Redistributed Manufacturing in Deployed Medical Care Network Plus

Abstract

Re-Distributed Manufacture (RDM) is defined as: "Technology, systems and strategies that change the economics and organisation of manufacturing, particularly with regard to location and scale" . In healthcare, RDM involves bringing production closer to the point of clinical need or use, enhancing the capability to deliver personalised medicine. The EPSRC funded Re-Distributed Manufacturing in Healthcare Network (RiHN- www.RiHN.org.uk) was launched in Feb 2015 and delivered by a consortium of six universities (UWE, Loughborough, Nottingham, Cambridge, Newcastle & Brunel), representing disciplines from both the social sciences and engineering and led an agenda to advance the impact of UK medical manufacturing.
One area of significant research interest identified by RiHN has been the potential value of implementing RDM approaches in deployed medical care. Whilst RDM includes the notion of a shift from centralised towards decentralised production, extant research continues to assume a degree of stable environmental conditions and fixed static location (e.g. hospitals/clinics/home). However, RDM can deliver life-saving benefits in mobile medical scenarios, where there is urgent and unforeseen demand in changing and remote locations, such as in response to natural disasters and emergencies (e.g. the Ebola crisis, Cyclone Idai and recent terrorist attacks), and the rapid treatment of injured military personnel in the field. This often requires a reverse supply chain perspective, starting with the requirement based on patient need. There are a variety of medical conditions that could benefit from rapid response treatments closer to the site of a medical emergency or in field hospitals; these include (but not limited to) blast injuries to skin and tissue, haemorrhagic shock and fractures.
The project will be led by a multi-disciplinary team bringing together subject matter experts that rarely meet, such as experts in trauma care, emergency medicine, innovation management and manufacturing, and will achieve the following outputs:

- A review of current research capability and knowledge- sharing of current breakthroughs
- Enhancing understanding of the use of RDM in deployed medical care
- Identification of and development of promising RDM technologies that support treatment pathways in deployed operations
- Capture of military and civilian emergency medical requirements and applications for RDM
- Systematic identification and comparison of needs, applications, technologies and resources

The overall outcome for the research will be the creation of 'communities of practice', producing real world demonstrators covering both the business and technical aspects of implementing RDM in deployed medical care, bringing the potential to advance the UK's capability in RDM, delivering impact to clinicians, practitioners and patients. The research will help shape how the UK Ministry of Defence, prime contractors in the defence sector, emergency and urgent care services and UK Research Council proceed with future R&D investment and resourcing. The outputs of the project will also be disseminated through a website, presentations at relevant national and international conferences, and research centres and a Special Issue journal (Health Services Management Research) and presenting the findings of the project at a showcase event. A final report will be produced, sent electronically to all interested parties, and made available digitally and via social media.

Planned Impact

In summary, during a series of sandpit and workshop events, run by the EPSRC funded RiHN, the potential value of implementing RDM approaches in deployed, frontline operations was identified. Working with MoD Defence Medical Services (DMS), members of the RiHN team seek to open up a new line of investigation that has the potential to radically transform support to medical treatments in deployed operations, where there is urgent and unforeseen demand remote and changeable locations. The proposed Network will allow us to bring together a number of key experts and disciplines and advance a robust, structured agenda for further research with the aim of advancing the UK's capability in RDM and delivering impact to clinicians, practitioners and patients. This agenda will help shape how MoD, defence prime contractors, emergency services and UK Research Council proceed with future R&D investment and resourcing.

Beneficiaries include SMEs and defence prime contractors (suppliers of 3D printers and scanners, and manufacturers specialising in customised parts and therapies), clinicians within the NHS and MoD Defence Medical Services, who will define requirements and will be end users of new technologies and and systems that will allow the rapid, tailored and localised treatments of life-threatening and debilitating injuries, resulting in better patient outcomes. If there were a significant adoption of RDM, soldiers would receive an enhanced level of critical care when it is most required, which has the potential to reduce complications and long-term care costs incurred by the Ministry of Defence.

Clinicians operating in deployed operations often have to administer treatments in extreme conditions, facing needs that may help define future take up by the NHS. The practice of emergency and military medicine can be employed to evaluate novel healthcare approaches, and identify where RDM can add the most value in healthcare delivery.

Finally, the academic community will benefit from the Network, which will bring together those from diverse backgrounds to investigate applications beyond the mainstream clinical settings. For those from the social sciences, this will allow them to explore the role of lead users in shaping novel manufacturing technologies and systems, and an improved comprehension of the diffusion of disruptive innovations. For those from engineering and science, they will be provided with an insight into the working environment and issues of emergency and military medicine. All academic participants will have the opportunity to explore and critique their ideas leading to further proposals investigating the feasibility of RDM applications.
The proposed Network is well connected to key researchers in engineering and management, centres of innovative manufacturing, and specialists in military and emergency medicine - ensuring effective participation, dissemination and exploitation.

Description So far the Network has funded 13 feasibility studies that are undertaking research into redistributed manufacturing in four priority areas: clinical fluids manufacturing, additive layer manufacturing,cell and tissue manufacturing and pharmaceutical manufacturing. Some of these feasibility studies have completed and their findings presented in their final reports which areavailable on the RiHN website. We have also undertaken additional research to understnad the operational requirement for RDM-led applications in dployed military operations and present the business case for RDM in deployed operations
Exploitation Route The outcomes of the feasibility studies should hopefully lead to further funding of larger scale research projects. Some of the techologies being developed by the studies have gone to be field tested in a deployed medical scenario and following the outcome of the testing may move towards being used by practitioners of deployed medical care. The programme has identified the redistributed manufacture of blood as a priority area and we have received dditional funsing to explore this in more depth, we hopethis will lead to larger funded projec looking at the manufacture of bllod products acrossteh UK healthcare system

Description The Programme has been impacted by the Covid-19 pandemic and so did not commence until the beginning of July 2020. However, Co-Is on the Programme, notably Prof Harris Makatoris, Prof Phillips and Dr Sam Roscoe have been interviewed by various media outlets regarding the impact of Covid on supply chains and healthcare manufacturing. Dr Sam Roscoe has provided recommendations to the International Trade Committee regarding building resilience into UK supply chains and these recommendations have been taken up by the UK Government and have resulted in a foundational level supply chain capability development programme which taught members of the DIT's Global Supply Chains Directorate how to identify the early warning signs of supply chain disruptions and develop strategies for overcoming disruptions. Building on the finings ofthe RiHN programme, the mHRA are about o launch a new point of care regulatory framework to support the manufacture of healthcare products and medicines close to the point of need. Our finings have also been used by UKK defence to identify priority areas for researchfor the use of RDM in deployed operations

The emergence of products manufactured and supplied at the POC were identified through a range of sources including engagement with the Engineering and Physical Sciences Research Council (EPSRC) funded project to evaluate manufacture through the Redistributed Manufacturing in Healthcare (RiHN) network. Print paths were directly created as machine control code (GCode) using FullControl GCode Designer, bypassing the conventional multi-stage translation of a design from CAD to an STL file, to slicing software and finally to GCode. This allowed precise parametric control of all aspects of the manufacturing procedure, including aspects that would not be possible with conventional slicing software such as control over the specific order and direction of printing each line, and the offsetting of specific printed lines on each layer in the Z direction by half the layer height. The printing quality of the structures was investigated with optical microscope and X-ray micro-computed tomography (micro-CT). Microscale changes to the extruded-filament geometry were characterized before and after filament-crossing points in the square grid structure, and internal cavities were identified. Printing speed was found to be a crucial parameter that should be carefully considered for lattice structure applications that require high manufacturing quality in terms of minimizing microscale geometric defects. Offsetting crossing extrudates by half the layer-height in the Z direction resulted in more consistent microscale geometry and greatly improved quality.

Results and Impact Workshop irganised by Innovate UK's Transforming Medicines Manufacturing (TMM) Programme which aims to support the development, de-risking and commercialisation of transformative innovation in medicines process development and manufacturing. The workshop drew together around 50 experts with experience and knowledge in NHS supply chains to contribute towards the designing of a programme which will be relevant to sector needs. During the workshop, participants were asked to scope and define a competition in the space of "Simplified Delivery of Complex Medicines", with the objective of addressing challenges associated with administering complex medicines by needle-free methods. Discussion points included:
• Are we there yet? What are the translational challenges which need to be addressed prior to reaching the point of manufacturing.
• What are the development and manufacturing challenges associated with the formulation of complex medicines?
• How do we improve the properties of the delivery vehicles thereby enabling simpler methods of delivery?

Results and Impact RiHN held its first funding webinar on the 17th September 2020 with +70 participants attending. The participants represented over 27 different higher education organisations and a few industry/public sector members.

We aimed to provide potential applicants with an overview of the funding competition and an opportunity to ask questions about the research themes and application process. We invited UK researchers to actively engage with us, learn more about our aims and how we are collaborating with stakeholders in the deployed medical care ecosystem.

The outcome of this webinar was a high request for application documentation, building on our RiHN membership base to 250 members and later on, we received 22 applications from attendees.

Results and Impact RiHN is hosting online a series of short talks on the business case for redistributed manufacturing (RDM) in clinical fluid, and cell & tissue manufacturing.

Talks from Prof. Nick Medcalf - Innovate UK and Dr. Ifty Ahmed - University of Nottingham, on Cell & Tissue Manufacturing at the point-of-care. Followed by talks from Prof. Darren Reynolds and Dr. Jenni May, on clinical fluid manufacturing and the challenges facing blood transfusion for frontline treatment.

With senior stakeholders in RDM on the steering panel for this special interest group, their involvement lends weight to the conversation and depth to what "point-of-care" manufacturing means to them. We hope to tackle some of the unique challenges in implementing and validating RDM solutions, determine where in the economic model does RDM makes the most sense, and articulate the benefits to both the providers and end-users.

Results and Impact RiHN is meeting in person to debate on the business case for redistributed manufacturing (RDM) in clinical fluid, and cell & tissue manufacturing. Talks from Prof. Bill Scott - Newcastle University, Lt. Col. Dr. Paul Hunt - DMS, Prof Richard Bibb - Loughborough University. With senior stakeholders in RDM on the steering panel for this special interest group, their involvement lends weight to the conversation and depth to what "point-of-care" manufacturing means to them. We hope to tackle some of the unique challenges in implementing and validating RDM solutions, determine where in the economic model does RDM makes the most sense, and articulate the benefits to both the providers and end-users. Part of the workshop will be part of a technology roadmapping exercise we hope to contribute to ALM business case to adopt RDM for healthcare and where it can make an impact.

Being able to meet in-person allows us to showcase facilities of partner organisations such as the centre for process innovation (CPI), home of the national biologics manufacturing centre and the RNA centre of excellence. CPI will be hosting tours of both facilities and an opportunity to meet with some the technologists.
