Abstract

We are trying to find out what it is about where people live that influences their health and ability to lead a healthy life. Knowing more about what matters and whether it matters more for some people compared to others, for example those living in deprived areas compared to more affluent neighbourhoods, men or women, adults or children, means that we will have valuable information for policy makers to plan services that can make a difference to people’s health and daily lives. We are particularly interested in how children’s environments might influence how physically active they are, as this is associated with the likelihood of them being overweight and there is little knowledge on how the local environment impacts on this. To research this we are asking over 1500 10 year old children across Scotland to wear a device (Actigraph) to measure their physical activity and another device (Geographical Positioning System (GPS)) to detect the places they are most active over one week. This is a unique study which will shed light on the ways in which children use their local neighbourhood and which aspects e.g. greenspace, impact on their physical activity, thereby providing useful evidence for planning services.

Technical Summary

Aims and objectives

Interest in neighbourhood-level influences on health has burgeoned with research showing that although ‘who you are’ explains much geographical variation in health outcomes, there is also an effect of ‘where you are’. Neighbourhoods are also an important site for health improvement efforts, and population health monitoring. Although policy makers recognise that people’s health, wellbeing and resilience is affected by the quality of their environments, including the availability of green and open spaces, transport, housing, access to good quality food and social isolation, finding levers to bring about health improvement by changing neighbourhoods remains a significant research challenge.
The aim of this programme is to further our understanding of the ways in which aspects of the local social and physical environment interact to influence health and the ability to lead a healthy life.

Research plan and methodology

The programme addresses two main research themes:

1. Living in deprived neighbourhoods is associated with poor health and life chances and there is an expectation that improvement to housing and neighbourhood conditions will improve health, however the evidence base to date is weak. Through face-to-face surveys and drawing on a range of existing data on levels of amenities and facilities in local neighbourhoods, we will investigate how area regeneration and housing investment affect individual and household health and wellbeing and explore the processes that support cohesive, sustainable communities. Although focused on regeneration in Glasgow, the aim is to produce findings that are transferable to other regeneration settings.

2. There is growing interest, and increasing policy attention, in the role of the neighbourhood context in determining the health and life chances of children and adolescents. Most studies to date have been conducted outside the UK, limiting their usefulness for policy development here. We propose to explore neighbourhood and community influences on a range of children and young people’s health and behaviours, with a particular focus on gathering objective measurements of physical activity (using accelerometry) and their location (using Global Positioning Systems) as well as developmental and behavioural outcomes. Moreover, although much attention is given to urban neighbourhoods, there have been calls to develop a better understanding of the differences between urban and rural life today and their importance to health, particularly among children and young people and identify the potential policy levers which could be tailored for different groups and contexts.

Through our work on these two themes we will apply the knowledge gained to develop and evaluate interventions in collaboration with policy, practice and community stakeholders.

Impact Paths for All is a charity which champion everyday walking as the way to a happier, healthier Scotland. Its aim is to significantly increase the number of people who choose to walk in Scotland - whether that's leisure walking or active-choice walking to work, school or shops. It works to develop more opportunities and better environments not just for walking, but also for cycling and other activities. As a board member, with special responsibility for health and health inequalities, it is my role to influence and guide the activities of the charity based on best evidence. PFA is very successful and is used as key delivery mechanism for improving physical activity levels by the Scottish Government. In 2016-17 for example, 4960 new walkers joined one of the local groups we run, 1,261 walk leaders were trained and there were 371,595 health walk attendances. This is a role in which I can directly transfer research evidence to practice.

Dr Olsen was invited to a subsequent workshop where the Scottish Government will seek to apply the data into the policy.

Title Changes in the frequency, experiences and barriers to green space use in the United Kingdom during the COVID-19 pandemic (November 2020 follow-up) 
Description The aim of the survey was to be able to explore how the COVID-19 pandemic and restrictions/'lockdown' were impacting the UK population's visits to green space in November 2020 compared to data collected during the initial 'stay at home' phase of restrictions in April/May 2020 (http://dx.doi.org/10.5525/gla.researchdata.1038). The survey was conducted using an online interview administered to members of the YouGov Plc UK panel of 800,000+ individuals who agreed to take part in surveys. The survey was completed between 25th - 26th November 2020, with a nationally representative sample of 2,246 respondents. In the survey, respondents were asked about their frequency of green space visits, as well as their experiences within green space and barriers to visiting green space during the current stage of the pandemic and restrictions. Questions on garden access, pet ownership, household size, urban/rural status and long-standing illnesses/health conditions were also included. Demographic data was provided by YouGov (including sex, age, geographic region, ethnicity, social grade, marital status, working status, the number of children in household, government region and social media usage). Title Changes in the frequency, experiences and barriers to green space use in the United Kingdom following restrictions on movement during the COVID-19 pandemic 
Description The aim of the survey was to explore how the restrictions on movement were impacting the UK population's visits to green space, and how these behaviours differed between population groups (e.g. by ethnicity, age, and health status). The survey was conducted using an online interview administered to members of the YouGov Plc UK panel of 800,000+ individuals who agreed to take part in surveys. The survey was completed between 30th April to 1st May 2020, with a nationally representative sample of 2,252 respondents. In the survey, respondents were asked about how their frequency of green space visits changed following restrictions on movement, as well as whether experiences and barriers to use changed. Questions on garden access, pet ownership, household size, urban/rural status and long-standing illnesses/health conditions were also included. Impact he model is designed to test and appreciate the difference in infections and casualties with and without social distancing/lockdown measures and, more importantly, to test the effectiveness of infection mitigation strategies such as large scale testing and contact tracing apps. Specifically, the complex interaction between the availability of testing and different levels of app adoption can be usefully explored with this model. The model also shows that, when we assume that the viral transmission runs predominantly through one's social network, the dynamic of the infection is different from that emerging under the assumption of most SEIR models of an equal probability of everyone infecting everyone else.

Description 'Transport, Housing and Well-being' (THAW) is a repeat cross-sectional (1997&2010) random stratified sample of adults in eight local authority areas in the West of Scotland. THAW examines three objectives, firstly, the statistical associations between long term morbidity and mental health and well-being on the one hand, and housing tenure and car ownership on the other (while controlling for socio-demographic and psychological characteristics); secondly, the role of housing quality, residential environment, and use of cars, in influencing illness and psychological health; and thirdly the meaning of housing tenure and car ownership in people's daily lives.

Title West of Scotland Twenty-07 Study: Health in the Community 
Description Extensive health and social survey data of 3 cohorts consisting of 4510 individuals interviewed on 5 occasions over 20 years. although no more survey data are being collected, deaths data are being added on a quarterly basis. Enhancements to the data set are continually made and the data are been shared with several current collaborators. Latest enhancement include data on multi-morbidity which was shared and resulted in the award of PhD for the collaborator. The data have also been combined with data from our latest sub-study: Seniors USP (Understanding Sedentary Patterns), included in a separate entry.

Impact In the last year alone, several publications using these data - both by researchers in our Unit and by external collaborators. Results and Impact On Monday 28th November, I attended Actify CIC's strategy day as one of their non-executive directors. Actify is a vital organisation in the sports and physical activity sector, working with the Scottish Government, Public Sector and Third Sector to provide bespoke training and complementary digital materials to help people become more active, more often.

The day brought together their staff, directors and key partners to shape their future directions as an organisation. I facilitated part of their day which focused on thinking about a whole systems approach to addressing physical activity. The outcome of the strategy day was to improve and change the way Actify think and operates in relation to their work which will have a direct impact on their clients.

Results and Impact This short blog aimed to share our thoughts on how we can assist with Scotland's greener vision of child development, health and wellbeing using Outdoor Early Learning and Play. Our primary audiences were practitioners, policymakers and the third sector. The blog provided an overview of the current context in relation to COVID-19, related research and our novel approach on utilising a whole systems approach to building an evidence base in this area. The blog was posted on Friday 5th June and by Wednesday 10th June it had 220 unique page views. The blog was promoted through Twitter and retweeted by a diverse audience, including researchers, a senior Government Minister (Maree Todd - Minister for Children & Young People), policy, practitioners and the Third Sector. Comments on social media highlighted the novelty and importance of our research in this area.

Results and Impact I created a blog which detailed the background, methodology and findings of the built environment change (BEC) project, while promoting the BEC atlas and interactive mapping tool. The Built Environment Forum (BEF) and the Scottish Policy Research Exchange (SPRE) requested use of the blog on their websites also. The BEF informed us that the blog would feature on their website and in their news bulletin, which goes out to over 600 people working across the built environment sector in Scotland, and that they would promote it via Twitter and LinkedIN. SPRE provides a curated blog of research-based content of interest to the policy community, so allowed publication on their website allowed our blog to reach a wider audience.

Results and Impact As part of the Children Create project, we organised a CPD event - ran by Children's Parliament - to introduce researchers and third sector organisations to children's rights-based participatory approaches in research. Results and Impact Our inaugural steering group meeting brought together representatives from research, policy and practice in the field of greenspace, including colleagues from Public Health Scotland, Greenspace Scotland, Forest Research etc. The meeting aimed to: i) establish our steering group; ii) discuss the wider GroundsWell project and the role of the cross-cutting theme on health inequalities; and iii) provide feedback on a theoretical model exploring factors that drive low and non-use of urban green and blue spaces. The meeting sparked discussion on barriers and facilitators to greenspace for those, in particular, who don't use or have low-use of these spaces. We will continue collaborative discussions in this area to advise on wider applicability and ensure the relevancy of our work.

Results and Impact This blog explored a key question in neighbourhood and health research: if there is a facility or amenity close to someone's home, is it OK to assume they use it? Surprisingly, this assumption is at the heart of a lot of health and environment research. We presented initial findings from the SPACES study that will contribute to future research and publications. The blog enabled us to highlight the work and workstream here at SPHSU around GPS and urban mobility to a wider international audience.

This was through an application to Universities UK. The GreenZone was accessible to the public, third sector organisations, academics, policy professionals and schools. The activity was promoted on twitter, Facebook, the Actify hub (https://www.actify.org.uk/module/887), and will be part of the 2020 Midlothian Science Festival Online Schools Programme (Oct 2020) and the Glasgow Science Festival (Sept 2020). We received feedback during the piloting of the activity that the children felt they gained knowledge of the links between the environment and health.

Results and Impact I was invited to attend and present evidence to the National Strategic Group on Sport and Physical Activity regarding child physical activity assessment. An SPHSU led report indicated that current methods employed within the Scottish Health Survey were significantly overestimating physical activity levels in children and as a result a proposal was tabled, and presented to senior policy members and the CMO, to edit the survey to allow for greater flexibility in the results presented in future.

Results and Impact Laura Macdonald undertook a lecture to post-graduate students on the Masters in Public Health course on 'the environment and inequalities in health and well-being'. The lecture provided students with information on health inequalities more generally and also allowed Laura to showcase our programme's recent research on inequalities in the built environment (i.e. work on walkability, environmental bads etc). The lecture led to questions and discussion amongst the students about research into health inequalities.

Results and Impact As part of the MRC Festival we organised a Question Timeââ'¬â"¢ style debate on how the media represents health issues entitled 'Health in the News: Fact or Fiction'. The panel included academics and journalists from Scotland and England. 93 people registered for the event and 48 attended. Three polls were conducted during the debate, before the debate people were asked: Health in the news: Fact or fiction? - 7% responded fact, 53% fiction and 40% unsure. After the event the same poll was conducted and 21% responded fact, 7% fiction and 72% unsure. The poll showed a clear change in respondents views of media reporting of health news. A specific question was asked: Will this debate change the way you interpret the next health story you read? - 67% responded yes, 13% no and 20% unsure. All panel members reported they would change the way they reported health news and share academic work with journalists.

Results and Impact Working with the SPHSU Engagement and Communications team, we developed a bespoke SPACES webpage and created four infographics. The purpose of these was to showcase the SPACES project to a general audience, present key findings and highlight the impact it has had to date.

Since the new SPACES webpage was launched on 10th February it has had 205 hits, with 164 of those being unique page views. In addition, the infographics have been looked at approximately 35 times.

Results and Impact Working with the SPHSU Engagement and Communications team, we created a short video where Paul McCrorie briefly described SPACES, its impact to date, and the next steps. This video was posted on YouTube, Twitter (cut to 3 separate videos) and added to our Webpage. This video aimed to showcase the SPACES project to a general audience and highlight the impact it has had to date.

Over a few weeks, the YouTube video had 34 views. The reach was more significant on Twitter with a total of 191 engagements (video 1 = 146; video 2 = 27; video 3 = 18). We have also been contacted by two people to understand more about the SPACES project and potentially collaborate.

Those in attendance will be presented with the main updates to the new Chief Medical Officers physical activity guidelines (for children and young people) and will be tasked with discussing how these will be implemented in their sector. We will discuss facilitators, barriers, and support, and these will be collated and presented in a conference report by the organisers (University of Edinburgh).

Results and Impact I was invited to attend a Scottish Sports Future event in Glasgow where I had the opportunity to engage with members of the Scottish Government (Active Scotland division), SportScotland, and a number of third sector groups (F.A.R.E, PEEK, Agile) about the SPACES project, its purpose, and it's potential for assisting with national physical activity agenda. As part of this, I spoke with Aileen Campbell (MSP), the Minster for Public Health and Sport, discussing at length the contribution we could make with our SPACES work. She invited us to set up a meeting with her Active Scotland colleagues to discuss further.

Results and Impact As part of a seminar Dr. Almagor and Dr. Picascia presented an agent-based that simulates children's movements and activities that I am currently developing. The seminar included participants of 3 research centres: CRESH, PAHRC, OPENspace. The seminar took place at Edinburgh University. The presentation included an introduction to the methodology of ABM, and the main ideas behind the model that is currently under development. This engagement may lead to future feedback and corporation with other researchers interested in the model.
