Abstract

Our research informs the management of health risks related to employment and feeds into Government policy and/or decisions in occupational health care. One focus is the influence of psychological risk factors on common work-attributed disorders such as low-back pain and non-specific arm pain. We hypothesise that much illness/disability blamed on workplace exposures does not arise from organic pathology, but is psychologically mediated and shaped by personal characteristics, culture, beliefs and expectations. In support of this, we have found that distress over common everyday physical symptoms predicts pain in the back and arm, and especially disabling pain, and also the occurrence of symptoms shortly after use of pesticides; that arm pain sufferers blame symptoms on work more often than can be explained by known occupational risk factors; and that UK workers have much higher rates of musculoskeletal disability than Indian counterparts performing similar work. Our research often feeds into practical policy. Thus, a study of hospital admissions for pesticide poisoning highlighted the importance of safer pesticide storage and led to tighter controls. In another analysis we compared national patterns of occupational mortality over time, finding a substantial fall in work-related deaths but several areas in which more preventive effort was needed.

Technical Summary

This programme applies epidemiological methods to inform the management of health risks related to employment. It addresses problems with relevance to Government policy and/or clinical decisions in occupational health care. Over the past five years one focus has been the influence of psychological risk factors, including culturally determined beliefs and expectations, on common work-ascribed disorders such as low-back pain and non-specific arm pain. We hypothesise that much chronic illness and disability attributed to injurious exposures in the workplace does not arise from organic pathology, but is a psychologically mediated response to an external trigger, conditioned by individual characteristics and cultural values, beliefs and expectations. In keeping with this, we have found (in two cohort studies and two cross-sectional analyses) that somatising tendency - distress over common somatic symptoms - is a strong predictor of: incident and persistent regional pain, especially disabling pain, in the community; arm pain presenting to primary care and physiotherapy services; and various acute symptoms within 48 hours of using a pesticide. We also found that arm pain sufferers blamed their symptoms on work 3.8 times more often than could be explained by known physical occupational risk factors. To explore cultural influences, we compared people performing similar occupational tasks in different countries and found that rates of musculoskeletal disability were substantially higher in UK workers than their Indian counterparts. Our hypothesis carries important practical implications resource invested on ergonomic controls may be less beneficial than expected, perhaps even counter-productive if reinforcing beliefs in a serious hazard; while attempts to modify beliefs and expectations could be more beneficial. Much of our work feeds into practice and policy, as in the above case. To give four other examples: 1) The data on pesticides suggest that illness ascribed to low-level pesticide poisoning may have a psychological basis, and have informed pesticide regulation. 2) Separately we have collected data on incident hospital admissions for accidental pesticide poisoning and highlighted the important contribution from poor storage of pesticides: this has led to tighter controls. 3) In a registry analysis commissioned by HSE and ONS, we compared patterns of occupational mortality in 2.8 million men from England and Wales during 1979-80 and 1982-2000. Although work-attributable mortality declined substantially, several current priorities for prevention were identified, including diseases caused by asbestos, sino-nasal cancer in woodworkers, and motor-vehicle accidents in lorry drivers. 4) Previous case-control studies on knee osteoarthritis and occupational lifting, kneeling and squatting have contributed to the introduction of new state compensation for coal miners with the disease.

Working under the leadership of an independent expert Chair, it considers chemical and physical hazards, and human, behavioural or organisational factors in the workplace (for example shift work) leading to physiological and psychological ill health. It does not consider wellbeing, sickness absence management or rehabilitation as these issues are dealt with elsewhere in government. Nor is it able to consider individual cases of ill health. WHEC will assess the evidence base concerning health issues within its remit and will seek to effectively and appropriately communicate the resultant health risk.

Impact VIBRISKS has contributed to the prevention and control of vibration-induced disorders by disseminating new methods, new findings, and experience from the project to occupational health professionals, standards committees (national, European, and International), and to employers and their workers. The VIBRISKS Final Report and all 21 Annexes are publicly available to assist others undertaking relevant research. The VIBRISKS protocols for diagnosing vibration-induced injuries, and the VIBRISKS guidelines for the health surveillance of persons exposed to hand-transmitted vibration or whole-body vibration, are available for application by occupational health workers across Europe and worldwide. Two international conferences and workshops were organised by the VIBRISKS project team to disseminate information from the project: (i) the 3rd International Conference on Whole-body Vibration Injuries in Nancy, France (7-9 June 2005) and (ii) the 2nd International Workshop on HTV Injuries in Gothenburg, Sweden (6-7 September 2006). The results are also presented in many papers already published or awaiting publication in scientific journals.

We won this award which will allow us to recruit 200 patients who are currently struggling at work because of their musculoskeletal health or their mental health. These people will be offered employment support and we will observe outcomes over three and nine months.
Collaborator Contribution The City Councils provide the employment support and project management for this study.
Impact We were successful in winning the competitive award of £376,000. An evaluation report is being prepared currently and DWP staff are coming to visit early March 2020.

Results and Impact Face to Face: Cathy Linaker organised this training day, chaired the afternoon session and gave a talk on confidence building and communication skills. The event was aimed at ARUK / MRC CMHW PhD students from the various spokes of the centre around the country. Talks were given by members of the MRC LEU (including from Work and Health, Keith Palmer, Karen Walker-Bone and Georgia Ntani). These talks included introductions to occupational health; musculoskeletal disorders; epidemiology; plus information for the students on research; confidence building and expectations of students/supervisors, followed by a Q and A session.

Evaluation sheets were given to the participants at the end of the day. Positive feedback was received with requests for future training days, possibly extended to 2 days so that more interactive sessions or workshops could be included.

Results and Impact Findings from our research on acute symptoms and long term neurological symptoms following work with pesticides fed into ACP deliberations on human health effects on pesticides.

The findings indicated that excess symptoms associated with exposure to pesticides may occur through non-toxic mechanisms, and this was taken into account when considering the need for tighter regulatory control.

She explained her role as a stem ambassador and her involvement in research. This provoked much discussion and interest, particularly in terms of encouraging women to study a STEM subject and their aims to promote women professionally by promoting education, empowerment and opportunity. Before Cathy left plans were made for future related activities and she had requests for further information.

Results and Impact CL has participated in 4 separate workshops during the past year and led several 15 minute discussions with groups of 12 secondary school students about careers/working in science. She has also given a talk to post graduate students. 24 students attended the talk about a career in science.

Questions and discussion about the various jobs within the field of science were asked. Positive feedback was also received from schools after their visit to LifeLab.

Results and Impact A study to validate methods for assesssing past exposure to depleted uranium underpinned the retrospecive exposure testing programme for Gulf War veterans provided by the Depleted Uranium Oversight Board, and biomonitoring of military personnel who have served more recently in Iraq.

These biomonitoring programmes have now been sucessfully completed. The results were covered by the media.
