Abstract

Our objective is to establish a state-of-the-art integrated Clinical Research Imaging Centre to
enable translational clinical research performed within the University of Edinburgh and
particularly within the internationally renowned Centres for Cardiovascular Science, Inflammation
Research, Reproductive Biology and Clinical Brain Sciences. Our vision is that the co-localisation
of 3T whole-body magnetic resonance imaging with positron emission and computed
tomography imaging will provide an unparalleled resource which will markedly enhance clinical
research and experimental medicine within this highly productive and unique grouping of worldleading
basic and clinical scientists, and feed directly into the newly established Clinical
Research Networks.
We are establishing the Clinical Research Imaging Centre in the University’s new Queen’s
Medical Research Institute and have already secured the provision of combined positron
emission and computed tomography, and a dedicated on-site cyclotron. We request funding and
support for the provision of a 3T whole-body magnetic resonance imaging scanner and
associated support costs. This enhanced magnetic resonance capability will build upon the
University’s highly successful clinical neuroimaging infrastructure already established in
Edinburgh through the Brain Imaging Research Centre and complement the juxtaposed
Biomedical Magnetic Resonance Imaging Unit with its pre-clinical 7 Tesla magnetic resonance
imaging scanner.

Technical Summary

Funding for a 3T MRI & associated support costs to enable expansion of existing MRI research capability to build upon infrastructure model developed through Brain Imaging Research Centre in juxtaposition with adjacent 7T Biomedical MRI unit.

Major research themes: translational & experimental medicine investigating disease within the cardiovascular, inflammatory and central nervous systems. Plan to maximise benefits of 3T MRI with novel imaging strategies: MRI contrast agents consisting of small & ultrasmall superparamagnetic particles of iron oxide to label specific cell types ex vivo & in vivo; perfusion & diffusion-weighted imaging to charaterise physiological & pathophysiological processes involved in tissue inflammation, ischemia and infarction in cardiovascular and CNS; magnetic resonance spectroscopy for metabolic studies of brain, liver, heart, skeletal muscle and other organs including potential examination of metabolism within atherosclerotic plaques.

Impact Evidence before this study Prior to study inception, we searched PubMed for terms that included "computed tomography", "coronary angiography", "angina pectoris" and "coronary heart disease". We identified a high-quality Health Technology Assessment (HTA) comprehensive systematic review that assessed the role of 64-multidetector computed tomography coronary angiography (CTCA). In keeping with previous analyses from the European Society of Cardiology, it confirmed the diagnostic utility of computed tomography coronary angiography in the identification of coronary heart disease. However, this systemic review highlighted several areas that need further research and in particular highlighted the need to evaluate the usefulness of multidetector computed tomography coronary angiography in patients with suspected coronary heart disease. Shortly afterwards, the National Institute for Health and Care Excellence (NICE) outlined recommendations for the assessment and investigation of recent onset chest pain or discomfort of suspected cardiac origin. These guidelines specifically called for research into the clinical and cost effectiveness of computed tomography coronary angiography in the diagnosis of angina pectoris due to coronary heart disease. One study has assessed the effect of CTCA on the management of low-risk patients presenting to the emergency department with acute chest pain. It suggested improved decision making but an increase in further downstream testing and healthcare costs without any impact on cardiovasular outcomes. In contrast, the benefit of implementing CTCA for patients presenting to the cardiology clinic with stable chest pain has not been systematically evaluated. Added value of this study In the SCOT-HEART trial, we investigated the impact of CTCA in patients referred to the cardiology clinic with suspected angina due to coronary heart disease. In a large pragmatic multicentre randomised controlled trial, we showed that CTCA markedly improved the diagnosis, management and outcome of patients referred to the cardiology clinic with suspected angina due to coronary heart disease. This was the first description of an imaging modality impacting on outcome in this group of patients. Implications of all the available evidence The addition of CTCA to standard clinical care markedly clarifies the diagnosis of angina due to coronary heart disease. This reduces the need for further stress testing, leads to more appropriate use of invasive coronary angiography, and results in more focused treatment regimes that are associated with a halving in future rates of fatal and non-fatal myocardial infarction.

Title Magnetic Resonance Imaging with Ultrasmall Superparamagnetic Particles of Iron Oxide 
Description In a multicentre prospective observational cohort study, we have demonstrated that USPIO-enhanced MRI predicts the rate of aneurysm expansion, and the risk of abdominal aortic aneurysm rupture and repair. Although it does not provide independent prediction of aneurysm expansion or clinical outcomes in a model incorporating known clinical risk factors, this is the first demonstration of a cellular imaging technique that can predict clinical events in patients with abdominal aortic aneurysm. Whether clinical outcomes can be improved by treatment decisions based on this novel imaging approach remains to be established.
