Abstract

The overall aim of this programme is to understand the detailed relationships between physical activity and disease, particularly obesity and metabolic disorders. Further, we are also examining how the environment, early life factors such as birth weight, and genes affect the associations between physical activity with obesity and metabolic disorders. We have lead the development and evaluation of an objective method based on the combined measurement of heart rate and body movement for measuring physical activity precisely in free living individuals. This technique is now implemented in our epidemiological studies and in collaboration with researchers in UK and abroad. We will use this method in future studies. We seek to understand population levels of physical activity and how activity levels might change by time and between sub-groups of the population. We have shown that physical activity is positively associated with metabolic disorders in both children and adults and this association is independent of aerobic fitness. This has important public health implications as it may be more feasible to encourage the population to increase their levels of activity rather than improving aerobic fitness which is likely to include more vigorous exercise. We will seek to further examine the importance of physical activity in the causation of disease and how these associations differ between sub groups of the population. For example, we will include precise measurements of physical activity and body composition from early age in various birth cohorts. Finally, we will examine the heritability of activity in twin studies and the genetic determinants of activity in large scale observational studies.

Technical Summary

Physical activity is assumed to be a key determinant of both obesity and metabolic diseases. However, the detail of this association is not fully clear. A major reason for this is that physical activity is typically poorly measured in epidemiological studies. The overall goal is to understand how physical activity and sedentary behaviour affect the health of people across the lifespan. AIMS The physical activity programme is divided into three different areas of research and the overall aims of the each of the areas are; 1. MEASUREMENT OF PHYSICAL ACTIVITY; Develop and evaluate methods for assessing physical activity in epidemiological studies 2. PHYSICAL ACTIVITY, SEDENTARY BEHAVIOUR AND CAUSATION OF DISEASE; Understand the role of physical activity and sedentary behaviour in the causation of obesity, diabetes and other chronic diseases 3. BIOLOGICAL DETERMINANTS OF PHYSICAL ACTIVITY AND SEDENTARY BEHAVIOUR; Understand the determinants, specifically the biological determinants (genotype and early life factors), of physical activity and sedentary behaviour and how these variables may modify the association between physical activity, obesity and related metabolic disorders MEASUREMENTS OF PHYSICAL ACTIVITY Measurement precision of exposure variables such as physical activity is key in epidemiological research. We are evaluating various objective methods for assessing physical activity including a combined heart rate and movement sensor (the Actiheart) for use in large-scale epidemiological studies. Our results show that this method is more accurate than other comparable objective activity assessment methods in both children and adults for assessing physical activity energy expenditure. However, these studies are restricted to confined settings and we are currently undertaking free living validation work using doubly labelled water (DLW) as the criterion method in UK children and adults. Preliminary data from these studies suggest that the combined heart rate and movement sensing is valid for estimating free living PAEE without any systematic error. It is, however unknown whether the validity of combined heart rate and movement sensing and other movement sensors is influenced by age, culture, climate and other environmental factors. We have therefore initiated studies to examine how different environmental conditions may affect the validity of this method. The next generation of objective measurement instruments will have the possibility to detect different types of activities performed. We have initiated work on developing a high frequency (80 Hz) sampling movement sensor in collaboration with industry for activity type classification. Our work on self-reported physical activity include the development self-report physical activity instruments specifically fit for purpose with the aim to establish a portfolio of questionnaires developed for different age groups, time frames, and assessing different domains of activity. This includes testing the reliability and validity of the European Prospective Investigation into Cancer (EPIC) physical activity questionnaire across 10 countries in Europe. PHYSICAL ACTIVITY IN THE CAUSATION OF DISEASE We aim to examine the complex interrelationships between sedentary behaviour and physical activity with morbidity, mortality and intermediate metabolic traits, throughout life. We aim to explore the direction of associations between physical activity, sedentary behaviour and health outcomes and how physical activity and sedentary behaviour may interact in relation to health. A long term goal is to understand how precisely measured physical activity and sedentary behaviour in childhood may influence on adult health. Impact The work undertaken as part of this Expert Group was leading up to the re-evaluation of the UK Physical Activity guidelines. A paper Dr Tessa Strain had written (at previous institution, University of Edinburgh) had highlighted that the muscle strength and balance guidelines were often forgotten. This group was convened to gather the evidence on a number of issues connected to the guidelines (e.g. relationship to health, current surveillance practices). She was involved in work (including an academic publication) identifying methods that other countries use to measure compliance to these guidelines. This work will feed into the wider guideline review ongoing in 2018.

Title GWAS summary statistics for risk-taking 
Description Risk-taking propensity is a trait of significant public health relevance but few specific genetic factors are known. Here we perform a genome-wide association study of self-reported risk-taking propensity amongst 436,236 white European UK Biobank study participants. We identify genome wide associations at 26 loci (P<5 x10-8), 24 of which are novel, implicating genes enriched in the GABA and GABA receptor pathways. Modelling the relationship between risk-taking propensity and BMI using Mendelian randomisation shows a positive association (0.25 approximate SDs of BMI (SE: 0.06); P =6.7 x10-5). The impact of individual SNPs is heterogeneous, indicating a complex relationship arising from multiple shared pathways. We identify positive genetic correlations between risk-taking and WHR, childhood obesity, ever-smoking, ADHD, bipolar disorder, and schizophrenia, alongside a negative correlation with women's age at first birth. These findings highlight that behavioural pathways involved in risk-taking propensity may play a role in obesity, smoking, and psychiatric disorders

Results and Impact Stand "A healthier you in a digital future"at Cambridge Science festival hands-on event at Cambridge Guildhall, including adults only session on the evening of Saturday 18th. Posters on factors influencing food choice, wearable fitness trackers and their research counterparts, and use of antibodies in our research. Activities included the "Things can only get Bitter" bitter taste test, the Race to 20,000 steps challenge using fitness monitors, and making antibodies out of play-doh and magnets. More than 500 visitors, including more than 300 children, attended our stand over the two days, and participated in activities and talked with Unit staff. The response was very positive. We also gave away over 150 copies of a MRC Cambridge colouring book and flyers for the MRC Festival in June.

Results and Impact Science Festival Stand "It takes a village to build a healthier future" at the 2-day Guildhall hands-on event for families https://www.sciencefestival.cam.ac.uk/events/it-takes-village-build-healthier-future. Stand included information on physical activity quidelines for children and tips on how families can be more active, as well as the Little Snackington and Great Snackington games http://www.mrc-epid.cam.ac.uk/snackington/, and the "Catch your 5-a-day" and hula-hoop challenges.

The stand was visited by about 250 families across the two days. More than 200 copies of the Great Snackington and Little Snackington leaflet were handed out, and 200 copies of the Cambridge MRC Activity book were also taken by visitors to our stand. Saturday was not as busy as Sunday though this allowed stand volunteers to have good conversations with visitors to the stand.

Results and Impact The ISPAH Sedentary Behaviour Council ran a one day workshop in Cambridge on Saturday 13 October, just prior to the ISPAH Congress in London.

During the course of the day we examined established and emerging evidence on the biological mechanisms via which sedentary behaviour may affect disease risk. Understanding biological pathways is critical for causal inference. Better mechanistic understanding may also provide insight into the best way to alter sedentary behaviour (reduce total duration vs 'break up' extended bouts) in order to optimise disease prevention.

We assembled an exciting programme, which included Professor Nick Wareham as our keynote speaker. Our morning session featured talks from distinguished and emerging researchers who shared new findings from mechanistic studies. In the afternoon, we highlighted new methods that can help extend this field of enquiry. Speakers included:

Results and Impact This involved a 45 min talk on the topic of "Evaluating physical activity in epidemiology studies" on the 19th July 2019. There was a mixture of practitioners, academics, and industry workers, working in a wide range of areas, although physical activity/movement features was a common topic of interest for many. TS was able to discuss many of the issues for epidemiological studies, which was a good contrast to many other talks that focused on small scale interventions or measurement issues. TS made contact with Google employees in London on the PPG heart rate monitoring in the google watches. The talk will be repeated to them.

Results and Impact Nutrition and exercise for public health: making sense of the science Nutrition and exercise are paramount for improving health and preventing disease. However, controversy exists over what constitutes a healthy diet. "Chocolate is good, Butter is back " Misinformation in the media is confusing. Alexander Mok, MRC Epidemiology Unit, aims to demystify the research and evidence base underpinning the public health recommendations for a healthy lifestyle. Alexander's research focuses on the combined influence of Nutrition and Physical Activity on health, chronic disease and longevity using large population-based studies. He graduated from the University of Cambridge with a Master's Degree in Epidemiology and subsequently completed interdisciplinary training in Public Health at Harvard University. Prior to starting his PhD studies at the MRC Epidemiology Unit, University of Cambridge; he worked in the Clinical Nutrition Research Centre within the Singapore Institute for Clinical Sciences and was awarded the National Science Scholarship from A*STAR, Singapore to pursue his doctoral studies.

Free talk "Making sense of a healthy lifestyle: how should we eat and exercise?" followed by Q&A. The venue was booked out, with about 230 people attending the talk. The response from the audience was very positive, and about 40 audience members stayed for an extended Q&A session with AM afterwards. Results and Impact Dr Tessa Strain co-chaired two seminars at the Scottish Physical Activity Research Collaboration conference 2018. She did this in her role as leader of the surveillance expert group for the UK physical activity guideline revision. The purpose of the seminars was to inform health professionals about the impact of the proposed changes. It was also to gather their opinions on the impacts they may have. They collected responses and will use them to guide further work in implementing changes to UK physical activity surveillance.

Results and Impact Our group organises and provides the lectures of the Physical Activity Module of this MPhil Course. This module provides the students with elementary knowledge on the measurement of physical activity and fitness in epidemiological studies, as well as the aetiological role of physical fitness and activity in determining a range of health outcomes. Students also collect, process and analyse their own physical activity and fitness data and present their findings during several practical sessions. This ensures their engagement in physical activity and fitness related research, which they can then apply in future careers in epidemiology or public health. Some students stay on as PhD students in our Unit.
