Assessment of the feasibility of a comprehensive web-based system for patient self-management in Type 2 Diabetes

Abstract

Type 2 diabetes (T2D) is a pandemic disease with major health and economic consequences. In the UK, 2.6 million people have T2D, with an annual cost of care of £8.8bn. Patients with chronic diseases engage in behaviours that affect their health, and disease outcomes depend to a large degree on self-management. The Department of Health has stated “The Government’s ambition is to achieve healthcare outcomes that are among the best in the world. This can only be realised by involving patients fully in their own care…” As the availability of tools for patients to do this in an effective way is very limited, MapMyHealth is developing an innovative and comprehensive web-based patient self-management system for T2D. This project will assess the feasibility of the approach by building a test-model system for T2D patient self-management and evaluating it in a Primary Care setting.
