Abstract

Our mission is to build capacity for public health research, strengthen the evidence for public health policy and practice and to improve the health of the population. Our aims are to ensure that our research is excellent while extending its reach and impact. Therefore our strategy and objectives are:

(1) to expand and develop our staff and equip them with the skills for public health research;
(2) to build on a platform of excellence in key areas {exploiting existing data; social causes of health and disease; and health behaviours}, extending our research infrastructure and strengthening themes that are important to our partners and funders;
(3) to ensure our partners are actively engaged in the research process;
(4) to work more closely with the other UKCRC Centres of Excellence to ensure that the initiative delivers its objectives for national and international leadership in the field;
(5) to incrementally increase our grant income to ensure longer term sustainability.

Our priority will be to use the unique opportunities that exist in Northern Ireland, with its distinctive approach to integrated health and social services and well connected civic and public sector structures, to research the social and biological causes of chronic disease and the main causes of inequalities in health experienced in the UK. We will leave a legacy for future public health research by:

(6) surveying and following up a large sample of school children in Northern Ireland to study the neighbourhood, school, family and individual influences on their behaviour, their health and achievements;
(7) by surveying and following up a large sample of older people to find out what influences their ability to work, when they retire, their independence, their health and their social and economic wellbeing;
(8) working out how best to undertake public health research by using linkages between data already collected by public bodies like the NHS and others; and
(9) by showing the benefits of designing and scientifically evaluating novel public health interventions in partnership those working directly with the public, across different public sectors, and with the public themselves.

In so doing we are creating opportunities for young researchers to practice the skills necessary for high quality public health research and for them to establish a body of credible work that will help them become independent researchers. We will also demonstrate the added value of bringing researchers from many different backgrounds together around a common purpose and provide a bridge between the academic and non-academic communities to ensure that new knowledge is translated into actions that can improve health.

Technical Summary

In so doing we will demonstrate the added value of stronger multi-disciplinary public health research networks and provide a bridge between the academic and non-academic communities to ensure better knowledge integration and translation.

Planned Impact

Promoting trans-disciplinarity and ensuring stakeholder engagement will help us to deliver high impact research, with multiple beneficiaries, including: citizens of GB and Ireland; carers and commissioners; policy makers across GB, Ireland and at EU Level; third sector advocates; health & social care practitioners; participants of our studies and their families.

The Public will benefit through increased knowledge of ongoing research. We aim to increase public engagement through events such as the ESRC Festival of Science, public lectures and our work with STEMNET. The importance of the media in informing public opinion cannot be understated, the Public Attitudes to Science Report 2011, commissioned by Department for Business, Innovation and Skills (BIS), found that only 43% of respondents said they felt informed about science. Thus, we will continue to work through the Royal Statistical Society to help deliver the Science Training for Journalists Programme. This type of engagement can occur throughout the research cycle, however, the public will also benefit, albeit at a later stage, from any research that is successfully translated into policy. Members of the public may also benefit from participation in research studies, receiving advice in areas such as staying active and maintaining physical and mental wellbeing. We will encourage participant representation on research panels and will work with the regional Health and Social Care Patient and Public Involvement Programme to ensure that research is relevant to the public and conducted in a responsive and appropriate manner.

Commissioners and Policy makers will find the research of value in assessing the health and social care needs of the local population and in planning health and social care services to meet current and emerging needs. Our plans to research the use of Evidence Aid resources can have impact on a global scale, while locally the establishment of record linkage infrastructure will facilitate the analysis of a variety of policy interventions. While the timelines for research and policy may not always be aligned, we will maintain our strong relationships with key decision makers to ensure that that have access to updates on new research and emerging results.

The Third Sector will benefit from a more effective knowledge exchange process that allows stakeholders to engage in and influence the research from its inception. For example, the Community Development and Health Network will play an important role in guiding the development of a community based intervention to build resilience and mental wellbeing. Opportunities will exist for Third Sector organisations to build capacity by improving research literacy and understanding of appropriate methodologies. They can also exploit the knowledge gained by our students as part of the NHS Evidence Student Champion Scheme (coordinated at Queen's by the Centre of Excellence) which trains students to disseminate information on the searching and use of evidence.

Health and Social Care Practitioners will benefit from knowledge gained through workshops delivered as part of the implementation of the Public Health Agency's Knowledge Management Strategy. We will promote the role of specific PHA staff as research champions. For example, Dr Lynsey Patterson's contribution to research on AAA screening will create an important bridge between research and practice and help to strengthen research capacity in the PHA. The NI Public Health Research Network will also provide a forum for practitioners to engage in multi-sectoral research collaboration.

Title Inside out: What you don't see - {RMB} 
Description 'Inside Out: What You Don't See' was an exhibition held at the Naughton Gallery, Queen's University Belfast. It consisted of fifteen pieces of art produced by prisoners in HMP Maghaberry who attended the Donard Day Centre (a mental health facility). The artwork explores issues of mental health and how prison impacts on the well-being of prisoners and their families. The project was a collaboration between the Northern Ireland Prison Service (NIPS), the South Eastern Health and Social Care Trust (SEHCT), Prison Arts Foundation and Queen's University Belfast.

Title Breast cancer cohort - {MOR} 
Description This dataset of over 5,000 patients compiles the clinical/pathological tumour characteristics of all breast cancer patients diagnosed in Northern Ireland from 1st Jan 2009 to 31st Dec 2013. In addition a nested-case control study within this cohort (relating to the work of the current CRUK award) has additional information on lifestyle/risk factors and surgery and oncological treatment details for approximatley 1,200 women.

Title Over-indebtedness and health - {DF} 
Description Five Northern Irish credit unions located in Greater Belfast and Newry, Co. Down participated in our study. They identified 1,091 members with either loans in arrears for greater than 9 weeks or loans that had been rescheduled. Only one participant per household was interviewed. The survey was carried out from January to April 2014 by a local market research company until the sampling frame was exhausted resulting in 499 completed surveys. The survey consisted of modules on household demographics, employment, income and benefits, debt, expenditure, savings and assets, financial literacy and financial characteristics as well as a number of health-related questions. The survey was carried out face-to-face and lasted approximately one hour.

PI Contribution This project involved the setting up and completion of a qualitative research project, the purpose of which was to use photo elicitation and focus group methodologies to capture the everyday lived experiences of adolescent mothers from their own perspectives. We recruited 27 young mothers in three locations (Belfast, Bristol and Durham) and conducted focus groups to identify the key barriers to, and facilitators of, positive health behaviours in this population; this produced a body of evidence to use as a framework to develop future interventions, particularly those targeting the health behaviours of young mothers.
Collaborator Contribution The research project was completed at three locations (Belfast, Bristol and Durham) therefore all collaborators have made a similar contribution to the research project.
Impact This research was completed at the end of 2015 and we are currently working to disseminate study results. Three papers have been written to date; these are in preparation/under review. Collaborators are from different disciplines and Institutes: Dr Kimberly Jamie (Durham University) is a Lecturer in Medical Sociology; Dr Lucy Hackshaw-McGeagh (University of Bristol) is a Research Associate in Cancer and a Chartered Health Psychologist; and Dr Roisin O'Neill is a Research Fellow in Nutrition and Cancer Epidemiology.

PI Contribution This is a knowledge translation project, seeking to translate the findings of previous physical activity research, into a web-delivered physical activity tool for patients with pre-diabetes, to increase their level of physical activity. We are reviewing previous pre-diabetes programme, building a web-delivered physical activity tool to meet the specific needs of pre-diabetes patients, and evaluating the implementation of the tool as part of the South Eastern Health and Social Care Trust's pre-diabetes programme. This project is funded by the NI HSC R&D Division (Knowledge Transfer Scheme).
Collaborator Contribution Our partners in the South Eastern Health and Social Care Trust, who have responsibility for the pre-diabetes prevention programme, are providing guidance for the web-tool, and access to patient groups to evaluating the implementation of the tool.
Impact This is a multi-disciplinary collaboration involving the Health Improvement Officers in the South Eastern Health and Social Care Trust, academic GPs and public health scientists. This has led to the development of the Each Step Counts Webtool.

After the sharing of contact details of migrant organisations, contact was made with these organisations and they expressed interest in being involved in further research, which is to be followed up once ethical approval has bee received for qualitative research with migrants on health and well being and their experiences and knowledge of registration with a GP and completing the census.

Results and Impact The talk focused around my research and to raise awareness of the types of mental health research going on within the Centre for Public Health. It spurred a long question and answer session afterwards with an exchange of information regarding local community based reseach also goin on which may relate to my work.

I have been put in contact with two prominant local researchers who are organising another public enagement event and it is expected I will be invited to speak at that. All individuals present and mentioned are also now invited to the Centre of Excellence's away day to allow for on-goin discussions and hopefully future joint projects.

Results and Impact 26 people attended a seminar by Prof Paul Elliot of Imperial College London on "Aircraft noise and cardiovascular disease near Heathrow airport in London: small area study". The audience was compromised of academics and members of the public and the community and voluntary sector. There was good interaction between the speaker and participants in the discussion after the presentation.

Results and Impact Dr Cantwell presented an invited lecture on "The Epidemiological Perspective for Diet and Breast Cancer" to over 300 undergraduate and postgraduate students and staff from a national audience. The aim of the lecture was to engage the audience in critical appraisal of epidemiological studies related to diet and breast cancer and to stimulate thinking in terms of causal relationships. The lecture was followed by a Q and A session which created lots of discussion through audience participation. It also was and opportunity for Dr Cantwell to highlight research on diet and cancer which is on going within the Centre of Excellence.

Dr Cantwell received several enquiries from undergraduate students about the possibility of completing a PhD in the area of diet and cancer within the Centre of Excellence at Queen's University Belfast. Dr Cantwell has been invited to participate in future Alpro events which will increase awareness of the research carried out on the impact of diet and cancer risk and progression.

The sandpit has lead to the development of a collaborative research project between QUB, University of Bristol and Durham University. A £15,000 grant was awarded to fund this project.

This involved lectures, practical sessions and discussion of the application of the taxonomy in designing and evaluating interventions and policies. Results and Impact 30 researchers, practitioners and policy makers attended a seminar given by a researcher, Emily Shortt, from University College Dublin entitled "The influence of health behaviours and psychological well-being on maternal weight during and after pregnancy".

A research collaboration has been developed, not directly with UCD, but with other pregnancy researchers in ROI. Policy makers (Public Health Agency) became aware of survey work in ROI around the acceptability of different policies to tackle obesity (another aspect of Emily's work), which they were previously unaware of.

Results and Impact 25 Practitioners attended a training course on Ankle Injury Management.
This course was led by Dr Chris Bleakley. Part of the course is dedicated to rehabilitation providing a medium to share the development of the Ankle App (KT App) and publicise its existence to an international audience.

A number of practitioners were interested in the App and were directed towards the download site on Google play.
In addition, it also provide a medium to receive feedback from a very diverse and international group of people (from 10 different Nationalities)- this included their opinion on the potential content and its applicability within health care systems outside of the UK and Ireland.

It was followed by a well-supported Q&A session and further discussions on the next stages in the study..

The event enabled us to share early findings not only with our key stakeholders in the Credit Unions but with other regional stakeholders; Community Development Health Network, our community partner in CoE; Public Health Agency; OFMDFM. The event also facilitated networking and information sharing between staff and Board members from all Credit Unions involved in the study. A Credit Union staff member has asked to be involved in product testing.

Results and Impact Members of staff engaged with the public on the topics of diabetes, nutrition , physical activity and macular degeneration. The audience comprised members of the public and included a large number of families, the engagement resulted in valuable discussions around sugar and fat content of foods, portion sizes and the nutritional value of foods. The public were also very interested to learn about research into macular degeneration and to try glasses simulating the condition. Advice was given on risk factors for diabetes and management of the condition.

2015 - Researchers cycled on a multi-person pedal bike and orally presented a lay summary of their work. We also provided promotional materials for members of the public as above

Members of the public learned about the centre and the research conducted. There was a change in attitudes in students and staff towards being more positive about public engagement

Results and Impact Twenty people (a mixture of school teachers, principals and researchers) attended an event targeted at teaching professionals, in both primary and secondary level education, to understand their experience of social science research and understand and tackle participation barriers and survey fatigue in schools. The first half of the event showcased current school-based social research in both Queen's University and the University of Ulster. The second half of the event focused on eliciting the views of schools who have participated in research.

Delegates were asked to complete a detailed feedback from for the event provided by the ESRC. Specific comments we received were:

- Very useful event which will benefit my future research in schools.

Results and Impact The findings of this study indicate that there is a need to further investigate the impact of early life factors, such as family environment, in terms of a 'whole system' of risk factors that could potentially lead to poor mental health outcomes. ~25 people attended the talk and questions were raised concenring the extent of the impact of familial environment on the manifestation of self-harm behaviours.

Results and Impact Two workshops were held to build capacity in Ireland and Northern Ireland. The first was held in Belfast on the 5th and 6th of June and the second in Dublin on the 9th and 10th of June with around 30 participants at each event.

The workshops brought together international speakers, researchers and practitioners from the island of Ireland in the field of tobacco control. The programmes comprised a blend of key note presentations, case studies and discussion on topics such as e-cigarettes, plain packaging and the development of an all-Ireland network for tobacco control. Dr Barrett was a member of the organising team, acted as a facilitator during the events and presented on the work of the NIPHRN and the potential for formation of an all-Ireland group.

Participants showed support for an all Ireland tobacco control network to share knowledge and experience from both Northern Ireland and the Republic of Ireland. A follow up event is being planned to launch the new network

Results and Impact Panel discussion involving the mental health commissioner for the Public Health Agency, the director of offender policy and operations (Northern Ireland Prison Service), a former prisoner who works as an outreach worker for a charitable organisation, and a retired art therapist who previously worked for the Probation Board Northern Ireland. 91 people attended 20 from an academic background, 56 from the public sector, and 15 from the 3rd sector. Question and answer session with panel and audience surrounding the themes of mental health in prison, the benefits of arts based interventions and the need for greater public debate regarding public health in prisons.

Provided platform for networking between university staff and students with public sector and 3rd sector stakeholders.

Results and Impact Grace O'Malley presented the results of her PhD research (Professor Mike Clarke supervised) to an audience of 25. Sixteen of those were from Queen's University Belfast and the University of Ulster, seven came from the public section and two were in attendance from the third sector. breakdown. The audience had the opportunity to discuss the issues arising from the presentation and also had the opportunity to ask Grace questions.

This seminar increased the awareness of the ethical consideration in conducting health-based research with young people.

The seminar consisted of a 45 minute presentation with a 15 minute question and discussion session. Twenty-five people attended. One person attended from the third-sector the rest were from Queen's University and the University of Ulster.

The seminar's main impact was information dissemination. A colleague approached me after the presentation and asked for my slides and said that 'I think that your study would be of interest to members of the public health research network and was wondering if it would be possible to add a link to your presentation in our next ezine and on the website'.

Results and Impact The talk was entitled 'Mortality in patients diagnosed with diabetes during childhood in Northern Ireland, UK'. The role of illegal drug use in deaths of young people with diabetes (previously reported from a study based in Leeds) could not be confirmed in this Northern Ireland study and instead alcohol was reported to have made a significant contribution to deaths in the Northern Ireland cohort.

Clinicians from many countries who care for children and adolescents with type 1 diabetes were in attendance and this talk raised their awareness of the role played by excessive alcohol consumption in deaths among young diabetics.

Results and Impact Dr John Hughes published the report 'CHD Mortality projections to 2020, comparing different policy scenarios' in February 2014 on European Heart Network website with subsequent dissemination of results at European conferences and seminars. The project included reviews on policy recommendations for future cardiovascular prevention strategies by representatives of government Departments, commissioning groups, Heart disease charities, general practitioners and leading Academics in nine EU countries.

Final results were forwarded to senior policy makers in each of the participating countries (including Northern Ireland) to inform future heart disease prevention strategies.

The study has enabled the production of a number of comparative papers (at an advanced stage and to be submitted to high ranking journals in the next few months) which provide interesting insight into how NI policies on cardiovascular disease prevention compare to policies in other countries (i.e at both a British Isles and at a wider EU level).

Results and Impact We had a stand at a careers evening in secondary schools in Northern Ireland to showcase the work of the Centre for Public Helath and to encourage students to think about taking up a career in science allied to some the broad disciplines in the Centre. I spoke a little about my own career to date and the purpose of my current CRUK fellowship funding. Feedback from the event was positive with students thinking more widely about their future subjects and career prospects.

A teacher (head of biology) had asked for representation from our centre at next year's career's event as the work of the Centre seemed to be well recieved amongst parents, staff and pupils.

I spoke to approximately 200 school girls about the role of iodine for health.
This talk resulted in 65 school girls giving their consent and taking part in the study.

Results and Impact This was an invited talk to the Public Health Agency's 'Patient and Public Involvement' representatives forum, to outline the Centre of Excellence approach to PPI and demonstrate using examples of ongoing or forthcoming projects. 7 of the 10 members were in attendance.

The PPI reps gave very positive feedback on our approach to date and noted that the quality of our mechanisms for enagaging the public in research. They gave us some further advice on how we might improve engagement with members of the public via the some of the organisations they are involved with. Finally, some of the PPI reps present indicated a willingness to join study steering committees for upcoming trials.

Results and Impact 40 pupils attended a presentation on tobacco and the dangers associated with smoking. The presentation provided an introduction to a health promotion module which was part of an AS in Health and Social Care. Students used the information to develop a health promotion activity.
