Aetiology of type 2 diabetes and related metabolic disorders

Abstract

It is suggested that type 2 diabetes arises from a complex interaction between genetic factors, obesity and lifestyle factors such as dietary behaviour and physical activity but the detailed mechanisms underlying these interactions have not been identified. We aim to study the interplay between these factors using a range of different epidemiological study designs across multiple studies around the world. We aim to identify genetic variants associated with risk of type 2 diabetes and related metabolic traits which can identify novel pathways to disease. Identification of these pathways can enable development of new therapies to help prevent and treat type 2 diabetes. A broad range of lifestyle, behavioural, anthropometric and biological risk factors are associated with future risk of type 2 diabetes. However, before we intervene on any of these risk factors, it is important to understand which ones are causal (i.e. would changing the risk factor actually result in reductions in risk?). Through the use of genetics, we can prioritise which of these risk factors is most likely to be causal and thereby define which risk factors are likely to be most fruitful for intervention.

Technical Summary

The overall aim of this programme is to use epidemiological methods to investigate aetiological pathways to type 2 diabetes and to translate that knowledge into preventive action.

The descriptive epidemiology of type 2 diabetes (T2DM) and its variation in time, place and person suggests that it arises from a complex interplay between innate susceptibility and lifestyle behavioural factors. Although global hypotheses have been put forward to explain these variations, the detailed mechanisms and pathways that underlie them are largely unknown. This programme aims to investigate the basis of these interactions by studying the genetic aetiology of obesity, T2DM and related metabolic disorders, the association between lifestyle factors such as diet, nutrition and physical activity, and lastly to study how the interplay between these various factors leads to disease. These goals are studied using a range of different epidemiological study designs including large case-control and case-cohort studies and population-based cohorts with detailed quantitative metabolic characterisation. The study of gene-lifestyle interaction requires a synthesis of data from a range of different studies including a large prospective case-cohort study nested within an existing cohort (EPIC-InterAct) and detailed metabolic phenotype cohorts with objective assessment of key lifestyle behaviours (Fenland study).

Specific objectives are currently to (i) Investigate the genetic basis of T2DM (ii) Study the genetics of intermediate quantitative traits in order to identify and validate potential modifiable pathways to diabetes risk and (iii) Study the basis for gene-lifestyle interactions in the causation of T2DM and related quantitative traits.

Title Data from: Genome-wide association analysis of type 2 diabetes in the EPIC-InterAct study 
Description Type 2 diabetes (T2D) is a global public health challenge. Whilst the advent of genome-wide association studies has identified >400 genetic variants associated with T2D, our understanding of its biological mechanisms and translational insights is still limited. The EPIC-InterAct project, centred in 8 countries in the European Prospective Investigations into Cancer and Nutrition study, is one of the largest prospective studies of T2D. Established as a nested case-cohort study to investigate the interplay between genetic and lifestyle behavioural factors on the risk of T2D, a total of 12,403 individuals were identified as incident T2D cases and a representative sub-cohort of 16,154 individuals was selected from a larger cohort of 340,234 participants with a follow-up time of 3.99 million person-years. We describe the results from a genome-wide association analysis between more than 8.9 million SNPs and T2D risk among 22,326 individuals (9,978 cases and 12,348 non-cases) from the EPIC-InterAct study. The summary statistics to be shared provide a valuable resource to facilitate further investigations into the genetics of T2D.

Title GWAS summary statistics for risk-taking 
Description Risk-taking propensity is a trait of significant public health relevance but few specific genetic factors are known. Here we perform a genome-wide association study of self-reported risk-taking propensity amongst 436,236 white European UK Biobank study participants. We identify genome wide associations at 26 loci (P<5 x10-8), 24 of which are novel, implicating genes enriched in the GABA and GABA receptor pathways. Modelling the relationship between risk-taking propensity and BMI using Mendelian randomisation shows a positive association (0.25 approximate SDs of BMI (SE: 0.06); P =6.7 x10-5). The impact of individual SNPs is heterogeneous, indicating a complex relationship arising from multiple shared pathways. We identify positive genetic correlations between risk-taking and WHR, childhood obesity, ever-smoking, ADHD, bipolar disorder, and schizophrenia, alongside a negative correlation with women's age at first birth. These findings highlight that behavioural pathways involved in risk-taking propensity may play a role in obesity, smoking, and psychiatric disorders

Stand included information on physical activity quidelines for children and tips on how families can be more active, as well as the Little Snackington and Great Snackington games http://www.mrc-epid.cam.ac.uk/snackington/, and the "Catch your 5-a-day" and hula-hoop challenges.

The stand was visited by about 250 families across the two days. More than 200 copies of the Great Snackington and Little Snackington leaflet were handed out, and 200 copies of the Cambridge MRC Activity book were also taken by visitors to our stand. Saturday was not as busy as Sunday though this allowed stand volunteers to have good conversations with visitors to the stand.

Results and Impact Cambridge University Fun Lab at the Big Weekend on Parker's Piece. Activities run included the Sugar Cube Challenge, Fortune Telling Molecules and Snackingtons game. Posters about the Unit's research were also presented, as well as leaflets and booklets.
About 100 families (250-300 people) visited stand over 5 hours (104 children completed Sugar Cube challenge). All activities were popular, and many conversations over course of day. Had conversation with consultant who is working with Brighton & Hove City Council on improving transport and sustainability, including cycle infrastructure, and pointed him to Propensity to cycle tool and work being done by James Woodwock's programme (provided contact details). Also spoke with Cambridge City Councillor Rosy Moore, who is interested in the research related to planning and infrastructure.

Visitors could also do tours of our volunteer testing facilties and try different measurements. Around 80 visitors attended with half of these also touring our laboratory facilities.

Excellent feedback, with 10% of visitors completing a feedback form. The event was very popular and we had many requests to repeat the day again in the future. We were also approached by a public health worker to provide learning resources as they felt out materials were very helpful and accessible.

Friday 28th was quiet, with limited footfall and perhaps 20 conversations, except when a party of about 100 schoolchildren attended a demonstration/show and most visited our stall afterwards. Saturday 29th was far busier, with a steady stream of families, couples and individuals. Or volunteers had about 100 good quality conversations, some involving more than one visitor, with about 250 people visiting the stand over seven hours. A Peterborough council transport team member was interested in our work and took away a leaflet for the Propensity to Cycle Tool. Peterborough Today reported on Life Lab, with a photo of the Unit stand.

Luca Lotta gave an ovreview of the Unit's research programmes, and an introduction to his own recent research on the discovery of biomarkers and causal mechanisms for the metabolic consequences of obesity. This was a joint event involving both the MRL and MRC Epidemiology Unit. Mentimeter online voting used to decide winners and get feedback from audience on what they thought when they heard the word "metabolism" before and after the talks, and also to get feedback on the event. Registration was free. Event was attended by 50 people, so the venue was almost completely full. The feedback was very positive, with attendees commenting that they found the event both very entertaining and informative.

Results and Impact MRC Festival of Medical Research - Science bus event at Ely College (18 June) and North Cambridge Academy (19 June), in collaboration with Cambridge Metabolic Research Laboratories/MRC MDU. Three Activities - Fat dogs activity on testing for genetic variations (gel loading and electrophoresis/genetics), Fortune Telling molecules (biomarkers of metabolic disease/concept of risk), and Physical Activity measurement. In addition to the activities we gave the students MRC bags with Snackington booklets and MRC Epidemiology Unit leaflets and IMS Staff profile/careers booklets.
Approximately 110 students from Ely College participated (mostly Year 9, some year 8), and about 80 at North Cambridge Academy (Year 9), each in 5 one-hour sessions of 15-30 over the day. We also had 60 Girl Guides and Brownies visit the Science bus and Gazebo on the evening of Wednesday 19th for 90 minutes. We received very positive feedback from participating schools and Brownies, and have been invited back to North Cambridge Academy to repeat the visit in June 2020.
We applied for and received £1,500 from the Medical Research Council to fund this activity.

Results and Impact Maastricht public health student visit. In total 70 undergraduate students visited the unit to find out about our public health research and tour the facilties as part of a two day trip to London and Cambridge.

We received excellent feedback from the students and tutors who organised the study tour. The students found trip interesting and enjoyable and the tutors felt it fitted well with the aims of the trip and was an excellent balance of theory and practical aspects. We have been asked to repeat the visit.

Results and Impact Chunxiao Li and Maik Pietzner assisted at the Cambridge Science Festival hands-on event for families at CAST with the unit stand "Diabetes risk factors: can we tell your health fortune?". About 150 children visited the stand and took part in activities, with a similar number of adults over 5 hours. About 70 families left suggestions fro lowering risk of diabetes in the population. We handed out about 100 copies each of the Great Snackington game and MRC Cambridge Units Activity book, as well as about 40 copies of Epigram, our annual newsletter, as well as several other leaflets including FEAT, Propensity to Travel and the MRC Epidemiology Unit trifold. The number of good quality conversations with stand visitors seemed higher than previous occasions, with volunteers reporting that they were able to discuss the various approaches the Unit takes to studying type 2 diabetes and related disorders. Having activities (including Gt. snackington) that highlighted different areas helped these discussions.
