Technical Summary
The Confidence in Concept scheme is a key part of MRC’s translational research strategy and provides annual awards to institutions, to be used flexibly to support the earliest stages of multiple translational research projects. The award can be used by the institution to support a number of preliminary-stage translational projects. The projects supported should aim to provide sufficient preliminary data to establish the viability of an approach –– before seeking more substantive funding. It is intended to accelerate the transition from discovery research to translational development projects by supporting preliminary work or feasibility studies to establish the viability of an approach.

| Description | The South London and Maudsley NHS Foundation Trust (SLaM) is the largest provider of secondary mental healthcare in Europe, serving a population of approximately 1.2 million people. Fully electronic health records (EHRs) were implemented in SLaM during 2006. Between 2007 and 2008, the Clinical Record Interactive Search (CRIS) application was developed to generate a psuedonymised version of SLaM's EHRs for use as a live-updated research database; CRIS data has since been extended through the development of NLP applications. The current project therefore sought to develop an NLP tool to identify instances of violence and abuse. |

The Interictal Epileptiform Discharge (IED), a hallmark graphoelement of epileptic EEG, is thought to play a key role in the formation and evolution of pathogenic networks in epilepsy. Although IEDs have previously been studied, their network attributes are still to be fully elucidated. Here, we perform a dynamic network characterisation of IEDs with high temporal selectivity, regarding this as a readily available window into the epileptic brain state. Methods Commonly, brain connectivity studies of epilepsy using EEG analyse either entire recordings, including pathogenic but also normal brain activities, or lengthy rare events, such as epileptic seizures. For the first time here, we estimate pathogenic networks that characterise very brief IEDs lasting a few 10's of milliseconds, alone. We use an inverse power method for estimation of sparse principal component analysis in order to reveal connectivity properties that are consistent across subjects. Finally, we employ centrality measures (eg. betweenness, eigencentrality, closeness) to identify areas that are either influential or play an important role in the IED network function. Results In a preliminary analysis of four exemplar patients, we examined their first EEG at initial epilepsy onset; subsequently, two proved to be treatment-responsive and two were treatment-resistant. We show a difference in their network features. Specifically, IED networks differ in their modularity. Conclusion We conclude that network investigation of IEDs may reveal predictive biomarkers for early identification of treatment outcome. |

| Results and Impact | We held a workshop on the information mental health practitioners need in order to respond to domestic violence victimisation and perpetration safely. A draft contents of a potential manual for mental health services was provided for the participants to comment on and presentations were given on current evidence base. The workshop was attended by safeguarding nurses, directors of nursing, chief executives of domestic violence charities, senior practitioners at number of different mental health trusts, policymakers at the Department of Health and Social Care, academics and mental health service users. There was agreement that an up-to-date valuable would be helpful for mental health trusts and the lead for domestic violence at the Department of Health and Social Care agreed to disseminated to all mental health trusts. NHS Improvement and the CQC have also expressed interest and details of the manual have been widely disseminated within our own organisation e.g. King's Health Partners newsletter, NHS England, CQC, DHSC etc. |

| Results and Impact | This meeting was set-up in order for Louise Howard and Emma Yapp to explain the LARA intervention, both in terms of the pilot intervention and current progress, to Heather Caudle, Director of Nursing for Improvement at NHS England. Heather Caudle expressed concern about overall lack of identification of domestic violence within mental health services, and about the failure of nurses in both primary and secondary care to assist in this practice. LH stressed the importance of working with the domestic violence sector in delivering interventions, and summarised the LARA pilot, leading to further discussion of improved the response to domestic violence within nursing. Louise Howard and Emma Yapp to attend next meeting of Strategic Leads recently set up across the Home Office, DoH and the NHS. |

| Results and Impact | This informal workshop aimed to discuss the opportunities and barriers of identifying and developing new biomarkers for asthma diagnostic tests and determine the next steps required in addressing the defined challenges through collaboration. The development of new diagnostics is embedded within the fifteen Key Research Priorities, as identified by the European Asthma Research and Innovation Partnership (EARIP). Challenges 1. Developing low-cost diagnosis devices that can provide rapid proteomic and metabolomic analysis of serum, EBC and urine 2. Further research into assessing VOCs using electrosensor (eNose) to phenotype asthma and predict asthma attacks Discussion • It was decided early in the discussion to focus solely on Challenge 1, as researchers should be looking at a broad range of metabolites in all matrices at this stage in the search for a gold standard diagnostic test. • The group noted that many drugs currently available failed their initial tests because patients enrolled in trials weren't stratified. They agreed that the development of new diagnostics will necessarily come from a personalised medicine approach. Next steps • There is a need to tackle conflicting guidance on how to diagnose asthma (whether it is physiology-based or symptom-based). Asthma UK are responding to the new NICE guidelines. • Researchers should develop a consensus paper detailing the current diagnostic landscape in terms of the available technology and biomarkers. From that, a working group would be better informed to define the needs and challenges ahead more precisely and crucially determine what information they wish to obtain from a diagnostic test. • Researchers should aim to identify three or four biomarkers from any matrix. An ideal biomarker should sit within the mechanistic pathway and be associated with the disease symptoms. It should be able to phenotype patients and will itself be altered by the treatment so that response is measurable. • Researchers require better access to biobanks and existing patient datasets and these barriers should be tackled. Simon Young at Astra Zeneca UK stated that they have access to cohort data and invited researchers to approach them for access. • When trials are designed, they must include multiple measures from the same patients over a long time period to understand variation in biomarkers. • GPs require better implementation of existing biomarkers and clinical risk prediction tools, and it was recommended that they explore the possibility of utilising/developing diagnostic hubs to overcome this. |

| Results and Impact | This activity was to relate our MRC funded research to patients through art. After describing our work and hosting a lab visit the patients were taught to express their understanding of the research in art. The designs they achieved were then printed into scarves that each patients could keep. The patients really enjoyed the workshop and the interaction with the scientist. To quote one patient " I just wanted to reiterate how much I enjoyed Tuesday's workshop, I absolutely believe that it's such a great thg for both patients and researchers to have more interaction with each other" Moreover this workshop really allowed patients to understand our research as exemplified by the quote 'Cells are so influenced by the environment, aren't they?' We had planned to continue these workshops into 2020 but the Covid-19 pandemic inhibited this. As an alternative we are in the process of developing a whiteboard video describing our research that will be loaded onto YouTube. |

| Results and Impact | Soapbox science in a national public outreach platform for promoting women scientists and their work. I co-presented our mathematical method and its application to health care with the post doc who was employed by the KHP R&D challenge fund (MRC confidence in concept award). This took place on a soapbox on Brighton beach and members of the public were able to gather and listen to the different speakers. Questions spanned from the value of big data in healthcare to how our work may help patients in critical care. Through this exhibition, i was contacted by a lead for STEM girls club and have since participated in a number of school events around STEM including a career workshop about STEM subjects and a further schools talk about the work we do. |
