Abstract

Older people seeking to continue living at home independently, and people living with chronic long-term conditions, can both benefit from the use of telehealth systems which offer a way to monitor and support daily activities. But some people who live in extra care housing experience loneliness and social isolation. The aim of this project is to create a 'virtual community' of connected carers, older people and people living with long-term conditions, through the use of technology and associated applications. Such an approach can reduce social isolation and help people maintain a healthier and independent lifestyle by encouraging a change in behaviour.
