Entero-endocrinology including the effects of bariatric surgery

Abstract

The gut releases signals that coordinate how it handles food, how nutrients are distributed in the body and how much food is consumed. Modifying these signals presents an opportunity to treat obesity and high blood sugar and it is thought that changes in these signals are important in the positive effects of bariatric surgery. We use mice that have been genetically altered to either label the cells that produce these signals or the cells that receive the signals, enabling us to identify and characterise changes when these mice undergo bariatric surgery. We hope that understanding these changes will, in the long term, enable us to modify gut-derived signals to treat metabolic diseases in humans without the need of surgical intervention.

Technical Summary

The enteroendocrine system is a key player in the control of postprandial nutrient homeostasis, modulating the transit and digestion of nutrients within the intestine, the disposal of nutrients in peripheral organs and affecting future feeding behaviour/appetite. Some peptide hormones, notably glucagon-like peptide-1, are already exploited to treat diabetes and obesity, whilst changes in enteroendcrine responses are thought to contribute to the beneficial metabolic outcomes of bariatric surgery. This programme uses genetically modified mouse models to address the mechanisms underlying enteroendocrine changes in response to bariatric surgery with the aim to modulate the enteroendocrine system for the treatment of metabolic disease. We compare in vivo results in mice after vertical sleeve gastrectomy with observations in human volunteers, which have undergone gastrectomy with Roux-en-Y type reconstruction of the upper small intestine and in vitro studies with primary mixed epithelial cultures and intestinal organoids of both human and murine origin. Transcriptomic, mass-spec-based peptidomic, optical single cell second messenger dynamic and electrophysiological investigations are combined with pharmacological and/or genetic manipulations to identify and characterise signalling mechanisms underlying enteroendocrine hormone release and action.

Title Alteration of the plasma peptidome of gastrectomised patients during an OGTT 
Description Performing gastric bypass surgery on patients severely changes their gut hormone response to glucose. The plasma concentrations of Insulin and a number of associated incretin peptides are significantly raised shortly after ingestion of glucose. Typically the plasma concentrations of these peptides are monitored using immunoassays. A simple and high throughput plasma peptidomics approach was used to simulatneously monitor a large number of these peptides. Comparison of gastresctomised and control individuals showed significant increases of insulin, glucagon gene products, PYY and neurotensin.

Title Expected values for gastrointestinal and pancreatic hormone concentrations in healthy volunteers in the fasting and postprandial state 
Description Background: Gastrointestinal hormones regulate intestinal transit, control digestion, influence appetite and promote satiety. Altered production or action of gut hormones, including glucagon-like peptide-1 (GLP-1), glucose-dependent insulinotropic polypeptide (GIP) and peptide YY (PYY), may contribute to the biological basis of obesity and altered glucose homeostasis. However, challenges in analytical methodology and lack of clarity on expected values for healthy individuals has limited progress in this field. The aim of this study was to describe expected concentrations of gastrointestinal and pancreatic hormones in healthy volunteers following a standardised meal test (SMT) or 75g oral glucose tolerance test (OGTT). Methods: 28 healthy volunteers (12 men, 16 women; mean age 31.3 years; mean body mass index 24.9 kg/m2) were recruited to attend a hospital clinic on two occasions. Volunteers had blood sampling in the fasting state and were given, in randomised order, an oral glucose tolerance test (OGTT) and standardised mixed liquid meal test with venepuncture at timed intervals for 4 hours after ingestion. Analytical methods for gut and pancreatic hormones were assessed and optimised. Concentrations of gut and pancreatic hormones were measured and used to compile ranges of expected values. Results: Ranges of expected values were created for glucose, insulin, glucagon, GLP-1, GIP, PYY and free fatty acids in response to a standardised mixed liquid meal or OGTT. Intact Proinsulin and C-peptide levels were also measured following the OGTT. Conclusions: These ranges of expected values can now be used to compare gut hormone concentrations between healthy individuals and patient groups.

Title Glucose-dependent Insulinotropic polypeptide receptor-expressing cells in the hypothalamus regulate food intake 
Description Ambiguity regarding the role of glucose-dependent insulinotropic polypeptide (GIP) in obesity arises from conflicting reports asserting that both GIP receptor (GIPR) agonism and antagonism are effective strategies for inhibiting weight gain. To enable identification and manipulation of Gipr-expressing (Gipr) cells we created GIPR-Cre knock-in mice. As GIPR-agonists have recently been reported to suppress food intake we aimed to identify central mediators of this effect. Gipr cells were identified in the arcuate, dorsomedial, and paraventricular nuclei of the hypothalamus, as confirmed by RNAscope in mouse and human. Single cell RNAseq identified clusters of hypothalamic Gipr cells exhibiting transcriptomic signatures for mural, glial and neuronal cells, the latter expressing somatostatin, but little proopiomelanocortin or agouti-related peptide. Activation of Gq-DREADDs in hypothalamic Gipr cells suppressed food intake in vivo, which was not obviously additive with concomitant GLP1R activation. These data identify hypothalamic GIPR as a target for the regulation of energy balance.

Title Peptidomic analysis of pancreatic neurodendocrine tumours (Glucagonoma) 
Description A peptidomic analysis of plasma from patients with well characterised pancreatic neuroendocrine tumours (PNET) was performed. A nano LC-MS analysis identified a number of peptides from the glucagon gene, which were idenfiried in a previous case study using multiple immunoassays. Peptides to other proteins involved in peptide secretion were also identified. Plasma from a second glucagonoma patient was analysed using a high flow approach, and this identified similar peptides to the nano analysis. In order to demonstrate the potential clinical application of LC-MS to characterising neuroendocrine tumours, a large cohort of plasma samples were analysed to demonstrate the ability of this approach to identify glucagonoma patients, and differentiate from a single insulinoma patient

Its aim is to give members of both Houses of Parliament an insight into the outstanding research work being undertaken in UK universities by early-career researchers. Prizes are awarded for the posters presented in each discipline which best communicated high level science, engineering or mathematics to a lay audience. The audience was made up of MPs, senior scientists, industry representatives and early career researchers.

Results and Impact VBL presented a poster at a local Cambridge Neuroscience Association meeting at Robinson College on March 14th, 2019. It was called "Cambridge Neuroscience Seminar 2019: Signalling, Sensation & Sentience" and the poster title was "ATP is released from GLP-1 secreting enteroendocrine cells and signals to vagal afferent neruones". It was a valuable experience sharing our work on gut-brain signalling to the larger Cambridge neuroscience community.
