Abstract

Public health decision makers and the public are exposed to a wide range of research evidence, that can often conflict or be uncertain. The way this evidence is presented, in scientific journals, political debates and in the news, influences how people understand, use, and act on it. We shall explore how information about health is communicated, and what people know and think about issues and policies, to better understand how we can communicate research findings into policy debates.
There has also been much debate in recent years about how research evidence can be made more useful and relevant to decision makers. We shall investigate ways in which we can improve the reliability and usefulness of studies that bring together large quantities of complex evidence.
Decision makers can also face difficulties with the quality of evidence available to them. Even large-scale evaluations of public health interventions are often inconclusive or hard to apply to real-life decisions. The elements of a more ‘fit for purpose’ approach to evaluation are well known, but rarely combined into a coherent package. These include stakeholder involvement, careful priority setting, research designs that can withstand external shocks, best use of routinely collected data, and the reporting of findings in ways that policy-makers and practitioners can readily apply to their own decisions. We will combine these elements into robust approaches to evaluating public health policies and programmes

Technical Summary

Aims and objectives
We seek to understand how new policies and interventions are publicly debated and influenced; how research evidence is best synthesised and presented for use in the policy making process; and what approaches work best for generating high quality, relevant, timely evidence. Our aims are to better understand and influence emerging social and public health debates, to improve the way research evidence is reported, synthesised and then used in the policy-making process, and to apply robust approaches to evaluating policy interventions.
We address these questions through three overlapping workstreams focused on: understanding emerging health debates; understanding policy and using evidence; and evaluating health and social policies. Our aim in the next five years is to combine high quality, translational public health science with an in-depth understanding of the political context of the policy process. In the longer term, our vision is to become a leader in the field of improving the timely translation of scientific knowledge into policies and interventions to improve population health and reduce inequalities.

Research plan and methodology

We have identified a series of guiding questions to address our objectives.

To study how health issues, risks and evidence are understood and presented in major emerging public health and policy debates, we will ask questions such as ‘How can we better examine emerging public health debates by combining research on actors, processes and contexts?’

To improve the reporting, synthesis and translation of public health evidence into policy and practice, we will ask questions such as ‘How can we best combine high quality, translational public health science with an in-depth understanding of the democratic and political nature of policy-making?’ and ‘How can we improve the synthesis of the complex, heterogeneous evidence typical of population health intervention research?’

To identify, develop and apply robust approaches to evaluating major policy interventions and strategies, we will ask questions such as ‘What methods work best in the evaluation of complex policy interventions’ and ‘Are there novel approaches to evaluation in disciplines such as political science and economics that can be applied to public health evaluations?

Title Production of pack of cards to explore ideal support design 
Description Visual prompts are often used as part of coproduction approaches and where one of the research questions sought to explore participants ideas on support gaps it made sense to provide a set of visual prompt cards to help break the question down. Rebecca Phipps designed a pack of 30 cards; the content of which was informed by a review of existing support services. Each card featured an image on the front, and a few words on the reverse side. Content is purposely ambiguous in order to allow participants the freedom to assign their own meanings and use them how they wish.

Title Course in Developing and Evaluating Social and Public Health Interventions 
Description A small team from Glasgow University, Danny Wight, Cindy Gray, Andy Baxter and Jo Halliday, delivered a nine day workshop in Moshi, Tanzania, to strengthen East African capacity to develop and evaluate complex interventions. The workshop presented, reviewed and adapted the GU Master's module 'Improving Health and Society' and trained the ten experienced researchers from Malawi, Tanzania, Uganda and Kenya to further deliver it in their own contexts. The module trains researchers and intervention developers in the Six Steps for Quality Intervention Development (6SQuID) model, an evidence-based approach to the process of developing programmes to improve health and society, and the key principles of evaluation. Participants of the workshop are planning to build this into courses for postgraduate students, research staff, and government and NGO researchers and programmers to promote better development, evaluation and scale-up of effective projects and policies. Additionally, this first trial of the workshop will result in reproducible materials to run similar training sessions for other academics to further develop courses in new international contexts.

Population health and policy interventions are legal, fiscal, structural, organisational, environmental, and policy interventions such as the regulation of unhealthy commodities, health service reorganisation, changes in welfare policy and neighbourhood improvement schemes. The checklist items comprise: the materials used; what processes were involved and how the intervention was planned and delivered; who was involved in the provision of the intervention; the location and scope; planned and unplanned variation; how fidelity was measured; and the extent to which the intervention was delivered as intended.

Description Discourse Network Analyzer (DNA) is a software package developed by Philip Leifeld of the School of Social & Political Sciences at the University of Glasgow. We used this software to create data sets that capture the public discourse in relation to fiscal policies intended to improve population health, as they appeared in the UK media, ie: the Soft Drinks Industry Levy and Minimum Unit Pricing for Alcohol. These data sets have subsequently been used to create visualisations of the network of stakeholders and coalitions evident in the debates and to draw comparisons between debates.

Description This database contains research data collected in Emergency Departments in February 2018, October 2018 and February 2019 for the evaluation of MUP in Scotland. Description This is the second database for the evaluation of MUP in Scotland (the other is the Emergency depts interview data). The data are anonymous and come from patietn completed surveys in sexual health clinics. There are three clinics in Scotland and three in England. This supports the natural experiment design. Data cover alcohol consumption, source of alcohol, spend on alcohol, and illegal drug consumption by individually named drugs. Data were collected for 3 week periods in Feb 2018, October 2018 and February 2019.

Collaborator Contribution Professor Stephen Mshana from Bugando Medical Centre participated in several workshops on AMR, hosted a visit by delegates from the University of Glasgow, facilitated the signing of an MoU and contributed to the writing of a grant proposal that is under consideration by MRC. His team will also contribute Klebsiella isolates to the MRC funded SpARK project, funded as JPI-AMR project and led by Prof. Ed Feil at the University of Bath.

Description Sugar-sweetened beverage tax media analysis: Collaboration with University of Liverpool

PI Contribution This collaboration arose due to a shared interest in the media debate surrounding the development of the sugar-sweetened beverage tax policy as part of the UK Government's strategy to deal with the escalating problem of obesity and associated non-communicable diseases. The SPHSU team initiated the concept for the study, led the development of the protocol and methodology, data collection and analysis, visualisation, interpretation and write up of the results.
Collaborator Contribution The University of Liverpool team provided intellectual input to the development of the methodology based on their previous experience of conducting a media content analysis on this subject at an earlier time point. They also provided additional resources for the data analysis phase. Simon Capewell provided particularly insight to the policy debate based on his extensive experience as a professor of Public Health.
Impact Two articles have been written and submitted to PLOS One for peer review, titles: Paper 1: The palatability of sugar-sweetened beverage taxation: A content analysis of newspaper coverage of the UK sugar debate. Paper 2: How proponents and opponents of the Soft Drinks Industry Levy used the news media to influence the policy debate: a critical discourse analysis. Results and Impact Laura McDonald created a blog which detailed the background, methodology and findings of the built environment change (BEC) project, while promoting the BEC atlas and interactive mapping tool. The Built Environment Forum (BEF) and the Scottish Policy Research Exchange (SPRE) requested use of the blog on their websites also. The BEF informed us that the blog would feature on their website and in their news bulletin, which goes out to over 600 people working across the built environment sector in Scotland, and that they would promote it via Twitter and LinkedIN. SPRE provides a curated blog of research-based content of interest to the policy community, so allowed publication on their website allowed our blog to reach a wider audience.

Results and Impact Event title: Changing price, preventing cancer in the UK and US: optimisation of message framing
Dissemination event for a project part funded by CRUK, examining the impact of optimising message framing to advance cancer prevention pricing interventions in the UK and US. The project was conducted in collaboration with the MVLS Dental School and other universities in the UK (University College London) and USA (Florida International University, University of North Carolina - Gillings School of Global Public Health and East Carolina University), all of whom were represented at the event which was held in London on 14th November 2019.
The event was introduced by policy representatives from CRUK and the keynote was given by Professor Shona Hilton. Almost 30 people attended representing organisations with an interest in public health policy, including NGOs, policy makers, advocates and academics.
The event included an interactive discussion with audience members calling on their own experiences of framing cancer prevention pricing interventions.
Key outputs included influencing policy makers and advocates thinking on the framing pricing interventions and learning from their experiences. The Scottish Government Head of Public Health Policy Team, Health Protection Division, has requested that we organise a follow up event with her team.

This mini-symposium was part of a workshop series aimed to develop research collaboration within Glasgow University and beyond, especially on incorporating data science and network analysis methods to understand young people's lived experiences and inequalities in the "digital age". The mini-symposium offered an opportunity for the workshop participants to present their initial ideas for a funding bid/research project on these themes, along with some pilot data and results.

Results and Impact A public debate on the role of the media in understanding health issues. This was a question time style panel discussion with invited guests from the worlds of journalism, academia and public health advocacy. Members of the public were invited to put their questions to the panel and explore a range of opinions on the impact of health issues and related research as reported by the media. Panel members were: Clint Witchalls (Health Editor of The Conversation) Helen Puttick (Health Editor of The Times), Kevin McKenna (former Deputy Editor of the Herald and Executive Editor of the Daily Mail in Scotland), Dr Shona Hilton (Deputy Director of the MRC/CSO Social and Public Health Sciences Unit) and Professor Linda Bauld (Professor of Health Policy University of Stirling and Chair of Behavioural Research for Cancer Prevention, Cancer Research UK). The discussion was moderated by Pennie Taylor, a freelance journalist and broadcaster who specialises in health matters.
We had interest from 108 people, 93 confirmed and 48 people attended (38% general public and 20% health professionals) as well as the panel member and an additional 7 members of unit staff who were involved in organising and running the event.
At the end of the debate 67% of people answered yes to the question "Will this debate change the way you interpret the next health story you read?
The question "Health in the news: Fact or fiction?" was asked before and after the event with the following results:

Results and Impact Presented a paper for the Media and Health Policy panel at the International Public Policy Association's 3rd International Conference on Public Policy held at the Lee Kuan Yew School of Public Policy in Singapore in June 2017. Mass media play an important role in policy processes by setting the public agenda and framing narratives. Evidence-based theory illustrates how media content influences which topics audiences are attentive to, and how they understand the problems, societal groups and potential solutions associated with those topics. Understanding media content is useful to policy advocates as they seek to both understand public and elite perceptions of issues and to influence debates by engaging with media narratives. I gave a talk on natural experiments for an audience of approximately 40 people at the Admiral Bar, Glasgow, on 25 March 2019. My purpose was to introduce a wider audience to the concept of natural experiments and how they can be used to understand the health effects of policy changes. The talk was 10 minutes followed by 20 minutes of lively questions about public health policy. The unveiling marks the start of British Science Week (11th - 20th March).

Results and Impact Co-developed and co-facilitated a session on Health Inequalities at St Denis' Primary School in Dennistoun as part of a collaborative project between the Institute for Health and Wellbeing and Glasgow City Council called "Determined to Animate" which resulted in pupils producing a short film animation on the topic of health inequalities. The films were judged by a range of experts in animation and public health, and St Denis' were awarded the first prize.
