Technical Summary

With improved survival due to antiretroviral therapy, HIV infection now presents the challenges of chronic disease management. For historical and cultural reasons, collaboration between primary care and specialist HIV services is underdeveloped and poorly understood. This contrasts markedly with the management of other chronic diseases in the UK.

We will use the General Practice Research Database (GPRD) to explore both the extent to which diagnosed HIV is disclosed, and the management of HIV infected individuals, in the primary care setting. Use of the GPRD, which provides demographic, diagnostic, prescribing, referral and consultation data on over 3 million individuals (including 6,000 HIV positive individuals) will allow us to describe the extent and quality of existing management of HIV related problems disclosed and/or diagnosed in primary care.

The results of this analysis will be used: (i) to compare epidemiological trends in primary care with surveillance data from specialist services, and thus estimate age and sex specific rates of HIV disclosure and new diagnosis; (ii) to estimate HIV associated morbidity and costs in primary care (including referrals); (iii) to inform HIV commissioning by estimating costs in the primary care sector; and (iv) to inform the development of relevant training for primary care practitioners in the management of their HIV infected patients.
