Epigenetic Programming of Metabolic Health Across the Life-course

Abstract

We aim to understand why babies who are exposed to a poor diet in the womb are more likely to develop type 2 diabetes as adults. Diabetes is a major health problem in the UK and western countries. Over 2 million people in the UK alone are affected by this disease.
We know that babies who were not well nourished during development are more likely to develop type 2 diabetes and associated diseases (heart disease, kidney failure). These conditions have a major impact on the quality of life of the individual and how long they live. Therefore, it is very important to understand how early diet has such a profound effect on health and lifespan as this may help us develop new medicines for these common diseases.
We know that one way in which a poor diet during early life impacts on health is through “epigenetic” processes. This is the way the environment controls which parts of DNA are used to make proteins and therefore how well a cell works. Using a rat model, we have already looked at a single important gene for diabetes and shown that early nutrition has a major impact on how much protein the cell makes from this gene. Recent advances in genome research will now allow us to look at all the genes in our DNA (~22,000). This represents a major advance in this area of science. It will allow us to identify faulty genes that cause diabetes because of poor early diet. By doing these studies we will be able to find individuals at risk of developing type 2 diabetes and develop treatments to prevent them developing disease.

Technical Summary

We aim to define the epigenetic mechanisms mediating the relationship between early suboptimal nutrition and metabolic disease in later life.
It is well established that patterns of early growth are associated with risk of metabolic diseases such as type 2 diabetes and obesity in later life, conditions which are major health care issues of the 21stcentury in both the developed and developing world. Strong evidence, from human and animal studies suggests that the early environment, including early diet, plays an important role in mediating these relationships. However the precise molecular mechanisms by which diet during critical periods of development programmes metabolism and consequently influences long-term health remain poorly defined. Permanent changes in epigenetic regulation of gene expression have emerged as an important contributing mechanism. Our recent studies demonstrated that transcription factors and miRNAs represent key nodes of epigenetic regulation leading to modulation of expression of complex gene networks. Our recent findings also demonstrated (using the Hnf4a gene as a candidate locus) that changes in promoter-enhancer interactions induced by changes in epigenetic modifications represented a novel programming mechanism. Furthermore we demonstrated that the dynamics of the age-associated changes in these modifications was modified by early nutrition. In the current proposal, using adipose tissue from a well-established rat model of developmental programming and state of the art epigenetic technology including promoter-capture Hi-C, we will test our hypothesis that modulation of promoter-enhancer interactions is a common mechanism underlying developmental programming and that non-coding RNAs represent particular “hot spots” within the genome for such regulation. We will define the function of these non-coding RNAs in vitro using a combination of primary cell cultures and adipocyte cell lines and in the longer-term transgenic mouse technology. To confirm that the programmed changes identified in the animal model are conserved between rats and humans, we will quantify the programmed epigenetic changes in adipose tissue biopsies from an extensively characterised cohort of control and low birth weight young men. We will then determine if these programmed changes are also present in peripheral blood mononuclear cells from the same individuals. Any programmed epigenetic changes that are translated to such a clinically accessible tissue could, in the future, be used prognostically, diagnostically and therapeutically to target individuals at risk of metabolic disease resulting from suboptimal early life programming. The findings could therefore help combat the growing burden of complex diseases such as type 2 diabetes.

Results and Impact We used a game based on throwing bean bags (representing glucose molecules) to a display with drawn cells to explain to children how diabetes affects the ability of insulin sensitive cells to uptake glucose. Kids and their parents could figure themselves which of the cells was a representation of a diabetic one and how blood glucose is increased in diabetic patients.

Results and Impact Be a scientist. Visitors put on lab coat, gloves & goggles. They were then instructed to pipette small volumes of coloured water. Pipetting accuracy was determined by the volunteers and commented on individual success. The purpose was to teach children the importance of reproducibility in science and to discuss aspirations to seek a career in science.

Results and Impact Clare College runs education outreach days to inform sixth form students at state schools about what it is like to go to University and the perks of going to Cambridge, etc. During the day the students had a couple of lectures, I delivered one of these lectures on the topic of epigenetics and health and disease. Around 50 students from Coventry listened to the lecture and many asked questions afterwards which showed that they both understood the content of the lecture and were interested in the topic. The students then had to get into groups and discuss how they will research a question on epigenetics (I provided 4 possible questions). They will present a poster or presentation on their research as a group in July 2017.

Results and Impact This Life Lab event was an event for the general public to learn about biology and how science works, a lot of activities were hands-on and gave the visitors (mainly families) a chance to explore the world of science.
We adapted an activity we used for the MRC Science Festival in schools (see previous entry). An animation was designed by a company to visualize the background of our activity (obesity in dogs as an example to explain genetic variation and how to analyse it). Visitors were then able to put on a labcoat, use a pipette and load samples onto a DNA electrophoresis gel themselves.

Results and Impact For the MRC Science Festival a group of researchers from the institute went to two schools (Ely College and North Cambridge academy) to engage with classes of 13 - 14 year olds. We planned and organised a lab activity for the students so that they could load DNA electrophoresis gels themselves and get a feel for basic techniques used in research.

The activity was centred around obesity in dogs as an example to show how genetic variation/mutations can influence behaviour. We showed how these genetic differences can be detected by isolating DNA from the saliva of dogs, treating it with a restriction enzyme and analysing it on an electrophoresis gel. The activity led to an understanding that our DNA builds the instructions for our body to make all essential proteins in our body. It showed that mutations can alter the functions of these proteins and that scientists can identify these mutations.

It was a very interesting experience and the students were very engaged. It was interesting and motivating to see that the students came up with very relevant and good questions. And most of them enjoyed pipetting for the first time.

In addition to this activity two other departments ran sessions with the students as well. The feedback from the teachers was very positive and they were asking whether this can be run again.

Results and Impact The World Health Organization (WHO) recently designated Zika virus disease a global health emergency. Our results demonstrate the need for comprehensive and coordinated public health policy to reduce the fear and anxiety women experience due to the threat of Zika infection, as well as the need to provide women with a full range of options to implement their reproductive choices quickly, effectively, and safely. Our findings allow policy-makers and clinicians to anticipate and prevent similar situations likely to arise in other global regions where Zika may spread and where abortion is unavailable through formal healthcare settings.

These research findings were published in the New England Journal of Medicine and subsequently picked up by national and international media outlets: Television appearances: BBC World News, BBC News, Cambridge TV. Radio appearances: BBC Radio 4, BBC Radio Cambridgeshire, French National Radio, NPR. Print reporting: Reuters, AP, Telegraph, Guardian, Times, Washington Post, New York Times. Other coverage/editorials: New Scientist, Science Daily, The Verge, Dallas Morning News, Time. Policy impacts: ACOG statement, Anis Brazilian legal challenge. The study authors also presented the work at multiple national and international scientific conferences to ensure that the scientific community were as widely informed about the findings as possible. The original NEJM paper is the 31st most highly cited article on social media ever published in the NEJM.

Some were very surprised by the findings and did not seem familiar with our field of study. These individuals might be more interested in our lab's findings, or other findings within our field, in the future.

Results and Impact Volunteered to be part of the science festival. Stalls were set up to enagage children with science and the research taking place at the university. Activivties were adapted depending on the age of the child in order for them to gain as much as possible from experience. Actvities were hands on and interactive. Also chance to promote healthy messages about CVD + diabetes/obesity prevention.
