Obesity strategic network: whole systems approach

Abstract

Increasing volumes of 'data' about individual behaviours are becoming available through social media, mobile phone geotagging, store loyalty cards and purchasing transaction data. There are many opportunities to use these data to benefit society. One key example is to better understand how the environment in which we live influences our behaviours leading to health concerns such as overweight and obesity. This is something which the Chief Medical Officer and other policy makers have begun to recognise as important. A better understanding of these complex interactions offers scope for designing interventions to addressing this significant 'wicked' challenge.
Obesity continues to be a problem in the UK with the most recent figures reporting 67% of men and 57% of women classed as overweight or obese. Overweight and obesity are serious health concerns and are risk factors for other non-communicable diseases such as cardiovascular disease, type 2 diabetes, hypertension, osteoarthritis and certain cancers. In many places the environment in which we lives makes it easy to gain weight. For example, fast food outlets are regularly closer to our homes and workplaces than supermarkets with healthier food choices and it is often safer or more convenient to drive to work rather than walk. These factors encourage individuals to eat unhealthy foods and be sedentary rather than physically active, contributing to weight gain and subsequent overweight and obesity. This setting has been termed an 'obesogenic environment'.
In order to promote healthier eating and more physical activity; groups of professionals need to work together to better understand how to modify these environmental influences so that it is easier to eat a good diet and be more active. These groups of professionals include researchers with an interest in diet and physical activity as well as involved with promoting active travel, reducing crime and improving retail planning. Real changes also need the involvement of local government, planning organisations, retailers, charities and health practitioners. Experts in data analytics are required to harness the volumes of information available and use these in a meaningful way.
This network aims to do just that - bring together many different types of researchers with policy makers, retail and third sector organisations to work together collaboratively in order to plan how to make best use of the large volumes of data now available to inform research, policy and practice in the prevention of obesity.
A network like this is a prime example of how the whole is far greater than the sum of its parts and together there is great potential to make a difference.

Planned Impact

To ensure maximum impact of the research generated by this cross disciplinary, multi-site network a range of stakeholders will be involved. These stakeholders include academics, local authorities (Leeds City Council), charity organisations (Association for the Study of Obesity and NESTA), industry partners (Anytime Fitness, Marks and Spencer's), practitioners (MoreLife) and policy makers (Town and Council Planning Association, Public Health England and the Department of Health, Scottish government Paths for All). Through these network partnerships there is great potential to co-produce new work with wide reaching impact. The innovative use of large data and novel methods in data analytics across many disciplines areas will be of benefit to many.
Who will benefit from this network?
Academic beneficiaries of this network will fall predominantly into two areas:
1) Those with interest in data analytics methods
2) Those with interest in a systems approach and the environmental factors influencing obesity.
There will also be cross over between the two groups. The network will generate new ideas and methods for how to best handle the large volumes such as those which will form the backbone of this network such as: the ESRC Administrative Data Research Centres from Scotland and Northern Ireland, the Health and Social Care Information Centre and the data held within the Leeds Institute for Data Analytics from both the Consumer Data Research Centre and the Medical Bioinformatics Centre.
Other beneficiaries will include a range of professionals working towards the reduction of obesity and associated improvements in health. The long term benefit would be societal if research generated could lead to changes in the environment making is easier to maintain a healthy weight and more difficult to gain weight.
The network will explore how the analytical methods and big data can be applied to address important interdisciplinary research questions around the obesogenic environment. Outcomes of these research questions will be of benefit to researchers from a range of areas including but not limited to: geography, spatial epidemiology, nutritional epidemiology, public health, sport science, active lifestyles, active travel, nutrition and diet. The applied methods will be of benefit to quantitative researchers and data analysts worldwide from within and beyond academia.
Policy makers and practitioners will benefit from this network as they will be able to contribute to inform research questions generated and ensure that these are applicable to their target populations. These collaborations will ensure that pathways to impact for the research will be clear and well informed.
How will they benefit from this network?
This network proposes to generate proof of concept work and collaborative funding proposals for further research using big data to answer important questions about the influences of the environment on obesity. Proof of concept results and methodological developments will be published in peer reviewed journals. Other news will be shared via virtual networks such as twitter, LinkedIn and JISCmail in attempt to ensure widespread impact of the network.
Through co-production with non-academic partners who are central to defining activities, including local authorities, industry partners and practitioners, benefits of the network will have a direct path to society for them to reap the benefits where possible. These can be operationalised through examples such as local council initiatives. Societal benefits will likely be longer term dependent upon the outcomes of collaborations beyond the network funding period.

Description Four achievements relating to our original objectives are as follows:

1) Engage researchers at different career stages and facilitate development of ECRs
The involvement of Early Career Researchers in the seminar series was very successful. We used a simple but effective devise of matching ECRs with a more experienced partner to jointly design and deliver individual elements of the seminar and workshop programme. The partners were typically not acquainted with one another at the start of the project. Two of the applicants including the network director (Morris) were ECR. Writing teams for our paper series were well balanced between early career and established researchers.
2) Share knowledge to facilitate development of new research approaches
The network drew an excellent mixture of contributions from policy-makers (e.g. TCPA, Local authority heath teams, J Sainsbury) and academics from a range of disciplines (medicine, health science, diet and nutrition, sports and exercise science, health economics, transport studies, geography). This in turn brought in a diversity of methods from agent-based modelling and microsimulation to causal inference, machine learning, hazard modelling and decision science. International colleagues from the Netherlands, US, Ireland and New Zealand contributed talks and broader strategic advice. The discussion of new approaches and recommendations particularly around the exploitation of Big Data for obesity research led to a new approach for reporting results from Big Data studies (the 'BEE-COAST' framework) and a Delphi forecasting process to determine consensus around definition of big data in obesity research and future research priorities, with both of these advances reported in the peer-reviewed research literature.
3) Generate a policy paper and shape future policy guidance
A paper was produced and published in the Journal of the Town and Country Planning Association. A series of six papers and two editorials, reporting a range of outcome across the project has been accepted by the International Journal of Obesity. The first two of these papers have been accepted for publication with the remainder at various stages of editing and review.
4) Generate a proof of concept research model
Using our network of research collaborators as an expert review team it was possible to direct a range of activities towards the design of a model of active lifestyles and obesity with data analytics and coding of the model structures being progressed between workshops. Data for the student populations of a University campus (Leeds) has been connected from a variety of sources including loyalty cards for University catering and gym membership usage. Further development of this model is progressing beyond the life of the project using resources of the ESRC's national Centre for Consumer Data Research.
Exploitation Route Now that approvals have been obtained work on the model demonstrator is being taken forward by a postgraduate research intern in the Leeds Institute for Data Analytics.
The engagement of commercial organisations has led to a PhD student which is being sponsored by a supermarket retailer including both financial support and provision of data from a loyalty card. Another PhD project will start in September 2018 in association with a lifestyle partner targeting weight loss interventions for healthy living.
Two pilot projects with the UK Prevention Research Partnership would allow for a significant scale up across a number of academic partners if approved for the main funding phase. A major research programme with the Wellcome Foundation involving a number of network partners is at an advanced stage of preparation.

Description New insight for the student services team managing food provision at the University of Leeds has been generated as a result of the network proof of concept project. Changes are currently under discussion within these teams in order to try and improve student access to healthier choices.

Results and Impact This workshop was a Data Science Engagement event involving an international company, Callcredit (and CDRC data partner), with data scientists, researchers and academics at the University of Leeds. A tour of the facilities in the Leeds Institute for Data Analytics was followed by six short presentations (from individuals at the University of Leeds and Callcredit) and an extended round table discussion.

Results and Impact The purpose of this workshop was to introduce early career researchers in the obesity field (interdisciplinary group including health care professionals) to the opportunities for using big data in their research. The workshop was very well received and positive feedback provided to the Association for the Study of Obesity. Participants reported that the workshop opened their minds to opportunities that they had not previously thought of as available to their research field.
Colleagues involved in the session also reported that their perspectives had been widened.

Christina Vogel et al. wrote the attached article as to why this is an important food policy. The purpose was to raise the profile of the research with local authority and third sector stakeholders in Greater Manchester and in Manchester City in order to increase awareness of the research plan, interest in our methods and the intended outcomes for the local community. This was also an opportunity to talk to stakeholders about recruitment of public contributors from the Manchester area, and it was successful in doing this.

Results and Impact Presentation followed by a meeting with software demonstration. Approximately 20 delegates from the China National Centre for Food Safety Risk Assessment attended my talk: The role of big data in health research: case studies of obesity and foodborne illness.
Questions and discussion on aspects of dietary assessment, syndromic surveillance and population modelling followed. In a meeting with the nutrition team, following the talk, I provided a demonstration of the myfood24 online dietary assessment tool and further discussion on techniques for population modelling, including spatial microsimulation. Detailed discussion on data sources in China took place.
