School Health Action Research Partnership and Network (SHARPEN)

Abstract
Secondary schools are important settings for health improvement, providing access to young people during a critical period when health risk behaviours markedly increase. Yet despite sustained effort to promote health through schools, the evidence for school-based interventions that effectively address issues such as obesity, smoking, alcohol use and mental health is limited. Health improvement research in school settings is challenging, with trials currently implemented in an ad hoc and inefficient manner. This is in stark contrast to research in primary care, which was greatly enhanced by the advent of primary care research networks (PCRNs) which facilitated an increase in the quantity and quality of randomised trials, improved research capacity and provided support for practitioner-led research. Such a step change is urgently needed to advance school-based research in the UK.
The project aims to improve the quantity, quality and efficiency of public health research in schools by developing and evaluating a School Health Action Research Partnership and Network (SHARPEN). The research to evaluate and refine SHARPEN has 3 strands. In the first, researchers from Cardiff, Bristol, Oxford and Swansea universities will work in partnership with the Welsh Government, Cancer Research UK, Public Health Wales and Cardiff and Vale University Health Board to establish a network of up to 90 secondary schools that are 'trial ready', by developing more efficient recruitment, consent and data linkage procedures. We will identify the infrastructure and processes necessary to make the network efficient, effective, acceptable and rewarding for both schools and researchers. We will explore the barriers and facilitators to making the network sustainable. Students in network schools will complete the Health Behaviour in School-Aged Children (HBSC) survey, a school environment schedule will be completed for each school, and we will pilot a system that uses these data as a basis for providing regular, tailored feedback to schools on pupil health behaviours and the school environment.
In the second strand we will test the feasibility of establishing school-based action research partnerships to see whether they add significant value to the network model. Five schools will each form an action group of pupils, teachers, parents, health professionals and academics and over the course of a year each group will review their HBSC survey and school environemnt data, identify health priority areas, discuss the links between health and educational outcomes and develop and implement a school health action plan. Action plans will draw on the project partners and local resources and adopt a whole-school approach to health improvement. We will evaluate the action research partnerships to capture how they worked, the factors that hindered and helped them, and whether schools and other stakeholders found them feasible and useful. During this strand, student and parent views on data linkage will also be sought. Issues around informed consent and anonymity will be discussed with students and parents and if possible, data linkage will be piloted.

Technical Summary
Schools are important settings for young people's health improvement, but research in UK secondary schools has become increasingly challenging with health improvement intervention trials undertaken on an ad hoc basis. A network of 'trial ready' schools with research literate staff would support more efficient health improvement research that could better inform public health policy and practice in schools.
With the aim of facilitating a step change in the quantity and quality of school-based health improvement research and informed by the success of UK primary care research networks, this project will develop and evaluate a School Health Action Research Partnership and Network (SHARPEN) in Wales. Planned Impact
The SHRN aims to develop an infrastructure to improve the quantity and quality of school based health improvement trials, increase research capacity and support school based Transdisciplinary Action Research. To achieve this, the SHRN's partnership approach maximises impacts through ongoing, active engagement of policy, practice and public partners. If successful, impacts will accrue for academics, policy makers, school practitioners and, ultimately, the public.
Academics
Academics will benefit from the availability of a pool of research ready network schools, with streamlined consent procedures, systematic context data and data linkage to provide long term health and education outcomes. Involving policy, practice and public partners in intervention design, evaluation and translation will facilitate evaluations that retain scientific rigour and high external validity and maximise policy and practice impacts.
Policy makers
The strategic partnership will benefit policymakers through an improved evidence base for school health improvement, which will address current policy shortcomings in tackling priority health behaviours and addressing health inequalities. The SHRN's approach facilitates the development and evaluation of process based capacity building interventions to achieve sustainable whole system change. Important intermediate impacts will therefore focus on increases in policy, practice and public engagement in intervention development, evaluation and implementation. The network will provide a platform for longer-term impacts, with the identification of sustainable cost effective interventions addressing multiple risk behaviours.
Practitioners
In pilot work, schools consistently highlighted impacts relating to improved pupil wellbeing. Schools will be better informed about pupil health behaviours and their school's health improvement activity, allowing them to formulate and support effective wellbeing plans tailored to their needs and capabilities. They also highlighted the benefits of school level data for school inspections of wellbeing activities.
The network will also contribute to the continuing professional development of school staff in research literacy and evidence based practice and minimise school research burden through avoidance of low quality studies and imefficient research processes.
The strategic partnership with CR-UK also provides schools with curriculum enhancing activities. In turn, the network will provide CR-UK with opportunities to formulate and evaluate school engagement approaches to support cancer prevention activity throughout the UK.
Public
In the short term, pupils will benefit from CRUK resources enriching the STEM curriculum and a RCUK school engagement initiative facilitating extra-curricular activities. SHRN data will also be used in the classroom to promotes engagement in science and with pupil voice groups and parent teacher associations to enhance school engagement.
In the longer term, there has been increased policy recognition of the need for early years' interventions to address determinants of health, social and economic outcomes into later life. Schools have been identified as important in influencing child behaviours, health and wellbeing, so the SHRN's platform for evaluating school-based interventions is highly scientifically and policy relevant, with the potential to significantly affect population health and reduce inequalities.

| Description | Purpose The study of mental wellbeing requires reliable, valid, and practical measurement tools. One of the most widely used measures of mental wellbeing is the Warwick-Edinburgh Mental Wellbeing Scale (WEMWBS). Our aim was to examine the psychometric properties of SWEMWBS (a brief seven-item version) in a 'real-world' population sample of young people. Methods We used data from the 2017 School Health Research Network Student Health and Wellbeing Survey, completed by 103,971 students in years 7 to 11 from 193 secondary schools in Wales. We first estimated polychoric correlation matrices for the whole sample and by school year, and undertook a principal components analysis to check for configural invariance. Subsequently, we used a multiple-groups structural equation model with successively greater constraints to test measurement invariance. To examine external construct validity, we calculated correlations between the SWEMWBS score and four covariates: life satisfaction, somatisation, school pressure and bullying victimisation. Results Parallel analysis suggested that extraction of one factor was appropriate both overall and in each year group. Inspection of standardised loadings suggested that four items had progressively stronger correlations with the factor as students are older, but change in fit indices between models suggested that loadings and thresholds, but not residual variances, were invariant by age group. SWEMWBS scores were moderately correlated with measures of life satisfaction and somatisation, and weakly to moderately correlated with school pressure and bullying victimisation. Conclusions This study adds to the growing evidence that SWEMWBS is appropriate for measuring mental wellbeing in young people and suggests that SWEMWBS is appropriate for tracking the development of wellbeing across adolescence. |

With Public Health Wales we helped develop a framework to help structure the work on Health and Wellbeing and how it could reinforce the whole school approach through its implementation. We helped the lead at Welsh Government understand the Welsh Network of Healthy School Schemes and its National Quality Award to ensure she understood the current context of health and wellbeing in the school setting. In 2017/18 we have met regularly with Lloyd Hopkin (Welsh Government) who is leading development of the the Health and Wellbeing Area of Learning and Experience to offer advice. |

| Results and Impact | Jordan and Lianna met with ALPHA to discuss the proposal of developing a primary school version of the School Health Research Network (SHRN). They introduced the topic by explaining how the research is based around the Wellbeing of the Future Generations Act 2015, the Welsh curriculum reform and the Mind over Matter report. The focus of the research is to expand the SHRN into primary schools to gain a mental health and wellbeing perspective on one of the most important environments in children's lives. There will be a focus to put children on a positive pathway and equip them to deal with a variety of situations in an appropriate way. Long term goals for the project included: - Looking at collected data from the children and how they progress and transition to adolescence. - Impacts of COVID-19 on mental health - Support pandemic recovery - Integrate primary and secondary school work into a single coordinated network. |

| Results and Impact | The overall aim of this meeting, which is to set out the vision for Public Involvement (PI) within DECIPHer 3 and to identify suitable and sustainable approaches for developing our key PI activities. In the outline plans for DECIPHer 3 we stated that the PISG will oversee the strategic development and oversight of PI. Included in this outline plan was three proposed approaches for PI: 1. Continue the development of ALPHA - We will continue to refine our PI approach and practices through research and the collection of monitoring data. This will be set out in the PI Monitoring and Evaluation strategy. It will assess: the number of Research Development Groups considering PI; number of funded bids including PI; feedback from funding boards; type and quality of PI undertaken; PI impacts on research conducted; dissemination and knowledge exchange, impacts and public benefit. 2. Develop a National Student Advisory Group (NSAG) - Develop the scale and scope of PI by establishing an all-Wales Young People's Involvement infrastructure via SHRN. This will build on current ESRC and HCRW studentships examining PI and engagement in primary and secondary schools. Working with schools in north and south Wales we will test the feasibility of key aspects of the infrastructure and PI innovative methods (face to face and online engagement tools) to assess their value in involving school PIs in four key aspects of research projects: 1) research to PI prioritisation; 2) design of research methods, such as selection and formulation of questions within surveys; 3) opportunities for young people to use research findings from DECIPHer research projects to inform school level policies and practice; 4) ways in which young people can be involved in DECIPHer's work to inform national policy. 3. Establish a Parent/Carer involvement group - Establishing this group will complement and build on the successful model developed by the ALPHA group. The group will be key in enabling us to further develop parental involvement in research studies. In particular, the group will play an important role in shaping the design and conduct of studies led by SHRN (e.g. evaluation of school-based interventions with a family component) and the development of the Healthy Social and Community Networks programme. We will involve parents/carers in all stages of the group's development and recruit members from across Wales to help with the formation and workings of the group One of the aims of this PISG meeting was to gather members' feedback on both the NSAG and Parent/Carer involvement group. To do this the members were split equally into three break out rooms. In these breakout rooms we encouraged each member to discuss the NSAG and the potential strengths, weakness, opportunities and threats, or ideas that we may not have thought of. The feedback obtained from each group during this breakout session will help shape the strategy for involving different audiences for the NSAG. |

| Results and Impact | We submitted an application to run a series of ESRC Festival of Science engagement events with the aim of showcasing the involvement techniques associated with the new National Student Advisory Group. This event was also an opportunity to demonstrate different methods for using the bespoke tailored School Health Research Network (SHRN) reports. Our goal for running these engagement events, with the focus on capturing students and teacher's opinions, will allow us to obtain and consolidate feedback to identify the limitations and constraints with each of the three involvement techniques, which will be key to refining the structure of the National Student Advisory Group. We have a unique opportunity to work with a well-established organisations who have direct access to a large number of young people from a variety of socioeconomic, geographic and ethnic backgrounds. This reformatting of ALPHA to a National Student Advisory Group, within SHRN, will allow for us to gather more comprehensive, broader, inclusive perspectives of research from young people. This in turn will make our research more relevant, understandable and acceptable which can lead to enhanced outcomes. We ran a series of engagement workshops with school students designed to encourage discussion on the involvement techniques and provide them with an opportunity to shape and design a service that allows them to provide feedback and opinions on research matters that directly affect them. From these engagement events we gained a more comprehensive understanding, of the need and work, involved for establishing a national student's advisory group, with the aim of increasing on-going engagement in public health research across Wales. We allocated the schools up to 30 places for each session, this meant that the workshops were easily manageable enabling us to collect reliable and meaningful feedback from the students. We asked the schools to provide a variety age range of pupils from different groups such as Welsh Baccalaureate students, school councilors and PSE class members. |

| Results and Impact | We ran a series of ESRC Festival of Science engagement events with the aim of bringing together Health School Practitioners (HSP) and local School Health Research Network (SHRN) school students to share experiences and good practice of using the SHRN data to tackle public health topics in their school, thus demonstrating health promoting school techniques. Showcasing and trialing the Action Plan toolkits with the local SHRN school students will enable them to learn about the key concepts of research, from why it is undertaken to discussing different data collection techniques and a how to run research projects. These tasks will help to improve understanding of the fundamentals of research and encourage and educate local SHRN school students to consult and collaborate on public health topics. It will also be an opportunity for social science researchers to engage with non-academic audiences and to establish working relationships to bridge the gap between theory and practice. Our goal for running these engagement events, with the focus on capturing students and teacher's opinions, will allow us to obtain and consolidate feedback to develop an action plan toolkit which will help young people to tailor and develop their own action plans to tackle public health topics that need addressing, highlighted from the bespoke SHRN reports. We have a unique networking opportunity for practitioners to share their experiences, good practice and exchange information. It will enable practitioners to increase their awareness of news and trends and identify new ways of working. To do this, we will develop a programme to illustrate how HSP and YP can use the SHRN data and research to promote the health of children and young people in schools. The information gathered from this event will equip HSP and YP with toolkits and techniques to aid promotion and development of local healthy school schemes within a national framework. The engagement activity was designed to encourage discussion about student's school research data that is supplied via the SHRN report, which the schools will be receiving. The tasks involved students discussing the SHRN report at length and identifying areas of Public Health that needed addressing and improving. A checklist was used to help identify a public health topic to look at in more detail, encouraging them to start thinking about how they can be involved to address public health concerns in their school or community. |

This UK wide project will build a youth engagement component for the Health Behaviour in School-Aged Children (HSBC) survey. The project has three key aims: • Identify topics and questions young people identify as important. • Empower young people to use data from the survey to influence policy and practice. • Help young people to think critically about research on health behaviours, and how young people can influence decisions. | Results and Impact | Peter Gee (PG) spoke with Alun School, Flintshire & Joan Roberts (JR) in west Wales to trial the web conferencing feature for the new National Student Advisory Group (NSAG). PG spoke Alan school to identifying the feasibility of running online focus groups within schools. The trial was hoped to identify what worked, what needed improving and what the students thought. PG ran a simple focus group exercise designed to identify and gather students understanding of public health topics and what topics need addressing more than other. The aim is that this will help DECIPHer identify Research Priorities (RP) of young people. Therefore DECIPHer can tailor our research to have the greatest potential public health benefit. This interactive model offers us the ability to give, receive and discuss information in real-time. It is extremely useful for conducting ALPHA sessions remotely with the potential to attract students from more than one school. There are a number of possible benefits for conducting research in this way, including increased speed of data collection, lower cost, and improved opportunity for some population groups to participate in research. The first task was to test the feasibility of the web conferencing system and identify any limitations with the service, it was also an opportunity to demonstrate the strengths of the service and showcase the potential impact. We held an online meeting with myself in Cardiff, JR in west Wales and the school council from Alun School in Flintshire. PG ran through some of the functionalities of the service, including PowerPoint presentations, uploading PDF documents, and sharing the split screen. |

| Results and Impact | The new Interactive Video Drama (IVD) for the Jack trial was previewed to the ALPHA members who attended the April residential meeting. It was a chance for the Young People (YP) to see the IVD for the first time and an opportunity to provide their opinions and feedback. Unfortunately only one ALPHA member was able to attend this session. The YP, with Honor Young and Peter Gee, watched the video and presented their feedback to Aine Aventin and Marrow Communications via teleconference. The below information summarises the feedback from both the YP and the Jack trial researchers. Initial feedback Overall it was agreed that the new IVD was considerably a lot better than the previous one. It was clear that the comments made during the residential weekend was taken into consideration and used to update the video. There were some minor concerns around certain aspects of the video but not significant enough to prevent the IVD main aim of preventing teenage pregnancies. Positive: 1. Relationship between Jack and Emma can be seen, it's believable and natural and liked that there was more ethnic diversity. The relationship is no longer perceived as forced this helps to show that Emma and Jack are in a sexual relationship. 2. Locations and filming scenes are more suitable to the target audience. I.e. walking round housing estates. 3. The acting was a better-quality which made the IVD seem more realistic and you could imagine yourself in that situation. 4. The accents of the actors are easier to understand which helps to make the video seem more realistic. 5. Agreed the school scenes were much improved and very realistic. It was more natural with pupils walking passed having their own conversations whilst Jack and Emma were talking about their options, not hiding anything and not being secretive. 6. Technology - Text on screen worked well, it will help to prevent the video from aging. Also the text language was suitable and appropriate. 7. The actors were relevant to the age that they were portraying. Jack, Emma and their friends all looked 16. 8. Lack of music made the video seem more serious and it helped to keep the focus on the drama. 9. Liked the diversity throughout the video, both with Jack and Emma and with Jack and his friends. Minor Concerns: 1. Posters in Jacks bedroom look cheap and printed off the computer (not a major concern as you only see them for a matter of seconds). 2. Some narration was unnecessary and interrupted the flow of the film. Felt odd that Jack was narrating the actions that he was doing. For instance, when he put his arm around Emma he stated that he put his arm around Emma. No need for narration when you have a visual image. 3. During the friend's scene, no one mention about Emma and asked "how does Emma feel?" there was also some negative comments about Emma. Aine said that this might be rectified by adding a question about Emma and her friends at this stage. 4. The ALPHA member also had some concerns with the parents' reactions - whilst they were appropriate and suitable the YP thought the scenes could be improved if it was clearer that each thing a parent said was a different possible reaction rather than one response from a parent that included two different reactions. They thought perhaps more of a gap between the reactions might help. Honor also felt that there appeared to me more 'negative' than 'positive' reactions from parents and wondered if it would help if positive and negative reactions were mixed rather than having negative reactions at start and positive reactions at the end. 5. Change the format of the parents' reactions. They felt quite negative towards the beginning of the scene, which may be misleading to the audience. 6. Agreed the counsellor scene was good but felt some of the 'questions' Emma asked aloud were a bit unnatural i.e. like things she would be saying in her head, rather than out loud. This could be improved if Emma said 'some of the things that have been going around my head/I've been thinking about are. Aine asked if they felt this was an essential change and it would probably involve reshooting the scene. Consensus was that it was not essential. 7. Text worked well on screen, but the last text was a bit odd. It was too formal, it needed to be more conversational and a longer message. The final text message was slightly brief considering the gravity of what he is communicating. Suggested the following: 'Hi Emma. I've done what the counsellor asked and I've made my decision. I know what I think'. 8. Honor thought that perhaps Jack's accent was too 'posh'. Neither Pete not the young person agreed. 9. At 08:26 you can hear the director say, "yep good". |

Several schools joined the School Health Research Network following the presentations. These meetings have continued in 2016 and 2017 in more local authority areas and resulted in further schools joining the Network. School leaders in Denbighshire are supporting a pilot of how pupil referral units and special schools could be involved in the School Health Research Network and how school level health data might be shared with other local agencies. |

| Results and Impact | The School Health Research Network manager has visited 9 schools that joined the Network but could not attend the launch event in June. She has also met with representatives from all schools in Denbighshire and the Denbighshire Healthy Schools coordinator and met the Wrexham Healthy Schools manager. At each meeting teachers reviewed and commented on the school health feedback reports and resources being offered by the Network. 2016: School visits are ongoing as part of recruitment to the Network. Nearly all schools that are visited go on to join the Network. Denbighshire schools that were not members of the Network asked to join. Schools suggested multiple ways in which they could make use of their reports for teaching, pupil voice and parent engagement. |

The teachers gave very valuable practical information and said they learnt a lot themselves about genetics and the research process, so it was a very valuable 2-way process. |

This was part of a programme of CPD jointly designed by the School Health Research Network and Public Health Wales. | Results and Impact | ALPHA are worked with SHRN to develop an engagement event for the ESRC festival of Science. The event involved going into 6 different schools throughout Wales and running workshops which encouraged the students to discuss public health topics and how it can influence their lives. This ALPHA session was a chance to test out the planned tasks and consider suitable ages for the activities. ALPHA helped refine the tasks and provided suggestion on how to involve all students in the sessions. |

| Results and Impact | Two researchers met with the ALPHA group to ask for feedback relating to the added questions for the School Health Research Network (SHRN) Questionnaire. This session was an opportunity for ALPHA to provide response and comment on the questionnaire. The group were asked if they thought the questions were age appropriate and asked for suggestions to improve the wording of the questions. The findings from this session helped inform SHRN of the changes that need to be made. These suggested changes have been inputted into the original set of questions in a track changes format. |

| Results and Impact | In April 2018 Rachel Brown meet with ALPHA to discuss the Tobacco Product Directive (TPD) and the changes made to the legislation. Including plain packaging, health warnings and smaller refill sizes. Researchers have met with focus groups of young people with plans for the groups to meet again in another year. Rachel Brown worked with ALPHA again, to share some of the final analysis of the TPD study data so the group can see how pupil behaviour changed between the first phase (the one that the group commented on) and the following year. We also discussed the findings from the TPD study and whether or not ALPHA agreed with them. Rachel also asked for ALPHA views on several additional questions for the SHRN survey 2019. We wanted ALPHA to review the questions for sense checking and language used and to make them more suitable and appropriate for the target population. |
