The social, cultural and economic impacts of the pandemic on ethnic and racialised groups in the UK

Abstract
The Covid-19 pandemic has thrown existing patterns of ethnic inequality in Britain into sharp relief, highlighting the vulnerability of ethnic and racialized communities not only in terms of poor health, but across a range of social and economic arenas. The Centre on the Dynamics of Ethnicity (CoDE) has pioneered innovative, policy relevant and impactful multi-disciplinary and mixed-methods social research on ethnic inequality since 2012. Building on this track record, and on new research on the pandemic, the programme will examine the impact of the crisis on ethnic and racialized people and communities, in the context of Brexit, an economic downturn and the increased awareness of racial inequality in the wake of the Black Lives Matters protests, and the rise of racist and anti-racist activism.
The programme offers new insights into key policy areas highlighted as concerns as the pandemic has progressed: health inequalities and ageing populations; housing and welfare; education, employment and policing; cultural politics, activism and resilience. The projects employ innovative quantitative and qualitative methods to examine emergent forms of ethnic inequality at multiple scales, from the household and community (micro), institutional (meso), to economic and demographic change (macro). The programme includes work packages which use a survey and the new 2021 census data to explore emerging patterns of racial and ethnic disadvantage related to the pandemic. It will also track the impact of Covid-19 on both physical and mental health of racialized groups, with a focus on older and vulnerable migrant communities. This work will identify gaps in our understanding of the vulnerability of racialized groups and ethnic minorities to Covid-19 and identify routes to filling these gaps, including linking data and the need for the collection of new data. Work packages will explore the impact of austerity and Covid-19 on housing inequalities in specific places as well as tracing the impact on labour market inequalities and their routes in education. The use of algorithms in education and policing will be explored. There will also be examination of the nature of memorialising and mourning in the wake of the pandemic in the context of new forms of community and political activism.

Ethnic inequalities in COVID-19 clinical outcomes: a global systematic review and meta-analysis of over 200 million participants

| Description | The stream of work under this award (The social, cultural and economic impacts of the pandemic on ethnic and racialised groups in the UK) alongside an allied award of Exploring Racial and Ethnic Inequality in a time of Crisis has undertaken a large amount of work into the impact of the Covid19 pandemic on racialised minorities, as well as the effect of the resurgence of the Black Lives Matter movement since 2020. One major part of this work is the Evidence for Equality National Survey (EVENS) which is the largest survey of ethnic and religious minority experience during the pandemic. The final weighting of this data is almost complete and will be released in May 2023 along with findings in the areas of health, housing, employment, education, experiences of racism, political and cultural engagement and identification and representation. Other research from the two awards includes a body of work in the areas including health, education, criminal justice and the judiciary although the research is not yet complete and final publications and associated findings are yet to be released. In the area of health, examples of the finding include those from the most comprehensive analysis of global ethnic inequalities in COVID-19 health outcomes to date, CoDE research published in The Lancet: eClinicalMedicine found that ethnic minority groups experienced higher rates of severe illness and death during the pandemic because of their greater risk of infection. The research analysed results from 77 research studies, covering 200 million people from around the world. Earlier research showed a higher risk of severe illness or death for people from ethnic minority groups, but it was not clear if this was due to higher infection risk, poorer prognosis once infected, or both. The study found that the biggest driver of ethnic inequalities was in infection rates. Compared with the White majority group, South Asian people were 3 times more likely to test positive for infection, Black people were 1.8 times more likely, and Mixed and Other ethnic groups were each 1.3 times more likely. Among studies that looked at the risk of severe illness or death from COVID-19 in the whole population, Black people were 1.5 times more likely to be admitted to hospital than the White majority, Indigenous people were 1.9 times more likely and Hispanic people were 1.3 times more likely. The risk of needing intensive care was also higher: South Asian, East Asian, Indigenous, Hispanic and Black groups all had more than triple the risk than White majority groups. Indigenous people had twice the risk of dying than White majority people, with the Mixed ethnic group at 1.4 times the risk and the Hispanic group at 1.3 times the risk. The fact that more people from ethnic minority groups were admitted to hospital and more of them died seems to be due to a greater proportion of people being exposed to the virus and becoming infected - when looking at studies that only include people infected with COVID-19, the risks of severe illness and death are similar for ethnic minority groups and the White majority group. However, after being admitted to hospital with COVID-19, ethnic minority people were more likely to be admitted to ICU compared to White people. These higher rates may be a reflection of poorer health before infection with COVID-19, or inequalities in access to healthcare or service quality - all of which are a result of structural and institutional racism. Higher infection rates can be driven by socioeconomic inequalities experienced by ethnic minority groups, that were exacerbated by the pandemic, and occupational risks. Different patterns of employment, income and housing impacted the level of risk faced by people from different ethnicities - people from ethnic minority groups are more likely to have public-facing jobs, less likely to be able to self-isolate or work from home, more likely to live in overcrowded housing and less likely to have access to open spaces. These factors all increase the risk of COVID-19 infection. Data was also shaped by inequalities in policy and service provision. While positive test rates for White majority and Hispanic people were similar, antibody tests - the best indicator of previous infection - showed that Hispanic people were twice as likely to have previously been infected with COVID-19. This suggests that Hispanic people may not have had adequate access to tests. The findings emphasise the importance of using disaggregated ethnic categories, as the use of broad groups can mask differences in health outcomes. The study found that South Asian people had a higher risk of infection, whereas East Asian people did not, and these differences may not have been found if a broad 'Asian' group was used. Similarly, in research on the take up of the covid vaccine, CoDE research suggests that racism (including institutional, community and individual level factors) is a fundamental cause of ethnic inequalities in vaccine hesitancy. Using data from the UK Household Longitudinal Study, findings show that institutional-level factors (socioeconomic position, area-level deprivation, overcrowding) explained the largest part (42%) of the inequity in vaccine hesitancy for Pakistani or Bangladeshi people, and community-level factors (ethnic density, community cohesion, political efficacy, racism in the area) were the most important factors for Indian and Black groups, explaining 35 per cent and 15 per cent of the inequity, respectively. Our findings suggest that if policy intervened on institutional and community-level factors - shaped by structural and institutional racism - considerable success in reducing ethnic inequities might be achieved. A rapid review into ethnic inequalities in healthcare (examining more than 13,000 research papers over 10 years of research) revealed vast inequalities across a range of health services. Some of the largest of those inequalities were found for mental healthcare where treatment for people from Black ethnic groups was particularly poor. Kapadia and Salway also found a lack of research into specific areas including how outcomes may differ for ethnic minority babies in neonatal healthcare settings, where they found just one study that had looked into health inequalities in the care of ethnic minority new-born babies. A common and prevalent issue identified by the findings was poor communication between women and those with responsibility for caring for them during pregnancy and after giving birth. The lack of accessible and high quality interpreting services was certainly a common issue, but this was not the only barrier to quality care as poor communication was also commonly an issue for British-born ethnic minority women, and migrant women who could speak English. Communication problems were compounded by a lack of trust, insensitive behaviour, failure to listen and to bridge cultural differences. Further, reports of negative interactions, stereotyping, disrespect, discrimination and cultural insensitivity were also common. A rapid review of ethnic inequalities in healthcare and within the NHS workforce was also conducted with the NHS Race and Health observatory (https://www.nhsrho.org/publications/ethnic-inequalities-in-healthcare-a-rapid-evidence-review/). This found that ethnic inequalities in access to, experiences of, and outcomes of healthcare are longstanding problems in the NHS, and are rooted in experiences of structural, institutional and interpersonal racism. It found that the health of ethnic minority people has been negatively impacted by: lack of appropriate treatment for health problems by the NHS; poor quality or discriminatory treatment from healthcare staff; a lack of high quality ethnic monitoring data recorded in NHS systems; lack of appropriate interpreting services for people who do not speak English confidently and delays in, or avoidance of, seeking help for health problems due to fear of racist treatment from NHS healthcare professionals. Findings were detailed for inequalities in Mental Health services; in maternal and neonatal healthcare, in digital inclusion and access to health services; in genetic testing and genomic medicine studies; and in the NHS workforce although there were areas with significant gaps in the evidence and data. In the area of education, an article examining the 50th anniversary of Bernard Coard's book 'How the West Indian Child is Made Educationally Subnormal in the British School System, found that institutional factors persist in shaping educational disadvantage for Black Caribbean young people in England, particularly in secondary schools, particularly in the areas of academic tracking, teacher diversity and expectations and school discipline. Work on security and policing on university campuses is still being analysed. In the area of criminal justice, findings on policing in the pandemic which founds that policing during the Covid19 pandemic undermined public health measures where there was disproportionate targeting of Black and Minority Ethnic communities (Harris et al). In a separate study, drawing on the findings of a new survey of 373 legal professionals, as well as existing research, including JUSTICE's work on judicial diversity, research (Quinn et al) found that racial bias plays a significant role in the justice system. Many of those surveyed regarded discriminatory practices as commonplace among the judiciary. Racial discrimination was found to be most frequently targeted toward people from Black communities. It also found inadequacies in diversity initiatives and training. Race training is not compulsory, and under half of judges surveyed reported receiving any race training in the past three years. Findings highlight how the appointment of judges appears to depend to a large extent on ethnicity, with judicial appointment rates for Asian and Black candidates 37% and 75% lower than for successful white candidates. A report on Race Equality in Greater Manchester: An analysis of key issues (https://pure.manchester.ac.uk/ws/portalfiles/portal/225856034/Race_Equality_in_Greater_Manchester_An_analysis_of_key_issues.pdf) found that where there was successful action to address inequality in the city region, it was driven by local activities by those committed to racial justice, but that these are vulnerable to being discontinued as individuals leave their positions. It found that there was a need for the Greater Manchester Combined Authority (GMCA) to be more open to challenge, make senior staff accountable for reducing racial inequality, to take account of the experiences of people with lived experiences of racism and to diversify the GMCA workforce and leadership. |
| Exploitation Route | Outputs from the research on the impact of the Covid19 pandemic will come in a variety of forms. There are a number of academic publications, particularly journal articles planned in a range of disciplines. There are also short films available on the CoDE website and YouTube channel (https://www.youtube.com/channel/UCZGYunEQLW2VPyhSmdNXUbw) as well as recordings of seminars and discussions which are freely available. The findings from the EVENS survey combined with the workpackages will be published in a book with an intended crossover academic and policy/public audience. Covid Briefings have been brought out with The Runnymede Trust on 'Ethnic minority older people; ethnically diverse high streets; legislating statues; racism and policing; apprenticeship policy; covid mortality, with more forthcoming (see https://www.ethnicity.ac.uk/discover/briefings/covid-briefings/). The EVENS survey data, once weighted, will be released and freely available to use and is likely to be of interest to a variety of organisations. The findings from the research include a range of recommendations which could be taken up by the central, local and devolved government; the NHS; police and the judiciary and schools and universities. |

| Description | Findings from the research have contributed to public debate, including in over 60 different engagement events during one year. These have included public talks, debates, blogs and interviews. Events organised in collaboration with the Stuart Hall foundation were attended (on line) by more than 3000 participants, with a further more than 2,000 facebook and YouTube views of the recordings. Social media accounts including twitter, Instagram and facebook also have a wide reach. We have a youtube channel with more than 30 videos, including animations, commissioned films and recordings of seminars and events which has reached almost 5,000 views. Our research and findings have been covered in a wide variety of media sources, including Sky News, ITN, The Guardian and many local and ethnic minority targeted outlets as well as specialist media, for example directed at legal audiences. We have submitted evidence to enquiries within the House of Lords and Byrne participated in the Government Office's Foresight Resilience project. James Nazroo was also co-opted onto and supported the work of the SAGE ethnicity subgroup on COVID and provides ongoing advice to the NHS Race and Health Observatory. Dharmi et al published a major systematic review on race inequalities within the health service. The findings from our research have been cited in parliamentary questions and in questions to ministers in parliamentary committees and we have had discussions on our findings with individual MPs, political parties and the Lord Chief Justice. |
