Abstract

Both diabetes and obesity are major public health problems, with an alarming increase in the numbers of people with these conditions. It has long been known that there is an important balance between energy intake in the form of food intake, and energy expenditure. However, the nature of the association between food intake and the risk for diabetes and obesity is poorly understood. There are confusing messages about what type of diets to eat, and what types of food to consume in what quantities, and what their effect is on the risk for diabetes or weight gain. Dietary intake is very difficult to measure, compounded by the limitations of the methods that have been used in the past to ascertain intake. Our research is important because it will help to address some of these pertinent issues by (1) using improved methods to assess dietary intake including measurement of blood-markers (called biomarkers) of food intake which are more accurate, (2) studying the association between diet and diabetes/obesity in large well conducted studies such as the EPIC-Norfolk cohort study, and (3) by collaborating with other scientists and groups who are doing similar research to increase further the size of studies and the resources available to adequately answer these questions. Examples of such collaboration include the InterAct Study with 12,403 cases of diabetes, making it the largest study in the world of these associations.

Technical Summary

The aim of the nutritional epidemiology programme is to understand the aetiological associations between dietary and nutritional factors and the risk of type 2 diabetes, obesity and related metabolic disorders in order to inform preventive action. The contribution of dietary factors to the risk for diabetes and obesity is acknowledged to be important but major uncertainties exist about the nature of the relationship. This has often been the result of limitations in methods used to assess dietary intake, and has led to mixed, and often confusing messages about the role of dietary factors in promoting disease risk. Our objectives are to study the association of food intake and nutritional content with the risk for diabetes and related endpoints using dietary assessment instruments with greater precision as well as using biomarkers of dietary intake. Our methods include detailed dietary assessment by the self-reported semi-quantitative food frequency questionnaire as well as by the more precise 7-day prospective food diary. From these we derive key nutritional factors. To overcome issues of measurement error and reporting bias we have also measured candidate biomarkers of nutritional status such as plasma vitamin C, serum ferritin (body iron stores), and also serum vitamin D (a hormone, partly derived from the diet). We also await completion of measurement of serum fatty acids in the large EPIC-InterAct Study in European populations, which should provide invaluable information on the association between different types of dietary fat intake, which is particularly difficult to quantify from self-report because of the “invisible” fat in the diet. We use large-scale observational epidemiological studies including EPIC-Norfolk Study, the Fenland Study and InterAct, a case-cohort study of 12,403 incident cases of type 2 diabetes nested within the EPIC-Europe cohort. We also address the critical problems of confounding by using genetic variants as instrumental variables to assess the causal nature of the relationship between nutritional biomarkers and outcomes including diabetes, obesity and quantitative metabolic traits. We have also initiated a randomised double blind placebo controlled trial of the supplementation of vitamin D among people at high risk of diabetes, to assess whether vitamin D might be causally related to diabetes risk. In order to translate the findings of this programme into preventive action, we will also work as part of the Centre of Excellence in Diet and Activity Research (CEDAR) to develop collaborative work on understanding the determinants of dietary behaviour, and the development and evaluation of individual and collective dietary interventions.

Impact The Cambridge Seminar was held from 31st March till 6th April 2019, imparting training to public health professionals including clinicians and non-clinicians on the public health and epidemiological aspects of diabetes. It was a privilege to organise and host this week-long residential training seminar that included participants from more than 20 countries, with a substantial representation from low and middle income nations where opportunities for such training are sparse. This Seminar is an important contribution to capacity building for public health research for diabetes and related disorders which bear a huge public health burden.

Impact We organised the Cambridge Seminar in August 2015, which was a week-long residential training programme for capacity building in the field of epidemiological and public health aspects of diabetes. 42 participants from all continents (including 27 countries) attended this Seminar, taught by an international expert faculty who voluntarily gave of their time for free in this educational endeavour. 81% of the participants reported that their knowledge was much improved as a result of the Seminar; 100% rated the Seminar as excellent to very good, and 100% reported that their learning objectives were completely or very well met. Some of the Seminar participants have sent feedback since the meeting that they are active in the diabetes epidemiology field, motivated by their attendance at the Seminar.

Title Measurements toolkit: online resource 
Description We launched a free online toolkit for the measurement of diet, anthropometry and physical activity. This toolkit serves as a one stop shop for researchers globally to access information on different methods and instruments available for using methods fit for purpose in the research context. There is a decision matrix and an instrument library, as well as detailed information on key concepts in measurement including issues of validity, reliability, and harmonisation of data across studies. This toolkit updated, expanded and improved a prior version of the toolkit which was funded by the MRC PHSRN and the InterConnect Project. The current DAPA toolkit is supported through the MRC Epidemiology Unit, and the Cambridge NIHR BRC theme on nutrition, diet and lifestyle.

Title Data from: Genome-wide association analysis of type 2 diabetes in the EPIC-InterAct study 
Description Type 2 diabetes (T2D) is a global public health challenge. Whilst the advent of genome-wide association studies has identified >400 genetic variants associated with T2D, our understanding of its biological mechanisms and translational insights is still limited. The EPIC-InterAct project, centred in 8 countries in the European Prospective Investigations into Cancer and Nutrition study, is one of the largest prospective studies of T2D. Established as a nested case-cohort study to investigate the interplay between genetic and lifestyle behavioural factors on the risk of T2D, a total of 12,403 individuals were identified as incident T2D cases and a representative sub-cohort of 16,154 individuals was selected from a larger cohort of 340,234 participants with a follow-up time of 3.99 million person-years. We describe the results from a genome-wide association analysis between more than 8.9 million SNPs and T2D risk among 22,326 individuals (9,978 cases and 12,348 non-cases) from the EPIC-InterAct study. The summary statistics to be shared provide a valuable resource to facilitate further investigations into the genetics of T2D.

Title GWAS summary statistics for risk-taking 
Description Risk-taking propensity is a trait of significant public health relevance but few specific genetic factors are known. Here we perform a genome-wide association study of self-reported risk-taking propensity amongst 436,236 white European UK Biobank study participants. We identify genome wide associations at 26 loci (P<5 x10-8), 24 of which are novel, implicating genes enriched in the GABA and GABA receptor pathways. Modelling the relationship between risk-taking propensity and BMI using Mendelian randomisation shows a positive association (0.25 approximate SDs of BMI (SE: 0.06); P =6.7 x10-5). The impact of individual SNPs is heterogeneous, indicating a complex relationship arising from multiple shared pathways. We identify positive genetic correlations between risk-taking and WHR, childhood obesity, ever-smoking, ADHD, bipolar disorder, and schizophrenia, alongside a negative correlation with women's age at first birth. These findings highlight that behavioural pathways involved in risk-taking propensity may play a role in obesity, smoking, and psychiatric disorders

PI Contribution Chronic non-communicable diseases such as diabetes, cardiovascular disease and cancer are responsible for the vast majority of deaths in all but the poorest low and middle income countries (LMICs). They are particularly rising in LMICs and are higher in many LMICs than in high income countries. A key driver of this health burden is poor quality nutrition, including diets high in energy dense and highly processed foods and low in fruit, vegetables and fibre. There are several factors that may contribute to poor diets, including poor conditions for growing, storing and transporting fruit and vegetables, and a reliance on relatively expensive food imports. In small island developing states (SIDS) the relationship between a reliance on food imports, poor nutrition and a high burden of chronic diseases is particularly apparent. Community-based agricultural food production initiatives are one possible approach to improving food and nutritional security and to reducing the risk of chronic diseases. They can also help to improve 'food sovereignty' by increasing the ability of populations to influence their own food systems in ways that are sustainable and promote health. Such initiatives may take several forms, but are owned, organised and managed locally, and produce either fresh or minimally processed food for local consumption. While the potential contribution of such initiatives is recognised, there is very little evidence available on what their impacts actually are in terms of health, social well-being, economics or the environment. There is an urgent need for improved evidence on their impacts, and this proposal is concerned with developing and testing methods to contribute towards filling this evidence gap. The proposal is designed to help establish a transdisciplinary research programme with the conceptual and methodological skills necessary to undertake robust evaluations of community-based food production initiatives in LMICs. The evaluation of the initiatives will include their contribution to promoting health and social wellbeing, their economic and environmental impacts, and their sustainability. The development work proposed here will be undertaken in two contrasting middle income SIDS, St Vincent and the Grenadines and Fiji. Both are actively engaged in promoting local food production for local consumption, and their well-defined geographical boundaries and relatively small size provide ideal settings to undertake development work. However, the overall goal is to develop a framework and methods that are readily adaptable and applicable to other LMIC settings in which nutritional insecurity is a major contributor to the risk of chronic diseases, including many settings in sub-Saharan African, where levels obesity and diabetes are already high and increasing. In this project we will update and expand existing evidence reviews on community-based food initiatives in LMICs. In the two chosen settings, we will develop and test research methods to investigate the current nutritional adequacy of local diets and the contribution of community-based food production, and find out who is involved in and supports local food production. We also aim to develop methods to estimate the environmental impacts such as on local water use and carbon emissions. Finally, we aim to determine the major challenges and successes in establishing and sustaining selected initiatives, and how barriers to success might be overcome. The findings will enable us to develop an evaluation framework and refine methods to design a programme of work to robustly evaluate and compare community-based food production initiatives in Africa, the Caribbean and Pacific from the perspective of their impacts on chronic disease risk, social and economic well-being and the environment.
Collaborator Contribution Relevance: Chronic non-communicable diseases (NCDs) are responsible for the vast majority of deaths, and age-standardised prevalence rates of obesity and diabetes and mortality rates from cardiovascular disease and many cancers are higher in many low and middle income countries (LMICs) than in high income countries. The burden of NCDs is an urgent concern beyond the public health field for governments, society at large, and regional and international organisations. The UN high-level declaration on the prevention of chronic non-communicable diseases (NCDs), for example, has committed governments around the world to taking concerted action, recognising the direct impact of NCDs on the economic and social development in particular for LMICs and calling for a multi-sectoral response. Food security and sovereignty are central upstream determinants of NCDs and community-based food production a poignant example of such multi-sectoral effort required. Who will benefit: While a multi-sectoral ('whole of government' and 'whole of society') response has been identified as playing a vital part in comprehensive approach to prevent and control NCDs, multi-sectoral collaborations require evidence and guidance to develop, implement, evaluate and sustainably support their initiatives. Direct beneficiaries of this project will be civil society organisation as organisers of community-based food production initiatives. Further direct beneficiaries will be governmental and non-governmental organisation as initiators and supporters of such initiatives. This includes governments in the selected countries but also regional policy-makers as both in the Caribbean and the Pacific setting intergovernmental regional collaborations are established to pool resources and expertise. Furthermore, policy-makers of governments in regions that are similarly vulnerable can benefit from the findings of this project. For example, our South African partners have a strong interest in this research programme and see relevance for policy-making in their setting. Finally, research partners will directly benefit from this project. There is an increasing interest in natural experimental studies to evaluate policy responses and 'real life' initiatives that tackle upstream determinants of health. As a relatively novel research field, this project will contribute to further develop methods for such evaluations, with strong guidance within the MRC Epidemiology Unit who contributed to the MRC guidance on evaluating natural experiments. How will they benefit: As a development project, the short term benefit will be to identify specific needs to support the development and maintenance of community-based food production initiatives, particularly for relevant key organisation in the two identified regions. The impact plan outlines how these will be engaged. In more medium term, with the successful establishment of a comprehensive programme of research, we can cumulate robust evidence for their complex impact, including potential unintended consequences for the settings such as the potential negative environmental impact in terms of water pollution or carbon emission. With robust evidence beneficiaries across sectors will be able to make a case for further support of such initiatives and identify concrete factors that might need addressing to reduce negative impacts. The researchers directly involved in the project, at the University of Cambridge, the University of the West Indies, the University of the South Pacific and the University of Witwatersrand will be involved in highly innovative methods development work; this will include local colleagues and postdoctoral researchers. Researchers beyond the direct collaboration will benefit will be access to tools, conceptual framework and related guidance made publically available (see academic beneficiaries).

Impact There is a disproportionately high burden of non-communicable diseases in South Asia. The South Asia Biobank study, led by Professor John Chambers is funded by NIHR and seeks to understand the determinants and drivers of type 2 diabetes and cardiovascular disease in South Asian countries including Bangladesh, India, Pakistan and Sri Lanka.

Impact The collaboration is multi-disciplinary including experts from the fields of type 1 diabetes, type 2 diabetes, obesity and childhood development and growth, as well as specialists in data information and data harmonisation fields. The project has just started recently and outputs are awaited.

Description Intervention Co-creation to Improve Community-based Food Production and Household Nutrition in Small Island Developing States

PI Contribution One in five members of the United Nations are small island developing states (SIDS): 38 countries with a combined population of 61 million. The majority of these are poor, eligible for official development assistance, and over a quarter are 'least developed' countries. They have high burdens of malnutrition, including overweight and obesity alongside anaemia in women of reproductive age, and additionally some, such as Haiti, also have high burdens of childhood stunting. Over the past 3 decades malnutrition in SIDS has been exacerbated by an increasing reliance on food imports, the majority of which are of low nutritional quality. SIDS Governments have committed to increasing the local production and consumption of nutritious food as a way of increasing food security and sovereignty and addressing the high burden of malnutrition related morbidity and mortality. This proposal is intended to add to the evidence base and research capacity to support these policy commitments. The proposal builds on development work undertaken in partnership with the Universities of the South Pacific and West Indies in two SIDS: Fiji and St Vincent and the Grenadines (SVG) respectively. Through this work evaluation methods were developed and pilot data collected. In addition, relationships were established with two local and very well established NGOs, the Foundation for Rural Integration and Enterprise Development (FRIEND) in Fiji and Richmond Vale Academy in SVG. In this proposal we will co-create community based interventions to improve local food production and consumption with these two NGOs and evaluate their impacts on household nutrition and household expenditures. Mixed methods will be used, including quantitative household surveys, qualitative in-depth group interviews and photo voice. The design of the interventions will be informed by preliminary work that identifies and appraises available evidence on current dietary intakes in those settings, and the nutritional value of locally produced foods. The design will also be informed by 'mapping' the food systems in Fiji and SVG. This will be done by engaging stakeholders from across the food value chain in a participatory process, including a workshop, in which the factors, and the relationships between them, contributing to the burdens of malnutrition will be drawn. In addition, to the full development and testing of interventions in Fiji and SVG, new work will be undertaken in Haiti. In Haiti available dietary and nutritional evidence will also be collated and appraised, and stakeholders engaged in mapping the food system. Pilot data will be collected in the Nippes region of Haiti, including on household nutrition and the opportunities for increasing household and small holder food production as a means to improving nutrition. This work will inform the design of an intervention ready for further roll out and evaluation.

SIDS also lack food sovereignty and are highly reliant on food imports. An urgent concern for social and economic development, governments committed to concerted action, and have committed to promoting nutritious local food production and consumption. Civil society organisations are highly active in supporting local communities in subsistence food production to positively affect their health and livelihoods but require substantial cross-sector support for sustainable impacts. Who will benefit: Direct beneficiaries of this project will be civil society organisations who deliver community food production initiatives within the specific sites (Saint Vincent and the Grenadines and Fiji) and beyond across the region (including Haiti). Further direct beneficiaries will be governmental and non-governmental organisation as initiators and supporters of such initiatives. This includes governments in the selected countries but also regional policy-makers, including the Caribbean Community (CARICOM), the Caribbean Public Health Agency and the Pacific Community. Finally, our Caribbean and Pacific research partners (The University of the West Indies, of the South Pacific and State University of Haiti) will directly benefit from this project establishing new partnerships and new learning as part of a truly multi-disciplinary research team. How will they benefit: As an intervention study, the short-term benefit will be to co-develop innovative community-based food production interventions with cross-sector partners to directly support the countries' commitment to promote local food production. The impact plan outlines how these will be engaged. The project also has a strong capacity building aim. There is an increasing interest in multi-level, complex interventions and natural experimental studies to evaluate 'real life' initiatives that tackle upstream determinants of health. The co-investigators and early career researchers will be involved in highly innovative methods development work; this will include local colleagues and postdoctoral researchers. Innovative approaches include participatory systems approaches with stakeholders for developing programmes and participatory visual approaches to involve communities in their evaluation. Researchers beyond the direct collaboration will have access to tools, conceptual frameworks and related guidance. In the more medium term, with the successful establishment of a comprehensive programme of research, we can accumulate robust evidence for the interventions' impacts, including importantly potential unintended consequences at community or household levels, whether social, economic or other. With robust evidence, beneficiaries across sectors will be able to make a case for further collaborative work between researchers and civil society organisations to develop and evaluate such initiatives.

PI Contribution Cardiovascular disease and diabetes are extremely common in the UAE and throughout the Arab World. While cohort studies have made tremendous contributions to scientific knowledge of the epidemiology and determinants of diabetes, cardiovascular disease and cancer, none have been done in Arab populations. To study the causes of these diseases and other diseases common to Emirates, we have established a prospective cohort study, the UAE Healthy Future Study. It will be a resource for researchers throughout the UAE and the wider region with data accessible for all research which will be of benefit to the local population.

Results and Impact Nita Forouhi was filmed for an interview by the BBC for a documentary "The Truth about Fat", that was broadcast on April 2, 2015. The programme has also been shown on various international media since then.
To help shape the content of the TV programme on dietary fat and health, Nita had extensive discussions with a BBC TV journalist by phone and also met with the programme producer to explain the complex science in simple terms, and advised on which aspects to convey in a popular science programme. "The Truth about Fat"" programme was broadcast on 02 April 2015, and NF was able to present some of her programme's recent research on dietary fats in a substantial segment of the broadcast.
Numerous colleagues have contacted Nita to congratulate her on the exciting research from her group and on the clear messages in the programme. At various national and international meetings people come up and identify Nita to tell her how much they enjoyed the programme (some have watched the programme in the Netherlands for instance, others in the UK and elsewhere).

Following this she also provided the journalist with the latest evidence in the form of some key scientific publications so they could use this in the piece on saturated fat and other articles. Results and Impact Big Biology Day. Unit members participated in two IMS stands at the Big Biology Day event held at Hills Road 6th Form College. The first stand was a "Science festival" type stand with hands-on activities for families, for which the Unit provided the "Sugar Cube Challenge" and "Sweet Enough Challenge" activities which we had developed earlier. Unit members (e.g. Professor Nita Forouhi) also contributed to a "faces of the IMS" booklet which gave examples of different career paths at the IMS, and which were handed out at a stand in the careers fair part of the event. The hands-on stand had over 250 visitors, and hundreds of CEDAR and MRC Epidemiology leaflets were handed out. (as well as most of our MRC Rulers and pens!) and queues to take part in activities for most of the day. There was a higher proportion of teens compared to the Cambridge Science Festival in March. There were fewer visitors to the stand at the careers fair, though still over 50 in the course of the day. Subsequently received request for work experience from 6th form student who visited careers stand.

Results and Impact Stand "A healthier you in a digital future"at Cambridge Science festival hands-on event at Cambridge Guildhall, including adults only session on the evening of Saturday 18th. Posters on factors influencing food choice, wearable fitness trackers and their research counterparts, and use of antibodies in our research. Activities included the "Things can only get Bitter" bitter taste test, the Race to 20,000 steps challenge using fitness monitors, and making antibodies out of play-doh and magnets. More than 500 visitors, including more than 300 children, attended our stand over the two days, and participated in activities and talked with Unit staff. The response was very positive. We also gave away over 150 copies of a MRC Cambridge colouring book and flyers for the MRC Festival in June.

Results and Impact Prof Forouhi was part of a small delegation that included the Vice Chancellor from the University of Cambridge and local politicians (the Mayor, James Palmer and MP, Daniel Zeichner) to the Houses of Parliament to meet with MPs and Lords to promote our research and its policy implications, combined with the launch of the "Research Horizons" initiative. She was a "Table Host" for the topic of Health, and several parliamentarians and thought leaders approached that table to discuss what type of research she focuses on and why. This provided a fantastic opportunity to explain the importance of public health focused research, and in particular the relevance of dietary and nutritional research and that of other modifiable behaviours. It was a tremendous platform to disseminate information and promote knowledge exchange on public health with MPs and Lords and others in the audience.

Friday 28th was quiet, with limited footfall and perhaps 20 conversations, except when a party of about 100 schoolchildren attended a demonstration/show and most visited our stall afterwards. Saturday 29th was far busier, with a steady stream of families, couples and individuals. Or volunteers had about 100 good quality conversations, some involving more than one visitor, with about 250 people visiting the stand over seven hours. A Peterborough council transport team member was interested in our work and took away a leaflet for the Propensity to Cycle Tool. Peterborough Today reported on Life Lab, with a photo of the Unit stand.

This was an important contribution to clarify that it is better to stick to the 5-a-day message about consuming at least 5 portions of fruit/veg a day, rather than increasing the guideline amount to 7 or 10 pieces a day, for which there was not convincing evidence and it could potentially reduce overall intake if people think the goal was unrealistic or unachievable.

Following this presentation I was invited to write a review article for a prestigious scientific journal on the topic of my talk. This was published in 2015.

I have been asked to write up the content of my talk into a research paper, which is now submitted. Also, other invitations have arisen on that topic as a result of the talk.

NF explained why it would be better to shift the focus away from dietary fat intake (the nutrient) to considering foods that contain fat as this is more relevant for public health. Following this they collected information from her about differences between dairy and meat as food sources rich in saturated fat, and their health effects. They went on to do one programme on meat intake and health with other researchers, and were thinking about returning with a programme separately on dairy products. The BBC were very appreciative of the information that NF provided.

Results and Impact About 150 children visited the stand and took part in activities, with a similar number of adults over 5 hours. About 70 families left suggestions fro lowering risk of diabetes in the population. We handed out about 100 copies each of the Great Snackington game and MRC Cambridge Units Activity book, as well as about 40 copies of Epigram, our annual newsletter, as well as several other leaflets including FEAT, Propensity to Travel and the MRC Epidemiology Unit trifold. The number of good quality conversations with stand visitors wseemed higher than previous occasions, with volunteers reporting that they were able to discuss the various approaches the Unit takes to studying type 2 diabetes and related disorders. Having activities (including Gt. snackington) that highlighted different areas helped these discussions.

Results and Impact I was interviewed on the BBC1 "The One Show" during a prime time viewing slot in the evening. I was able to get important public health messages out to a large audience about the need to consume sugar sweetened beverages in moderation, based on our findings from the InterAct Study. We were able to reach an estimated 4 million viewers.

Many people were able to understand the potential detrimental effects on risk of diabetes from drinking sugar sweetened beverages regularly. There was positive feedback from many individuals by way of verbal or email communication.
