Sourcing Community Solutions to Antibiotic Resistance in Nepal

Lead Research Organisation: University of Leeds
Department Name: Sch of Languages, Cultures and Societies

Abstract

This project will develop, pilot test and evaluate a high-quality intervention aimed at preventing and controlling antibiotic resistance in Nepal. Community-led solutions to the growing problem of antibiotic resistance will be promoted through a participatory digital film-making intervention that will lead to a health-education campaign within communities, and to an advocacy campaign targeting policy makers. This will directly support the delivery of the UN's Sustainable Development Goals, in particular SDG 3: 'ensuring healthy lives and promoting well-being for all at all ages'.

In Nepal, antibiotic-treatable infections are a significant public health burden. A recent review of studies examining antibiotic resistance to common bacterial diseases in Nepal indicates that antibiotic resistance is a growing threat to public health (Basnyat 2015: 6-10). Indeed globally, as the World Health Organisation warns, "without urgent action we are heading for a post-antibiotic era, in which common infections and minor injuries can once again kill" (WHO 2016). Although the development of new medicines is critical, without behaviour change antibiotic resistance will continue to be a major threat to population health. Our project seeks rigorously to evaluate the potential of participatory arts interventions in supporting such behaviour change.

The project has three phases. Phase One will focus on the development of methodologies of co-production and participatory film-making, a health-education campaign, and an advocacy campaign. Phase Two will focus on pilot-testing the participatory film-making, health-education campaign and advocacy campaign in one urban and one rural setting, and on evaluating the approach for reach, implementation fidelity, and acceptability to a variety of stakeholders. Phase Three will focus on refining the approach prior to scale-up, evaluation of outcomes and impact.

Our specific intervention consists of four stages:
1) The co-production of short comics (drawing on the 'grass-roots comics for development' methodology) which identify critical barriers to preventing and controlling antibiotic resistance at the individual, household and community levels, and which develop community-led solutions to overcoming those barriers.
2) The development of these comics into documentary films that can be utilised both as health-education tools, which are appropriately tailored for the locality, and as advocacy tools, which are aimed at policy makers at the district and national levels;
3) The utilisation of the documentary films in a health-education campaign within localities;
4) The delivery of an advocacy campaign at the district and national levels.

Working at the intersection of humanities, social sciences and public health, and bringing together the expertise of The Centre for World Cinemas and Digital Cultures (Paul Cooke, PI), the Nuffield Centre for International Health and Development (James Newell and Rebecca King, Co-Is) and HERD International (Sushil Baral, Sudeepa Khanal, Co-Is), and working in collaboration with the Ministry of Health and Population in Nepal, this partnership will pilot an innovative approach to dealing with a significant public-health issue. Drawing on participatory-arts methodologies of co-production with community stakeholders, we aim to explore the powerful socio-cultural, political and economic issues at play in the misuse of antibiotics and support the development of community-led solutions to this issue, thus helping to increase the effectiveness of, and adding significant value to, government-led campaigns. The project will, moreover, help build capacity for the use of participatory arts as a tool for public-health campaigning, synergising the expertise of the project team to produce an outcome that none of the partners could achieve on their own.

Planned Impact

Our aim is to have impact both on the ground on the behaviour of the communities we will be supporting and on district and national-level policy. Engagement with non-academic stakeholders has been central to our project's design and is central to our impact strategy.

From the outset, we have discussed the aims of this project with the Nepal Ministry of Health and Population (MoHP), with whom we have a long-standing and trusted relationship. Nuffield and HERD have reputations in Nepal for delivering research findings that address priority problems and are policy-relevant, and for committing to supporting large-scale take-up of interventions that we have demonstrated to be effective.

In developing this project, we have held discussions with Mr Bhogendra Dotel, Director - MoHP, Policy Planning and International Cooperation Division, Mahendra Shrestha, Chief Public Health Administrator, MoHP, District Public Health Office Kathmandu, Rajendra Prasad Pant, MoHP Assistant Director, Department of Health Services, Kathmandu and their teams, who are the appropriate policy-makers and programme implementers in MoHP. Avoidance of antibiotic resistance in Nepal is a high MoHP priority (Government of Nepal 2015).

To ensure that the intervention we develop is acceptable to the intended users (that is, the general population), and hence more likely to be effective, we will obtain information from members of communities including locally elected representatives involved through interviews and focus group discussions to ensure that their knowledge and experience informs intervention design. Issues of gender and equity, to which MoHP is committed (Government of Nepal 2015), will be critical here, so we will take care to involve both men and women, and identify and involve other key equity groups.

Equally key to achieving impact are the concepts of feasibility, appropriateness for context, scalability and sustainability. To ensure that the intervention we develop is feasible, appropriate to the context, scalable and sustainable, and hence more likely to be adopted nationally, we will closely involve MoHP and District Authority officials and their development partners (who play a vital role in shaping policies and resource distribution) in design of the intervention. They are best placed to know the realities and constraints of the health system within which the intervention must be integrated. Ensuring their ownership of the project is key to facilitating impact, and care will be taken to ensure their opinions are respected and guide intervention development. The campaign package will include guidelines, materials to be used by presenter/facilitators, training materials, and training methods, to ensure that we provide an integrated package ready for assessment and use without further modification.

We will mirror implementation as it would be if going to scale, to avoid any 'demonstration site effect' of funding, human resourcing or monitoring that would not be feasible at scale. To inform future assessments, we will investigate how to measure multiple potential outcomes - in a complex intervention such as this, assessment of effectiveness and cost effectiveness will not be sufficient, since there are also likely to be wider impact benefits/dis-benefits related to community cohesion, gender relationships, health ownership etc.

To monitor our impact activities, which are integral to the whole project, we will hold regular virtual meetings between partners. This will ensure any problems are identified early and mitigation can be initiated early.

Title CARAN delivery manaul
Description The CARAN delivery manual is a research tool to guide the re-production of this intervention and promote its scalability. The manual is available in hard copy or as a pdf file in English and Nepali (although wider translations are being considered for the future). It describes all activities in detail and list the resources, space, time and personnel requirements for each. The manual is designed for the use of participatory video to explore AMR however it can be modified to tackle other One Health issues such as climate change and eco-anxiety as explained in an additional entry (see engagement activities).
Type Of Art Artefact (including digital)
Year Produced 2019
Impact The manual is currently being utilised by a PhD student who will be investigating the role of gender in AMR behaviours and adopting participatory video as her engagement tool. This will result in a better understanding of AMR related behaviours and allow gender-specific interventions to be actioned if necessary. The manual has also been utilised by a project using participatory video to explore people's perception of their natural environment. The impact of this has been a better understanding of how communities in semi-rural areas value and engage with nature, allowing more specific interventions to project environmental features to be actioned. Activities from the manual (particularly those in the ice-breaker section) are now regularly used by the project team at conferences and events to warm up the audience and create a more open and trusting working environment.

Title Nisha's Story
Description Nisha is a Voluntary Female Health Working, working in Chandragiri Municipality, outside Kathmandu. This 20 minute documentary shows how she's working to stop her community misusing antibiotics. This is a massive problem that is leading to antibiotics no longer working, which could lead to a world where infections can't be controlled. For many in the medical world, this is a bigger threat to humanity than climate change.
Type Of Art Film/Video/Animation
Year Produced 2020
Impact The film has been submitted and shown at a number of global film festivals. It was a finalist at the Eurasia International Monthly Film Festival and Mindfield Film Festival, Albuquerque and won the award for best short documentary at the Model N Movie International Short Film Festival and The Impact DOCS Awards

Title Post-intervention participant experience film
Description Professor Paul Cooke is currently working with the Nepali-based team to develop a short follow-up film which re-connects with CARAN participants to discuss their experiences, learning and potential attitudinal change around AMR since being involved with the CARAN project. The film-making is being led by the same film-making who engaged throughout the CARAN project from our Nepali-based partner organisation HERD international. He and Professor Cooke are creating and editing the film together with input from the community participants.
Type Of Art Film/Video/Animation
Year Produced 2020
Impact The film has not yet been finished but the intended impact is to reach a wider audience within our research community including local government, policy makers, NGOs, the general public and schools children. We hope that the film makes the issue of AMR more approachable and tangible to these community members but also encourages them to take their own actions against it based on the examples of CARAN participants within the film. Wider impacts may also include making research more approachable to community-level individuals within Nepal, long-term this may help other research teams to recruit diverse participant pools for similar creative or health-based research projects.

Description The CARAN project has been a wonderful example of how interdisciplinarity can allow people both to play to their disciplinary strengths and yet develop an innovative approach to an established research problem. The disciplinary expectations of public health are very different to those of participatory video. All participants engaged in a detailed process of negotiation, where every member of team had to continually revaluate what they considered to be the core purpose of the project. Initially, I thought that this was going to be very similar to work I'd undertaken previously. The community would define how they wanted to engage with the topic and this would change with each group we worked with. There would be little need for a detailed 'manual' and the project would develop iteratively in response to the creative direction the groups wished to go in. This, I think it's fair to say, made our colleagues in public health somewhat nervous. They were particularly concerned that an entirely community-led filmmaking process could lead to the spread of misinformation about ABR. They also initially saw the project primarily as about data-collection on attitudes to ABR, rather than about how film can generate advocacy. Through detailed discussion, where everyone involved was very keen both to see the collaboration work, and also to ensure that their core disciplinary values were maintained, we developed a working method that we feel will genuinely bring something new to the discussion about the role of participatory arts within public health. Through the use of film, we feel that the project has genuinely provided new research insights, foregrounding the specific cultural barriers to antibiotic misuse within specific communities that are not generally engaged in policy development. Our project has provided a way of engaging communities on their own terms, while also ensuring that the project is rooted in an understanding of the facts about ABR. This, in turn, has allowed us to develop a manual on the specific value of participatory video in public health campaigns. At the same time, we have also investigated how social science methodologies can work together with arts practice to engage policy makers. In so doing, we are now discussing the thorny issue of 'scalability', exploring the value of 'micro' knowledge in a world of 'big data'. This was a focus of the workshops on days 2 and 3 of our international event in Kathmandu in June 2019 where projects using creative, community and participatory approaches to health problems came together to discuss developments and challenges in the field. Key to our thinking in this project has been a reflection on the nature of 'equitable partnerships' within a participatory project, where all members of the team, from public health specialist to filmmaker to community participant understand why they want to participate in the project, what they bring to the table in terms of expertise and what they hope to gain from it.
Exploitation Route The aim of the project was to use our findings both the raise awareness of antimicrobial resistance (AMR) at community level and also to engage national healthcare policy in Nepal. We have achieved these aims by co-producing participatory videos which illustrate some of the common behaviors linked to antimicrobial resistance (AMR) within two Nepali communities http://www.herdint.com/projects/29/read-more. These films have been showcased at the local level to spread awareness of AMR which we know has been impactful due to focus group discussions following the showcasing in which participants discuss their learnings on safe antibiotic use. The films have also been shown at policy level, influencing the development on the Nepali national action plan on AMR. Finally the films and project manual https://ce4amr.leeds.ac.uk/projects/caran-2/ are available as free-to-use resources, the aim being to encourage the use of creative methodologies such as participatory video in other health interventions.

Description The findings of this study are frequently used as case study examples of community engagement and participatory research methods in health. Members of the team have shared findings at online webinars for networks such as ReACT action on antibiotic resistance, the SONAR Global social science network and the One Health EJP. SONAR global have also included findings of this project within their AMR curriculum. Films resulting from this project are shared with Ministry of Health and Population officials in Nepal as examples of developing equitable partnerships in AMR research. Discussions continue within Nepal regarding how to scale up the impact of this study to other areas and particularly within the field of education. Through an AHRC funded follow project we have developed a facilitation and resource pack to enable Nepali school staff to engage students on the topic of antimicrobial resistance (AMR) these packs are in the final stages of translation and will shortly be available on the CE4AMR website. There are also several short films, info graphics and AMR educational presentations that were created for our online information sessions - these are now being modified to share on the CE4AMR website so that other projects may use them. Finally, we have spent a large amount of time training our participants on how to use online technologies such as zoom. Due to COVID-19 almost all our engagement was online including initial information sessions, consent taking, Q&A session, orientations, and engagement activities. We have a set of resources and learnings which we are now refining into guidance for other projects.

PI Contribution The MESH community engagement platform engaged via the CE4AMR twitter account (set up during the CARAN project) to discuss ways of disseminating and sharing information on community engagement methods with a specific focus on AMR. The MESH and CARAN project team now share 3-monthly skype calls to update on each other's progress, new resources and news. The MESH platform helped to launch the official CE4AMR website by providing a press release and social media content. The CARAN team use the CE4AMR twitter account to promote new MESH resources, webinars and updates to their network.
Collaborator Contribution This relationsip is collaborative and works on the basis of promoting eachother's outputs where they are relevent to both the MESH and CE4AMR audience. The MESH platform sign-posts to CE4AMR for AMR specific resources whilst the CE4AMR platform signposts community engagment projects to MESH. Both MESH and CE4AMR host some of the same resources but are trying not to overlap too much. CE4AMR will signpost projects without a specific AMR focus to the MESH platform as their content better fits this website.
Impact Press release and website launch tweets from MESH for the CE4AMR website 3-monthly meetings on progress, news, updates, resources, meetings between MESH and the CARAN/CE4AMR team

Results and Impact In July 2021 the HERD and UoL team prepared a blog to reflect on our initial experiences of engaging community participants online and the challenges of encouraging genuine participation through this method. Because of COVID-19 there was no other way to engage participants safely but we worried about the co-creation and participation nature of our work. The blog was well-received by the CE4AMR network and twitter following with many other research groups asking for support with this approach, the CARAN participants were also pleased to see that their involvement had already allowed the project to develop outputs and learnings.

Results and Impact On June 26th 2021 the CARAN participants were engaged in an online workshop to refresh our shared knowledges of AMR. The CARAN participants were encouraged to remember their time on the CARAN project and what they had learned about AMR. our research team then discussed the aims to work in schools and the CARAN participants were very excited about this. We discussed what the most important AMR information to share would be and what sort of methods we should use to engage students. CARAN participants were asked if they would be happy to talk to teachers in a future meeting, most were very keen and one who didnt want said they would make notes so their ideas could be shared.

Results and Impact The CARAN project has a dedicated page on the CE4AMR website which describes all project methods, findings and outputs. This is intended to support others interested in using the participatory film-making process in health research and to showcase the project manual in action. The manual is also included in the resources section of the website (See additional entry). The website was officially launched in January 2020 and has been well promoted on social media and through partner organisations, we estimate around 500 people have viewed the CARAN page or manual so far. The primary audience of the website is researchers and practitioners working in the area of AMR however those involved in community engagement, participatory research and One Health research have also been targeted. Under- and Post-graduate students at the University of Leeds are likely to have come across the website due to members of the team being involved in teaching and supervision. The CARAN page has also been promoted within tweets linking to behaviour change, policy interventions on AMR and the sustainable development goals which should drive traffic from policy and governmental areas as well as other research organisations, NGOs, charity and social enterprise groups interested in both AMR and creative practices.

Results and Impact Following the success of the CARAN project the team have become invested in an AMR-centred research agenda, particularly the use of community engagement and participatory approaches to tackle this challenge. This has resulted in the development of the CE4AMR (community engagement for antimicrobial resistance) network of (currently) 60 researchers and practitioners who share this aim. A twitter account was set up with in this name ahead of our June 2019 event in Nepal which brought together researchers and practitioners using CE methods for AMR and other health challenges. The twitter account was primarily used to drive interest toward this event and live-share learnings from the workshops and presentations at the event. Where possible tweets on projects and workshop content were linked to the sustainable development goals to attract more interest. The twitter account was incredibly successful during this event reaching 11083 accounts and creating 59184 impressions over the 3-day event. We currently have 285 followers which has happened in an organic fashion, however a drive to recruit more followers is planned for later this spring.

The event created space to discussed alternative (and mainly creative) methods of research dissemination and the CARAN film was seen as a key example of how to use video to do so. The project team briefly explained the background to the project and why the research on AMR was much needed in the Kathmandu region of Nepal. Audience members were very taken with the participatory video approach and asked a high number of questions around the method, its feasibility and how it could be applied to other research questions. The project team had brought hard copies of the CARAN facilitator manual to distribute at the event and were able to use some of these questions as prompts to talk through specific sections and activities within the manual.

Results and Impact This project is follow-on funded and hinged on the engagement of participants from the previous CARAN project to work with us to co-create an education programme on AMR for Nepali school students. However due to the COVID-19 situation the CARAN participants had not yet been contacted. Our HERDinternational colleagues spend several weeks in June 2021 phoning previous participants and explaining the project. Information sheets were also sent as physical copies.

After several rounds of coordination 6 of them agreed to participate in the project, these participants were then contacted over zoom to provide their consent to take part. The Zoom and internet costs were covered by the project, phone support was needed to orientate participant's on the use of Zoom and the consent taking process was lengthily to ensure all participants fully understood what they were being asked to do. This process was also an essential part of the project itself as it gave the CARAN participants essential skills on Zoom allowing them to fully engage with the rest of the project which was almost exclusively run online
