Abstract
The project address the 'urban challenge' of delivering better, more cost effective healthcare in cities by enabling better resource planning based on data. It will develop a 'proof of concept' (POC) planning support solution for hospitals and health facilities using anonymised, aggregated mobile network data (extrapolated to provide insights representative of the total and segmented population in an area, including demographics, footfall and movement), together with health sector data (e.g. types of condition linked to demographics, environmental factors) and environmental data that can impact on health (weather, air pollution forecasts etc). The project will be led by O2, building on their Dynamic Insights platform, with Cambridge city and Addenbrooke's hospital providing the use case. This approach has been successfully applied in other sectors, such as retail and transport where it has enabled improvements in the nature and location of offerings based on detailed knowledge of the customer demographic. This will be the first time it has been used in the health sector. Successful demonstration of the POC (and supporting business models) will lead to commercial roll-out across the UK.
Planned Impact
The main beneficiaries from this project will be NHS A&E departments, and UK health boards
Demand on healthcare services is growing due to changing demographics (ageing population, migration to cities, movement of people within and between cities for work/social reasons etc). Combined with public spending limits this is leading to stresses on health and care services. Healthcare providers have only limited ability to predict peaks in demand at services such as A&E, GP surgeries etc. This leads to resourcing difficulties, bottle-necks, delays and increased costs. The situation could be partly addressed through improved planning, such as the ability to establish local drop-in clinics in strategic locations, encouraging better use of GP surgeries and improving capacity planning at A&E. However, to do this effectively, reliable data for factors linked to demand for services are needed (e.g. movement of people over time, routes taken by people visiting health facilities, weather, air pollution, links between illnesses & demographics). There are 180 A&E departments and 8,000 GP surgeries in England (plus walk-in centres and minor injury units). Demand is rising. Demographic pressures to 2017 will cost £1.1-1.4 bn extra pa to maintain quality (King's Fund). 18.3 million accident and emergency attendances were recorded at major A&E departments, single specialty A&E departments, walk-in centres and minor injury units in England in 2012/13; an increase of 4.0% from 2011-12. In Oct-Dec 2014 7.4% of patients waited more than 4 hrs from arrival in A&E to discharge, admission or transfer (47% up on the previous quarter and the poorest performance since quarter 3 of 2003/4). On average there are about 10,000 admissions each day. The cost of a hospital bed is c.£500 per night. One benefit of the application of the Dynamic Insights model to A&E would be to reduce hospital admissions. A 10% reduction in admissions (1,000 daily across England; about 5 per A&E) would lead to savings of £500,000 each day (£190m pa) if used in all A&E departments. We are targeting 20% of A&E departments over 5 years (£40m savings pa). Additional savings will occur in other parts of the UK and in walk-in clinics etc. Dynamic Insights will cost £50-100k pa for each hospital/trust (40-50 targetted in 5 yrs leading to an ROI of up to £5m pa).

| Description | 1. Under legislation in force at the time the project was conducted, the majority of secondary uses of CDR data (provided that this was conducted at macro level with aggregated data) would be legal. Since the introduction of the General Data Protection Regulation and the UK Data Protection Bill, the position has become less clear. Whilst the contract between a mobile phone operator and a customer does give legitimate grounds for some data processing, it is unclear whether use of data for ancillary purposes such as secondary analytics could reasonably be described as necessary for the performance of the contract. Specific legal guidance on a case by case basis is recommended, and in all cases privacy notices should be employed. 2. The public engagement workshops conducted within the project indicated that the majority of participants were happy for CDR data to be used for health research provided their annoyminity was safeguarded. There was more ambiguity about other uses of the data. 3. The data modelling work undertaken in the project showed that where aggregated CDR data was used in isolation there was a low risk of an individual's privacy being compromised. The risk increases significantly where CDR data is combined with other datasets, but this can be overcome in most cases by the introduction of a robust governance framework for handling the data. |

These offer an overview of legal issues, general good practice and can be used to inform future study design for projects using mobile phone data. |
