Construction of an Imaging Centre of Excellence (ICE) at the new South Glasgow University Hospital

Abstract
The synergy between imaging and genomics technologies has been recognised as being an important driver to the future development of Stratified or Precision Medicine. The benefits of stratified medicine are now globally recognised, and Scotland is uniquely placed to capitalise on this potential, owing to its strong academic research base, advanced electronic health records within the NHS, and a strong industry presence in genomics, bioinformatics and diagnostics. The South Glasgow University Hospital represents an investment to Glasgow and Scotland in the region of £1Billion. When it opens in 2015, the new hospital will be the largest in Western Europe and will include maternity, paediatric and adult services on a single site. The Imagine Centre of Excellence (ICE), located at the new hospital, will uniquely accommodate a 7 Tesla MRI scanner together with other imaging modalities for clinical and research use, including 3T MRI and high resolution multi-slice CT, all networked and sharing staff with the large number of service scanners within the main hospital. This state-of-the-art equipment will be housed alongside clinical academic imaging specialists, NHS clinical physics expertise, innovation space for industry, and potential for expansion of precision medicine activities.The University of Glasgow led the development of the Scotland-wide £20M Stratified Medicine Scotland Innovation Centre (SMS-IC), which will be based adjacent to the ICE building. The co-location of ICE and SMS-IC will therefore provide significant added value to both centres.

Technical Summary

The benefits of stratified medicine are now globally recognised, and Scotland is uniquely placed to capitalise on this potential, owing to its strong academic research base, advanced electronic health records within the NHS, and a strong industry presence in genomics, bioinformatics and diagnostics. Planned Impact
The ICE building is being designed to allow direct ambulatory access for patients, and the building will be linked to the new Clinical Research Facility to allow clinical trials to benefit from the advanced imaging technologies. The Glasgow MRI facility will offer a resource that will be uniquely valuable for researchers across the country, and central to its goals will be the application of advanced technology to understanding of human disease. The ability to undertake the most advanced imaging across the entire spectrum of clinical problems, encompassing acutely unwell or dependent patients, is a key principle of the research imaging facility.
Importantly, NHS Greater Glasgow & Clyde's catchment area covers a population of 2.8 million, and represents one of the most epidemiologically problematic areas of the UK including high co-morbidity and chronic disease. Therefore the clinical environment is certainly unique within the UK. Of course, for maximum clinical impact any imaging facility needs to be embedded within the hospital environment - so, for example, a 7T MRI scanner based in Oxford would not be of use to the large cohort of stroke patients in the West of Scotland. However, our ability to network with other 7T sites in the UK, as these develop, will contribute to the development of 7T skills, knowledge and expertise across the UK.
Ultimately the impact of ICE will extend to the whole patient population through the development of innovative diagnostics and medicines. It will also ensure that the new hospital becomes a UK exemplar for life sciences industry. NHS Greater Glasgow & Clyde already has the largest Clinical Physics NHS department in the UK and the largest imaging modality assets in Europe. PI Contribution | Using clinical samples obtained in the ORBIT trail, the research team generated the initial bioinformatic signatures (from RNAseq data) that were able to stratify rheumatoid arthritis patients into responders and non-responders to biological therapies. This enable the research team to create a new gene list for testing in the BioSpyder TempO-seq platform. |

The rationale for this is that to translate our initial bioinformatic signatures into a useful clinical test requires us to use a assay that has the potential to be used in the clinic. Results and Impact | Objectives: • To showcase the University of Glasgow's innovation activities and provide Innovate with a platform to launch Scotland's engagement with the Industrial Challenge Strategy Fund • To broker introductions between academics, industry and NHS through facilitated workshops and match making • 155 delegates attended (89 academic, 66 NHS and Industry) • 4 Interactive workshop sessions including 2 tailored around identified training needs and opportunities for collaboration • Provided a forum for engagement of industry with Scotland's BEIS Science and Innovation Audit on Precision Medicine • One attending SME has re-located to Glasgow's Clinical Innovation Zone and partnered with the University to bring an international IEEE Conference on "Blockchain for Clinical Trials" to Glasgow - attracting 170 registered attendees. Industry Day Workshop "Industrial Strategy Challenge Fund - Developing Connections" Objectives: • To provide an overview of the Industrial Challenge Strategy Fund, in particular the 'Data to early diagnosis and precision medicine challenge'. • The opportunity to present Offers (e.g. expertise, technologies, approaches, cohorts, infrastructure) to enable identification and development of potential consortia to apply for ISCF funding. • The opportunity to identify specific Needs (e.g. skills, products, expert advice, access to data, samples etc) to support current activities/potential consortia aligned with ISCF funding opportunities. • The opportunity to meet and talk with partners from different sectors, and consider potential collaborative funding bids. Outcomes • Needs and offers presented by 20 participants facilitating discussion and further networking • A pan Scotland consortium was established and successfully bid for the ISCF Centres of Excellence in Digital Pathology and Radiology - resulting in £15M of funding for a consortium of 17 partners from Industry, NHS and Academia. |

The physical installation of the 7 Tesla scanner was a dramatic event and gained extensive TV and press coverage in Scottish TV stations and the regional press. In terms of raising the profile of the regard the event has been an outstanding ICE centre and by extension the wider Queen Elizabeth University Hospital including our clinical innovation zone. |

Results and Impact | Project planning and development day The purpose of this all day event was to bring together all of the internal and external stakeholders connected to and active in the delivery of the Imaging centre of excellence objectives. This comprised a selective group of academic NHS industry support staff with an active presence from the City of Glasgow. The day opened with an overview of the vision for ICE from industry partners Siemens and Professor David Porte, newly appointed Chair of MRI Physics. It then split into sub groups aligned specifically with the delivery work streams of the project encompassing the scientific and economic delivery work-streams with the overall ICE project plan. These engaged with a wider group of prospective users and advisors and addressed and stress tested a wide variety of issues related to delivery (technical issues, public engagement, industry partners, risk analysis etc). These sessions were moderated by an external facilitator and the results collated and will into a significant revision of the on going ICE delivery plan to be presented to the Board April 17. |

Results and Impact | This visit hosted by Professor Dominiczak and welcomed senior Scottish Government Ministers Shona Robison MSP and Michael Russell MSP to the new Imaging Centre of Excellence in early February 2017. Ms Robison, the Scottish Government Cabinet Secretary for Health, and Mr Russell, the Minister for UK Negotiations on Scotland's Place in Europe, first met with Vice Principal and Head of College, Professor Dame Anna Dominiczak, and were then given a tour of ICE by Dr Carol Clugston, the Chief Operating Officer of MVLS.The MSPs were also there to discuss the potential difficulties posed by Brexit and the implications for the NHS and EU funding for NHS and research collaborations. The visit provided and excellent opportunity to impress upon these senior policy makers the long term vision and impact of the ICE project and influence policy at a Scottish Government level. |
