Development and Evaluation of a Training Package to support the Remote Assessment and Management of People with Movement Impairment and Disability

Abstract

Many people experience physical disabilities throughout their life, requiring advice and management from clinicians such as physiotherapists. Usually a hands-on detailed movement assessment is taken to help formulate a plan to deal with each individual's needs. For most this face-to-face approach has not been possible during the Covid crisis, and this is likely to continue for some time with social distancing, particularly in older people and those with health issues. In response to the crisis clinicians have found new ways of working. They have used telephone & web-based consultations (known as telerehabilitation) to help people continue to rehabilitate at home. This approach, however, has many challenges when assessing people with movement disabilities, such as those experienced by people recovering from Covid-19, with long term neurological conditions, arthritis or diabetes. For example there are risks associated with assessing balance and mobility in frail older persons at risk of falls. Yet this information is essential to create a plan to ensure recovery occurs as quickly and fully as possible. Unfortunately relevant guidance and training for NHS/Social care staff about this is extremely limited, relying mainly on experience gained on the job. No specific guidance is in place to ensure telerehabilitation is delivered effectively and equitably for people with physical disabilities. We will work closely with NHS/Social care staff to quickly develop an assessment toolkit and training to provide them with practical guidance to increase their skills and confidence. This is important now & for the future.

Technical Summary

The problem:
Physical disabilities are common throughout life. Usually, hands-on detailed movement assessment results in a targeted rehabilitation plan, with ongoing evaluation required as needs change. During Covid-19, many people, of all ages, have received no rehabilitation, and rehabilitation need is escalating with recovering Covid-19 patients; creating a "tsunami of rehabilitation need". In response, clinicians are rapidly adapting and creating telerehabilitation solutions to manage caseloads and prioritise those at risk of deterioration, with little specific guidance, training or support. Professional bodies and clinical networks highlight the marked variations in approaches used, expressing concerns about potential inequity and inefficiency. Our rapid literature review underlines this problem, citing lack of specific guidance/training as barriers to effective telerehabilitation implementation. This has immediate and long-term relevance given the "new-norm".

Research aim:
Develop a Toolkit & Training package for the current/future workforce, wherein telerehabilitation will be integral to rehabilitation assessment and management.

Study Design:
Knowledge to Action Framework, methodology commonly used in implementation science. Key steps include: (i) literature review to inform guidance/provide examples of best practice; (ii) national online survey to identify clinicians' needs, (iii) production and evaluation of the Toolkit through iterative cycles of data collection, evaluation and refinement (initially in the SouthWest); (iv) national roll-out/evaluation, (v) production of final training package for use within clinical/educational arenas.

Study outputs:
Practical guidance, process pathways and training to optimise the confidence and competence of clinicians to effectively implement telerehabilitation in people recovering from Covid-19 and those with physical disability from across the lifespan.

Description The results of our scoping review confirmed there is minimal specific guidance available to inform telerehabilitation assessment via video in people with physical disabilities/movement impairment, with recommendations that further guidance and training is needed to optimise implementation.

The results of our national survey of UK health and social care practitioners demonstrated that the vast majority (84%) or practitioners reported they had used video-based consultations, typically supported by telephone and e-mail. Of these, 71% had experienced practical problems with seeing or hearing patients remotely in order to conduct an effective assessment. Additional obstacles commonly experienced included technology-related issues such as poor internet connection (87%), lack of patients' access to technology (73%) and low digital literacy in patients (72%). Practitioners reported that family members and carers played an important role in helping to overcome these obstacles, by providing technological or physical support during assessments. Of the 247 respondents, only 46 (19%) had received formal training in conducting remote consultations.

Our service evaluation highlighted that the "chaos" experienced in the earliest days of the pandemic subsided relatively quickly and practitioners adapted services to conduct remote appointments within a matter of weeks. Practitioners have since continued to learn by experience and improve how remote services are run. They expressed some concerns about the effectiveness of remote physical assessment, particularly the reliability of outcome measures, but acknowledged that the uptake of remote working was a long-needed change. Many reported that patient non-attendance rates fell as they increased their remote appointments. Patients recognised the necessity of remote appointments and saw this as a useful part of a larger treatment package, particularly in the long-term management of chronic conditions. The convenience of not needing to leave one's home for an appointment was viewed as being helpful particularly in rural areas where long travel times were viewed as a barrier to treatment. Practitioners and patients expressed concern about remote appointments being inferior to face-to-face appointments but this concern was usually (but not always) rendered moot after a successful remote appointment.

Although practitioners and patients identified obstacles to remote assessment, particularly when directly compared with face-to-face assessments, there was general agreement that it is a useful, convenient way to offer patients assessment and treatment. There was emphasis by practitioners that commensurate time and training is required to adapt to this shift. Also highlighted was the need for reassurance to patients that remote assessments can provide a convenient, reliable service that offers some benefits over face-to-face options such as joint consultations with multiple practitioners.

Impact It is anticipated that the Telerehab Toolkit will help to improve the knowledge, skills and confidence of a range of health and social care practitioners in telerehabilitation and specifically in conducting remote physical assessments. The resource is also available for access by patients and the public, with relevant guidance for these groups. The Toolkit is being disseminated widely to approximately 35 professional networks, 77 UK universities, and 213 UK health Trusts. The website receives approximately 1,750 views (1,250 unique views) per month, and although it was designed primarily as a UK resource, reach has been wide with access from Europe, Asia, North America and Australasia. Feedback from practitioners and patients via the research methods and via e-mail (ongoing) has been encouraging and suggests there has been a positive impact on knowledge, skills, attitudes and behaviour towards telerehabilitation.

Results and Impact Prof. Jenny Freeman presented at a symposium in collaboration with researchers at the Universities of Manchester and Oxford. The presentation was entitled: "The Digital transformation in physiotherapy: Ready, willing and able?". The audience was approximately 1200 physiotherapists.
