DECIPHer: Centre for the Development and Evaluation of Complex Interventions for Public Health Improvement

Abstract
To develop effective interventions to improve population health requires an understanding of what works, for whom, under what circumstances and why? This necessitates the development of epidemiologically and social scientifically informed complex and multi-factorial interventions that are effective across settings and behaviours; the rigorous evaluation of complex interventions, often using pragmatic controlled trial designs with nested process evaluation and including natural experiments of new policy programmes and the use of routine data to develop and target interventions and to provide sources of data on contexts and long term outcomes for intervention studies. The centre for the Development and Evaluation of Complex Interventions for Public Health Improvement (DECIPHer) brings together a team of world class researchers with expertise in a wide variety of quantitative and qualitative evaluation methods and intervention technologies to address these issues. Building on a strong track record in working with partners from public health policy and practice, and in public involvement in research, we will develop, test, evaluate and then implement interventions that are effective in improving the health of the population across settings and behaviours and which address health inequalities, with a particular focus on children and young people.
DECIPHer's aims are to conduct research that:
1. will lead to measurable improvements in the health of children and young people
2. identifies policies and interventions that are readily adopted, implemented and maintained in the real world and are effective in improving health and reducing health inequalities.
We will also provide high quality training programmes to develop the skills and careers of public health researchers, and a focal point for collaboration with policy and practice.
For its second five-year period of funding, DECIPHer's core Centre resources will be focussed on developing a high quality cadre of early career researchers who, with a multidisciplinary team of senior scientists will take forward four research programmes:
The first will examine multiple risk behaviours in young people and their antecedents to develop complex interventions which address multiple risk behaviours and their causes.
The second will develop and conduct a programme of research on organisational approaches to promoting health in schools and other youth settings.
The third will focus on further development of methods to develop and evaluate complex interventions, with an emphasis on applying randomised trials and other high quality research designs to evaluating interventions in a way that produces rigorous evidence that is useful to public health decision makers.
The fourth programme is also methodological in nature, and is concerned with maximising the use of routine data from population surveys and other administrative sources to improve our understanding of the determinants of population health and facilitate the evaluation of interventions without the need for expensive collection of new data.
These four complementary programmes will provide a platform for multiple applied research projects, funded through additional competitive grant funding, which will be strategically co-ordinated within and across three broad priority areas relating to the health of children and young people: Tobacco, Alcohol, Drugs; Obesity, Physical activity, Diet; Mental health and wellbeing. DECIPHer will address health behaviour in its widest sense, encompassing individual risk and protective behaviour and the contexts and structures that improve, sustain or undermine health and well-being. Adopting a socio ecological approach, the focus will be on multiple behaviours and on the design and evaluation of interventions that take full account of the interdependencies between individual, social, family, community, organisational and policy factors.

Technical Summary
Multiple risk behaviours: Complex analyses of large scale longitudinal data sets including latent class analysis, with further analyses attempting to identify longitudinal relationships between multiple correlated determinants at multiple time points on the one hand, and multiple risk behaviours and their development over time on the other. Systematic reviews of the literature, including new methods which systematically identify and synthesise relevant social theory.
Health promoting settings: Transdisciplinary action research partnerships including: repeated measures data collection to inform needs assessment and strategy development, longitudinal evaluation and a data rich environment for system dynamics methods to be developed and applied to the modelling and adaptation of interventions; engagement of stakeholders to maximise ownership and maintenance of interventions; multilevel programmes, following a standard functional design based on evidence and theory, but optimised for local circumstance and need.
Complex intervention research: Mixed methods randomised trials of complex interventions. Further development will be in the analysis and dissemination of how to design and deliver such studies in the real world, contributing to methodological literature in policy trials and implementation science; developing the concept of realist trials; improving reporting and synthesis of complex trials; modelling of complex systems and relationships - agent-based modelling, social network analysis, and of long term cost-effectiveness.
Data linkage and natural experiments: Exploitation of data resources created through data linkage to facilitate natural experiments of population health interventions. This will include development and refinement of novel research designs, informed by engineering based systems science methods, to use repeated measures data to monitor, evaluate and improve interventions. We will further develop the Welsh Electronic Cohort for Children.

Thus, impacts will accrue for academics with improved research methods for evaluating complex interventions and increases in policy relevant research, for policy makers and practitioners with the development of evidence based cost effective interventions through partnership working and ultimately, for the public, improvements to population health and reductions in health inequalities.
2. How will they benefit.
Impacts for academics: DECIPHer's leadership in developing innovative complex evaluation methods and processes to maximise high quality policy trials, natural experiments and data linkage to provide long term outcomes across multiple domains, will address calls for improvements in the evaluation of complex intervention and policy. DECIPher will continue to have a major impact on policy relevant research by developing knowledge and expertise in how to design and conduct evaluations that understand how interventions are implemented in real life. In this way, evaluations retrain both scientific rigour and high external validity. Involvement of policy and practice partners in all stages of intervention design, evaluation and translation will maximise policy and practice impacts. These issues are germane to many related and important areas of inquiry. For example, educational research in the UK can learn greatly from the schools based work that DECIPHer will conduct.
Impacts for policy makers and practitioners: DECIPHer's focus on multilevel processes and multi-level interventions will address policy/practice shortcomings in tackling priority health behaviours and addressing health inequalities. Important impacts will be the identification of sustainable cost effective interventions addressing multiple risk behaviours by understanding the extent to which risk behaviours occur together, identifying common underlying factors and intervention pathways, and assessing the potential impact of universal and targeted approaches. Such approaches have the potential, not only to improve population health, but also to reduce health inequalities. DECIPHer' s approach facilitates the development and evaluation of process based capacity building interventions to achieve sustainable whole system change. Important intermediate impacts will therefore focus on increases in policy, practice and public engagement in intervention development and implementation

There is also evidence to indicate that intervention in childhood can be very cost effective, yet it is not clear which strategies work best, for whom and under what circumstances. Although, attention has focussed on pre school and parenting interventions, schools and other youth settings have also been identified as important in influencing child behaviours, health and wellbeing. Therefore, DECIPHer's focus on children and young people, on multifactorial settings-based interventions and on interventions that may influence multiple risk behaviours not only remains highly scientifically and policy relevant, but in the long term has the potential to have significant effects on population health and reduce inequalities.

Evaluation of the course showed positive responses, with all aspects of the course receiving an average score of at least 4 out of 5 (with 5 being the most positive score). |

Impact | Evaluation results for the Primary School Free Breakfast Initiative have had a direct impact on the level of funding of the policy in the context of austerity. In the foreword of the evaluation of the pilot PSFBI, the Welsh Government noted that: "The feedback we have received, both through this report and directly from those participating, has helped to identify areas where further consideration is needed. The Finance sub-group has been established for example in response to concerns about the levels of funding. The Minister for Education and Lifelong Learning has stated that 'a sum of £3.5m is available for 2005-06 rising to £8.5m in 2007-08. We will revisit these funding levels once we have considered the evidence from the pilot and the independent evaluation.' " [1] Additionally, Jane Davidson, the then Education Minister, stated of the PSFBI that "We have always said we will revisit the funding levels once we have considered the evidence from the pilots and the independent evaluation"[2]: to date, the funding of the initiative has risen from £3.5m.for 2005/6 to £12.7m. for 2012/13. Evaluation results have been also been used to support the breakfast club initiative in the face of cross-party disagreement. Responding to criticism by Welsh Conservative shadow education minister Paul Davies, an Assembly Government spokeswoman quoted the results of the PSFBI evaluation, pointing out that "overall the results are positive regarding the outcomes in terms of number of healthy food items consumed at breakfast and more positive attitudes towards eating breakfast". Responding to Davies's claim that the PSBI did "not address the root causes of poor diet linked to poverty", the spokeswoman cited the continuing work by Dr Murphy on the PSFBI evaluation, including "a detailed examination of the association between socio-economic position, diet and school performance", highlighting its on going impact and most recent research publication [3]. Consequently, financial and political support for the initiative has been maintained by the subsequent coalition government. Speaking on the BBC, Education Minister Leighton Andrews stated "One Wales [the Labour-Plaid Cymru assembly government coalition deal] commits us to maintaining the programme of free school breakfasts and we look forward to even more schools taking up the scheme." [4]. This is reflected in government and school action - by 2012, 73% of primary schools across Wales had implemented the scheme and on one of the census days that year, 69,200 pupils ate at least one weekly free school breakfast (36.9% of them being eligible for free school meals). 1. Impact | The UK Baby Friendly Initiative is based on a global accreditation programme of UNICEF and the World Health Organization. It is designed to support breastfeeding and parent infant relationships by working with public services to improve standards of care. In 2012 we revised our standards; for more on the history of Baby Friendly in the UK, please see the dedicated section on our website. Why are we needed? Breastfeeding reduces the risk of babies becoming ill. It is also associated with a reduced risk of later childhood disease and protects the mother's health. It has been suggested that the lower incidence of illness associated with higher breastfeeding rates could also lead to significant cost savings in the treatment of illnesses such as gastro-enteritis. While UK breastfeeding rates are increasing, they are still among the lowest in Europe. At birth, only 81 per cent of British babies are breastfed. This figure falls to 69 per cent at one week. Just one in three babies are still receiving breastmilk at six months, despite recommendations that babies need nothing other than breastmilk for the first six months of life. Best practice The UNICEF UK Baby Friendly Initiative provides a framework for the implementation of best practice by NHS trusts, other health care facilities and higher education institutions, with the aim of ensuring that all parents make informed decisions about feeding their babies and are supported in their chosen feeding method. Facilities and institutions that meet the required standards can be assessed and accredited as Baby Friendly. Implementing Baby Friendly standards is a proven way of increasing breastfeeding rates. It also means health professionals can give mothers the support, information and encouragement they need. |

Impact | In 2015 and in 2016 our research findings from the Department of Health Policy Research Programme (Building Blocks trial of the Family Nurse Partnership, FNP) formed the basis of two consultations (meetings) with key policy, practice and academic stakeholders. These included policy leads from Department of Health (England), Public Health England, Scotland and Northern Ireland, the leadership team from the FNP National Unit, local service commissioners, practitioners and academic researchers (from UK, US, Europe, Australia). Our results have led to further service developments in the evaluated scheme to address deficits in programme derived benefit. Importantly the evaluated showed an overall lack of benefit. The impact of the research on modifying practice and policy is expected to be gradual and at this stage (four months post publication) is still emerging. Impact | This research safety protocol was successful in managing the potential risks and avoiding harm to the school-based adolescent research participants. This work is designed to help address some of the current research barriers to completing adolescent self-harm prevention intervention research in schools. It also contributes to the emerging knowledge-base and discussion highlighting the complex ethical issues that can arise when completing adolescent self-harm research with young people. The research safety protocol was presented at the International Society for the Study of Self-Injury 2020 conference. It will also be presented at the 18th European Symposium on Suicide and Suicidal Behaviour in Rome (this symposium was to take place in September 2020 but has been delayed due to COVID-19). |

For the first time in 2019, the previously separate 'Developing' and 'Evaluating' courses were combined into one 5-day course. 38 participants attended the course in Cardiff in June 2019, with participants representing the following areas of research and practice: - Public Health - Health Sciences - Community Health Services - Occupational Therapy - Physiotherapy - Psychiatry - Palliative Care - Renal Medicine - Pharmacy - Midwifery - Dental Hygiene and Therapy - Speech and Language Therapy. Participants came from the following countries: - Wales - England - Scotland - Northern Ireland - Austria - Denmark - Italy - Australia - Peru - South Africa. Evaluation of the course showed positive responses, with all aspects of the course receiving an average score of at least 4 out of 5 (with 5 being the most positive score). |

Description | Nurseries in Bristol were invited by email to take part in the test-retest mediator (parental and nursery staff knowledge, motivation and self-efficacy) questionnaire study and an invitation to take part was posted on the website www.netmums.com. Inclusion criteria were self-identifying nursery staff and parents or carers who worked with or had children aged 2-4, respectively. Participating nursery managers recruited nursery staff and parents via email, and the respective questionnaires were sent out as a weblink and completed twice (a week apart). A small number of parents were recruited online via www.netmums.com. The acceptability of the questionnaires was explored by looking at response rates and missing data. Cronbach's a coefficients were used to look at the internal consistency of the four scales (nutrition self-efficacy, PA self-efficacy, nutrition motivation, PA motivation). Test-retest analyses (weighted kappa coefficients and paired t-tests) were used to determine whether the items/scales are reliable. The analyses demonstrated that the scales show good internal consistency and test-retest reliability. |

The three stages of the framework are: 1) Evidence review and stakeholder consultation; 2) Co-production; 3) Prototyping. The framework extends current guidance on intervention development by providing step-by-step instructions for co-producing and prototyping an intervention's content and delivery processes prior to piloting and formal evaluation. This framework addresses a gap in existing guidance for developing public health interventions with regards the early stages of intervention development that have previously received little attention. The framework could be transferred to co-produce and prototype other public health interventions. The framework was developed during the ASSIST+Frank study which led to the development of two new secondary school-based drug prevention interventions. |

Impact | Since publication in December 2017 this paper has been cited 27 times by a broad range of population health researchers, demonstrating its applicability beyond school drug prevention. The methodological impact of this paper is evidenced by its inclusion within a guidance document produced by the Canadian Institutes of Health Research and National Institute for Health Research on taking account of context in intervention development. The framework formed the basis for an invited keynote talk by the project Principal Investigator (Dr James White) at the 2018 European Society for Prevention Research conference as well as an invited guest lecture by the lead author (Dr Jemma Hawkins) at Karolinska Institute (March 2019) and an invited keynote talk at the 2019 Nordic Advances in Health Care Sciences Conference. The published paper is a key reference within DECIPHer's short course on developing and evaluating complex interventions in which delegates are taught about the development of the framework and how to use it. In 2019 the course will be delivered across several countries internationally including Ireland, Sweden, Jordan, and South Africa. We have been informed by researchers at Aarhus University that the framework is being utilised in their research and teaching, and the lead author has been invited to present the framework within the University's doctoral training for intervention development. |

Description | The realist diagnostics is adapted from the realist synthesis method where an intervention is evaluated on the basis of its success in enabling positive change for the population. As part of this the evaluation identifies CMO (Context - Mechanism - Outcome) configurations, and which mechanisms have been targeted by the intervention with the aim of triggering change that will lead to positive outputs. The realist diagnostic amends this traditional approach by mapping the mechanisms that exist in the natural system, the contextual factors that have the potential to trigger these mechanisms and the outcomes that result. Once mapped there is the potential to identify mechanisms that could be the target of future interventions. This was developed for a study of young carers mental health. The diagnostic identified and screened past young carers studies, before analysing the resulting 45 papers to identify configurations that exist. |

Title | Remote digital food photography in nurseries |
Description | We developed a protocol for collecting food/drink from children aged 2-4 in nurseries using remote digital food photography to give an estimation of the type and quantity of food served and consumed. We tested the feasibility of the method in four nurseries with 51 consented children to determine the feasibility of data collection before the child had eaten anything and after to capture leftovers, with additional photographs if additional food was served. A standard credit-card sized marker was included in all photos to estimate portion size. Different foods were separated to aid portion size assessment. A reference meal of composite dishes was photographed, and ingredients of the meal weighed on scales and recipes documented. The type and number of drinks was recorded and the volume of a standard cup was measured. Nursery staff were asked to complete a questionnaire about their views of the acceptability of the data collection. The photographs were submitted for analysis to an organisation specialised in estimating food portion and nutrient data using food photography. Trained staff used the photographs and annotations to match foods to a UK food composition database. Portion size was estimated by comparing the photograph to standard photographs where portion size (grams) was known. Questionnaire data from nursery staff were summarised using descriptive statistics. It is feasible and acceptable to children, parents and nursery staff to collect diet data in a nursery setting by taking photographs. |

Anonymised survey data were deposited within the Secure Anonymised Information Linkage (SAIL) Databank and linked with a series of routine medical datasets (the primary care dataset (GPD), hospital inpatient dataset (PEDW), emergency department dataset (EDDS), and hospital outpatient dataset (OPD)) to obtain mental health related read codes. Patient records were accessed between 1st Jan 2009 and 31st August 2017, providing full coverage of all datasets. |

Title | The Building Blocks family cohort |
Description | A pseudonymised research database is being established by the research team and hosted at SAIL, Swansea University. This currently includes the original Building Blocks trial data set (baseline and outcomes to age two years) and now further follow-on data (from age 2 years to six years old) for children in the trial and their mothers. Data will be drawn from NHS Digital and Department for Education's National Pupil Database following section 251 approval and a dissent model of access. Once fully established and validated, the database could in principle be expanded to extend breadth and duration of follow-up and to provide an accessible well-chacterised dataset of families for other researchers to apply to undertake further analysis upon. Update: 2020 The BB study cohort is now established in the SAIL databank. In addition to being the data required for the main BB2 study analysis and report to NIHR, it is now to be used for two further studies (funded by ERC and by Nuffield Trust). We are completing the set-up process for this (the principal component being amending our data sharing agreement for data provided by NHS Digital). |

Prior research has demonstrated that the topic of adolescent self-harm is excluded in many schools from the whole-school curriculum which negatively impacts the delivery of any whole-school self-harm prevention activities and support. The stigma model generated in this small scale project may be one explanation for this exclusion. |
