UK Ethics Accelerator: Coordinating and Mobilising Ethics Research Excellence to Inform Key Challenges in a Pandemic Crisis

Abstract
The COVID-19 crisis demands that policy-makers, researchers, health and social care workers, and members of the public address unprecedented ethical dilemmas on a daily basis. Resolving these is hard, and it is risky. The complexity and speed of ethical challenges are leading to harms-some inevitable, some avoidable-on a significant scale. The main aim of the Ethics Accelerator (EA) is to harness and mobilise existing UK ethics research expertise to bear on these multiple, ongoing ethical challenges. The EA will rapidly provide evidence, guidance and critical analysis to decision- makers, helping to improve decision-making over the evolving pandemic response. A second aim is to enable systematic public deliberation around key ethical challenges. A third aim is to identify strategies to embed ethics at the core of future epidemic preparedness. The EA will leverage and promote a broad network of UK and international researchers to create flexible Taskforces that deliver rapid guidance and responsive advice to leadership in government, science, medicine, and public health. It will establish virtual fora for public discussion, deliberation and information about arising ethical challenges. In coordinating and focusing existing national investments in ethics research, the EA adds significant value and scales up the potential impacts of ethics research in science, medicine, policy and society. Primary outputs will be rapid research reviews; policy guidance; commissioned research; a broad peer review body; and stakeholder engagements. Main outcomes will be decision-making that is informed by ethics expertise, and is more transparent and accountable, thereby improving public trust.

| Description | We have achieved significant public engagement by publishing lay-accessible summaries of ethical reasoning on key ethics challenges in popular media venues. By way of example, in just the Prioritisation workstream, we have published 42 news articles on five blogs. Over the course of the project, the 22 articles that this workstream has published in The Conversation alone have had over 665'000 readers. Our publicly-oriented work has addressed ethical questions concerning vaccine mandates, selective lockdowns, mass testing, and childhood vaccination, amongst other topics. Members of this workstream have also made 80 international media appearances, providing content for the Science Media Centre, and appearing on national and international news television and radio shows. All of this work has served to enhance public understanding of a range of different ethical challenges that our society has faced over the course of the pandemic. We have delivered responsive advice to decision-makers via four written pieces of evidence to government calls for consultations on the introduction (and eventual revocation) of mandatory vaccination in the healthcare sector. Prof. Wilkinson was also consulted by the Cabinet Office regarding the mandatory vaccination policy. We have delivered detailed rapid reviews of existing ethics literature to inform decision-making on key challenges in the pandemic, including vaccine prioritisation, payment for vaccination, childhood consent for vaccination, and vaccine nationalism. In addition, these detailed reviews were supplemented by 22 short pieces published in the Conversation that aimed to provide publicly accessible and timely ethical analysis. We have produced 10 peer-reviewed publications addressing emerging ethical issues in the pandemic in leading scientific journals and specialised ethics journals (including featured target articles in The American Journal of Bioethics & The Journal of Medical Ethics, as well as standard articles in the Journal of Bioethical Inquiry, Journal of Law and the Biosciences). These papers will be published alongside solicited commentaries by international experts, evidencing how our work has helped to set the research agenda for pandemic ethics research over the course of the project. Our work has also revealed that public trust in data has been eroded during the pandemic. Building back trust in data and the institutions, organisations and systems that control data should be prioritised in government and NHS data strategies. Pandemic data systems will continue to be an important ethical, political and public health concern because public data will continue to influence how we respond to immediate and long-term impacts of covid-19. Debate is needed about the public value of maintaining these data systems. The (continuing) development and dissemination of frameworks and ethics reviews will be of enduring value, for explaining and evaluating ethical issues arising in the longer term in this and future pandemics, which will help planning and decision-making. The project has been good for the academic development of the ECRs and the public engagement exercise has been outstanding. As well as posing practical challenges to engagement activities, our work on the pandemic has demonstrated tensions in political authority, health communication, and public values, which influence the policies that are eventually implemented. |
| Exploitation Route | Here are further recommendations for how the outcomes of the funding could be usefully taken forward: 1) Re-balance inequalities that covid-19 has exposed and exacerbated: Address disparities in healthcare, particularly for those experienced by people with Black, Asian and minority ethnic heritage, and combat poverty which has worsened alongside intolerable inequality. 2) Build trust and transparency into government policies and actions: For example, through greater collaboration across the home nations to provide consistent and clear messaging and communications for citizens across the UK. 3) Public involvement in policy making: To create a society which is resilient in the face of future pandemics, the public want to be involved in shaping future policies. 4) Accessibility: Ensure continued access to materials that we have produced, so that it can usefully improve preparedness for future pandemics. 5) Recognition of the relation between ethics and governance: A fuller appreciation of the unavoidable place of ethical values in policy (whether or not these are referred to as such in terms), and better incorporation of ethics expertise, both explanatory and advisory, in future. |

| Description | Our award is still ongoing, but nevertheless the impact of our work is broader than academia. Some of the recommendations from this project have been adopted by government policy - for example, the decision to put on hold the plan for mandatory COVID vaccination by healthcare workers. Importantly, we argued in our analysis (and in a separate paper submitted for peer review), that some of the ethical arguments given for mandatory vaccination during the pandemic would apply (and would apply more forcefully) for mandatory flu vaccination of health care professionals. At the time of writing, that suggestion has not been taken forward. Furthermore, some of our academically-oriented work has already generated published responses, and we expect that much of our academic work will continue to be taken forward by other academics with an interest in public health ethics. Looking ahead, our findings have important implications for how government departments and agencies (such as the UK Health Security Agency) and international organisations (such as the World Health Organisation) might seek to manage future public health emergencies that may lead to scarce medical resources. We have sought separate funding for a project investigating how artificial intelligence might be used to incorporate public values with ethical analysis into policy for future emergencies. |

| Results and Impact | This six-part podcast series is hosted by David Edmonds and features interviews with members of the Ethics Accelerator research team, covering some of the themes that emerged from our research. All episodes can be found at the University of Oxford website. Episode 1: Jamie Webb. During the pandemic, there were social gatherings in Number 10. This seriously undermined trust in government. But what exactly is trust? And why is trust so vital during a pandemic? Jamie Webb explains. Episode 2: Beth Kamunge-Kpodo and John Coggan. The pandemic had disproportionate impacts when measured by ethnicity, gender and geography. Beth Kamunge-Kpodo and John Coggan are both legal scholars, and both are interested in inequality. Episode 3: Sarah Cunningham-Burley. At the start of the covid pandemic there was little time for officials to consult the public. Sarah Cunningham Burley oversaw some public dialogues to assess public attitudes to the pandemic, and to the government's response. Episode 4: Melanie Smallman and James Wilson. During the height of the Covid pandemic we became accustomed to watching, listening to and reading about experts in health statistics. J. Wilson and M.Smallman have been researching the use, and sometimes misuse of pandemic data. Episode 5: Johnny Pugh. Vaccines to combat Covid were developed in record time. Policy-makers then faced a tricky question. It was impossible to vaccinate everyone immediately: so who to inoculate first? Jonny Pugh says there were complex trade-offs. Episode 6: Ilina Singh. The Pandemic Ethics Accelerator programme was led by Ilina Singh, an Oxford Professor of Neuroscience and Society. In this interview she explains what the programme was, what it was designed to achieve and whether it succeeded. |

Mark and his guests Anjana Ahuja, Prof. Sarah Cunningham-Burley, Rev Charles Kwaku-Odoi, Prof. Christina Pagel, explore why trust matters, who do we trust most and how we can better prepare for future pandemics. Episode 2: Were Unequal Outcomes Inevitable during Covid-19? In this episode, Mark and his guests Charlotte Augst, Halima Begum and Dr. Beth Kamunge-Kpodo discuss unequal outcomes during the Covid-19. With Prof. Sir Michael Marmot, and Pastor Mick Fleming. Episode 3: How Many Deaths Are Too Many? From the outset of the Covid-19 pandemic, the British Government made it clear that a baseline level of mortality from Covid was being "priced in" to its decision making. In this episode Mark and his guests Anjana Ahuja, Prof. Martin McKee, Prof. Dominic Wilkinson, Ceinwen Giles and Matt Fowler reappraise this approach. Episode 4: What Would an Ethical Pandemic Look Like? As sure as night follows day, we will face another pandemic, so how can we learn from the mistakes made during Covid-19, to ensure our response next time is not only more effective, but also more ethical? In the final episode in the series, Mark and his guests Prof. Ilina Singh, Prof. James Wilson, John Prideaux, and Cath Joynson debate how ethicists and ethical thinking could play a more central role in deciding how to respond to the next pandemic. Bonus Episode: Interview With Sir Michael Marmot on Covid Inequalities. Professor Sir Michael Marmot has been researching health inequalities and their relationship to social injustice for more than 50 years and was named a Companion of Honour in the 2023 New Year Honour's List. Today Prof Sir Michael Marmot speaks to Mark about Covid-19 and health inequalities as well as his decades-long research into this field.
