GCRF Action against Stunting Hub

Abstract

The global community aims to decrease the number of stunted children under five by 40% by 2025. While targeted and specific, we know that this is presently unachievable. Part of the problem is that over the last three decades, the search for the 'silver bullet' or the specific driver, which if addressed could solve this intractable problem, has narrowed praxis. This is not to say that gains have not been made, but rather our efforts have not been entirely impactful. While substantial disciplinary advances have occurred, often they have not been joined-up. And while systematic reviews abound, pan-disciplinary understandings, do not. Hence, child stunting is an intractable problem, waiting for a unified solution.

If we perceive child under-nutrition as a mosaic, we have knowledge of many of the individual elements. For example, biological advances demonstrate there is an identifiable 'epigenetic signature' of stunting. Children who are stunted also have immature gut micro-biomes and we also know a large proportion of the global malnutrition burden is caused (either directly or indirectly), by infectious agents ranging from viruses and bacteria to protozoa and helminths. Food-borne toxins also impact stunting. Equally, we know a range of elements can help to prevent stunting from animal source Foods (ASF) to behavioural elements from dietary choices to feeding practices to water, sanitation and hygiene (WASH). Yet overall, it may be argued that we are missing the shape and structure of the mosaic and the synergies between the component parts. In many quarters, the literature on child under-nutrition is viewed as 'siloed' and non-relational (Perkins et al., 2017). But equally importantly, 'integrated' nutritional programmes have often not had the expected impacts.

Herein lies the problem and the related solution: we urgently need to understand the 'cascade' of factors driving child stunting and the synergies and inter-relationship between drivers. And equally importantly, we need to better understand the 'tipping points' or the critical points along this cascade where healthy linear growth diverges to slow or no growth. To do this, we propose to transform our exploration of child under-nutrition from the component parts to the 'whole child'. Where the biological, social, environmental and behavioural context in which stunting occurs is understood in its entirety and where the strength and directionality of these drivers, inform related interventions. Based on this joined-up approach, we will explore the ability of a range of child-centred interventions to disrupt the cascade of factors that inhibit the ability of a child to grow. These actions and outcomes will then form the basis of a decision-making platform to enable users to identify the ex-ante and ex-post impacts of potential interventions. Embedded in this process, however, is a values-based approach that ensures that from the outset, our research directly connects to and betters the lives of the children, families and communities involved.

We will work across three countries: India, Indonesia and Senegal in over 50 communities. We aim to decrease child stunting by up to 10% our communities. Finally, to enhance our impact and legacy our Hub, we will engage a range of end-users in both our outputs and in the wider 'whole child' approach. We will support new regional platforms on maternal and child nutrition proposed by UNICEF linking the work of FAO/The World Bank/WHO at the country-level. We will also engage our network of over 100 civil society organizations in our findings. Over the course of the project, we expect to positively impact the lives of up to 1 million children.

Technical Summary

Abstracts are not currently available in GtR for all funded research. This is normally because the abstract was not required at the time of proposal submission, but may be because it included sensitive information such as personal details

Planned Impact

Our goal is to reach over 1 million children via both our outputs and outreach over the life of the Hub. Our specific aim is to reduce the prevalence of stunting by 10% across the communities we work in. We will measure progress toward this goal by identifying three 'control' communities participating in more traditional nutritional research/development projects. Additionally, we will work with our partner, 3iE, to develop a range of indicators to compare impacts and to assess our reach. To ensure we meet our targets, we have developed specific 'impact and engagement' strategies for the following stakeholder groups:

a. In-country actors including, but not limited to, decision-makers, practitioners, policy makers, clinicians and other health sector workers.
b. Multi-lateral, bi-lateral, UN agencies and national governments.
c. Children and their families across the three study sites.

In-country actors: We will invite end-user groups ranging from international and local NGOs, CBOs and local government officials to community-level meetings across the study sites. Utilising our shared-values lens, we will explore these actors' differing needs and priorities within a context of national-level policy priorities. We will share lessons and preliminary findings between and among all in-country stakeholder groups across the project cycle. Our aim is to enable policy and decision-makers to formulate predictive, rather than reactive, 'whole child' nutritional policies and actions. The development and dissemination of the decision-support tool is key to meeting this objective. By creating and publishing an open-source decision-support tool, policy makers will have access to a synthesis of existing, national-level data as well as project-level baseline and intervention data. Users will also be able to discern the ex-ante and ex-post impact of interventions. We will measure our success by the total number of users across key time periods: initial launch, after 12 months, after 24 months.

Global Agencies: Our Hub will directly inform and support nutritional programs at the global level as part of efforts to meet SDG 2 and the World Health Assembly (WHA) targets. As such, Hub indicators will align with those detailed by WHA (see WHO, 2014). Furthermore, we have created an End-User Committee to facilitate the uptake of our outputs across a range of international agencies such as UNICEF, WHO, the World Bank, IFAD, SUSTAIN, the Global Panel, EAT etc. Current members include Jonathan Wadsworth, Senior Advisor, World Bank; Rose Ndolo, Head of the Knowledge Hub, World Vision, Antonio Rota, Senior Advisor, IFAD, and Basil Rodriques, Senior Advisor, UNICEF, Fatiha Terki, Deputy Director Nutrition, WFP. We will embed our findings in these institutions' priorities and programmes and leverage committee members' wider networks. By linking this committee to our Flexible Fund, we will ensure that our impact at the local level will be both accelerated and durable.

Local children and their families:We anticipate that the impact on this community of Hub-users will be twofold. First, our engagement will enable the voices of children and families to inform stunting praxis. Second, our range of bespoke interventions, specific to the interplay of drivers involved, aims to break the intergenerational cycle of stunting. As such, we will put in place a number of impact and engagement strategies for the children and families who will directly participate in Hub-related activities. We will design and deliver the world's first Citizen Science project, which will empower women to identify linear growth issues in their own children and enable them to receive bespoke advice. Core Hub components from our work on shared values to our educational and behavioural components will enable us to better understand, listen and engage this community of users. We will use this learning to underpin a range of engagement exercises with our partner, Science made Simple.

Stage I (Setting the scene) has been finalised. A Bayesian Network model of synergies and interactions of drivers of child stunting in India, Senegal and Indonesia has been completed with publications drafted/at the submission stage. Stage II (Populating the whole child evidence-base) and Stage III (Interventions) are underway. The work on Shared Values has been completed with related publications under development. The Shared Values legacy work will begin in September 2022. Shared Value outcomes have been disseminated to all academic Hub partners, ensuring this critical knowledge of the socio-cultural context of stunting in each country informs their ongoing work.
Exploitation Route Outcomes of Stage I:
The modelling work has been disseminated through a workshop with policy makers and researchers working with national-level data on child stunting, peer reviewed publications and through blog posts disseminating findings to the general public. Outcomes of the Shared Values work will be disseminated through a peer-reviewed publication on understanding values-based perceptions regarding child stunting across a range of stakeholder groups.

Outcomes of Stage II: The Observational Cohorts and interventions are underway: we anticipate this data on the Whole Child will transform praxis on child stunting specifically and child nutrition more generally. We anticipate that both global and national level policy makers will be key adopters of our work and the Hub has developed extensive networks across these communities within the field of child nutrition.

Our innovative All Hub Repository and related Interdisciplinary Aid will enable the wider research community to better understand the synergies and inter-relationships between the many factors related to child stunting.

Our SOPs and protocols will ultimately form part of a range of future publications informing the field.

Description The impacts and outcomes of our Hub from the outset have been geared to two populations: the global community of stakeholders working in child nutrition and the communities themselves. To meet these impacts and outcomes we are underpinned by an ethos of equitable partnerships. Hence, capacity strengthening has been a core activity across our Hub. We have held over 53 trainings covering all aspects of the 'whole child' from anthropometry and dietary training, tools to measure children's learning environments and cognition, nutrigenomics and nutrigenetics, bio-informatics, uterine growth trajectories, Bayesian networks, WeValue approaches and the use of electronic data capture tools. We have trained over 600 individuals. Further, by using a 'training of trainers' approach we ensured the impact of these trainings would cascade to wider communities of practitioners, particularly in relation to our dietary, anthropometric and shared values training. By building High Performance Computing Environments in each of our countries, our country teams are able to undertake cutting edge analysis of the epigenetic and microbiome data thereby enhancing critical skills in bioinformatics. We have also developed a variety of new academic collaborations to support the Hub: from our work with the University of South Carolina supporting the development of the Food Systems metric, to our partnershiip with the University of Nebraska and Southampton in adapting measures of childhood development to our in-country contexts. We have strengthened our relationships with the UN agencies particularly WFP, FAO and WHO. We have also accellerated the development of our 'impact networks' by working closely with GAIN, EAT Forum and Power of Nutrition on Action Against Stunting Day activities. We are committed members of the 1000days.org network and part of their community of practice. We have strengthened our partnership and fostered South-South learning by implementing weekly meetings geared specifically to sharing fieldwork challenges and successes. We believe strengthening this all-Hub communication platform has fostered equity and teamwork, enhanced knowledge sharing and forged a solution-orientated environment across our Hub. We have also significantly strengthened our outreach and engagement during this period. As noted above, we have held 17 large-scale webinars this year with over 2500 registrants. Our emphasis is on policy-facing events and workshops. The Hub has spearheaded a drive to 'Save SDG2': and we have held a range of workshops to Action Against Stunting Day. Indeed, Action Against Stunting day garnered over 42,000 impressions on Twitter and over 6000 views of our website. Our events have supported strong growth across all Hub social media channels. Our field activities are well underway.Together, over 2,600 women and infants are part of our Hub and we have collected over 10,200 samples across the Observational Cohorts and Interventions. Across each country, we have formalised agreements with laboratories and research institutes who will support the in-country analysis and storage of Hub samples. In response to COVID-19 we have devised and implemented a range of enhanced bio-security procedures across our data collection and sampling activities. Importantly we are also exploring how COVID-19 has impacted our communities. We are implementing two new cohort studies: one among pregnant women and one among children to explore the wider impact of the pandemic on these vulnerable populations. As we continue to build relationships at the community level, we expect the project will begin to yield impacts at the societal level. Public engagement is a key element of our project. We have developed strong networks with local TV and radio stations across our Hub who have been able to quickly amplify our messages regarding child stunting. Indeed, a local TV station in Senegal covered the initiation of our fieldwork activities reaching an audience of half a million. We continue to explore new approaches to engage our communities. The intended impacts of these activities are to foster two-way knowledge transfer for communities and project research partners.

Title 24 Hour Recall Dietary Data Entry and Decision-Support Tool: Senegal and India
Description The above dietary application is a bespoke application build to combine 24-hour food recall with local resources to create a local databank of recipes and the corresponding energy values and nutrients involved. The application is a web-based system, developed on MySQL or CKAN, with an intuitive front end to allow for easy data entry and navigation of the enumerators in the field. The algorithms and calculations necessary to put together the recipes will be programmed into the applications as well as the ability for designated users to add new foods and update the recipe database. This will also enable the re-use of the skeleton of the application to then build the same repository of local recipes and nutrition for the team in India and Senegal. The Dietary App collects- record level data for each participant, but will not contain identifiable information and will be directly linked to the All Hub Repository, as the resulting local recipe and nutrition database is an important outcome for the repository and a necessary resource for other workstreams (e.g. decision tool). The participant data will also be synced for data analysis within the hub.

Title Adapted Measuring Early Learning Environment (MELE) Tool
Description The Measuring Early Learning Environment (MELE) observation grid allows for the measurement of multiple dimensions of the learning environments related to quality. However, the MELE tool was originally devised for use in a higher-income country context. India, Indonesia and Senegal are low- or middle-income countries with under-represented ethnic and religious communities. It was therefore essential to adopt an inclusive approach in terms of the language used (the tool has been translated in Bahasa, French, Wolof and Telegu), and to align items with local norms and cultural values across our study sites. The MELE adaptations were created following a rigorous process involving: 1) profiling of the preschool educational landscape in each country via a documentary analysis, creation of an Evidence Gap Map, and discussions with country teams; 2) identification of national education standards; 3) translation of the original tool; 4) adaptation of the tool to national standards; 5) piloting and back translation; and 6) country teams were trained by Dr Dawn Davis (the creator of the MELE) and certified in the process.
Type Of Material Improvements to research infrastructure

Title Agrifood Linear Model and Policy Priority Tool
Description Our Agri-Food tool identifies foods that can address nutrient gaps in peoples' diets, using both dietary data and food environment data. Agri-Food is underpinned by the Optifood programme (a linear programming tool that simulates the extent to which alternative combinations of locally available foods can improve nutritional adequacy). Agrifood may also be used to collate the perspectives of different stakeholder groups from household members to agricultural extension workers, public health staff, food sellers/traders to government officials.
Type Of Material Improvements to research infrastructure
Year Produced 2021
Provided To Others? No
Impact The results of the Agrifood tool underpins the Food Systems theme of the Hub and will be used to a) guide the analysis and discussions for a market-based intervention development; b) identify the context relevant, priority food value chains to be assessed; and c) identify model parameters for the systems dynamic modelling to determine how to strengthen local food systems to improve nutrition in a safe and equitable manner.

Title Evidence Gap Map: The State of Pre-Primary Education in Senegal, India and Indonesia
Description Evidence Gap Maps and infographics on pre-primary education: Here the the Gap Maps serve as the basis to adapt the Measuring Early Learning Environments tool (MELE) to better reflect the educational system of each country. A key aim of the Evidence Gap Map is to stimulate discussions about the preschool system with stakeholders, including policy makers. A key outcome of the maps is to inform the forthcoming MELE analysis and build the capacity of stakeholders to engage with this assessment tool.
Type Of Material Improvements to research infrastructure
Year Produced 2021
Provided To Others? No
Impact This tool is informing the MELE testing and approach to understanding access to education and the availability and support children who are stunted are able to access at the community-level. We anticipate that these maps will be published and indeed, will form a critical part of the policy-engagement element of the our outputs in the near future.

Title Food Composition Tables (FCT) for Senegal
Description Food composition tables (FCT) are key for nutrition research and include detailed information on energy, macro and micro nutrients elements of certain foods. Country specific FCTs are preferable and these are available for both India and Indonesia. Currently, there are no Senegal specific FCTs and we are therefore collaborating with researchers from the Helen Keller Institute (HKI) to develop this. We firstly used the Household and Expenditure Survey (HCES) (Agence Nationale de la Statistique et de la Démographie , 2011) to ascertain the 50 most commonly eaten foods. We also included foods collected by the HKI group from a detailed survey. In-country experts have been consulted who added and excluded foods from the final list. Next we will use this list to obtain the energy, macro and micro nutrient values from other FCTs in the following hierarchical order; 1) West African FCT, 2) Gambian FCT, 3) Nigerian FCT, 4) Ugandan FCT, 5) South African FCT, 6) USDA and 7) McCance &Widdowson.
Type Of Material Improvements to research infrastructure
Year Produced 2021
Provided To Others? No
Impact Country specific FCTs will be incorporated into the Optifood linear modelling programme to develop food- based recommendations. Additionally, in Senegal, FCTs will be integrated in a dietary application to analyse 24-hour dietary recalls and aid with data quality collection in the field. Developing this FCT for Senegal will aid future nutritional research in the country. Developing a new collaboration with Helen Keller International will augments our in-country network of partners to share and disseminate Hub-related news and ultimately findings.

Title Hub Data Management Plan
Description The Data Management Plan describes the data infrastructure of the Hub in order to guarantee and secure data quality and to ease data usage. It also describes the design and development of the Action Against Stunting Hub All-Data Repository (a key interdisciplinary output) is being built. The aim is to create an integrated data ecosystem that supports comparative interdisciplinary data analysis of the Whole Child. Key featurs of the Repository include: • Accessibility: The Repository must be searchable using a standardized set of topics and tags which conform to recognized disciplinary ontologies. • Interdisciplinarity: The architecture underpinning the Repository must aid/accelerate interdisciplinary analysis. • MEL: the Repository must act as a tool to support Monitoring Evaluation and Learning across the Hub. • Decision Support: The Repository must underpin the decision support tool, a key output of the Hub.
Type Of Material Improvements to research infrastructure
Year Produced 2022
Provided To Others? No
Impact The Hub Data Management Plan provides documentation on large-scale, interdisciplinary data management and in this manner, can inform future projects. The integrated nature of the Data Management System is particularly important for monitoring and evaluation as it allows for feedback in real-time regarding progress such as data and sample collection and processing, and other project key performance indicators through the MEL dashboard. The interdisciplinary nature of the system put in place allows researchers to focus on actual data analysis while reducing the complexity of the formatting and transformations done between data systems. This then creates a set of innovative tools that will serve future projects.

Title Hub Data Management System

Title SENGSYN Study (Senegal Synbiotic Study)
Description Young children exposed to poor sanitation and hygiene develop a disorder of the gut called "environmental enteric dysfunction" (EED) characterised by abnormal histopathology, permeability defects, and inflammation. EED significantly impairs growth through reduced digestion and absorption of nutrients, increased susceptibility to infections and systemic inflammation that directly inhibits growth hormones. EED may already be apparent by 6 weeks of age. Dietary supplementation with a synbiotic may provide resilience to the developing gut microbiome against these adverse environmental factors. Synbiotics are prebiotics combined with probiotics. Prebiotics are non-digestible compounds that encourage the growth of healthy gut bacteria. Probiotics are live beneficial bacteria such as bifidobacteria and lactobacilli. We are evaluating whether administration of a synbiotic to infants up to age 6 months in Senegal improves linear growth through improved gut health. In order to do this we are undertaking an individually randomized, 2-arm, open-label, controlled study of a synbiotic in infants between 0-5 months of age in Kaffrine District, Senegal. To date, 182 infants have been recruited into the trial.

Results and Impact In the second edition of our annual awareness day, we brought together a global panel of policymakers and academics to commit to promoting research and interventions to reduce stunting by 2025. We explored how to advance a just transition to sustainable food systems and food security through appropriate policies, investments, and support and how could we make this year actionable. The AASD 2022 was held in a hybrid format at the London School of Hygiene and Tropical Medicine (LSHTM) to convene key changemakers in the field of nutrition to discuss how we can tackle child stunting and transform nutrition. After the event, we utilised the space inside LSHTM to host a photographic exhibition of our work in India by Liz Hingley. This exhibition included refreshments for all attendees at the school which provided a great opportunity for networking. Accomplishments from the day are multifaceted. Firstly, we successfully engaged a variety of individuals at an international level. This included connecting with each country branch of the AASH in Indonesia, India, and Senegal and having their respective country leaders attend to speak about their Hub's work in the discussion. This also allowed the central Hub to work with the respective country lead's communications offices to circulate the event and raise awareness about stunting. Moreover, the organisation of "Watch Parties" with international partners such as the Indian Institute for Public Health Gandhinagar (IIPHG), allowed for local researchers and participants to engage with each other's work and the main Hub and meet and collaborate with members of their team. This was crucial in our reach expanding to over 400 people.

We also connected with our national and local partners, such as the UK Research and Innovation (UKRI), Global Challenges Research Fund (GCRF), and the London International Development Centre (LIDC) respectively. These partners assisted in circulating the event and connecting us with their connections. Furthermore, our public engagement partners, Science Made Simple, participated in the event via assisting with the setting up of the networking event at the LSHTM's courtyard, and their engagement with members of the audience and students. This included simulating the work of our researchers on the ground via an innovative interactive display.

In regards to interdisciplinarity and the connections formed here, we invited many exceptional speakers from notable organisations such as, the World Health Organization (WHO), the Food and Agricultural Organization of the United Nations (FAO), the United Nations Children's Fund (UNICEF), the Global Alliance for Improved Nutrition (GAIN), and Global Panel on Agriculture and Food Systems for Nutrition (GLOPAN). Lastly, we collaborated with Liz Hingley, a photographer, curator and anthropologist whose practice is inherently collaborative in nature and seeks to create connections between disciplines, geographies and generations. Liz's projects have received numerous awards, including the Lens Culture portrait prize, PhotoPhilanthropy Award, Prix Virginia and Getty Editorial Grant. This made her photographic exhibition a large success with participants, to which we are now in the process of expanding the scale of her work for the purposes of a public exhibition.

Results and Impact The Action Against Stunting Hub and All-Party Parliamentary Group for Africa joined forces to hold a high-level parliamentary roundtable on Tackling the Rise of Child Stunting in Africa at the House of Commons. The meeting brought together over 40 participants including parliamentarians, leading researchers in the field, NGO workers, and the civil society to examine policy and regulatory solutions to stem the rise in child stunting. Our speakers were Professor Claire Heffernan (PI of the Hub), Professor Paul Haggarty (Deputy-PI of the Hub), and Rose Ndulu Ndolo of World Vision. The roundtable discussion allowed the Hub team to contextualise the problem of stunting, outline our efforts to create a typology of stunting, and argue for the need for a multi-stakeholder approach to tackling the problem. Accordingly, it supported our efforts to position the Hub as a leading authority on child nutrition research. Following this event, Pauline Latham MP, who chaired the session and sits on the International Development Committee, recommended that the Committee hold an evidence session specifically on the impact of childhood stunting. This positive commitment reinforces the importance of holding such events with key audiences.

Results and Impact Nine shared values workshops were held with local groups of mothers, fathers and early-years teachers in East Lombok, the field site for the cohort study in Indonesia. Each workshop involved between twelve to fifteen participants (including research staff from the Indonesian Hub team, who received training in the WeValue process). The workshop process is used to elicit and make tangible local shared values. In the context of the Hub, this relates to eliciting values-based perceptions regarding child stunting and exploring community-led legacies of the project. All researchers actively observed or assisted with the facilitation of 9 workshops, with three having achieved 75% of the training needed to become Certified Facilitators of the Shared Values WeValue InSitu process and a fourth researcher achieved 50% of the training. Future training is planned to enable all researchers to become fully certified.

Nine participants were involved in each of the ten workshops, usually made up of four(4) community members/beneficiaries, four(4) researchers from the Senegalese hub and one researcher from the UK hub. These shared values workshops were organized to obtain information on local/communal shared values regarding the project in Kaffrine. To this end, information regarding local perceptions/knowledge and context specific understanding of child stunting, breast feeding, health seeking behaviors and various public informational needs etc. were sourced and obtained from participants of the workshops. The overall data obtained from the shared values workshops will provide local/communal as well as cultural insights which are relevant to the themes of this research and beyond.
