Abstract
Far too many newborns continue to die in early life, particularly in low-and middle income countries (LMIC). Many die from infections, some of which could be prevented. During the late stages of pregnancy, antibody is transferred across the placenta from mother to child, providing protection to the infant in the early weeks of life. However, if the mother has insufficient protection against certain infections, the infant will also not be protected.
Vaccinating women in pregnancy can enhance the level of antibody in the mother, thereby protecting the mother, and increasing the amount of antibody transferred across the placenta - protecting the infant from birth too. This concept has already been successfully established for the prevention of tetanus, influenza and pertussis (whooping cough). Many investigators and stakeholders are interested in using this strategy to protect infants from other serious infections, for example group B streptococcus or respiratory syncytial virus (RSV), and vaccines against these infections are in development. Some fundamental gaps in our understanding remain about the immune response to vaccines in pregnancy and in newborn infants which are essential for us to address in order to accelerate development of new vaccines. It is also very important that the scientific community tackles how best to implement such vaccine programmes in both high-income countries and LMIC.
Our unique inter-disciplinary network represents stakeholders from basic science, immunology, vaccinology, social sciences, industry, public health and national and international policy makers who have come together in order to tackle the key challenges in the best use of vaccines in pregnancy and in newborns, and in the long term, improve maternal and newborn health.
Specifically, the IMPRINT network will implement collaborative research projects to answer fundamental biological questions about antibody transfer of antibody across the placenta, how to induce the "best" antibody, and its effect on the maturing newborn immune system. We will investigate if maternal vaccines interfere with the way in which the baby responds to vaccines later in infancy and childhood, to ensure that protection is not compromised at any stage. Co-factors such as nutrition or other infections might also change the way in which mothers and infants are protected by vaccines. Hence we need to take them into consideration if we are to ensure optimal protection of vaccines in pregnancy and early life. We will also find out why women decide to receive or not to accept vaccines in different countries in order to optimise the uptake of current and new vaccines. We will endeavour to work out how vaccine safety and effectiveness can be best monitored, given the different health care systems in different countries. Addressing these implementation challenges will enable us to better understand the issues and site-preparedness in the many geographical areas and populations in which IMPRINT investigators work.

Technical Summary
To improve maternal and neonatal health remains a focus of international investment in global health, given that Millennium Goals 4 and 5 were not achieved. 40% of all mortality in children under the age of 5 now falls into the neonatal period, with significant impact on economic development and welfare of the populations affected, primarily in LMIC. Infections are a major contributor to this burden.
There is increasing momentum to develop and implement maternal vaccines to prevent infections, in particular in LMIC, where the bulk of neonatal deaths continue to occur. In addition to its impact on the neonate, maternal vaccination offers the potential to improve maternal health and reduce stillbirths and prematurity, significant and under-appreciated problems in LMIC.

Planned Impact
The vision of IMPRINT is the widespread implementation and uptake of safe and effective pregnancy and neonatal vaccination programmes that lead to improved long-term neonatal and maternal health, in all relevant settings.
The potential academic benefits of IMPRINT are in the advancement of our understanding of the critical aspects of immunity in pregnancy and early infancy. Our focus is on the mechanisms of protection of maternal immunization and its impact on neonatal immune ontogeny, however our work is also of relevance to other maternal and neonatal infections, for which vaccines have not yet been developed. Achievement of the IMPRINT objectives concerning acceptability, safety and effectiveness of vaccination will facilitate our understanding of how to successfully implement vaccination programmes in all settings. Maintaining the trust of both vaccinators and the general public in a range of settings requires that clear and appropriate messages are conveyed to them, including that vaccine safety is being assessed robustly and proactively. There is clear academic benefit from this work to infant and childhood vaccination in general and also to other health interventions that may be required in pregnancy. These academic benefits extend to the ongoing work of all members of the IMPRINT network, some of which currently collaborate, but many of which currently do not. This is particularly so for members in LMICs and we aim within our pilot projects and fellowships to identify, support and train candidates who will then continue to develop and work in this field and bring added future value.

Results and Impact | The MatImms project consists of a multi-disciplinary research team of midwives, doctors and scientists that investigates vaccination in pregnancy. The project is led by Prof. Beate Kampmann. MatImms encourages vaccination in pregnancy to protect newborn babies from preventable infections. In the laboratory, MatImms studies the impact of vaccines on immunity in mothers and babies. In order to connect with pregnant women, the MatImms Smartphone App has been developed to improve vaccine information available, enabling pregnant women and their support networks to make informed choices. Results and Impact | We held our first full network meeting in Sept 2018 in Kingston upon Thames with a total of 87 participants. At that time, the network was running for 14 months and had a total of 200 members. Travel funds for young scientists, postdocs as well as grant holders were provided. 30% of the participants came from LMIC, 70% from HIC and from a total of 22 different countries. During the meeting, we not only had scientific talks by our pump priming and fellowship holders on their IMPRINT sponsored projects, but also from our GCRF sister network PIs and stakeholders from policy (Ghana Standards Authority and WHO), as well as the Group Strep B support group.. We also had an industry panel discussion with representatives from Pfizer, GSK and Sanofi Pasteur. We offered workshops on the topic of Patient and Public Involvement and grant writing as media training. A speed dating session for networking fostered the interaction between the participants. Additionally, we supported the career of our IMPRINT sponsored fellows by recording video interviews about their research and career which we will display at the IMPRINT website and provide to the fellows. We also recorded videos and interviews with our network members on our passion for research, our network of members, our innovational approach and impact. All videos can be seen on our website. |

Results and Impact | On 17 September 2019, IMPRINT held a session and network meeting in conjunction with the 5th International Maternal and Neonatal Immunisation symposium (INMIS) in Vancouver, Canada. INMIS takes place bi-annually and is the world leading conference in the field of maternal and neonatal immunisation and as such the most important conference for IMPRINT to reach our main target audience, namely researchers, industry partners, funders and other stakeholders in our field. IMPRINT was one of the sponsors for INMIS contributing travel funds for IMPRINT members and the IMPRINT Steering Group. The session took place directly after INMIS and was advertised to all INMIS attendees as well as the whole IMPRINT network. In total, the session was attended by over 70 people. The session consisted of presentations by all IMPRINT funded Pump Priming projects and IMPRINT Fellows, as well a select presentations by one of our newly funded Public Engagement projects. In addition to the research and engagement projects, The Vaccine Confidence Project presented on its measures to monitore vaccine confidence across media & social media, utilizing machine learning, digital analytics and confidence mapping. The project is co-funded by IMPRINT. Impact: The session offered excellent networking opportunity for all IMPRINT awardees to form future research collaborations. Both industry representatives (e. g from GSK) and representatives from the Bill & Melinda Gates Foundation attended the meeting fostering dialogue about potential collaborations on network level as well as sustainability for the IMPRINT network. In addition, the session led to a spike in membership applications. Due to our presence at INMIS, approx. 20 new members joined the network. |

The meeting focused on the achievements of the IMPRINT fellows, pump-priming and public engagement projects. Each fellow and project lead presented the final results of their project. The meeting also included presentations by other IMPRINT members on different perspectives of COVID-19 and pregnancy. Interactive sessions and panel discussions on the successes of IMPRINT as well as the network's role in the global developments of maternal immunisation offered insights into the main themes of IMPRINT and the network's contribution to the wider field of maternal and neonatal immunisation. |

Results and Impact | IMPRINT hosted a series of fours webinars on topics around maternal and neonatal vaccinations in 2021, many of them specifically responding to the COVID-19 pandemic. The webinars were open to the public, recorded and made accessible to the IMPRINT membership afterwards. (1) In the first webinar, Prof Asma Khalil, Obstetrics and Maternal-Fetal Medicine at St George's University Hospital (University of London), presented on the "Impact of COVID-19 pandemic on pregnancy" and IMPRINT Steering Group member Dr Beth Holder, Imperial College London presented on "Mother to fetal transmission of SARS-CoV-2 across the Placenta." (2) On 11 March 2021, Dr Chris Gale from Imperial College London provided new insights on the impact of COVID-19 on the health of newborns in the IMPRINT webinar series 'Maternal and infant immunisation in times of COVID-19.' In his presentation, Dr Gale described the incidence, characteristics, transmission, and outcomes of SARS-CoV-2 infection in neonates who received inpatient hospital care in the UK from a prospective population-based cohort study. Neonatal SARS-CoV-2 infection is uncommon in babies admitted to hospital. Infection with neonatal admission following birth to a mother with perinatal SARS-CoV-2 infection was unlikely, and possible vertical transmission rare, supporting international guidance to avoid separation of mother and baby. (3) On 19 April 2021, Prof. Ruth Karron from Johns Hopkins Bloomberg School of Public Health and Dr Judith Absalon from Pfizer, Inc. discussed the COVID-19 vaccine trials in pregnancy and the ethical framework around these studies in our webinar series "Maternal and infant immunisation in times of COVID-19." (4) In the fourth session of our webinar series 'Maternal and infant immunisation in times of COVID-19' on 17 May, Dr Natalie Shenker from Imperial College London spoke about challenges and opportunities in COVID-19 vaccination during lactation. Her presentation was complemented by a personal report from Dr Gemma Partridge, a consultant obstetrician at the Norfolk & Norwich University Hospitals NHS Trust, on her own experiences of receiving the vaccine whilst breastfeeding and her advocacy work to ensure access to the vaccine for women who are breastfeeding and women who are pregnant. |

Results and Impact | IMPRINT was promoted at the first London Maternity & Midwifery Festival that took place on February 13th 2018 with over 1600 participants. This national professional festival, with leadership speakers and exhibitions, is to be held annually to highlight developments in maternity and midwifery across the South East of the United Kingdom. Our IMPRINT network member Beth Holder and a team from London School of Hygiene and Tropical Medicine attended the event to promote MatImms, a free Smartphone App designed by leading experts of paediatric infectious diseases at London School of Hygiene and Tropical Medicine that aims to improve vaccine information available to pregnant women, enabling them to make informed choices and encourages vaccination in pregnancy to protect newborn babies from preventable infections. MatImms collaborate with pregnant women alongside academic, clinical and policy partners such as Public Health England and antenatal services, in the UK and globally through the Centre for International Child Health. Matimms is led by Professor Beate Kampmann. We aimed to attract midwifes to our network and spark interest of the general public. |

The purpose of the project is to monitor public confidence in immunisation programmes by building an information surveillance system for early detection of public concerns around vaccines. IMPRINT is featured as the Vaccine Confidence Partner on their website. We plan to open a call in the near future about patient and public engagement in vaccination, and the Vaccine Confidence project display gives us more visibility. |

The work is organized around developing a strategy, understanding the pipeline of vaccines in development, articulating the value of vaccines against AMR, developing methodologies to measure the impact of vaccines to reduce AMR, and assuring impact of vaccines at the country, regional and global levels. |
