The role of social anxiety on safety-learning: Bridging the gap from the lab to the clinic

Abstract

In the UK, anxiety disorders affect over eight million people and cost the economy approximately £10 billion a year (McCrone et al., 2008). Social anxiety disorder (SAD) is the most common anxiety disorder, except for specific phobia (Ruscio et al., 2008), and is characterised by excessive fear and avoidance of social or performance situations. SAD typically develops during late childhood or adolescence and therefore has a significant impact on a large number of young people, as well as adults (Beesdo et al., 2007; Detweiler et al., 2014; Stein et al., 2017). Further, some level of social anxiety is experienced by most humans at some point in their lives, for example when attending a job interview or having to give a presentation. People who suffer from social anxiety symptoms report poorer life satisfaction, physical health, mental health and have more clinical complaints and disabilities compared to people with lower social anxiety symptoms, even if they do not qualify for a diagnosis (Fehm et al., 2008). Therefore, the number of people severely affected by social anxiety may in fact be higher than current estimates of the disorder indicate (Jefferies & Ungar, 2020).

Cognitive Behavioural Therapy (CBT) is used for the treatment of SAD and includes exposure therapy as a main component. Exposure therapy for social anxiety includes face-to-face and imagined encounters with feared social situations, for example, starting a conversation or attending a social event, with the help of a Clinical Psychologist. Exposure to these social situations provide the patient with the opportunity to learn that the feared social situation will not always pose a threat. As the patient learns this information, they generate "safety" associations and social anxiety declines. However, treatment for SAD is often not successful. Many patients still experience symptoms or a return of fear after treatment. This suggests that there is room for improvement in the treatment of SAD. Laboratory-based research on safety learning, for example learning that something that was once threatening is now safe within an experimental task, can be carried out by researchers to further understanding of why fear may be maintained or return after exposure for people with social anxiety. Findings from this research can be used to inform Clinical Psychologists of potential methods of improving treatment for SAD. Therefore, there is a need to encourage collaboration between laboratory-based researchers and Clinical Psychologists to gather new ideas that might improve treatment outcome for SAD.

This fellowship aims to maximise the potential of my laboratory-based research to bridge the gap between laboratory science and clinical practice. This project is important because it may help us to design ways to adapt exposure therapy for SAD that might lead to improved outcomes of treatment. The findings of my work are relevant for clinicians using exposure-based treatments for SAD, and this fellowship will provide the opportunity to gain clinical experience and foster connections with clinicians to maximise the societal impact of my work. Further, the fellowship will enable me to develop my track record academically through the publication of two substantial review papers and an experimental piece and engage with patients and the public to ensure that future research is informed by and reflects the priorities of people affected by social anxiety. Therefore, the fellowship will enable me to develop the track record, networks and experience required to enhance the translational impact of my research from the laboratory to clinical practice. In the long term, this work has the potential to improve the quality of life of people suffering with social anxiety and decrease the substantial societal costs associated with the disorder.
 
Description The primary aim of my Postdoctoral Fellowship was to develop the skills, expertise and networks required to maximise the translational impact of my research from the basic lab-based science conducted throughout my PhD to clinical research. My research examines the biological and behavioural processes related to transdiagnostic and disorder-specific features of anxiety and has highlighted several novel explanations regarding why many individuals with social anxiety might remain symptomatic after intervention with exposure therapy. In doing so, my research extends theoretical understanding within my research field and lays a foundation for further psychopathology research focused on translating findings from basic laboratory science into clinical interventions.

Much of my Postdoctoral Fellowship involved seeking out opportunities for intra- and inter-disciplinary collaborations within and outside of my home institution while exploiting opportunities to develop the social and cultural impact of my research. For example, during my Fellowship, I established a collaboration with the Cherrycroft practice on a project that is examining the relationship between verbal comprehension and social problems in a population of children that have experienced severe trauma. I am also a named collaborator of the Recreational Fear Lab headed by Dr Mathias Clasen at Aarhus University, Denmark, and a member of The Oxford Psychological Interventions of Children and Adolescents Research Group headed by Professor Cathy Creswell, University of Oxford. Further, during my Fellowship, I was awarded a position on the Sealey Associate's ESRC Postdoctoral Development Programme (PDP). The PDP provided me with the opportunity to attend a Policy Challenge Workshop hosted by Sealey Associates, where I developed relationships with policymakers of the "Levelling Up" policy (Jagdeep Athwal and Prateek Buch) and ESRC Postdoctoral Fellows outside of my discipline (Dr Emily Marchant, University of Swansea and Dr Lindsey Collins, University of Bradford). This opportunity resulted in the award of additional pump-priming funding for a collaboration on a project that aimed to understand the individual- and place-based factors that influence well-being in individuals currently excluded from completing well-being surveys due to reduced communication abilities.

During my Fellowship, I was also invited by academics and clinicians outside of the University of Reading to present my findings in seminar series including the Neuroscience Seminar Series hosted by the University of Exeter, the Mood Disorder Centre Think Tank Talks hosted by the University of Exeter, and the TOPIC Research Group Talks hosted by the University of Oxford. In addition, I demonstrated leadership and commitment to the dissemination of my work through the organisation of two symposiums (Reading Emotions 2022 and "Driven by the unknown" at the EABCT Congress 2022 in Barcelona) that bought together internationally leading neuroscientists, clinicians, and researchers in psychopathology during the summer of 2022. I co-chaired and presented my research findings at both symposiums, greatly enhancing my research profile.
Exploitation Route As the research crosses subdiscipline boundaries, it benefits clinical researchers studying social anxiety and treatment processes due to findings that provide possible explanations behind the maintenance of fear associated with social anxiety after exposure. Further, my work benefits the field of clinical anxiety research going forward as my findings promote the importance of considering the specificity of trait markers of anxiety and negative affect on safety learning processes. This approach enables researchers to tease apart the overlapping and disorder-specific features of anxiety to better inform target-driven treatment strategies based on specific characteristics of the individual. Before the work of my PhD thesis and Postdoctoral Fellowship, this approach had not been utilised in the field of social anxiety and safety learning and therefore my work provides a conceptual and empirical foundation for future research in the field that aims to address similar research questions. Due to the potential of my research to inform and improve treatment options and raise awareness of the learning processes and behaviours characteristic of social anxiety, non-academic beneficiaries of my research include Clinical Psychologists, patients and individuals suffering with symptoms of social anxiety, parents and educators, young people and the general public more broadly.

Title The impact of social anxiety and instruction on avoidance 
Description This experiment examined the impact of social anxiety and instruction to not avoid on avoidance behaviour within a Pavlovian Conditioning paradigm. We measured skin conductance responses, expectancy ratings and avoidant behaviour during an associative threat learning and avoidance task. To assess social anxiety, intolerance of uncertainty and trait anxiety, we administered the Social Phobia Inventory (SPIN) (Connor, Davidson, Churchill, Sherwood, Foa, & Weisler), The Intolerance of Uncertainty Scale (IUS) (Freeton, Rhéaume, Letarte, Dugas, & Ladouceur, 1994) and the trait version of the State-Trait Anxiety Inventory (Spielberger, Gorsuch, Lushene, Vagg & Jacobs, 1968). The experimental task included four phases; threat acquisition, avoidance conditioning, avoidance test, and threat extinction. All participants completed all four phases. The study used a between-subjects design and participants were allocated to one of two conditions: "instruction" and "control". Participants in the instruction condition received the following instruction between the avoidance conditioning and avoidance test phases of the task: "Please press the non-avoidance button to test your expectancy whether an electrical stimulus and statement will occur". At the same time point, the control group saw the following message "you may take a short break and continue when you are ready". There were no other differences between the "instruction" and "control" conditions.

Results and Impact Nine professionals attended a stakeholder meeting organised by myself to discuss the barriers of measuring the well-being of individuals with reduced opportunity/or ability to communicate. Professionals from the What Works Centre for Well-being, Disability Rights UK, the Department of Work and Pensions, the NHS, and the Cantre for Thriving Places attended the meeting which resulted in the establishment of a network for future collaboration.

Results and Impact I organised, chaired, and presented at the annual Reading Emotions symposium in 2022 that focused on Avoidance and Exposure: from Brain to Clinic. 361 attended the online event, including academics, clinicians, individuals working for third-sector organisations, and interested individuals from the general public.

Results and Impact I was invited to attend Downe House School to give a presentation about my research to ~150 sixth-form students. Students were engaged, asked questions and several reported that they would like to apply for a Psychology course at University as a result of the talk. The school has asked whether I will come back to give another presentation next academic year.
