National COVID-19 Wastewater Epidemiology Surveillance Programme

Abstract

Wastewater-Based Epidemiology (WBE) requires relatively few resources compared to the systematic testing of populations. WBE is especially promising for novel infectious diseases, where asymptomatic cases might play a significant role in transmitting the virus. However, WBE is only now being used to monitor the spread of a pandemic infectious disease.

Early studies by ourselves and others have shown that SARS-CoV-2 RNA can be recovered from wastewater, including from wastewater treatment plants (WWTP) preceding local COVID-19 hospitalisation activity. Given the challenge of making available diagnostic tests to the entire UK population, WBE represents a potentially low-cost and immediate mechanism for understanding levels of infection within large geographic areas.

N-WESP aims to compare our methods with those of European & North American WBE teams in an inter-lab trial for understanding, supporting and improving the DEFRA COVID-19 measurements which will feed into the Joint Biosecurity Centre (JBC). We will also compare methods with DEFRA, the EA's and JBC whilst they explore options for finer geographical measurements. N-WESP will empower public health authorities with an optimised surveillance tool with maximal sensitivity and predictive power whose uncertainties have been well characterised.

N-WESP will determine whether SARS-CoV-2 RNA in wastewater and sludge is infectious, and to what extent there might be downstream risks to human health. N-WESP will exploit catchment and, uniquely, sub-catchment-scale longitudinal surveillance to understand temporal and spatial heterogeneity, relationships to human disease burden distribution and whether there is potential outbreak 'hotspots' by surveilling sewer system nodes.

Description Wastewater contains coronavirus that can be quantified using molecular approaches, such as qPCR.
At present it is not apparent that SARS-CoV-2 remains infectious in wastewater. This question remains an open question that is being investigated using a range of methods.
Efforts are on-going to link up wastewater estimates of viral load with cases of COVID-19 in the catchment. In many cases these linkages have been successful, however, there are numerous catchments where this linkage has proven to be a significant challenge. On-going work aims to understand what attributes of a WWTP catchment allow for better correlations between viral load and cases. Similarly, what attributes of a WWTP catchment reliably contribute to poor correlations.
On-going work aims to understand the factors that impact the survival of viral RNA in sewage. Early evidence suggests the following negatively impact on RNA recovery: industry with surfactant/detergent discharges or effluent with high/low pH. Pumps within the network can expose the viral RNA leading to loss. Longer transit times are likely to reduce survival.
Many members of NWESP are active engaged in the national WBE surveillance programmes (Wales, Scotland and England), where advice on sampling, storage, analysis and modelling is provided.
Exploitation Route The expertise build within the NWESP project is being used to help inform a permanent WBE capacity within the government.

Description NWESP has been directly feeding into governments' national wastewater-based epidemiology programmes and have delivered a report to SAGE. The knowledge and expertise developed within NWESP has facilitated expansion of WBE into school, prison and care home wastewater. Wider application of the approach for health surveillance is on-going and will help to inform the future capability within government for health surveillance. Many members of the NWESP team have since joined to submit a Program Grant on WBE as a direct result of our collaborative efforts on NWESP and the potential that the field represents.

Impact A number of significant evidence gaps hamper assessment of transmission risk of SARS-CoV2 through environmental pathways. While various studies have detected viral RNA signatures from environmental samples (from air, water, treated effluents and sewage and surfaces) using RT-PCR, infectivity has not been assessed in most cases. Very few studies have investigated the presence of infective virus using culture techniques. In addition, the infective dose of SARSCoV-2 is still uncertain. Assessments of risk and uncertainty therefore draw heavily on expert judgement and knowledge of other pathogens throughout this document. The level of risk of catching SARS-CoV-2 from the environment is highly dependent on the levels of infective SARSCoV-2 circulating in the population and its geographical spread.

Impact Key points taken from report Executive Summary: • Antimicrobial resistance (AMR)-when microbes (i.e., bacteria and fungi) develop the ability to defeat the drugs designed to combat them-is a public health threat and global priority. Resistant pathogens can cause infections in humans that are difficult or impossible to treat. • Scientific evidence shows that antimicrobials and antimicrobial-resistant microbes are present and can persist and travel (spread) through the environment (waterways and soils). Human activity can contaminate the environment with antimicrobials and antimicrobial-resistant microbes, which can accelerate the development and spread of resistance. • Contamination can occur from human and animal waste, pharmaceutical manufacturing waste, and use of antimicrobial pesticides for crops; however, the scale and risk associated with this contamination is not fully understood. There are outstanding scientific questions related to the presence and impact of antimicrobial-resistant microbes in the environment and the direct risk posed to human health. • The environment is a key element of the One Health framework. It is necessary to better understand risks, prioritize action to address antimicrobial-resistant microbes in the environment where potential risks to human health are greatest, and cultivate a collaborative global approach. • Scientific review suggests that there are actions that could improve understanding and guide action: - A better understanding of hospital waste treatment in different global settings is a priority, requiring establishment of evidence-based waste standards and implementation of effective waste management practices and capabilities where interventions are most needed. - Good hygiene and sanitation, including effective waste disposal and treatment, are important ways to mitigate the risk of antimicrobial-resistant microbes in the environment associated with human waste and wastewater contamination. - When feasible, contamination by animal waste could be reduced by improving antimicrobial use, developing alternative disease control methods (e.g., vaccines), and improving the quality of the rearing environment to help reduce the need for antimicrobials. - Agreement on a discharge limit for effluents leaving manufacturing sites and standardized monitoring and reporting of effluent levels could significantly reduce contamination and potential human health risks associated with exposure to resistant microbes in the environment. - Potential risks from using antimicrobials on crops could be reduced through greater global transparency of antimicrobial use, implementation of best management practices when applying antimicrobials as pesticides, and greater use of alternative disease prevention and treatment strategies. • To maximize potential impact, align activities to address antimicrobial-resistant microbes in the environment (e.g., addressing knowledge gaps) with existing global public health and development efforts, such as Sustainable Development Goals; the Water, Sanitation and Hygiene Initiative; and the Global Health Security Agenda.

Results and Impact Over the past year I have engaged with the media on the COVID-19 pandemic via the Science Media Centre and the Edinburgh University Press Office. This has involved a variety of interviews and quotes at the national and international level. A search done by our research office suggests considerable penetration internationally with a search returning mentions in 3016 articles online, 93 print articles, 73 broadcasted interviews and 6 blogs. While the bulk of these impressions was in the UK, there are further multiple impressions around the world (top 10 are the UK, USA, Canada, New Zealand, India, Australia, Ireland, Singapore, Jersey and Malaysia).
