Neighbourhoods and Dementia: A mixed methods study

Abstract

Dementia is often presented as a global issue with substantial economic consequences for all countries and societies providing diagnostic and/or supportive services. Whilst we believe this is necessary and important information, in our 5-year study we want to celebrate the achievements, growth and contribution that people with dementia and their carers make to society. To do this, we are putting the local neighbourhood and networks in which people with dementia and their carers live and belong at the centre of our work. We have designed a study on neighbourhood living that has 4 inter-linked work packages (WPs), an international partner , the Center for Dementia Research [CEDER] at Linköping University, Sweden, and strong user involvement through the EDUCATE and Open Doors groups [Greater Manchester, England]; The ACE Club [Rhyl, North Wales]; and the Scottish Dementia Working Group [Glasgow, Scotland]. In the UK our academic partners are situated in Manchester, Salford, Stirling, Liverpool and London and we have third sector involvement through the Deaf Heritage Project at the British Deaf Association, as well as a range of project partners which includes the North West People in Research Forum, the Citizen Scientist initiative and a Community Integrated Company that supports people with dementia through accessible technology [Finerday]. As this is a complex set of networks based around a neighbourhoods theme, each WP will use different research methods and partners to meet their primary aims and objectives. WP1 is a secondary analysis of the English Longitudinal Study of Aging database which will compile Neighbourhood Profiles that will be available for the whole country; these Profiles will include information on cognitive risk factors and clusters of population; WP2 will develop a set of core outcomes measures in dementia that will involve people with dementia and their carers in deciding what measures and priorities are important for them; WP3 will explore what makes a dementia friendly neighbourhood and will take place in Stirling, Salford and Linköping; WP4 has 3 interventions representing various stages of the Medical Research Council's complex interventions framework. Intervention 1 will be a full RCT of an educational intervention for general hospitals that several members of the project team have developed and piloted over the last 2 years. In this study, we want to find out if the educational intervention results in people with dementia leaving hospital for their neighbourhood home sooner, but with high levels of satisfaction. Interventions 2 and 3 are pilot trials. Intervention 2 will be conducted in Sweden and Manchester, UK and will use technology to help couples, where one person has a dementia, to better self-manage the condition and, more importantly, their relationship. In intervention 3, we are looking at the diversity of a neighbourhood and will develop the first digitalised life story intervention in the world for Deaf people (BSL users) who live with dementia. This will be the first intervention for this group in the world. In this programme of work we will develop a user research programme as some people with dementia have told us that they would like to work alongside the research team as co-researchers. We will therefore appoint a PPI co-ordinator for the duration of the study with a responsibility for identifying co-researcher training needs, running a regular co-research programme, mentoring co-researchers, ensuring user goal preferences are met and facilitating user dissemination. Through the implementation of a neighbourhood approach each WP will promote closer relations and working between professionals, lay people and people living with dementia. This study will also contribute to the currently limited evidence base for dementia friendly communities and provide knowledge and insights to support a robust theoretical framework of neighbourhood work that will have international scope and relevance.

Planned Impact

Building on our long-standing working relationship with 4 user groups: EDUCATE and Open Doors [Greater Manchester, England]; The ACE Club [Rhyl, North Wales]; and the Scottish Dementia Working Group [Glasgow, Scotland], the Neighbourhoods and Dementia study has integrated multiple pathways for members of these groups to engage throughout the 5-year programme. In consultation with each of the user groups, we have developed a Dementia User Involvement and Engagement Model and 6-module 'Co-researcher Education Programme' that prepares interested users to participate in all stages of the research programme. We have consulted with both the NIHR Research Design Service and the NIHR INVOLVE organisation, and their good practice guidance, to help us prepare both these innovations. Manchester Informatics will develop the software to assist users in their co-research work. Once suitably trained, user group members will take a full and active role as co-researchers in the work packages (WPs) and interventions outlined in the research programme. To date, user group members have expressed an interest in a direct data collection role in the dementia core outcomes data set (WP2), the dementia friendly neighbourhood work (WP3) and the pilot RCT aimed at developing a self-management education course to help couples, where one person has dementia, keep doing everyday tasks and activities together (WP4.2). Remunerated in accordance to NIHR INVOLVE guidance, co-researchers will also have the opportunity to act as consultants throughout each of the WPs offering advice and guidance to the PPI committee and PI (Prof Keady). We will facilitate separate co-researcher meetings for members to link-up outside the 'formal' research teams to discuss the project and set action plans. Through world-class videoconferencing facilities at the University of Manchester, we will organise an annual opportunity for UK co-researchers to meet those at our international partner based at the Center for Dementia Research [CEDER] in Linköping, Sweden, enabling cross-national learning, networking, project advice and friendship circles to emerge. As recommended by the NIHR EQUIP study, we will appoint a PPI co-ordinator (Dr Swarbrick) for the duration of the research programme with a responsibility for identifying user training needs, overseeing the co-researcher education programme, mentoring co-researchers, ensuring user goal preferences are met and facilitating co-researcher involvement in dissemination and evaluating impact. Dr Swarbrick has worked closely with EDUCATE and Open Doors over the last 4 years and Prof Keady helped establish the Open Doors position/network and has also worked with the ACE Club since their inception in 2002, as evidenced through joint publications with Club members. As the ACE Club is funded through donations, we have costed a part-time (0.2 WTE; over 5 years) project coordinator to be based with ACE Club members to facilitate involvement and liaise with the Dr Swarbrick and Prof Keady. Adapting user components of the NIHR Mental Health Research Network toolkit, Dr Swarbrick will monitor and facilitate the evaluation of user involvement in the Neighbourhood and Dementia study through evaluation forms administered every 6 months and at monthly face-to-face meetings. This information will be collated, analysed and fed back to the user groups and any identified points of action discussed directly with individuals, user groups, Prof Keady and the PPI Committee. All co-researchers will be invited to participate in an 'Impact on Policy' event, run by the Rt Hon Hazel Blears MP who is Vice Chair of the All Parliamentary Group on Dementia (and Project Partner) and Prof Alistair Burns, who is the National Clinical Director for Dementia for the Department of Health (Co-I). The event will give the opportunity for co-researchers to explore the potential contribution of the research programme and their own involvement on national guidance and policy.

Description	Work Programme 2 [of 8] is the only programme on the Neighbourhoods study to have completed their work. These key findings therefore only relate to work programme 2 which is titled: 'Generating Neighbourhood Profiles of Cognitive Ageing & Service Use Using Nationally Representative Population Data' Supporting Narrative on Findings The main team responsible for the activity is Dr Gindo Tambupolon, Professor James Nazroo and Professor Neil Pendleton, an interdisciplinary group of medical, social gerontologists, geriatrician and social statistician. The aim of work programme 2 on the Neighbourhoods study, using representative longitudinal cohort study in England, is to determine neighbourhood characteristics associated with good/poor cognitive ageing. We also show individual and area level associations with these different paths in ageing cognition. For this we have used the population representative sample of adults, the English Longitudinal Study of Ageing (ELSA). Using data from the Office of National Statistics and linked dementia prevalence data we have combined out-put areas into 310 local authorities or districts across England producing geographical maps. These show geographical variation in dementia prevalence across England. We followed this with a multilevel model to determine individual risk factors or area features associated with dementia prevalence. The results suggest that area differences in dementia prevalence are determined by individual risk factors such as gender, education, and physical activity but not area features such as index of deprivation or land use mixes. Of course this is the first analysis and cannot examine all area features effects regarding dementia prevalence. Description	With the award concluding at the end of April 2019 and those work programmes with approved extensions at the end of October 2019, the last reporting year has been much quieter in response to non-academic impacts. However, as one update from the previous 2021 narrative statement, the Open University Press/McGraw-Hill have now commissioned a submitted book proposal about the Neighbourhoods study that was submitted to their 'Reconsidering Dementia' series (Professor John Keady as Editor). A number of the non-academic impacts created by people living with dementia, such as the animation on stigma and the animation on place-making in Salford [see: https://vimeo.com/343246616], were placed in the book proposal and will be available to future readers once the book is made available in 2023. All eight work programmes and all completed PhD students on the Neighbourhoods study (there were 4) have independent chapters in the commissioned book, alongside the main project administrator, and will focus on the substantive impacts arising from the Neighbourhoods study. A post-study website has been created to sustain the legacy of the Neighbourhoods study and non-academic impacts of the study also feature on this location, particularly in work programme 1. This innovative method addresses the need to get the perspective of people living with dementia on core outcomes for community based non-pharmacological interventions.

We have worked with the Royal College of Psychiatrists National Audit on Dementia team to coordinate the distribution of the survey to specialist staff contact lists. This has contributed to national momentum for the evaluation of hospital dementia training, at a time of policy drive to improve provision of services to dementia patients in these settings.

We have worked with the Royal College of Psychiatrists National Audit on Dementia team and Royal College of Nursing to encourage the distribution and completion of the survey. Given the strong steer of work programme 1 to maximise the voice and representation of people living with dementia in our research, we involved people living with dementia and their carers from the early stages of study design and survey development.

PI Contribution	A research requirement for the development of a digital life story tool for use by members of the Deaf community is access to an historical archive of pictures and photographs. Our partner organisation the BDA had already received a heritage lottery grant to make publicly available an archive of films from Deaf history. We contributed to the digitisation of over 30,000 images/photographs from Deaf history and these are being made available back for our use in the project as well as to the wider general public.
Collaborator Contribution	We have provided the funds to permit the digitisation of the photographs to ensure the saliency and relevance of some of the materials to be used in one of the tools within within our research project.

PI Contribution	We provided specialist research expertise in dementia and Deaf people to the evolution of a pilot home based intervention aimed at Deaf carers of people with dementia. We became part o the delivery team for this pilot, carried out the underpinning consultation that supported the pilot and the initial evaluation of its process and outcomes.
Collaborator Contribution	The partners were the Alzheimer's Society Northern Ireland and the British Deaf Association Northern Ireland along with the central research and development team at the Alzheimer's Society. The central team initiated the collaboration in the first place, Alzheimer's Society Northern Ireland and the BDA Northern Ireland worked in partnership with us on initial scoping research, the evolution of the pilot intervention and its initial delivery prior to full commissioning of the trial service.
Impact	Internal scoping project report: Double time: Deaf carers of people with dementia. A commissioned journal article currently being written for publication later in 2018 based on the Deaf carer data.

Description	The Memory Friendly Neighbourhoods Knowledge Exchange programme

PI Contribution	I (Richard Ward) was co-PI on this knowledge exchange programme funding. The programme was aimed at bringing together academics, policy-makers, practitioners and people living with dementia to explore the issue of memory friendly neighbourhoods. The collaboration has led to the creation of an online resource to facilitate sharing of knowledge and good practice and we have recently secured funding for a second phase of the work, this time ensuring that the website is dementia-friendly by consulting with people with dementia and carers.
Collaborator Contribution	The partners were co-PI's and co-hosts for the programme.
Impact	The collaboration was interdisciplinary combining, social care/work, social policy, nursing, architecture, and town planning. Outputs to date include a presentation about the programme to the European Alzheimer Annual Conference in Glasgow 2014, a project report available on the funders website and a dedicated website which is due to be revamped and made dementia-accessible.

Description	The DemPower guide is a couple-management app aimed at enhancing well-being and relationship for couples where one partner has a diagnosis of dementia. The contents of the guide are based on experience, available research and practice evidence as well as common sense tips and advice. The guide covers aspects related to life at home and in the neighbourhood, physical and mental activities, social life, managing emotions and tips for maintaining a healthy relationship and lifestyle. It is an interactive guide that uses art illustrations, short films, music, games, suggestions for activities together with other useful information such as contact details for relevant organisations. This work has featured on BBC North-West TV news. A shorter version of this was then recorded on BBC online: Cartoonist offers new help to people living with dementia A leading cartoonist is helping researchers understand what matters most to people living with dementia. Tony Husband, best known for his work with Private Eye magazine, became involved with the project following the death of his father. The team at Lancaster University say incorporating his artistic talents could inspire a new way of working with people living with the condition.

Results and Impact	Working with a research team from Lancaster University, a Private Eye cartoonist is using his sketches to help dementia patients. This is the first study where researchers, led here by Dr Siobhan Reilly from the University's Division of Health Research, have worked with people living with dementia to prioritise what, they say, are the most important areas of life and apply these to health and social care research. Tony Husband became involved following the death of his own father from the condition. In 2014 he published a book of cartoons based on his experiences called "Take Care, Son: The Story of My Dad and his Dementia". At the same time Dr Reilly was just starting two studies within the Neighbourhoods and Dementia Programme, funded by the Economic and Social Research Council and the National Institute of Health Research. Dr Reilly met with Tony to see if there was a way of incorporating his artistic talents to document the study, which aims to identify what matters most to people living with dementia. His cartoons have appeared in many newspapers, magazines, books and websites, including Private Eye, The Times, Punch, and The Spectator. He has won more than 15 major awards. Dr Reilly said: "Back in 2014 we didn't know what the results of the study would be but we knew that bringing Tony on board to bring the study alive for the public would be a great idea - little did we know how integral his cartoons would become to the study! The response has been amazing." His cartoons have been used to: · capture the essence of what people with dementia were saying · express views in simple accessible images · help to open up a conversation on a difficult subject using humour · enable people with dementia to shape the research narrative · communicate the purpose and outcomes of the research to a wider audience. Last week Tony was invited to sketch attendees at a study event on campus. The BBC were there to film. This event provided an opportunity for people living with dementia to discuss the areas of daily life that are most important to them. Tony listened attentively to the discussions and drew many pictures of their daily lives and aspects vital to living a fulfilled life. Tony, who was keen to be involved in the study, said: "I'm happy to do anything I can to raise awareness of dementia and also to show how carers are often overlooked; if I can raise a smile at the same time, that's even better." He also recently documented the final study conference at University of Manchester at the culmination of the five-year project, where the Lancaster research team presented their research into the key outcomes relevant to the daily life of people living with dementia, as well as assessing the impact of dementia training in hospitals. This event also provided an opportunity for people living with dementia to discuss how contributing to the research had enhanced their lives. One of the conference speakers was Pam Begg, who was diagnosed with dementia some 10 years before becoming a Lancaster University co-researcher on the Neighbourhoods and Dementia study. She said: "I have begun to believe in myself again. My life is not over. It is worthwhile and I am able to give something back to the community. I feel that by being involved in research I am slowing down the progression of my dementia as I am keeping my brain active." This work is of national importance and a number of studies are already considering how these results will be incorporated in their choice of outcome measures for their studies. Results and Impact	John Keady (chief investigator), Andrew Clark (work programme 4), Ingrid Hellstrom and Therese Bielsten (work programme 6), Emma Ferguson-Coleman (work programme 7) and two BSL interpreters, were able to attend the Gerontological Society of America (GSA) Annual Scientific Meeting which, this year, was called 'The Purposes of Longer Lives.' The event was held in Boston on the east coast of the USA. The team delivered an hour and a half long symposium titled 'Neighbourhoods and a life with dementia: Navigating new understandings. There was a was real and genuine interest in the Neighbourhoods and Dementia study and in the range of qualitative and quantitative research methods that were employed to support various aspects of the research work.

We want to develop an outcome measure consistent with what matters to people living with dementia. Results and Impact	The Work Programme 3 research team was keen to find out which outcomes are important to people living with dementia. The team completed its Delphi survey in February 2018. This was a two round process involving people living with dementia, carers, health & social care professionals, policy makers and researchers (n=288 R1, with an 84% response rate for R2). Following the Delphi survey, a successful Consensus Meeting was held at Lancaster University to which participants in the Delphi survey were invited to attend. The team were delighted to have such a high proportion of people living with dementia and their carers attend the day and the meeting was chaired by experienced and skilled facilitator Katherine Cowan. The results of the Consensus Meeting were fed back to the participants in the meeting and will be written up and published over the coming months.

Results and Impact	Work Programme 5 recently participated in the British Society of Gerontology conference in Manchester, which was attended by researchers and interested audiences alike from all over the world. It took as its theme "Ageing in an unequal world: Shaping environments for the 21st Century". Dr Faraz Ahmed presented the DEMTRAIN work at the symposium, which highlighted "the role of co-researcher in the development of a hospital organisational and staff survey exploring dementia training in acute hospitals". Faraz in a separation presentation, described the work on the main theory underpinning the DEMTRAIN study, which is exploring the impact of dementia training in hospital settings in a presentation titled "Developing an initial programme theory for dementia training in hospitals: An experiential and consultation review exercise".

Results and Impact	The research team consulted by NHS Health Education England and been invited by Health Education England to present our latest findings to the Dementia Workforce Advisory Group. The DEMTRAIN research team have worked with people living with dementia and carers as co-researchers in all phases of this mixed method study. We have developed and will shortly launch the dementia training in hospitals resource (currently a working version): Dementia Training in Hospitals and Neighbourhoods: What leads to success? The DEMTRAIN team are working with the design company to create an interactive version (mobile and desktop). We are shortly to distribute this along with a bespoke report to 87 NHS acute hospitals in England who took part in the research. This material will help to inform NHS hospital dementia leads, trainers, champions and specialist clinicians in planning, developing and evaluating educational programmes in hospital settings. These materials are being endorsed by a major national charity (Dementia UK) and the Royal College of Nursing.
