Barretts metaplasia and associated carcinogenesis

Abstract

A type of oesophageal or gullet cancer, called adenocarcinoma, is rapidly increasing in the western world. Most patients do not seek medical attention until they develop swallowing difficulties and by then the cancer is usually advanced with a 5-year survival of <20%. However, if the cancer is detected early then the survival rate can be improved to >80%. These cancers usually develop as a complication of a condition called Barretts oesophagus which occurs in approximately 10% of those with heartburn symptoms. However, most people in the population with Barretts are not diagnosed and most will not develop cancer. Therefore research is needed to identify the patients at greatest risk for oesophageal cancer. |The specific issues being addressed are:| Can we identify high-risk groups for the development of Barretts oesophagus and oesophageal adenocarcinoma?| Can we understand the development and progression of oesophageal cancer at a molecular level so that we can develop effective clinical tools for patient management? |To do this research we work closely with the clinical service at Addenbrookes Hospital. Patients have the opportunity to take part in research by providing us with essential clinical specimens.|

Technical Summary

We conduct translational research on oesophageal cancer and the precursor lesion called Barretts oesophagus. This is important because the incidence of oesophageal cancer is increasing rapidly and the outcomes are dismal. The diagnostic tools and treatments available are limited, but the MRC Cancer Medicine Unit offers a unique opportunity for beneficial research because of its proximity to the clinical environment and its opportunities for scientific collaboration. Recent achievements include development of a non-endoscopic device coupled to an immunocytological assay which has gone all the way from conception to a clinical trial. Our over-riding objective for the future is to understand the molecular and cellular drivers for the development and progression of Barretts oesophagus and in so doing to provide tools for early diagnosis, accurate prognostication and therapy which can be applied to clinical practice.

Title Interview with BBC Radio 4's flagship news programme, PM 
Description On Monday the 13th March, Professor Rebecca Fitzgerald, was interviewed by BBC Radio 4's flagship news programme, PM. This interview with the renowned presenter Eddie Mair highlights research carried out in the Fitzgerald lab at the MRC CU over the past many years that has led to the fruition of the early detection device - Cytosponge ('Pill on a String') - which is now in phase 3 clinical trials and may revolutionise early detection of Upper GI cancers. This interview is perhaps particularly timely given the recent series of interviews with the, sadly now deceased, journalist Steve Hewlett. Steve's reflections about his journey, from diagnosis to his untimely death, through Oesophageal Cancer were broadcast every week on the same program and became hugely popular and highlighted the need for research into earlier detection of cancers - something that is reflected through Rebecca's research in this instance and is exactly the mission of the MRC Cancer Unit, overall .

Impact Radio 4 were doing a series that narrated the experiences of (the now late) Steve Hewlett (BBC reporter) whilst he was undergoing treatment for Gastrointestinal cancer. Professor Fitzgeralds interview was in this backdrop and was very timely in highlighting the need for and progress in research in this field of cancer. The whole series had a huge listenership and being part of this ensured that Prof Fitzgeralds message had reached a huge radio audience.
