Technical Summary

The UK Dementia Research Institute (UK DRI) is an initiative funded by the Medical Research Council, Alzheimer's Society and Alzheimer's Research UK. Funding details for UK DRI programmes will be added in 2019.

Clinical impact: Small vessel diseases (SVD) affect perforating cerebral vessels less than 500 microns in diameter resulting in diverse brain disorders. They cause most vascular dementia and worsen Alzheimer’s disease (AD), in total causing up to 45% of dementias. SVD also causes 25% of ischaemic and 90% of haemorrhagic strokes,1 worsens stroke outcome,2 and increases future risk of dementia, stroke and death.3 Patients present to different services, resulting in lack of awareness of SVD’s wide impact on the brain, mechanistic understanding or effective interventions.4 SVD and neurodegeneration: Visible SVD lesions, e.g. white matter hyperintensities (WMH), lacunes, microbleeds, perivascular spaces (PVS), recent small subcortical (lacunar) infarcts, atrophy,5 share clinical effects, epidemiology, pathology, and occur in AD6 as does microvessel dysfunction.6,7,8 SVD affects the brain diffusely: peri-lesion white matter is abnormal;9 global white matter integrity declines as SVD burden increases;10 SVD lesions cause focal/regional cortical,11 long tract and global brain atrophy.1 SVD is dynamic: acute lesions can disappear, remain a WMH or cavitate;1 WMH increase12 and regress, both in
sporadic13 and monogenic SVDs.14 Although lesion dynamics are poorly understood, progression worsens cognition and physical function, while regression resulted in less cognitive decline, disability or recurrent stroke in our cohort9 (in review).

White matter hyperintensity (WMH) segmentation and connectomics were performed in 47 patients with SLE from structural and diffusion magnetic resonance imaging (MRI) data. Network nodes were divided into hierarchical tiers based on numbers of connections. From each tier, and globally, brain network metrics were extracted for each patient. Voxel-based analyses of the spatial distribution of WMH in relation to disease indicators and network measures were also conducted. The data comprise all lesion distribution maps from the sample, the raw results (in nifti-1 format) of the voxel-based analyses of the WMH distribution in relation to disease indicators and network parameters, the MATLAB code to process the data, and the excel spreadsheets with the brain network metrics.

Imaging methods for brain arterial territories have evolved significantly from cadaver injection techniques to combinations of CT angiography with multiple MRI perfusion techniques. The variability of the brain arterial territories is well acknowledged in the literature. It has been attributed to: (1) haemodynamic factors related to the peripheral resistance of each artery; (2) anomalies of the Circle of Willis; (3) the involvement of leptomeningeal collateral (LMCs) arteries; and (4) enlargement of arterial territories due to previous stroke; among other factors. We separately conducted two systematic literature reviews searching for papers that 1) acknowledge, illustrate or analyse the vascular supply of the brain territories and the variability of their boundaries; 2) identify and discuss the source of this variability; and 3) present or evaluate useful resources to estimate the boundary of the arterial territories from brain MRI data. Each systematic search had slightly different aim and inclusion/exclusion criteria. This dataset contains the result of the searches and the data extracted. From analysing the data extracted, the primary source of variability in the arterial territories was the involvement of LMCs. These provide emergency blood supply across the surface of the brain, between arterial territories, following occlusion. Secondary sources included variation of the Circle of Willis and enlargement of arterial territories due to previous stroke. After analysing non-contrast FLAIR MRI from 45 stroke patients for border zone and LMCs involvement, inter-observer reliability analysis found the prospect of assessing LMCs from conventional structural MRI sequences to estimate the variability of these territories to be very low (Kappa Statistic = 0.1262). This dataset also includes this inter-observer analysis.

a) Analysis of longitudinal neuropsychiatric symptom data and white matter hyperintensity volumes using linear mixed effect models in adults with established dementia and mild cognitive impairment, using analysis of a large brain imaging dataset (Sunnybrook Dementia Study) which will be replicated in the Edinburgh Dementia Research Institute-funded Mild Stroke Study 3 study.
b) Harmonised study procedures for the parallel clinical SVD study which is taking place across Edinburgh and Toronto sites.
c) Observed study procedures in phase 2 trial of therapeutic intervention for Alzheimer's Disease.

a) Analysis of longitudinal neuropsychiatric symptom data and white matter hyperintensity volumes using linear mixed effect models in adults with established dementia and mild cognitive impairment, using analysis of a large brain imaging dataset (Sunnybrook Dementia Study) which will be replicated in the Edinburgh Dementia Research Institute-funded Mild Stroke Study 3 study.
b) Harmonised study procedures for the parallel clinical SVD study which is taking place across Edinburgh and Toronto sites.
c) Observed study procedures in phase 2 trial of therapeutic intervention for Alzheimer's Disease.

Results and Impact Stewart Wiseman made two trips during 2019 to the Roski Eye Institute, University of Southern California, as part of a UK Medical Research Council / USA National Institutes of Health joint initiative on "neurodegeneration" to foster internal collaborations between UK and US investigators. Roski are leaving experts in analysis of optical coherence tomography angiography, an advanced form of retinal eye imaging that aims to complement our understanding of small vessel structure and function in stroke and dementia. Stewart learned how to conduct detailed quality assurance on Edinburgh's small vessel disease images, and generate density and other vessel metrics.

Results and Impact Being officially recognised as a Research Centre in 2018, The Row Fogo Centre for Research into Ageing and the Brain launched a new website. The website is directed in multiple audiences. Content includes a comprehensive description of the Centre research activity, updates on recent publications, news and organised events. The website contains information for PhD candidates and medical imaging researchers about related programmes and courses, and for visiting staff and students on how to join SVD Research Groups. The website gives a breadth of ways to fundraise and donate to support SVD research.
