Abstract
HCV vaccine

Hepatitis C virus (HCV) is passed on through blood-to-blood contact and infects the liver. In most cases, after initial infection, it remains in the blood and the liver unless those who are infected receive treatment to clear the virus. Over a long period, up to 10-20 years after infection, liver disease can develop which may lead to liver failure or cancer. Since infection is not always obvious, people are frequently unaware that they carry the virus. As a result, HCV infection is a global clinical problem; in the UK, there are up to 215,000 infected people but only about half of them have been diagnosed.
Our programme aims to understand how HCV changes in people who are infected. This is important because some people can develop resistance to the drugs used to treat the virus. Currently, there are new but expensive drugs available to cure infection. However, not everyone responds because they become resistant. We are trying to understand what changes occur in the virus to make people resistant and why these changes happen. In addition, we believe that eradicating HCV needs a vaccine, especially for poor countries that cannot afford drugs. Therefore, we are also looking at the antibodies which are naturally produced during infection to make a vaccine that would protect people from the virus.

Technical Summary
HCV will remain a serious healthcare burden for the foreseeable future and despite attempts at intervention, new infections will persist, particularly in the 'people who inject drugs' (PWID) group. Emergence of antiviral resistance that could be transmitted in the community, and the impact of the inherent diversity within and between HCV genotypes on patient treatment are crucial challenges. In addition, virus evolution occurs in a genetically diverse host background, ultimately shaping response to infection. Fundamental questions on the contribution of host factors to pathogenesis and their role in determining the outcome of DAA therapy need to be addressed for those who fail to clear virus. Our core objective is to tackle viral evolution and host response as interconnected issues. We have developed initiatives and expertise which are central to our plans. To translate our research findings for clinical benefit, we will capitalize on HCV Research UK as a unique resource for clinical data and samples and utilize next generation sequencing (NGS) and bioinformatics pipelines to determine the evolution of viral resistance in those who do not respond to therapy. In addition, creation of gene knockouts by CRISPR technology combined with single cell sequencing will be pivotal for understanding the contribution of host factors as drivers of viral diversity, including DAA-resistance. Our future programme thus encompasses clinical to fundamental research, populations to single cells, incorporating in vivo, in vitro and in silico dimensions.
In the same vein, we set out a strategic plan for the rational design of a HCV vaccine from our studies on neutralising antibodies. Virus eradication will not succeed through antiviral therapy alone due to lack of availability and cost, and resistance may be problematic. Our aim is to isolate unique antibodies from niche patient cohorts from HCV Research UK that are otherwise difficult to access. Again, our objectives are rooted in extensive expertise allied to the benefits of core-funded sequencing platforms and cognate bioinformatics expertise. Moreover, the collaborative network established with a range of disciplines will energise the plans and bring unique ideas to confront the obstacles preventing vaccine development.

Following on from the first two episodes in this series, influenza virus in one such infection that can lead to sepsis. The only treatments for sepsis are indirect drugs that target the cause of infection - fine when good drugs are available, but this isn't the case for all pathogens. In this episode Connor and Jack are joined by Dr Kenny Baillie from the University of Edinburgh, who explains how it could be patients' own DNA which holds the key to targeting sepsis directly. |

| Results and Impact | The CVR hosted the 24th Glasgow Virology Workshop (GVW) on Friday VV June 2019 (Sir Charles Wilson Building, University of Glasgow). This annual one-day meeting aims to communicate the latest innovative science in the world of virology, and to promote interactions between scientists and a variety of stakeholders throughout the world. This meeting is an informal and welcoming one day event comprising a mix of presentations from both external and internal speakers, along with networking sessions. It is rounded off by an evening ceilidh to provide attendees with a more relaxed opportunity to engage with one another. The annual event attracts a wide age range of attendees (from undergraduate students through to healthcare professionals, members of the biotech industry to retired professors), providing an ideal opportunity to catch up with former colleagues, establish new collaborations, and listen to truly inspirational science. |

Rob's lab combines sequence data and software development to explore virus genomics and coevolution of viruses and their hosts. He applies virus genomics to help and guide treatment of HCV-infected patients; support genomic surveillance and outbreak response for bluetongue and rabies; and explore the diversity of endogenous viral elements in published genome sequence data, just to name a few of his projects. | Results and Impact | Each year the CVR runs a series of 6 individual workshops for Higher Biology school pupils. Around 100 pupils attend each workshop and travel from schools throughout Scotland. The workshops are fully booked each year and run in collaboration with the Glasgow Science Centre and supported by Thermo Fisher. The workshops require the support of around 15-20 volunteers from the CVR and wider MVLS each year. The workshops provide pupils with experience and knowledge they would not normally receive until attending university. |

| Results and Impact | In April, CVR postdoc Connor Bamford travelled to the Eden Project in Cornwall to participate in a cross-disciplinary workshop set up to think about new ways of engaging with people about how viruses move between different species. The day-long event was held at FoAM Kernow - a not-for-profit organisation "inhabiting the spaces between society, research, education and policy" - and was really in aid of a new endeavour led by Dr Ben Longdon (a Wellcome Trust Sir Henry Dale Fellow and Royal Society Research Fellow) who has begun to establish his lab at the University of Exeter Penryn campus on the peninsula. The whole idea of the workshop was to guide the development of a game (entitled: Viruscraft - explore its new website here) based on 'tangible interfaces' - linking the digital world with real-life movements - that would help players explore the concept of cross-species transmission of viruses. The entire project is supported by the Wellcome Trust via Ben's fellowship grant. Connor found this to be a positive development opportunity and is currently leading a project to apply for an intern/exhibition slot at the Eden project during the summer of 2018. | Results and Impact | Carol Leitch, at the request of Health Protection Scotland carried out the phylogenetic analysis of the 2 patient samples suspected of being infected by a healthcare worker in Lanarkshire. She was able to show that the HCV infecting these individuals was closely related and this was backed up by individuals from PHE who performed another analysis. This led to the realisation that these patients had likely been infected during a procedure carried out by the healthcare worker. This information formed the basis of a report submitted by NHS Lanarkshire to the UK Advisory Panel for Healthcare Workers Infected with Blood Borne Viruses and the national body endorsed the health board's proposal to carry out a patient notification exercise. 8383 patients treated by this healthcare worked will now be contacted and offered screening for HCV. As a result, the BBC featured a news story about the security alert- http://www.bbc.co.uk/news/uk-scotland-glasgow-west-35639826. This is an example how the work of the CVR has a direct influence on Policy, Practice, along with patients and public interests. |

She then went on to mention the CVR and what she had learned during the visit at a parliamentary meeting in Westminster later that week. Furthermore, she then went on to visit one of the other Biomedical research centres that forms part of the University of Glasgow to gain a further insight into some of the important work and research being carried out across the University. |

This ground breaking initiative has involved the development of a Bio-repository of clinical samples and cohort study of greater than 10,000 patients infected by Hepatitis C Virus (HCV). The database of information gathered about this cohort group is supporting the work of STOP-HCV and is also influencing prescribing decisions taken by the NHS. The information is also being made available for future research into HCV infection, thereby influencing and creating greater funding opportunities for research in this disease area. |

| Results and Impact | In July 2017 a group of CVR researchers attended an exhibition called Hepatitis See at the Glasgow Gallery of Modern Art. This was an invitation from the charity Hepatitis Scotland (who we had previously collaborated with) and an opportunity to engage with people who had previously attended an event for people affected by Hep C in 2016. We were asked to join the event as a result of social media promotion for the CVR colouring book. Our researchers took along pages of the book with craft materials, along with printed copies to share with visitors. This activity allowed our researchers to spend quality time with exhibition visitors and answer questions about their research. |

| Results and Impact | Over the course of two events in June 2013, we interacted with approximately 350 children and their families. We used these interactions to communicate some basic facts about viruses - focussing on their size, structure, routes of transmission and how to limit spread. As part of the activity, we asked participants to consider the question 'If you were a virology researcher what would you study?' and collected responses using post-it note feedback. This provided a good opportunity to discuss the impact of medical research (we often used vaccines as an example), and the importance of the support of the Medical Research Council to help scientists tackle some of the greatest health problems facing humanity. We have been invited to display the timeline at local schools and science centres across Scotland. |

Cabinet Secretary for Health and Sport, Jeane Freeman also attended. From the CVR Alain Kohl, Claire Donald and Ben Brennan displayed at a stand with a poster, film clip of mosquitoes and augmented reality viruses with tablets. Massimo Palmarini and John McLauchlan attended as guests in the room. The CVR also extended the invitation to Vice Principal for Research Prof Miles Padgett, Dean for Research Prof Graeme Milligan and Head of Institute Prof Iain McInnes. In the run up to the event, the CVR followed up with 9 MSPs informing of our presence at the event. We received positive responses from Miles Briggs and Patrick Grady. |

| Results and Impact | In September 2016, CVR postdoc Connor Bamford accepted an invitation to support a placement for an undergraduate microbiology student Josie Bellhouse. Josie came to work at the CVR on a part-time basis and completed a project focusing on public engagement as part of a project. Josie designed her project focussing on a questionnaire designed for CVR and University of Glasgow researchers and undergraduate students to assess attitudes on public engagement and barriers to involvement. Josie also designed a separate questionnaire for members of the public and took this to Explorathon at the Glasgow Science Centre. Josie also used the placement as an opportunity to produce to podcasts for the CVR blog. This was shared on social media via targeted marketing. Josie produced a report of her findings, which have subsequently been taken into consideration for the CVR 5 year strategy and annual communication plan. We also presented a summary of her findings during a presentation at the CVR staff meeting. |

| Results and Impact | In August 2017, Dr Ed Hutchinson provided support to a Glasgow School of Art student completing an MSc in Medical Visualisation and Human Anatomy in a course run jointly with the University of Glasgow. The focus of Naina Nair's project was to take structural representations of influenza and represent in an artistic format as an exhibit for the show. Ed worked with postdoc Connor Bamford to produce a blog post to highlight the project - http://cvr.academicblogs.co.uk/1344-2/, which was viewed 273 times.
