Maximising the public health impact of a human milk bank

Abstract

Human milk is hugely complex, containing thousands of factors that guide development, protect babies from infections, and train their immune system to recognise harms. Breastfeeding also gives major health benefits to mothers, but these are largely unknown and unexplained. Furthermore, human milk banks, which provide screened donated breast milk to sick infants when a mother's milk supply is insufficient, have been restricted in recent decades, meaning donor milk is now rationed to only the most preterm babies.

The UK has no dedicated centre of research into human milk, and few centres in the world focus on the maternal health impact of breastfeeding. Having founded the first independent non-profit human milk in the UK 2 years ago, I have laid the groundwork to launch an innovative programme of three research projects, each of which will lay the foundations for a larger body of impactful research and clinical advances. The three projects are summarised:

1: Genetic mutations are known to cause breast cancer (e.g. BRCA1 mutations), but much less is known about the epigenetic risk. Epigenetics describes the modifications to DNA that regulate how much of the DNA is turned into proteins. Epigenetic patterns are tissue-specific, so patterns detected in breast cells are different to other tissues such as blood. Previous research, including our own work, has identified epigenetic patterns of breast cancer risk in the DNA of blood DNA. We have found evidence that different cancer risk factors, such as smoking, alcohol and BMI, influence these patterns. However, it is difficult to extrapolate these findings to breast tissue. Breast milk contains a large number of cells from the lining of the breast ducts that are the same cell type that may develop breast cancer. Many of the molecular changes that cause breast cancer may occur long before the cancer has developed and may be detectable in these cells. First, I will recruit more subjects into our research cohort to reach a minimum of 300 subjects. We will assess known epigenetic markers of risk including aging, smoking, alcohol and BMI. This project will provide evidence of early changes in normal breast cells and provide important biological insights into how these increase risk. Finally, this work will provide the evidence needed to create a large study of milk from 20,000 women to develop new cancer screening tests.

2: The World Health Organisation recommends children are breastfed for at least 2 years, but only tiny amounts of data exists on the composition of human milk beyond 6 months. We will recruit 45 mothers to give samples of milk every 3 months over at least 2 years, and profile each sample for the 1000s of fatty acids to understand how milk composition changes, if at all. This information will guide milk donor recruitment, address the value of breastfeeding beyond 6 months, and inform future studies of targeting donor milk for babies with specific nutritional needs.

3: Babies born just a few weeks prematurely, so-called late preterm babies, have a higher risk of death and ill health than full-term babies. They are often a good weight and mostly do not need help with breathing, but still have subtle problems. One of these problems is a lower chance of establishing breastfeeding. Extremely premature babies who receive only mother's milk or donor milk do better in both the short- and long term compared to those who receive formula. Mothers whose babies receive donor milk see it as a 'bridge' to expressing or breastfeeding. We will recruit 120 babies and mothers born late preterm and randomise them to receive donor milk compared to formula with lactation support to assess whether this helps mothers to establish breastfeeding.

The three questions addressed in this Fellowship aim to help improve the UK's breastfeeding rates, which have been among the worst in the world for more than two decades. This Fellowship presents a unique opportunity to drive change.

Planned Impact

Academically, this research fits well within a growing body of interdisciplinary maternal and infant health research relevant to researchers in academia, public health and policy, and across multiple scientific and medical fields, including oncology, clinical genetics, paediatrics, midwifery, public health, nutrition, nursing, microbiology and policy. It will enhance current networks that I established over the course of my bench-based science career and strengthened since establishing the Hearts Milk Bank, and lead to new international collaborations. Findings will be disseminated in leading cross-disciplinary journals and at international conferences.

The combination of research is strongly translational with significant real-world impact for health professionals in multiple sectors and the families they support. The findings from this programme have the potential to change policy and practice, leading to increased breastfeeding rates, reduced rates of maternal breast cancer and more babies and families benefitting from receiving screened donor human milk (DHM), as well as a greater public understanding on the broader maternal health impacts of breastfeeding. In accordance with my work over the last 2 years, I will therefore extend the reach of this work to a far wider audience globally than can be reached via academic publications. From this start, I will ensure that the studies are relevant and feasible for those affected by involving health professionals, parents and policy makers in project-specific advisory group (working with Bliss, HuMBAG and the IPPV), asking for their support in developing our research tools. We will present our findings to them, again asking for their support in helping the optimal dissemination.

Furthermore, findings will be of relevance to:

1) Health professionals and students, engaged through conference and study day talks, university lectures, articles for professional publications and the design of educational and CPD material.

2) Charities and organisations supporting breastfeeding and families by speaking at study days and events and writing blogs and magazine material. I will work closely with the Baby Friendly Initiative, Breastfeeding Network, and UK and international milk banking associations (EMBA and HMBANA) to achieve this.

3) Mothers, infants and families through using social media, parenting forums podcasts (e.g., Parenting Science Gang, iLactation, etc.), parenting blogs and develop animations for broad dissemination, including through a project-specific website.

4) The general public through recognising the impact of wider public knowledge and attitudes and ensuring our messages are relevant to all, not simply those who are going through the experience. I will continue to work with the community interest company 'Tiny Humans CIC' and contacts made in the last year (Nesta, Cambridge Social Ventures, White October, Economist films) to create short films around the links between breastfeeding and cancer risk, and the impact of DHM use.

5) Policy makers through policy briefings, presentations to APPGs, select committees, etc.

Throughout the Fellowship, I will engage with the media through press releases, interviews for newspapers, television, internet and radio, while working with the relevant public advisory groups to determine and cocreate the key messages.

Pathways to impact will be enhanced by ensuring the findings reach those who matter and can be used, where appropriate, to influence policy and practice, and enhanced by the strengthening of local and international collaborative networks.

Description To date, the key findings are:

Donor milk use, which has been rationed for the last 40 years to extremely preterm babies, has been preliminarily evaluated in full term and older preterm babies.

All babies grew well on donor human milk.

Over 60% of mothers using donor human milk as a supplement to their own breastfeeding went on to establish their own milk supply in full.

Mothers reported a significant positive impact on their mental health, with abnormal and borderline abnormal scores for anxiety and depression, respectively, dropping into the normal range in a prospective study of parents accessing milk bank community-based services.

Parents whose infants were able to access donor human milk in hospital and at home reported feeling relieved at the perceived impact on their infant's health, listened to and cared for, supported in their decisions around feeding, and able to cope with complex emotions more easily.

A scoping review of the literature related to donor human milk from 1946 to 2021 highlights the intervention of the infant formula industry, who started to fund studies in the sector within 18 months of the first published association between formula feeding and the onset of necrotising enterocolitis.

An observational study of women with insufficient glandular tissue (IGT) were 2.66-fold more likely to have been overweight as a child than the reference group (95% CI 1.26%-5.73%). This is the first evidence of a potential causal link between childhood overweight and breast development issues that have a negative impact on lactation, and will inform future studies on prenatal counselling and feeding plan development.

A survey of UK medical schools showed that 85% offered 'compulsory training in lactation'. However, only 3% of medical students felt able to assess a latch confidently, which is a key contributor to poor breastfeeding rates.

The work is also focussing on understanding human milk composition, so we can build towards personalising donor milk as a therapy for babies and children with specific clinical conditions. Methodological work has continued in collaboration with others to sample
Exploitation Route A randomised clinical trial is now underway in Newcastle looking at the impact of provision on populations of babies born between 30 weeks and 34 weeks gestation. The milk bank I established and that is supported by this award will be supplying the donor milk into the trial.

I have also launched a prospective study into the impact of milk bank services on maternal mental health, in collaboration with Professor Amy Brown of Swansea University.

Dr Simon Cameron in London is building a programme of research around human milk innovations, including freeze-drying, microbiological testing, and compositional analysis. We cosupervise a PhD student, Simran Cheema.

My work was showcased at the Swansea University Research and Innovation Awards, and discussions have already started between local Health Boards, County Councils and Welsh Government about the future establishment of the first human milk bank in Wales.

I represent Imperial College London at the Microbiome APPG and Infant Feeding Inequalities APPG, and present regularly at the latter.

In 2020 I was invited to a reception at 10, Downing Street, met the Prime Minister Boris Johnson and described my work, and had a number of useful conversations about rolling out further milk bank operations based on the work of this Fellowship. SInce then I have had meetings with and formally advise DHSC, DEFRA and MHRA.

My work forms the evidence currently being considered by the BAPM Framework for Practice for the Use of Donor Milk, which includes a stream on planning future services.

Globally, the model created by my team at the Hearts Milk Bank is serving as a potential model for future milk banking services in other countries, whereby families facing breastfeeding challenges can access short-term donor milk as a bridge to breastfeeding, alongside provision for mothers with no capability to breastfeed. Novel findings of the supportive nature of milk bank services on their mental health warrants further investigation and validation by others.

Description Impacts in 2023: Donor human milk (DHM) is used to feed infants when maternal milk is unavailable or where initial supplementation is needed. Although milk banking services used to be more widely available, no national funding or human milk banking strategy has existed in the UK for over 40 years. Consequently, DHM has become rationed for only the most extremely premature infants (generally those born weighing less than 1500 g) where the risk is higher of developing gut complications with high mortality rates if human milk is unavailable. Some element of rationing exists for all milk banks, but the WHO published recent recommendations supporting all preterm infants to have access to DHM. In Brazil, investment in a nationally equitable milk bank service from the 1990s helped to support sharp increases in breastfeeding rates, alongside a whole-system approach of legislation and community support. Our recent research has shown marked positive impacts where DHM is available alongside high quality lactation support in circumstances beyond the neonatal unit. Findings to date include improvements in parental psychological wellbeing, normalised maternal anxiety and depression scores, and a high proportion of mothers going on to establish breastfeeding who would otherwise have stopped. RCTs are ongoing, but early data and revised WHO guidance as a result in part of our team's work has led to the first UK national guidance through the British Association of Perinatal Medicine, of which I was a member of the Working Party. Implementation of services is now key, with high-quality training needed for healthcare professionals supporting families, and an understanding of when DHM could potentially undermine breastfeeding. Donor milk should only be available under the most extreme circumstances, with lactation support upscaled alongside to meet the majority of clinical and cultural need around the support for mothers and birthing people in the perinatal period. Our work in the FLF extension phase will focus on implementation, with planned programmes across health economics, scalability assessments, and technological innovation across milk bank services. As required by circumstance, my work has entered other necessary areas. For example, human milk banks also play a vital role in food security. During the recent US formula shortages, US milk banks increased capacity four-fold to meet the need of vulnerable infants in hospital or unable to be fed with alternative commercial milk formulas. Currently the UK lacks human milk banking capacity to respond similarly to future formula shortages. The UK also lacks legislation to prevent commercialisation of the sector, with multiple documented ethical and safety harms. There has been a recent UK Health Security Agency product safety alert and FSA product recall from one of the first human milk commercial companies with subsequent involved of Trading Standards and the Chief Medical Officer's office. To meet the needs of all families cared for in hospitals or at home, significant scaling up of UK services is needed. Our modelling studies suggest that approximately 25,000 litres/annum are needed to meet the needs of preterm infants born up to 34 weeks. Current output from NHS-funded human milk banks is estimated at less than 10,000 litres DHM/annum. The Hearts Milk Bank, set up by NHS professionals and academics as part of the Human Milk Foundation, launched in 2018, now has a capacity of 5,000 litres DHM/annum, and is seeking to scale rapidly alongside established NHS infrastructure. UKRI-funded research is ongoing as to the most effective way to implement human milk banking services alongside hospital- and community-based lactation support, aiming to help reverse the UK's status as having some of the lowest breastfeeding rates in the world.

Impact By allowing vaccination of lactating women and others, people who had been previously denied access to vaccination were given the autonomy to make this choice for themselves. My collaborative work that showed functional COVID-19 neutralising antibodies in human milk, and this helped to inform national data collection by NPEU and the Yellow Card reporting system showing that the risk to lactating mothers and their infants were negligible. My expertise in lactation and immune responses helped to provide reassurance,. Sadly the opportunity for wider messaging around the immune response of human milk to protect infants was not adopted by public health bodies, missing an opportunity to give information to families to help them make informed choices..

Impact Increasing government attention has been drawn by healthcare bodies, Royal Colleges, third sectors organisations and the general public to the low rates of breastfeeding in the UK, with increased government investment announced in 2021 for community breastfeeding support. My research programme is laying evidence in the UK for the potential for human milk banking to play a supportive role in helping some women to breastfeed, by providing a much-wanted 'bridge to breastfeeding' for women who are motivated to breastfeed but facing short- or long-term challenges to establishing a full milk supply. It is estimated that improving breastfeeding rates by 1% could lead to annual savings to the NHS and wider society of £341 m. Therefore even small incremental gains in breastfeeding rates as a result of a) access to donor milk being more widely available, b) perceptions of human milk changing as a result, will be of societal and economic benefit to the UK, replicating the success of such services in other countries (e.g., Brazil halved child mortality within 20 years of establishing a national milk bank service).

Title Donor Portal 
Description As demand for donor milk increases, along with demand from women with surplus milk to donate it altruistically, milk bank teams are under increasing pressure to respond. As milk bank operations are scaling nationally, introducing challenges in supporting donors effectively. A fully integrated donor portal would improve interactions and facilitate efficiency and responsiveness, as well as freeing the team to spend more time engaging in face to face and virtual support. The HMB relies on bespoke software system (Li-LAC) developed by Savant to ensure donor anonymity and track and trace the processed milk and store records for 30 years. The portal allows donors either to register themselves, or request to be registered, on the Li-LAC milk bank system, and following the creation of a donor record (with associated information) they would be able to create and access information from the system, including donor events, personal data and additional data. The data exchange would be done via a set of "web services" - a set of procedures that allow for the exchange of secure data between client computers and a central server. This ensures the client machine doesn't require a database installed on the machine, and does not require access to the central database (and is more secure).

Results and Impact The theme of World Breastfeeding Week 2020, coordinated by the World Alliance for Breastfeeding Action, WHO and UNICEF, is Supporting Breastfeeding for a Healthier Planet as a result of the BMJ publication in 2019. I gave a lecture followed by a Q+A session to 614 attendees from across the public health infrastructure of Mexico. The lecture was titled "Supporting breastfeeding as an environmental imperative", building on my publication in the British Medical Journal and highlighting the work I supervised by a team of MSc students from the environmental technology MSc at Imperial College, looking at the carbon footprint of milk banking. The talk highlighted to a varied and senior audience, including the Minister for Consumer Affairs of the Mexican Government the environmental impact of infant formula production, transportation and use, while highlighting the potential for milk banks to support maternal breastfeeding. I anticipate this will lead on to future dicsussions with Mexico in the development of their milk bank service, also in alliance with the Iberico-American Milk Bank Association centred in Brazil and a partner in the Global Alliance of Milk Banks and Associations.

Results and Impact Since it was established in 2019, the Israeli National Milk Bank has been operating at only a third of its capacity. This is because neonatologists are reluctant to use donor human milk rather than infant formula. I was asked by the ministry of health of Israel to present on my work examining the broader impacts of donor milk availability, including in the support of parental mental health and as a bridge to breastfeeding when used in the context of optimal lactation support.

Since the talk, and suggesting other follow up speakers, the usage of donor milk has increased in Israel, and the Ministry are now considering extending the initial 3 year funding period.

Results and Impact Presented for 15 minutes and took part in a 45-min long panel discussion, explaining the research produced by my team and others on the public health impacts of milk bank services, work undertaken to understand how these could be delivered most effectively in the UK, and how a community programme alongside hospital provision means these services can become more cost-effective and impactful.

Results and Impact This engagement piece was published after my article in the British Medical Journal on the environmental imperative to support breastfeeding (Altemetric >500 after a week). This article re-emphasised the role of government in creating policies that meant the >90% of mothers who stop breastfeeding before they choose in the UK can receive better support. Results and Impact My team at Imperial, the Human Milk Foundation and Alive and Thrive developed a campaign that brought together for the first time milk banks from every populated continent to create a film that went viral (viewed over 180,000 times across 42 countries across all platforms). The film showcased the work of human milk banks, and how greater support could lead to better outcomes for mothers and infants.
