team can detect arterial pulses from the neck and pH levels from sweat located in the back of the body. The system uses organic thin film transistor (OTFT)-based signal amplification front-end circuits with modifications to accommodate the dynamic signal ranges obtained from the sensors. The OTFTs were manufactured on a low-cost flexible polyethylene naphthalate (PEN) substrate using a coater capable of Roll-to-Roll (R2R) deposition. This research tool/method was developed by Dr Salzitsa Anastasova, Dr Bruno Gil Rosa, Dr Benny Lo and Professor Guang-Zhong Yang.

Title Batteryless, wireless, implantable pH and lactate sensor 
Description We have developed a micro chemical sensing system that employs simultaneous detection of pH and lactate. The system is powered using inductive telemetry, which has been used to demonstrate effective, batteryless operation. Sensor readings can be carried out with smartphones and tablets equipped with Near Field Communication (NFC) hardware, supporting remote powering of the sensor interfacing electronics and data readout. This indicates the potential of this device for implantable use. The pH and lactate sensors were electrochemically stable throughout our experiments and demonstrate good linearity in the biologically relevant ranges.

Impact This equipment will enable us to fabricate fibres with various integrated functionalities. In addition to optical interface with the tip of the fibre that conventional fibres offer, these multimaterial fibres allow other means of communication with the sensors at the tip, such as electrical interface with metal elements in the fibre, and fluidic interface via micro-channels, all integrated into a single fibre. These fibres will help integrate and miniaturize multiple sensors, allow drug delivery, and will significantly enhance overall functionality of our sensors in this project. The emerging field of multimaterial fibers presents exciting opportunities and has potential to create many fundamental research areas.

Description Graphitic carbon-based sensors fabricated on flexible and stretchable polymeric substrates for a wide variety of sensing applications. Description Methods for the production of conductive structures on flexible/stretchable polymeric substrates for the fabrication of sensors and electronics. The approach is based on post-processing of graphitic carbon structures. It is low cost, simple, and provides high accuracy in the definition of geometries with a reduced number of steps (relative to alternative techniques) and without the use of additional chemical or photolithographic stages. This approach has a resolution limit of approx. 150 microns.

Description We report the development of a flexible microfluidic platform with fully integrated sensing for on-body testing of human sweat. The system can simultaneously and selectively measure metabolites (e.g. lactate) and electrolytes (e.g. pH, sodium) together with temperature for internal calibration. The platform is designed such that a continuous flow of sweat can pass through an array of flexible microneedle type sensors (50 µm diameter) incorporated into a microfluidic channel.

PI Contribution The Hamlyn Centre is developing a new generation of microfabrication techniques, based on multi-functional fibre could improve the rehabilitation of patient by allowing for the implants to directly reconnect severed nerves. To achieve this, the growth of cultured nervous cells along an implant axes will be induced and enhanced by the mean of topological, electrical and chemical stimulations. A unique set of techniques has been developed in the Hamlyn Centre for the fabrication, functionalisation, electrical connection and fluid delivery capability of fibre designed and realised on site. Those fibres of tuneable diameter and length can be used to meet the requirement of nerve regeneration implants. Our research teams brought our current research results regarding neuro-regeneration, robotic assisted surgery for spinal cord surgery and wearable devices to our collaborators for further discussion.
Collaborator Contribution The collaborators from Renji Hospital offered their advices to our research issues. This collaboration will be used to set up an experimental framework adapted toward our long term goal of in vivo study.

Impact This equipment will enable us to fabricate fibres with various integrated functionalities. In addition to optical interface with the tip of the fibre that conventional fibres offer, these multimaterial fibres allow other means of communication with the sensors at the tip, such as electrical interface with metal elements in the fibre, and fluidic interface via micro-channels, all integrated into a single fibre. These fibres will help integrate and miniaturize multiple sensors, allow drug delivery, and will significantly enhance overall functionality of our sensors in this project. The emerging field of multimaterial fibers presents exciting opportunities and has potential to create many fundamental research areas

Results and Impact The event os a annual public event organised by Imperial College with an aim to promote science and research to the public. The ESPRIT team has been invited to exhibit every year since the launch of the festival in 2012. With an intuitive game, the ESPRIT demo attracted many young adults and children, and which helped the team to explain the concept of pervasive sensing and how it can be used to improve sport training.

The festival has been a very well attended event well over 10,000 visitors visited each year. Among the different attendees, students were particular interested in the pervasive sensing research. Some of them visited the Hamlyn Centre open day subsequently and enrolled to our MRes programme to study body sensor networks.

Results and Impact Our senior research fellow Dr Matina Giannarou invited Prof. Itaru Kitahara to share the experience about "3D-CG Virtual Surgical Operation in University of Tsukuba" with our researchers in the Hamlyn Centre. Prof. Itaru Kitahara is leading the Virtual Surgery research project at the University of Tsukuba, Japan and his research focuses on Computer Vision and Mixed Reality. During the talk, he not only introduced "3D CG Virtual Surgery", which aims to realise a navigation system of surgical operation using Computer Vision and AR/VR techniques, but also presented the collaboration between their research team and the medical doctors in our university regarding 3D CG Virtual Surgery.

Results and Impact Professor Daniel Elson, our Hamlyn Centre Director of Studies, talked through a career that has mirrored the typical innovation translation pathway - from a PhD in laser physics, and time spent in the Faculty of Engineering developing imaging tools for key hole surgery, a move to the Division of Surgery saw him work directly with surgeons to begin translating those technologies into image guidance tools. He discussed progress made developing diagnostic methods and technologies, and the value of adopting a clinical, rather than purely technological driven, approach to innovation and problem solving, working with surgeons to identify challenges today and tomorrow.

Results and Impact Acting autonomously or in close collaboration with humans, Robotics and Autonomous Systems (RAS) have a broad range of new and established applications. As the underlying technologies are further developed and translated from laboratory settings to real-world applications, RAS is playing an increasingly important role in the UK's economy and it's future growth.

The UK-RAS Network was established with the mission to provide academic leadership, expand collaboration with industry and integrate and coordinate activities at the EPSRC funded RAS capital facilities, Centres for Doctoral Training (CDTs) and partner universities across the UK.

We are delighted with last year's inaugural UK Robotics Week and proud to build on its success with this year's event. The UK Robotics Week provides a spotlight on the UK's technology leadership in RAS, and engages the nation's schools, colleges and universities in developing the digital skills needed to drive the UK's future economy.

The UK Robotics Week also acts as a forum for discussion of technological, commercial, legal, ethical and social aspects of robotics. The wide range of events covered, from symposia, workshops, conferences, festivals, competitions and hackathons, contribute to a thriving programme across the country. We endeavour to provide promotion to other robotics related events in the calendar and aim to ensure robotics remains high on the government's agenda as one of the transformative technologies of the present and future.

Results and Impact There is a growing need for electronic and sensing devices that can conform to non-flat surfaces and that can accommodate mechanical motions and stresses. Such devices are needed for wearable electronics applications such as physiological monitoring of patients and athletes, but also for prosthetic devices (artificial limbs), robotics and brain-machine interfaces. There is a particular need for such electronic skins for surgical robotic devices. To introduce flexibility and stretchability, a move away from traditional materials and fabrication processes is needed.

This has given rise to the field of flexible/stretchable electronics. This workshop will cover the aforementioned aspects within the context of realising e-skins that can introduce sensing intelligence to robotics, surgical tools, artificial limbs and wearable devices.

The workshop features researchers from leading medical, science and technology institutions covering topics including clinical needs and the latest developments of sensing technologies and new materials in medical innovation and healthcare and their application to robotics. There is still great potential for advancing the state of the art, with a need for further miniaturisation, increased functionality, in situ computation and signal processing and sensing capabilities.

An open panel discussion will summarise these and bring the workshop to a conclusion.
