COVID-19 Cancer Attitudes and Behaviour Study

Abstract

With COVID-19 at the forefront, people may not be aware of the importance of seeking medical help for early signs of cancer or taking up screening, when available, to diagnose cancer sooner. In addition, some people may not want to be referred to a hospital for diagnostic tests due to fear of catching coronavirus in the healthcare setting. COVID-19 may also affect whether people take part in healthy behaviours that can reduce the chances of getting cancer. These factors may lead to more cancers occurring, and more cancers being diagnosed at a late stage when treatment may be less successful.

Over 18 months, we will carry out a large study to measure cancer attitudes and behaviours in adults aged 18+ across the UK. We aim to include adults from a range of different backgrounds.

Working closely with Cancer Research UK, we will carry out an online survey with at least 3,500 people to ask about any recent symptoms, cancer screening and health behaviours during the UK lockdown period. Survey questions will include the time taken to visit the GP with a range of possible cancer symptoms, attitudes to cancer screening, anxiety about seeking help in the current situation, other barriers to seeking help, health behaviours including smoking, alcohol, diet and physical activity, and preferred ways of receiving public health information.

We will also interview by telephone around 30 people who have taken part in the survey to understand their attitudes and behaviours in more detail. The survey and interviews will be repeated with the same people around six months later, to assess any changes in their views and information preferences.

Finally, we will link the survey data with medical records available in Wales to assess the number of GP referrals and tests for suspected cancer symptoms, whether people have taken part in cancer screening, whether people who smoke have quit, and the number and stage of new cancers diagnosed.

Our study findings will be used to rapidly develop clear public health messages encouraging people to act on the early signs of cancer, take up cancer screening when it becomes available, and engage in healthy behaviours. We think this will help to reduce the negative impact of COVID-19 on cancer outcomes in the longer term.
 
Description This UKRI Rapid COVID-19 funded study was successfully set up and started data collection within 6 weeks from the funding start date. This was a testament to the excellent partnership between Cardiff University and Cancer Research UK and the whole research team who were in a position to start immediately. By working in collaboration we set up two concurrent surveys (CABS and COVID-CAM). Key measures were aligned across both surveys. Recruitment took place between 6th August and 18th September 2020 and 7543 participants were recruited across both surveys. This surpassed our required sample size and the decision was made to close the survey early. Where appropriate survey data were pooled. Survey data captured participants experiences of potential cancer symptoms and help seeking, attitudes towards screening and engagement in health behaviours (diet, weight loss, smoking, drinking and exercise) during the first 6 months of the UK pandemic. A subsample (n=30) of participants were also interviewed to learn more in-depth about their experiences during this time.

Findings on symptom help-seeking and intentions to engage in cervical and bowel screening have been published in two peer reviewed papers (Quinn-Scoggins et al 2021; Wilson et al 2021) and were also published in two policy briefings which were shared with over 50 stakeholder including third sector organisations, NHS organisations and government agencies/officials and gained significant media coverage. Results indicated that almost half of those participants who experienced a potential cancer symptom did not contact their GP, even for red flag symptoms (i.e. coughing up blood) and that 1 in 5 were less likely to take part in Cancer screening now than before the pandemic. Study findings have informed messaging of two national public health campaigns encouraging people to contact their GP if they experienced any unusual or persistent symptoms (Cancer Research UK's 'Don't Ignore It' and Cancer Research Wales; 'Get Checked' campaigns). Prospective analysis of health behaviours data has now been completed and has been prepared for publication with the intention to submit this to BMC Public Health in the next few weeks. Findings indicate that participants were highly motivated to engage in healthy behaviours including: increased physical activity (52.4%), increased fruit/vegetable consumption (49.1%), weight loss (48.5%), reduction in alcohol consumption (19.0%) and stopped/reduction in smoking (51.5%/27.4%). Motivation to continue to engage in these healthy behaviours were sustained over a 6 month period.

Analysis of anticipated symptom attribution and associated help-seeking and prospective analysis of symptom help-seeking are currently being finalised and prepared for subsequent publication. We have now also secured approval to access routine data through the SAIL data bank and intend to investigate GP referral data. We have identified a Medical Intercalated student at Cardiff University who will be taking up this project in October 2022.
Exploitation Route We are calling for well evidenced, nationally funded and co-ordinated cancer awareness campaigns to get the message out to the general public that NHS services are open safely and to encourage people not to delay seeking help for any unusual or persistent symptoms. To do this we want to use the findings from this study to influence local and national campaigns and government messaging. We plan to engage with a range of stakeholder including third sector organisations, NHS organisations and government agencies/officials to ensure messages are well evidenced and uniform. This will ensure that the general public receive strong unified messages.

Description Both policy briefings received significant engagement from key stakeholders and resulted in further impact. The first policy briefing reporting on symptom experiences during COVID-19 received extensive media coverage, in particular, the findings were highlighted by prominent social media influencer Deborah Jones (@bowelbabe) and were featured in The Sun, The Daily Mail, BBC news and on Sky News to name a few. The study findings directly influenced three cancer campaigns. Cancer Research UK draw on the study findings when prioritising the focus for the 'Don't ignore it' campaign, while Cancer Research Wales amended their key messages and tone of their 'Get checked' campaign and Wales Cancer Network reiterated the key findings as part of their social media delivered with the support of rugby legends. Following the second policy briefing, reporting on cancer screening engagement, we supported Public Health Wales screening division in identifying key items as part of their screening strategy by drawing on barriers reported in both the symptom help seeking and cancer screening engagement papers. Furthermore, Professor Brain was invited to provide evidence at the Welsh Screening Committee meeting drawing on findings from the CABS study.

Impact A policy briefing was released on 25th February 2021 outlining the initial findings from the first survey on participants experience of potential cancer symptoms and help-seeking. The policy briefing was made available on the study website (https://cabs-study.yolasite.com/news.php) and also emailed directly to over 40 key stakeholders across the UK including policy makers, UK/Welsh/Scottish government, NHS organisations and third sector organisations. The policy briefing was accompanied by a press release. Following this, we received a large amount of media coverage and are now in contact with a range of stakeholders to plan follow-on meetings to discuss the next steps of this work. The findings were also brought directly to the attention of the First Minister of Wales during 'First Ministers Questions' on 2nd March 2021 (https://record.senedd.wales/Plenary/11181#C360574 ) by Caroline Jones MS who asked in light of these findings, and apart from reinforcing the messages about the NHS being open, what more the government could do to ensure the backlog in diagnostics is dealt with and improve chances of survival for those seeking treatment.

Results and Impact Cancer Research UK with support from NHS Wales and the Welsh Cancer Network ran a symptom awareness campaign called 'Don't Ignore it' with the aim to reduce barriers to help seeking. The mass media campaign was aired in two bursts in June and August 2021 in Wales for 6 weeks via TV and social media and drew on the barriers identified by the CABS findings (i.e. worry about wasting GP time, overstretching the NHS, worried about making a fuss). In developing the campaign, we shared a report with CRUK outlining a breakdown of the CABS symptom help seeking findings by UK nation. On launch of the campaign this was shared with local media and several news stories were published including in the Daily Express, ITW Wales and Wales Online to name a few, which reported the devolved nation findings.

An evaluation of the campaign is currently being conducted and will be available at the end of March 2022.

Results and Impact Dr Whitelock Presented a poster on the preliminary findings of potential cancer symptom experience and barriers to help-seeking during the first 6 months of the UK COVID-19 lockdown. This raised awareness of the CABS study and sparked questions and discussion. Dr Whitelock was also awarded the best poster presentation prize.

Results and Impact Ms Moriarty (Research Associate and CABS study manager) presented a poster on the CABS study design and planned analysis at the virtual National Cancer Research Institute conference on 2nd-3rd November 2020. The title of the poster was 'COVID-19 Cancer Attitudes and Behaviour Study: A UK wide prospective mixed-methods cohort study'. This raised awareness of the CABS study at a key national cancer research conference and audience.

This raised awareness of the CABS study and a copy of the poster is accessible on the PRIME centre website.

Results and Impact The first findings on participant potential cancer symptom experiences and help seeking behaviour was released on the 25th February 2021 via a policy briefing. This was accompanied by a press release which received extensive media coverage on a national and international scale and covered as a news story on Cardiff University and CRUK websites. There were over 200 news items in local newspapers or news websites covering these findings and 30 pieces of coverage on local/Welsh radio. Professor Brain was interviewed on the study's findings on two live UK radio stations (LBC and Times radio), one live UK TV programme (Good Morning Britain on ITV) and one pre-recorded Canadian TV news channel (CBC news). Sky News presented the findings with a live interview with Matt Black who was diagnosed with bowel cancer during the first lockdown and with prominent social media influencer Deborah Jones (@bowelbabe). The findings were also extensively covered via social media with the Cardiff University twitter account recording 13.8 impressions and 67 engagements, while the CABS study twitter account released a sting of tweets highlighting the key findings and recorded 11.6K impressions and 222 engagements.

Results and Impact A study Logo was developed to use for all future communications regarding the study. A twitter account (@CABSCovid19) was set up in July 2020 and a study specific website (https://cabs-study.yolasite.com/) was set up in December 2020. The study also has a presence on the Centre for Trials Research website (https://www.cardiff.ac.uk/centre-for-trials-resea rch/research/studies-and-trials/view/covid-health- and-help-seeking-behaviour-study) and CRUK website (https://www.cancerresearchuk.org/health-professio nal/awareness-and-prevention/the-cancer-awareness- measures-cam#can_i_use_the_cam0). Since setting up the Twitter account we have had 33.3K impressions with the highest number of impressions on release of the first wave 1 findings on 26.02.21 (18.6K impressions). Since setting up the study website, we have had 407 visitors across 559 visits and 938 page views.

Results and Impact Professor Kate Brain and public research partner Julie Hepburn delivered a webinar hosted by Wales Cancer Alliance on ''Public Awareness of cancer during COVID' drawing on findings from the CABS study. The webinar was open to members of the public and was attended by third sector organisations, academics and the general public. The webinar raised awareness of the study findings and sparked questions and discussion.
