A Shared Space and a Space for Sharing: A Transdisciplinary Exploration of Online Trust and Empathy

Lead Research Organisation: University of Sheffield
Department Name: Information School

Abstract

What do people choose to share online when they find themselves in difficult and precarious circumstances? What can we learn from this about how trust and empathy are established and maintained in online relationships? And what can all of this tell us about sharing, trust and empathy in the offline world? These questions lie at the heart of this innovative research project looking at the online relationships and behaviour of people affected by natural disasters; life-threatening or terminal disease; organ donation; emotional distress and suicidal thoughts; drug use and addiction. Why are we looking at these particular situations? This is because people in these circumstances are extremely vulnerable, and some of them may be seeking information support or help that could make the difference between life and death. That means it may be necessary to establish trust and empathy very quickly to make that difference. In these extreme contexts, people may end up over-sharing, or equally may be disadvantaged by not sharing enough. At the same time, others online need to know when they are responding to genuine distress. Understanding the way that trust and empathy works in these circumstances may help to transform the way in which resources and aid are distributed, locally and globally, and enhance individual and community resilience. But such changes may have both good and bad consequences, so working through the ethics of all this, at personal, global, institutional and governmental levels, will also be at the core of our project.
The project brings together researchers from a wide range of backgrounds: sociology, international relations, computer sciences, health informatics, philosophy, media studies and bioethics. By working together we hope to produce new 'transdisciplinary' understandings of the issues of sharing, empathy and trust online - in other words, insights that would not be possible using only a single disciplinary lens. All of this will be challenging, but we have begun to exchange ideas and are confident that we will generate findings and perspectives that are genuinely novel. Several themes clearly cut across our disciplinary boundaries and this has helped us to structure our approach to the individual case studies. Our overall approach has been shaped by the notion of 'Facet Methodology'. This suggests that, like light being refracted differently through the various faces of a gemstone, we can understand more about a social problem by viewing it from a number of different angles and perspectives.
As part of the research, we will be accessing online data from a wide range of sources and interacting with a variety of organisations, including UK charities, international NGOs and health providers. The charity, The Samaritans, will be directly involved as a project partner. The outputs from the research will not be aimed solely at academic audiences and we will be working hard to make them accessible and valuable to this wider set of stakeholders as well as to the general public.

Planned Impact

Who will benefit from this research?
Beneficiaries of this research will include policy-makers in national and international government and non-government agencies. Within the UK, the governmental beneficiaries include DSTL, the Home Office, the Foreign and Commonwealth Office, the Cabinet Office, and the Environment Agency, the Department of Health and the NHS and NHS Blood and Transplant. Within the public and third sector, the Citizens Advice Bureau, MacMillan Cancer Support and Marie Curie Cancer Care, as well as similar organisations providing support to people in extreme circumstances. The organisation, Samaritans, is a project partner and has a particular interest in the role that trust and empathy play in their email service and in peer communication within social media. At an international level, organisations such as the Overseas Development Institute (ODI), the United Nations Office for Disaster Risk Reduction, International Committee of the Red Cross (ICRC), the World Health Organisation (WHO), Organwatch, Oxfam and other relevant UN and NGOs agencies to the European Monitoring Centre for Drugs and Drug Addiction (EMCDDA) will be beneficiaries of the research. Given the popularity of, and interest in, the Web as a means of communication, and social media in particular, the results from the project will be of interest to the wider public, and to specific groups, e.g., people who have experienced extreme and life-threatening situations or conditions. In addition, within the commercial private sector, social media platform providers (such as, Twitter, Facebook, and YouTube, etc.) and mobile communications providers, e.g., Vodafone, will be interested in how their products/services are used for sharing, within the case study contexts, and more generally.
How will they benefit from this research?
The research will develop a better understanding of how people in extreme circumstances can be supported in online environments and the role of trust and empathy in this. This will be of value to government and non-government agencies that provide support to people experiencing natural disasters, drug addiction, life-threatening or terminal conditions and people contemplating suicide. It will enable them to develop and increase the effectiveness of their services and contribute to the nation's health, well-being and enhance quality of life, particularly among these vulnerable groups. In addition, and as part of WP6, a partner-specific output will be delivered to Samaritans; this will help inform the development of their current and future services and interventions online. The project also has the potential to impact on the working practices of a wide range of professionals engaged in the reduction of harm and amelioration of harms, or potential harms suffered by people caught up in variety of dangerous, risky or life-threatening situations.
During the project life-cycle (short/medium term), we will offer to talk or debate at public events, e.g., the Cafe Scientifique network, the Salon network and, at an international level, through liaison with the British Council. We will approach local public venues to offer a small exhibition. In the medium/long term, the project will offer input into global academic links, e.g., the U21 Global Issues Programme, a multi-disciplinary programme offered by the Universitas 21 network, will offer input into global citizenship and global issues dimension of the school curriculum, e.g., via the Global Learning Programme. The Samaritans organisation has also indicated its willingness, through its access to decision-makers, to support the project in ensuring impact on public policy and other service providers.

Title Photographic exhibition relating to the 'A Shared Space and a Space for Sharing" project.
Description During 2016, we worked with professional photographer, Anton Want, to develop a photographic exhibition based on the findings from the 'A Shared Space and a Space for Sharing' project. The exhibition comprised 12 (2 portraits per Work Package) A0-sized images, which were portraits of people, which had text superimposed containing messages from the Work Packages. In addition there were A0 posters summarising the project as a whole, and the individual Work Packages. The exhibition was displayed for 2 weeks during the ESRC Festival of Social Science in November 2016 in the Winter Gardens in the centre of Sheffield. The Winter Gardens has a very high footfall, hundreds of people passing through each day. The exhibition attracted the attention of the local media, and the Sheffield Star newspaper published a photo of Peter Bath and Anton Want against the backdrop of the exhibition in the Winter Gardens. Following this exhibition the exhibition was displayed further in Lincoln, Sheffield and London in 2017.

Title Play entitled: A Space for Sharing
Description During 2016, we worked with the DeadEarnest Theatre Company, who developed a play based on the findings from Work Package 3 of the 'Space for Sharing' project. The play featured five women diagnosed with breast cancer and how they used an online health forum to share information, experiences and emotions during their illness. The play was followed by a discussion of the issues with members of the public. The play has since been performed as part of the Dying Matters Awareness Week (Sheffield, May 2017), at the end of project conference (London, May 2017) and at the Cavendish Cancer Care AGM (November 2017).
Type Of Art Performance (Music, Dance, Drama, etc)
Year Produced 2016
Impact We collected feedback forms from the public and a currently evaluating these for how they informed the views of the public.

Description The project examined how people in extreme circumstances share information, experiences and emotions in different online environments and the role of trust and empathy in the process of sharing. We defined being in extremis as situations in which a person experiences a heightened sense of something critically important being at stake (this might be their life, but could also be their health, well-being or something more nuanced, such as their previously anticipated lifespan. The level of extremis can fluctuate over time and even one's experience of being in extremis may vary, even though the underlying situation might not change. Using online environments to share information, experiences and emotions, (sometimes leading to the sharing of physical goods in the offline world), can provide support to people who are in extreme circumstances, and this can offer advantages over offline provisions in terms of timeliness and extending social networks. Being able to share in online spaces can enable individuals to gain access relatively easily to different forms of knowledge and expertise: this offers the potential for different perspectives to be voiced and heard, both by people who are in extreme circumstances and those who are trying to help others in these situations. However, as well as the benefits associated with sharing information, emotions and experiences online, there are also some corresponding risks, and trust plays an important role in sharing. Being in extreme circumstances, and needing information and support from other people, gives people a reason to share themselves; however, trust gives people the confidence to do so. The structures and norms of specific online spaces can help to promote this trust, although the needs for privacy, safety and security need to be balanced with the desires and expectations of individuals to express themselves freely, without undue intervention or monitoring. Further key findings are presented in the overall project report under the Work Packages. From a methodological point of view, a key challenge for transdisciplinary research is the extent to which knowledge generated by multiple research areas and disciplinary approaches can be effectively integrated for the exchange of information and synthesis of findings. The transdisciplinary process that we developed throughout the project was designed to encourage openness among the researchers, stimulate conscious reflection, and allow for disagreement and different perspectives, in order for information and knowledge to be exchanged. Early in the project we held collaborative discussions during project meetings and all of the researcher teams contributed to the exchanges to examine the different disciplinary perspectives of the essential concepts within the project. These extensive discussions enabled a more nuanced and shared understanding of the central concepts to emerge. In addition to the academic outputs from each Work Package, we have now submitted a paper for publication from the Transdisciplinary work, co-authored by all the investigators and researchers involved in the specific work packages.
Exploitation Route At an academic level, the individual WP leads are continuing to develop their programmes of research through grant applications and research publications. At a non-academic level, we envisage that the research outcomes will be taken forward at an individual work package level, as well as for the project overall. For the overall project, we anticipate that one of the EMoTICON programme sponsors, Dstl will be interested in the overall project findings, and we look forward to sharing our results with them through the EMoTICON network programme of activities. For the individual WPs, Brownlie (WP6) continues to collaborate with Samaritans UK and is looking to make recommendations relating to social media guidelines for discussion of deaths by suicide. In WP5, Boothroyd is exploring possibilities for further collaboration with colleagues at European Centre for the Monitoring of Drugs and Drug Addiction, in Lisbon. In WP3, Bath is continuing to collaborate with and share findings with Breast Cancer Care and the Motor Neurone Disease Association. The play that was developed in conjunction with DeadEarnest Theatre Company, based on the findings of WP3, will be performed further to audiences of practitioners, charitable organisations and the general public.

Description Summary of how WP0 findings have been used: the transdisciplinary reflections from the project were presented to other academics and various key stakeholders at the end-of-project conference (Wellcome Trust, May 2017). A project photographic exhibition was commissioned and displayed to the public for 2 weeks in the Winter Gardens, Sheffield, as part of the 2016 ESRC Festival of Social Sciences. The exhibition was then displayed to the public in Lincoln, Nottingham and London (two venues). The findings from the analyses of the individual Work Packages have been of interest to external organisations, e.g., Samaritans UK (WP6), Breast Cancer Care (WP3), Motor Neurone Disease Association (WP3), European Centre for the Monitoring of Drugs and Drug Addiction (WP5) and Good Things Foundation (WP3). Summary of how WP3 findings have been used: regular updates of progress and findings have been communicated with Breast Cancer Care and the Motor Neurone Disease Association through email and visits to the organisations and at the MND Association annual conference. Further work to support both organisations in how they can use the findings is ongoing, this includes a Premiere Event being organised by Breast Cancer Now (formerly Breast Cancer Care) on 31st March 2020, which will involve broadcasting through their social media channels (Facebook). This will involve their users viewing the film of the play online live and people will watch along with it, as if it were live, and their comments on social media will be connected to the time at which they were made in the video. BCN asked to share other social media content around the play so that they can promote the benefits of the Forum and the impact it has, with the goal of driving more users. The screening of the play will be followed immediately by a discussion/Q&A session with Peter Bath. . The findings from WP3 were developed into a play 'A Space for Sharing' which was performed at the 2016 Festival of the Mind in Sheffield to an audience of 60-70 members of the public. Since then it has been performed at the Dying Matters Awareness Week, at the end of project conference at the Wellcome Trust, and at the Cavendish Cancer Care Annual General Meeting (2017). The play was performed at a workshop for >30 healthcare and professionals in South Yorkshire as part of the ESRC Festival of the Mind (November 2018). We then made a film of the play that is available online (https://www.sheffield.ac.uk/space4sharingstudy/play). On 11th September 2020, Breast Cancer Now organised one of their regular Premier Events to broadcast the film of the play live through their Facebook channel to promote the benefits of the Forum and the impact it has, with the goal of driving more users. A series of short podcasts about the research and the play created by Peter Bath were also made available on the Facebook channel. The event was advertised to 67,863 people. Overall, there were 16,763 views (8,097 during the live broadcast; 8,666 views in the following week), the predominant group was women aged 45-54 years old. There were 62 Shares of the film and 159 comments made about it and 641 reactions (501 likes, 126 loves, 2 haha, 4 wow, 8 sad). Amongst the feedback we received about how the play had changed their thinking or anything new that viewers had learned, one person indicated that "I am very grateful to the play for highlighting that radiotherapy can bring adverse side effects, I learned that other women can sometimes also have reactions to the treatment like I had. so might reach out for help to find and speak to them about it. It would be so good to find someone similar to me." and another responded "To speak up and be honest about your illness and how it affects you". In March 2019, an article entitled "Online Health Forums for people living with breast cancer and other health conditions" was published by Peter Bath and Sarah Hargreaves in 'The House' magazone. The House magazine is "Parliament's own" weekly publication, placed in the pigeon-hole of every member of Parliament and Peer in Westminster. Totally apolitical and answerable to a cross-Party editorial board, The House carries exclusive high profile interviews, unique articles from Parliamentarians and policy information. The findings from WP3 have also been developed into a series of information sheets that have been distributed to individuals and organisations providing support to people with specific health conditions, e.g., breast cancer. The information sheets are four pages long and provide advice and information for people who are considering using online health forums and for health care professionals who are sign-posting patients to these resources. A further version of the patient information sheet has been prepared in collaboration with Breast Cancer Care. Summary of how WP4 findings have been used: Moorlock's 2015 paper was the inspiration for a BBC Radio Four documentary called 'The Organ Beauty Pageant', broadcast on 2nd May 2017. Moorlock provided advice to the show's producer on the ethical issues associated with using social media to find donors, who acknowledged that the published paper was the impetus for the documentary. Moorlock & Draper (2018) was picked up by a number of news outlets in the UK and North America, resulting in an Altmetric score of 98 (placing it in the top 5% of all research outputs scored by Altmetric) (https://wiley.altmetric.com/details/34358826 ). The ELPAT (Ethical, Legal and Psychosocial Aspects of Organ Transplantation of the European Society of Transplantation) Living Donation Working Group is undertaking work exploring solicitation of living donation. Moorlock is leading on this, and will be discussing at the next meeting of the Working Group (in 2019). Summary of how WP5 findings have been used: Boothroyd met twice with Paul Griffiths (Scientific Director) and Andre Noor (Head of Section) at the European Centre for the Monitoring of Drugs and Drug Addiction (EMCDDA) in Lisbon to discuss project and findings. Some of the research data from this WP was shared with staff at EMCDDA and informed their own research interests and projects. The WP led to Dave Boothroyd's personal invited participation in a multi-agancy (police, probation services, drug policy/ lobby organisations conferences, such as ESRC-funded seminar series on 'New Drugs' held at Univ of Kent, and keynote address at Theorizing the Drug War' conference at Univ of Essex Centre for Law and Human Rights. Summary of how WP6 findings have been used: the key output has been the publication of a full research report for Samaritans' internal use (Brownlie and Shaw, 2016), an abbreviated online version of which was published by the organisation (Brownlie and Shaw, 2017). On request, the full confidential document was also shared with Dstl in July 2017. In addition, a research briefing has been published virtually and in hard copy by the Centre for Research and Relationships (CRFR) (Brownlie, 2017). The report which contained an endorsement of the research from the Chief Executive of Samaritans was also circulated by CRFR to 500 individuals and organisations in the UK and internationally, many of them leading organisations involved in the provision of online support. Using Twitter data collected as part of the collaboration between WP1 and WP6, Brownlie is currently working with several postdoctoral students on an article exploring the relevance for social media of media guidelines for reporting of death by suicide. This work will be shared with Samaritans on completion.

PI Contribution This collaboration yielded a working paper which is currently under review. In this paper, we are interested in understanding the interrelationships between mainstream and social media in forming public opinion during mass crises, specifi- cally in regards to how events are framed in the mainstream news and on social networks and to how the language used in those frames may allow to infer political slant and partisanship. we study the the dynamics of lingual choices for event framing and political agenda setting in mainstream and social media by analyzing a dataset of more than 40M tweets and more than 4M news articles from the mass protests in Ukraine during 2013-2014 - known as "Euromaidan" - and the postEuromaidan conflict between Russian, pro-Russian and Ukrainian forces in eastern Ukraine and Crimea. We focus on the most memorable terms - such as memes, symbolic labels, and personalities - which shaped the public discourse during the Ukraine crises and show that lingual choices in mainstream and social media can point to partisan behavior. We further exploit the state-of-the-art natural language Word2Vec representation to analyze at scale the linguistic markers which point to a particular political leaning in online media and show that political slant in news articles and Twitter posts can be inferred with high accuracy. These results contribute to the debate on the role of social media in fostering grassroots frames and partisan discourse during political crises and helps to uncover the extent to which the two media spheres - social and mainstream - compete or cooperate with each other as parts of a networked system of politicized frames and partisan language shaping the overall conversation
Collaborator Contribution We collaborated with Dr. Pryymak from Facebook London who contributed in collecting Twitter and news datasets and also in designing algorithms for identifying significant events in the data through natural language processing. We also collaborated with Ms. Lokot from the University of Maryland who - as an expert in media analysis during political crisis - provided crucial insights which assisted the design of machine learning models. She also significantly contributed in preparing the manuscript.

PI Contribution We have contributed to this collaboration by working on a paper about studying behaviour of online users across different social networks. The paper is summarised below.
Collaborator Contribution Our collaborators contributing in this paper by both conducting experiments but also by jointly preparing the manuscript.
Impact This collaboration produced a working paper which is currently under review. As social networks have proliferated, users have started creating profiles on multiple social networks. In such a setting, we investigate whether the profiles of a user on different social networks are redundant expressions of the same persona, or whether users adapt their profiles on/to each platform. Using the personal webpages of 116,998 users on About.me, we identify and extract 275,430 profile images and 229,365 profile selfdescriptions on several major social networks including Facebook, Twitter, LinkedIn, and Instagram. By analyzing these dataset, first, we establish that different social networks have different norms, both in terms of the expected kind of profile images used, as well as in terms of the language found in self-descriptions in pro- files. Second, we show that users do indeed try to "fit in" to individual platforms, and therefore adapt to the norms of a platform: A random forest classifier trained on the specific norms of a given social network can robustly identify a user's profile image (i.e., 0.657-0.829 in AUC) or self-description (i.e., 0.608-0.847 in AUC) on one platform from other profiles of the user on other platforms. However, despite this fitting in, the different profiles of a user appear to still be recognisable as expressions of the same persona. We demonstrate this by showing that a random forest classifier can separate a user's profile on a platform from those of up to k = 10 other users on the same platform (i.e., AUC ˜ 0.7), by training on the original user's profiles on other social networks.

Results and Impact During 2016, we worked with professional photographer, Anton Want, to develop a photographic exhibition based on the findings from the 'A Shared Space and a Space for Sharing' project.

Results and Impact This stake-holder workshop was organised in collaboration with the Tinder Foundation, a charity which supports digitally and socially excluded people to improve their lives through digital technologies. The aim of the workshop was for the stakeholders to develop an understanding of social science research, to discuss the 'A Shared Space and A Space for Sharing' project, to discuss some of the barriers to people using online spaces for sharing information, experiences and emotions and to explore ways of overcoming challenges to people using online resources. The workshop also sparked discussion for future research ideas and proposals. Following the workshop, the research team (Peter Bath, Melanie Lovatt and Julie Ellis) worked with Tinder Foundation (James Richardson) to write a blog about the workshop (http://www.tinderfoundation.org/our-thinking/blog/shared-space-and-space-sharing-online-health-forums-digital-exclusion) - this was published on the Tinder Foundation web-site.

Via the feedback forms, attendees reported many positive effects from the workshop. These included comments that the workshop had generated new knowledge and ideas for improvements - for instance about barriers to engaging with forums - for them to take back to their workplaces, which would ultimately benefit their service users / members. Other comments included the benefits of sharing knowledge with other stakeholders and learning more about online health forums.

Feedback from workshop participants include:

Results and Impact The event was one of series of seminars funded by ESRC on 'New Drugs' Speakers and participants were from both academic and professional services organisations - e.g. including Prison Officers Association, Police, addiction and abuse services. It was, amongst other things a opportunity to exchange views about the state impact of 'new drugs'/NPS and recent drug control legislation (Psychoactive Substances Act 2016), and how this was likely to dealt with by police and how these drugs were impacting prison officers and prison populations.

13.00-14.00: Roundtable: New drugs and the 'bigger picture' (Chaired by Dr. Kate O'Brien)

A collection of eminent drug researchers consider the impact that new drugs have had on the work that they do, for example in reconstituting the way that we think about legislating for the control of drugs, designing interventions to help people who use drugs, and understanding the way that drugs are bought and sold.

I spoke on a panel including the following: Professor Susanne McGregor (London School of Hygiene and Tropical Medicine), Professor Dave Boothroyd (University of Lincoln), Professor Shane Blackman (Canterbury Christchurch University), Dr. Axel Klein (Global Drug Policy Observatory, Swansea University), and Professor Alex Stevens (University of Kent).

Results and Impact The Lincoln work package of the S4S project was the focus of a set of research projects (funded and unfunded) and activities being undertaken with the School of Film and Media, each of which has an investment in 'participatory media' - both as a concept and as a practical project currently at particular stage of development. It was open to the public, all members of the University of Lincoln and to invited potential stakeholder guests. It comprised a poster exhibition and a series of presentations. The event was filmed and the filmed material will appear at a later date on the research webpages of the School and (likely) as part of the later S4S project exhibition. The event was organised by Professor Dave Boothroyd and was financially supported, and 'in kind', by technical staff and film students from within the School.

Results and Impact Social media, Activism, and Organizations 2015 (#SMAO15) brought together a large, international audience from a variety of disciplines. Founding chair, Dhiraj Murthy, was thrilled to receive such a strong set of submissions, which made for an exciting day of talks. Papers at #SMAO15 not only advanced our understandings of contemporary uses of social media use in social movements, but also the many ways in which we can critically reflect on their potential and limitations. Importantly, organizational communication perspectives were central to #SMAO15 and the contributions made give scholars in this interdisciplinary area much to think about. Questions around social media in formal and informal movement organizations were explored from a variety of methodological perspectives, including social network analysis, ethnography, surveys, participant observation and big data analytics. Papers covered regional case studies in Europe, North America, Africa, the Middle East, and Asia. In addition, #SMAO15 brought together practitioners, artists, and scholars. This unique constituency of attendees yielded new perspectives and approaches to studying social media, activism, and organizations.

Results and Impact This talk focussed on the experiences of people living with Breast Cancer and Motor Neurone Disease. The journey with the two conditions differ: people with breast cancer hope to be treated successfully and to move on in their lives; in contrast, Motor Neurone disease is currently incurable and, because of this, the diagnosis is particularly devastating. However, the story is not about despair, but about the hope and inspiration that people in these situations found on the Internet, and particularly in online support groups.

Results and Impact The activity was a workshop organised and funded through the ESRC Festival of Social Sciences at the Northern General Hospital, Sheffield. 30-40 healthcare practitioners and allied health professional, including hospital chaplains, particularly those involved in Cancer Care, attended the workshop. It involved an introduction about the "A Shared Space and a Space for Sharing", a performance of the 'Space for Sharing' play, followed by an audience discussion with the actresses and researchers. Then we posed a series of questions to the audience about the use of online health forums, and this led to an interactive panel discussion. The panel included a member from Breast Cancer Care. We collected feedback from the audience which is currently being analysed. In addition, we distributed copies of the information sheets that we have developed in collaboration with Breast Cancer Care: these include one for patients and carers of people with cancer and the other for healthcare professionals. The information sheets provide advice about using online health forums.

Results and Impact The workshop involved working in partnership with the National Health Services' South Yorkshire Education in Cancer & Long Term Conditions (SYEC<Cs) network. We delivered a workshop on OHFs in November 2018 to approximately 40 healthcare professionals. 89% of attendees reported that they had a better understanding of the benefits and risks of online forums after the event. Furthermore, attendees recorded actions that they would take away from the event, with responses including: signposting patients to forums, and sharing learning with colleagues. Attendees were surveyed about their willingness to signpost patients to OHFs before and after the event. The results indicated the power of the workshop to change practice, with 42% of attendees reporting a willingness to signpost patients before, which doubled to 87% after the event. The workshop has had a lasting impact. Six months after the workshop an attendee from a Teaching Hospital reported that patients attending the breast clinic are now signposted to online health forums as an additional form of support, and a lot more patients are now exploring this option. Another attendee from the IVF unit at the same NHS Trust reported that young women going through breast cancer treatments, who are referred to their services to preserve their fertility, are now signposted to online health forums as a form of support.
