Abstract
Vector-borne diseases are pervasive, causing massive morbidity and mortality and severely retarding economic development across the developing world. Many of our current tools are inadequate in twenty first century urban environments or failing due to insecticide resistance, and the evidence base for newer interventions remains weak. Selecting the most cost effective, affordable and acceptable tools to reduce the burden of vector borne disease requires an integrated approach involving vector biologists, economists and anthropologists, working in partnership with national ministries of health, planning and finance. In reality, most disease endemic countries lack the capacity to develop evidence based strategies to control these diseases. There is a particularly acute shortage of vector biologists but also an urgent need to attract leading scientists from other disciplines to tackle this major public health crisis.
This proposal will establish a Partnership to Increase the Impact of Vector Control. We will bring together leading research institutes and national disease control programmes from three African countries with exceptionally high burdens of vector borne disease, Burkina Faso, Cameroon and Malawi, to develop evidence based solutions for integrated vector control. Supported by long term mentorship from LSTM and rigorous evaluation of the capacity needs in-country, the Partnership will generate new knowledge and tools, and strengthen national decision-making leading to reduced disease burden and increased resilience for responding to outbreaks.

Technical Summary

Planned Impact
Capacity
A substantial increase in the critical mass of researchers with appropriate interdisciplinary training, capable of working across the various phases of translational research has been identified as one of the key factors required to increase the impact of scientific innovation on human health. We will invest in 17 young African and UK researchers from multiple disciplines who have the creativity, intellect and determination to make a lasting impact on reducing the burden of vector borne disease. We will allocate resources to promote cohesion between these researchers to increase opportunities for collaboration between disciplines and between countries. Each researcher supported will recruit and train at least two additional junior researchers, further building capacity.
The PIIVeC applicants have an exemplary track record in supporting the careers of young vector biologists in Africa and a firm commitment to providing long term support, e.g. 11 of the 14 recipients of Wellcome Trust Public Health and Tropical Medicine Training and Intermediate Fellowships in this discipline over the past four years were supported by Vector Biology Department at LSTM. However, recognising the challenges in establishing research careers in resource poor settings, this proposal will go beyond supporting the individual scientist by also strengthening institutional capacity to build a vibrant environment to retain excellent African scientists.
Knowledge
The outputs of the research of PIIVeC will lead to evidence-based strategies to tackle multiple vector borne diseases using the most cost effective, sustainable and locally appropriate/ acceptable tools. By strengthening the design and implementation of vector control trials, including the accreditation of at least one field site to international good laboratory practice (GLP) standards, we will accelerate the translational pathway. Enhanced surveillance tools, coupled with advanced mapping approaches will transform each country's ability to plan routine control programmes and respond to outbreaks of vector borne disease.
Integration
Rather than evaluating control tools in isolation, we will take a holistic overview of the strategies needed to design and implement an integrated, sustainable and effective package of interventions for controlling multiple vector borne diseases. Each of our three partner countries will establish multi sectorial technical vector control advisory groups (TVCAGs) to enhance integration of disease control planning and implementation. In all countries, control programmes are currently disease specific, and often run and managed by external providers. However many interventions can target multiple diseases and supporting and empowering the TVCAGs will lead to a more efficient and integrated disease control strategy in country.
Partnership
LSTM has an excellent track record of providing long-term support and mentorship to strengthen the research base in Africa. We have worked with each PIIVeC partner for multiple years on numerous projects but this call provides a unique opportunity to consolidate those partnerships by integrating expertise from multiple disciplines. Capacity to tackle VBDs is in critically short supply in all three partner countries, particularly in Malawi, where there is only one PhD public health entomologist currently active in the entire country (Mzilahowa, co-PI on the current application) and where capacity in allied disciplines is also in extremely short supply. PIIVeC will strengthen south-south partnerships, promoting the secondment of scientists from Burkina Faso and Cameroon, to build capacity in Malawi. In this way we will build three regional hubs, in West, Central and Southern Africa. By investing in the enhancement of research management skills, we will also strengthen our partners' capacity to successfully compete independently for significant international funding.

| Description | Objective 1: To reinforce capacity for the development and implementation of evidence-based solutions for the prevention and control of VBDs via the recruitment and mentorship of a cohort of African scientists from pertinent disciplines. This objective focused on individual capacity strengthening through a cohort of post-doctoral African Research Career Development Fellows (RCDFs) and their mentee Early Career Researchers (ECRs). The most significant achievement in relation to this objective was that both cohorts of researchers significantly surpassed the targets set for them at the programmatic level. We established criteria for "exceptional" levels of performance and targeted 1 male and 1 female fellow to achieve these levels. By the end of the programme, nearly half of the fellows (2 females, 3 males) met these criteria. Together, they received over £100,000 in external follow-on funding and several fellows have been recognised as global experts in their fields through invites to participate in WHO technical advisory teams. Objective 2: To increase interactions between stakeholders working on multiple VBDs, identifying and evaluating opportunities for integrated vector management This objective focused on the Technical Vector Control Advisory Groups (TVCAGs), facilitating interactions between vector-borne disease researchers and policy-makers responsible for defining national approaches to controlling these diseases. The most significant achievement related to this objective has been the identification of evidence gaps and commissioning of 17 operational research projects by these groups, both increasing the demand and supply for evidence on vector control at the national level in each country. In Cameroon, the establishment and implementation of the TVCAG was seen as such a success that the Ministry of Health has decided to support the TVCAG as a national guidance group on vector-borne diseases. This objective was not completely met, as we had targeted more frequent TVCAG meetings. Several factors prevented us from meeting this target, including 1) delays in TVCAG establishment, especially in Malawi 2) Covid-19 which prevented face-to-face meetings for approximately 1 - 1.5 years, 3) UKRI budget cuts which decreased budget available for all partners. Objective 3: To accelerate research uptake using evidence synthesis and policy analysis and strengthen linkages between vector control research and policy at global and national level This objective focused on stakeholder and policy engagement work through development of policy-relevant evidence products, "behind-the-scenes" advocacy work by members of the TVCAG, and promotion of African researchers to global policy-making fora. Several significant achievements related to this objective were met. First, the value of the TVCAG as a knowledge translation platform was recognised by the Cameroon Ministry of Health and adopted as a technical guidance group, ensuring the sustainability of knowledge translation for vector control research in the country. Second, the TVCAG group in Burkina Faso demonstrated their influence on policy through successfully advocating for government resources to support dengue surveillance after several years of outbreaks. Third, the PIIVeC early career researchers recognised the value of for policy engagement and demonstrated their intentions to continue to focus on impact of their research in their future careers: "I realized I need to be more open minds as a researcher and always put the beneficiaries of my research in mind when I am conducting my research." "PIIVeC has helped me to grow, career-wise.. I have gained confidence in writing analysing and interpreting data being involved in policy making has now been added to my career goals." Finally, several of our evidence reviews (one authored by an RCDF) have been cited in WHO's malaria guidelines and several fellows have participated in WHO technical guidance groups. Objective 4: Strengthen research management skills at national level This objective focused on the institutional capacity strengthening assessments and action plans. The most significant achievement related to this objective has been the successful development and implementation of four capacity strengthening action plans in our partner institutes. Programme-wide, 70% of the action plan objectives were met, which is close to our target of 75%. The institutes will continue to work on their action plans after PIIVeC and have their capacity assessment reports which they can use to leverage funding from other sources. |

| Description | PIIVeC has started to see some evidence of larger impacts on health of society starting to emerge from the work that was completed under the award. This has largely been facilitated by the Technical Vector Control Advisory Groups established in Malawi, Burkina Faso, and Cameroon. TVCAG members and recommendations from the group have influenced vector control and financing decisions at the national level in all three countries. Advocacy efforts by TVCAG members in Burkina Faso enabled the Ministry of Health to justify a change to next generation bednets to the Global Fund and successfully persuaded the Ministry to support dengue surveillance efforts after several years of outbreaks. Although these actions are difficult to quantify in terms of health impact, many empirical and modelling studies have shown that appropriate intervention choice saves lives. Another broad impact has been the increased influence that that TVCAG itself has on the policy-making process. In Cameroon, the TVCAG was established by a research organisation outside of the Ministry of Health. However, over the course of the programme, the Ministry has recognized the value of the TVCAG in supporting evidence-based decision making for vector control. For the final TVCAG meeting supported by PIIVeC, letters of invitation were sent out by the Ministry, demonstrating a commitment to this knowledge translation platform and a desire for it to continue operating in the country. In addition, our partners in Cameroon have been asked to provide technical support to Central African Republic to advise them in establishing a similar advisory group there. In Malawi, despite a slow start, the TVCAG remains operational and were recently called upon to lead discussions on the pathway to control Human African Trypanosomiasis in Malawian game reserves. We learnt many lessons about knowledge translation from the TVCAGs. This is summarised in a paper published in BMJ Global Health, but briefly, we learned that the appropriate model for a vector control knowledge translation platform will be different in different countries and can change over time within a country. The timelines for our policy engagement work were impacted by turnover of key ministers, periods of political uncertainty, and the COVID-19 pandemic. Although the timing and exact nature of these events is not predictable, it is highly likely that such setbacks will occur and therefore risk management and mitigation plans which consider this would be worthwhile. Individual research projects supported by PIIVeC have started to demonstrate uptake at the national level. The Ministry of Health in Cameroon has shown interest in a fellow's work on establishing the risk of dengue infection in urban centres. This is important as Cameroon currently does not have a dengue control strategy. Likewise, the Ministry of Defence is working with another fellow on establishing a strategy for control of sleeping sickness in areas where troops are stationed. These conversations have been facilitated by the TVCAG and will likely develop further over the coming years. |

The second TVCAG Consultative Committee (held on 8/8/2019) meeting was the occasion to validate the proccess of selection of operational research projects. Coordinators of national disease control programmes and experts in vector-borne diseases when through the short-listed projects and verify that the objectives and the outcomes were in line with national plan policies in vector borne diseases before the validation of the awardees. The second annual TVCAG was held on 25 Nov 2020 after being postponed due to COVID. The TVCAG received updates on research from all RCDFs, Operational Research awardees, as well as other researchers outside of PIIVeC. | Title | Flying In-formation: A computational method for the classification of host seeking mosquito flight patterns using path segmentation and unsupervised machine learning |
| Description | Vector control targets the insects and arachnids that transmit 1 in every 6 communicable diseases worldwide. Since the effectiveness of many vector control tools depends on exploiting or changing vector behaviour, a firm understanding of this behaviour is required to maximise the impact of existing tools and design new interventions. However, current methods for identifying such behaviours are based primarily on expert knowledge, which can be inefficient, difficult to scale and limited by perceptual abilities. To overcome this, we present, detail and validate a new method for categorising vector behaviour. This method combines existing path segmentation and unsupervised machine learning algorithms to identify changes in vector movement trajectories and classify behaviours. The accuracy of the new method is demonstrated by replicating existing, expert-derived, findings covering the behaviour of host-seeking mosquitos around insecticide treated bednets, compared to nets without insecticide. As the method found the same changes in mosquito activity as previous research, it is said to be validated. The new method is significant, as it improves the analytical capabilities of biologists working to reduce the burden of vector-borne diseases, such as malaria, through an understanding of behaviour. |

The program was provided as a joint effort with MLW as contribution from the Partnership for Increasing the Impact of Vector Control (PIIVEC) program from the Liverpool School of Tropical Medicine (LSTM). The course was produced and taught primarily by Dr. Marc Henrion of MLW, with assistance from Dr. James Chirombo of MLW, Dr. Augustine Choko from MLW and Dr. Jim Maas from LSTM. The target audience was primarily to support the needs of the PIIVEC Research Career Development Fellows (RCDF) in Malawi and staff and graduate students from Malawi and comprised some thirty individuals. |

| Results and Impact | Annually at the beginning of the year, the Malawi national malaria control programme (NMCP) conducts a malaria review in order to assess progress in implementation of several malaria control programmes and to discuss challenges. The meeting involved all the DHOs in the country, District Commissioners, the NMCP, Funders and other implementing partners including the academia. Two days were dedicated for the meeting and it took place at Riverside Hotel in Lilongwe. The key themes during the meeting were, case management, malaria in pregnancy, data capture & quality and malaria commodities. Besides the selected DHOs and the NMCP, additional presentations were made by MAC (Themba Mzilahowa) and LINK Project. The presentation by MAC focused on opportunities that ICEMR 2 and PIIVeC projects present to the programme for evaluating impact of interventions and efforts to harmonize disease vector control in the country. Besides the main meeting and the power point presentation, Themba had two side meetings at Ministry of Health Headquarters and WHO Country office. These two meetings were aimed at introducing the PIIVeC project, its objectives and to seek support for the project. Themba met Dr. Storn Kabuluzi at his office at "Capitol Hill" where they discussed the idea of integrating the various disease vector control technical working groups in order to effectively coordinate control efforts. Dr. Kabuluzi was very open to the idea but was quick to point out that it might take a while before programmes can be fully integrated. Further, he mentioned that the ministry has had similar plans for quite some time and therefore this project could be an opportunity to further advance those ambitions. Themba visited the WHO offices to discuss PIIVeC project with Mr. Wilfred Dodoli. WHO (Lilongwe Office) is willing to support the initiative. However, he pointed out some of the risks involved that would delay the initiative. He mentioned that the current set up of vertical programmes has created "territorial behavior" that would inherently create some challenges or derail the process. |

| Results and Impact | The 1st WAASuN was held from July 8th to 12th in Accra, Ghana. The meeting reunited representatives from 19 West African countries to present an update of the current Aedes and Arbovirus situation in their countries. There were discussions about the main challenges, the network weakness and Terms of Reference (TOR) to examine the future of the organization. PIIVeC was invited to attend this first meeting to present an overview of the project, insecticide resistance in Aedes in Burkina Faso, vector-borne diseases mapping and methods of Aedes collection to surveillance and monitoring. In addition, a hands-on training was delivered by CDC in insecticide resistance and using a phone app to do surveillance. |

| Results and Impact | The PIIVeC project established Technical Vector Control Advisory Groups (TVCAGs) in 3 African countries. These intersectorial working groups have remained active after the project finished . As an example, in Malawi in Feb 2022, the TVCAG were joined by additional stakeholders (around 30 people) working on the control of Human African Trypanosomiasis to discuss the way forward for control of HAT in the Vwaza Marsh Game Reserve. Feedback from the meeting was very positive and it was remarked that it was very rare to get so many stakeholder for HAT in a room; this is a positive legacy of PIIVeC where we invested considerable time in identifying and engaging stakeholders from research, government and civil society. |

The guest of honour was the Secretary for Health and Population, Dr. Dan Namarika. The first official to make a speech was the current chair of the TVCAG, Dr Lester Chitsulo. In his remarks he said that TVCAG is a multidisciplinary committee with various expertise and it is a way of bridging the gap between various sectors. He said that although vector control is largely to malaria and trypanosomiasis, it is can be used to monitor and evaluate impact of disease programs. The course was delivered with the support of two LSTM projects, PIIVeC (Partnership for Increasing the Impact of Vector Control) and GIVeS (Geographical Information for Vector Surveillance) and involved 25 participants from a range of Malawian organisations.
