Proactive Primary Care With Multimodal Data Integration

Abstract
This project aims to explore AI-driven approaches that may lead to innovative primary care services. We will make use of various forms of data, including NHS data and social care data. By exploring the added benefits of integrating primary care, secondary care, and social care data, we will showcase how human-centred explainable AI methods can bridge data silos and improve efficiency in healthcare. The outcomes of the project may eventually reduce healthcare costs through proactive predictions of care needs.
Our key objectives are:
Develop machine-learning models to predict clinical events (such as hospitalisation) for patients with chronic conditions, based on primary and secondary care data.
Investigate the integration of social care data with health care data to identify early indicators of health needs.
Develop user-dependent explainable AI interfaces tailored to patients and health professionals, enhancing collaboration in healthcare decision-making
