Developing an mHealth intervention to strengthen the community mental health system in the context of the COVID-19 pandemic in Peru

Abstract
The COVID-19 pandemic and the measures of mandatory social distancing presented a challenge to the Peruvian health system, which is not equipped to provide remote care to patients. This situation worsened an already fragile health system, limiting the population's access to adequate care. People with mental disorders are particularly vulnerable in this context, due to the impact of the pandemic on their mental health, as well as the limitations in accessing mental health services.
This proposal aims to (1) assess the impact of the COVID-19 pandemic on the provision of mental health services in Peru, (2) identify the strategies used to address this situation, particularly those involving mobile technology, as well as the main advantages and challenges faced, (3) design and test an intervention based on mobile technology (i.e. mobile phones, tablets) to provide mental health care, with the input from health providers, patients, and policymakers.
To achieve our objectives we will conduct a 2-year project with 3 phases: (1) Phase 1: We will examine the impact of the pandemic on the public mental health services and the strategies used to continue the provision of care, through the review of guidelines and protocols, as well as interviews with the patients, health providers and policymakers; (2) Phase 2: We will design and develop our technological intervention based on the information collected in the previous phase. In this process, users, health providers, managers and policymakers will provide their opinions to make the intervention acceptable, feasible and adaptable to the Peruvian public mental health care system; (3) Phase 3: We will perform a pilot study to field test the developed intervention in 3 community mental health centers in Lima, Peru.
The results of this 2-year project will help us to understand the viability of our idea, and potentially an implementation at a larger scale. Additionally, we will organize regular meetings with mental health policymakers to share our findings and discuss how they could be integrated within the public health system. Our research team, consisting of researchers from Peru and the United Kingdom, will also share the project results with an international audience, through conferences and publications in specialized journals.

Technical Summary
The COVID-19 pandemic and the mandatory social distancing have made visible the challenges the Peruvian health system faces to provide remote care. People with mental disorders are particularly vulnerable in this context, due to the impact of the pandemic on their health, as well as the limitations in accessing services. As a result, the Ministry of Health in Peru published guidelines to providing mental health care through telemedicine.
In this context, which is simultaneously a great challenge and opportunity for the community mental health system in Peru, we proposed a two-year project with the following objectives: (1) To assess the impact of the pandemic on the community mental health system in Peru, (2) To identify the strategies implemented to preserve the provision of mental health care during the pandemic and assess, using the Learning Health Systems framework, the readiness to deliver remote mental care, (3) To co-design and evaluate the performance of an mHealth intervention to deliver remote care within the community mental health system in Peru.
To achieve this, the study comprises 3 phases: (1) the formative phase, centered around the first two aims, will involve the review of local and international guidelines and evidence produced during the pandemic and a qualitative study with stakeholders. (2) The intervention development phase to co-design along with stakeholders the mHealth intervention based on the formative phase. Finally, (3) the pilot of our intervention, assessing implementation and clinical outcomes.
The pilot will recruit 60 participants (30 users and 30 relatives) who will receive the intervention delivered by 15 health providers. Recruitment will take place at 3 Community Mental Health Centers. Data will be collected at 3 different time points: (i) before starting, (ii) during and (iii) after finishing the intervention, through questionnaires, in-depth interviews, as well as data generated through the mHealth platform.

| Impact | The workshops conducted resulted in a set of prioritized outcomes and tools to assess the recovery of CMHC patients. These outcomes and tools were commonly selected by different stakeholders involved in the recovery process of people living with mental disorders (e.g. CMHC patients, practitioners, and decision makers). With these outputs, the MoH will be able to measure the results of their treatments in the recovery of patients. Addittionally, these workshops helped to raise awareness in policy makers about what recovery looks like for mental health patients and practitioners, understanding their own experiences and attitudes. |

| PI Contribution | We are collaborating with the Peruvian Ministry of Health in their implementation of Electronic Health Records (EHR) in the community mental health centers (CMHC) of Peru. Based on our project components (e.g., formative study, co-design, and pilot study), we contribute our expertise and intellectual input in research methods to collect data about the implementation of the EHR through the lens of a health systems framework. First, using questionnaires, we explored the implementation of the EHR in the first CMHCs using it, to learn from this experience and inform the national scale-up. Second, we co-prioritized with key stakeholders (i.e., patients, health workers, decision-makers) a group of outcomes that will be incorporated into the EHR to assess the progress and results of the patient's treatments at the CMHCs. Finally, we conducted a pilot study to assess the implementation of the EHR and explore its usability, feasibility, adoption, and acceptability, among other implementation outcomes in four CMHC by questionnaires, revision of clinical record, and semi-structured interviews. |
| Collaborator Contribution | The Ministry of Health contributes by designing the EHC (technology development) and coordinating its implementation in CMHC in Peru. They also gave us access to the data and health facilities to enable our data collection and analyses. Lastly, we interviewed them as part of the process evaluation of the EHR implementation and will inform them about its results so that they can use this information in their national scale-up. |

| Results and Impact | As part of the implementation of the Electronic Health Records (EHR) in the community mental health centers (CMHC), the Ministry of Health (MoH) prioritized 55 CMHCs to use the EHR routinely. To do so, the Mental Health Directorate in collaboration with the Technology Office of the MoH, trained the CMHCs' workers to use the EHR. These trainings were virtual and in-person, with representatives from each CMHC prioritized. The research team participated in various coordination meetings with the MoH to support their training strategy, and the joint their trainings to collect how they were conducted. Lastly, the research team sent some questionnaires to the CMHCs' workers to collect their experiences with the training received. |

| Results and Impact | The Principal Investigator met with a representative from the Pan American Health Organization (PAHO) involved in mental health surveillance in the Americas to present our project. The representative was highly interested in the implementation of the electronic health records in the community mental health centers of Peru, and he invited us to share our results in a further meeting with a bigger audience from PAHO. We have arranged a meeting on Mid-April to share these results. |

| Results and Impact | Since February 2023, we have presented our project to four Health Directions in Lima, Peru. The Ministry of Health began the implementation of the Electronic Health Record (EHR) in eight community mental health centers (CMHC) in Lima, corresponding to four Health Directions, which had to authorize our work in the CMHCs. To date, two of them have approved our activities and enabled us to contact the CMHCs of their jurisdiction. We are still waiting for a response from the other two Health Directions. |

We had meetings with the Mental Health Directorate, which overviews the provision of mental health services nationwide, and the General Office of Information Technology, which is in charge of developing and implementing the EHC in primary care settings, including the CMHCs. Afterward, we began collaborative work with both Directions to support the implementation of the EHR in CMHC by collecting and analyzing data that will inform their decisions and improve the uptake of the EHR in mental health services. |

| Results and Impact | From September to October 2022, the research team coordinated with the Peruvian National Institute of Mental Health the implementation of their software, named "MentalCom", to provide remote mental health care in Peru. This software was designed for community mental health centers (CMHC) during the pandemic, but the lack of funding to support its continuation stopped its use. When we met them, they used this software in their institution only, so we presented our project and offered to collaborate with them to assess the implementation of MentalCom in CMHCs. However, this collaboration did not continue because the Ministry of Health approached the Institute to communicate that they would implement Electronic Health Records (EHC) in primary care services, including the community mental health centers. Since this EHC would be used throughout the Peruvian health system, the Institute deemed that our project would be of better use to the Ministry of Health's initiative. |
