Efficacy and mechanism of thyroxine treatment on pregnancy and neonatal outcomes in women with thyroid antibodies:

Abstract

Technical Summary

Scientific Abstract|We conducted a systematic review exploring the association between thyroid antibodies and adverse pregnancy outcomes. For miscarriage, we identified 29 studies (9817 women), and 26 of these showed a positive association. Meta-analysis of the 29 studies showed more than doubling in the odds of miscarriage in the presence of thyroid antibodies (OR:2.4, 95% CI: 1.9, 3.1). For preterm birth, we identified 4 studies (2896 women). All studies showed a positive association and meta-analysis found more than doubling in the odds of preterm birth in the presence of thyroid antibodies (OR: 2.7, 95% CI: 1.9, 3.9). The two dominant hypotheses for these observations are that a) thyroid antibodies represent a state of global immune dysfunction and b) thyroid antibodies are associated with relative thyroid insufficiency. There is evidence that thyroxine treatment can alter global immune response directly, and it can also correct any relative thyroid insufficiency. We carried out another systematic review to identify studies of thyroxine replacement to improve pregnancy outcomes in women with thyroid antibodies: two randomised trials, including a total of 187 women, were identified; the studies showed a reduction in miscarriages (RR: 0.48, 95% CI:0.25, 0.92) and preterm birth (RR:0.31; 95% CI:0.11, 0.90) with levothyroxine treatment. However, our UK clinician survey (n=183) found that only 8% (15/183) of clinicians use or intend to use levothyroxine for the prevention of miscarriages in thyroid antibody positive women and that 85% (155/183) do not currently use levothyroxine, but would be willing to recruit into a randomised study.|We, therefore, propose a randomised, placebo-controlled, double-blind, multi-centre trial to test the hypothesis that in euthyroid women with thyroid peroxidase antibodies, levothyroxine (50mcg, oral, once daily), started pre-conceptually and continued to the end of pregnancy, compared with placebo, increases live births beyond 34 completed weeks of gestation by at least 10%. Nested within the trial, we will evaluate cytokine levels in the maternal circulation and decidua. We plan to randomise 900 women (450 participants in each arm): To detect a 10% difference in live birth beyond 34 weeks, for an alpha of 5% and power of 80%, 380 women will need to be randomised to each group. However, assuming and adjusting for a worst case scenario of a 15% attrition, the total number of participants required will be 900. The analysis will be by intention to treat, and according to a pre-specified analysis plan. The trial will be co-ordinated by an Accredited Clinical Trials Unit. Twenty one hospitals in the UK will participate in this trial, which will be led by a strong team of researchers with track record in conducting multicentre trials as well as miscarriage, preterm birth and thyroid research. The trial is supported by patients (78 interviewed), Miscarriage Association, British Thyroid Foundation, British Thyroid Association, the RCOG and others. If our study confirms the benefits shown in the existing trials, the NHS can save over £215 million/year.|

Impact Between April and June 2016, maternal health research became the highest scoring press theme of the academic year at the University of Birmingham. Overall, the campaign delivered approximately 1,200 website link clicks from 550,000 impressions. Facebook advertising was particularly effective, with over 600 link clinks generated from 50,000 impressions. On average people visited 2 webpages per session. The Twitter advertisement was retweeted 95 times. Our most popular advertisement strapline was "we are leading the largest early miscarriage centre in Europe." The campaign received praise and support on a variety of social media sites, from a variety of stakeholders including patients and families, students, academic peers, charities, professional practitioners and the Minister for Care Quality Ben Gummer.

The petition has over 250,000 signatures and is calling for the following recommendations from the Lancet series to be built into the women's health strategy: • Support should be available after every miscarriage in line with the graded model of care. • Mental health support should be available after each loss, as those who have suffered miscarriage are at a higher risk of developing anxiety, depression, post-traumatic stress disorder, and of suicide. • An end to the postcode lottery: tests and treatments must be standardised across the UK. • All miscarriages must be formally recorded as this information is required to inform our research.

Title Levothyroxine to reduce miscarriage risk in euthyroid women with thyroid auto-antibodies 
Description Financially supported by the NIHR EME competition, the TABLET trial (ISRCTN 15948785, EudraCT 2011-000719-19) sought to evaluate the effects of thyroxine to prevent miscarriage in women with thyroid antibodies but normal thyroid function. The trial was carried out at more than 30 centres across the UK.

Results and Impact In October 2010 at the Annual Meeting of the British Thyroid Association in London, Professor Mark Kilby took part in a national debate to contest whether the reproductive health of women with anti-TPO antibodies may be improved by treatment with thyroxine prior and during pregnancy. Professor Mark Kilby argued for this motion, and Professor Tony Weetman argued against it. The debate sparked audience participation and discussion afterwards.

The debate stimulated thinking and active consideration of the hypothesis among professional practitioners.

Results and Impact In April 2016 we launched Tommy's National Centre for Miscarriage Research. Events in London and Coventry were spearheaded by academic team leaders, celebrity guest speakers and patrons of Tommy's charity. We gained tremendous national media exposure and raised awareness of our efforts to understand miscarriage.

Results and Impact Despite one in four pregnancies ending in miscarriage, the silence around the subject means that many couples who have lost babies harbour unexpressed feelings of failure, isolation and guilt. This campaign seeks to tell the stories of all those who have experienced miscarriage.
