Programme 1: Musculoskeletal health in later life

Abstract
The aim of this programme is to reduce the burden of chronic non-communicable musculoskeletal disorders such as osteoporosis, osteoarthritis and sarcopenia (the commonest disorders worldwide of bone, joint and muscle respectively). Our approach integrates observational epidemiology and mechanistic understanding to develop novel interventions in later life aimed at optimising musculoskeletal health. Our work has already informed strategies developed by the WHO and IOF. Complemented by investigation of underlying mechanisms, we are evaluating interventions that might be widely delivered in older age, in order to successfully implement potential novel public health strategies which will reduce the health burden resulting from these disorders.
Technical Summary
Summary: This programme integrates observational epidemiology and mechanistic understanding to develop novel interventions in later life aimed at optimising musculoskeletal health, in order to reduce the burden of chronic non-communicable musculoskeletal disorders, principally osteoporosis, osteoarthritis and sarcopenia.
Objectives: To (a) understand better the risk factors for, and mechanisms which underlie the development of osteoporosis, sarcopenia and osteoarthritis; and (b) develop intervention studies aimed at modifying later disease risk.
Research plan: Our future work will therefore focus on four key areas: (a) The Hertfordshire Cohort Study (HCS) and a novel musculoskeletal ageing cohort in Southampton, will form the basis for investigations across the lifecourse (complementing work in the Southampton Women’s Survey [SWS] with Programme 3). These will include novel intergenerational analyses as well as behaviour-focused diet and activity interventions to reduce falls and fracture risk; trajectories of musculoskeletal health in the oldest old will be characterized, with linkage to incident events via NHS Digital; (b) We will elucidate the environmental and genetic risk factors for musculoskeletal disorders such as osteoporosis, osteoarthritis and sarcopenia, accounting for multimorbidity, in cohorts including MrOS, WHI, Health ABC and UK Biobank. These findings will inform the development of the next version of the FRAX fracture risk assessment tool, as well as optimisation of primary and secondary risk assessment strategies (SCOOP and CTF); (c) We will use similar approaches to characterise the most effective integration of muscle mass, strength and physical performance, to predict falls, fractures, hospitalisation and death, in a new harmonised cohort collection (collaborator: MRC Versus Arthritis CIMA, Sheffield), in order to derive a novel outcome-based consensus definition of sarcopenia, and related risk assessment tool; and (d) We will carry out epidemiological studies to identify individuals at highest risk of requiring joint arthroplasty for osteoarthritis, and characterise outcomes of these surgeries.
Impact: We will continue to link closely with Programmes 2 and 3 in the MRC LEC, to characterise relevant aetiologic factors across the lifecourse from development, through working life, into senescence. Diabetes Care published online 29 March 2021 doi;.org/10.2337/dc20-2515.) of the findings from the NiPPeR study in this paper received some publicity from both Nestle and Food in Progress as well as being reported by the University of Southampton. The findings show that a mother's diet before and during pregnancy is important for herself and gives her baby the best start in life. The supplementation given to the women taking part in the NiPPeR study was linked to reduced risk of pre-term birth. The national report presents evidence of declining health among children under five in the UK. It says taking steps to improve the health of babies and young children will have lasting benefits. The report outlines several key issues including: Rising rates of obesity, with over a fifth of children aged five now overweight or obese; one-in-four five-year-olds are affected by tooth decay; a rise in infant mortality in England; higher demand for children's mental health services; a decrease in children having vaccinations. During his interview he called on councils to do more to support new mothers to stay physically active. He led a study on the activity of 800 women in Southampton and found fewer than half were getting enough moderate to vigorous physical activity. |

Results and Impact | As part of the 50th Anniversary of the NIHR BRC, the Southampton Women's Survey were chosen to represent some of the research that has taken place in Southampton. The BRC funded an evening at the City Art Gallery in Southampton for 50 SWS participants and their family or friends. The speakers who talked about the study findings were Prof. Cyrus Copper, Prof Keith Godfrey, Prof Janis Baird and Prof Hazel Inskip who wrote and sang a song at the end of the presentation whilst playing her ukulele. SWS staff in attendance and who organised the logistics for the event were research nurses, Julia Hammond, Nicole Minette, Karen McGill and Tina Horsfall. They received requests about further participation or involvement, the audience reported change in views, opinions or behaviours, requests for further information. Children were also recruited for PPI for the BRC. |

Results and Impact | Press release - Southampton researchers will lead one of several new national UKRI research networks helping tackle challenges in ageing. The MyAge network will focus on pathways of muscle development, differentiation and decline. Over the course of the two-year project, scientists aim to build new directions and collaborations for future research. They will also develop a roadmap of changes that can help people live independently for longer. The 11 new national networks are funded by the Biotechnology and Biological Sciences Research Council (BBSRC) and Medical Research Council (MRC). MyAge is directed in Southampton by Professor Keith Godfrey, Professor Karen Lillycrop and Professor Peter JS Smith. It is also led by Professor Carolyn Greig (Birmingham), Dr Kambiz Alavian (Imperial College), and Dr Mathew Piasecki (Nottingham). |

Results and Impact | Announcement that the University of Southampton will lead one of the eleven new national research networks to tackle challenges in ageing, announced today by the Biotechnology and Biological Sciences Research Council (BBSRC) and Medical Research Council (MRC). Keith Godfrey part of a team of scientists at the University who will lead the MyAge (Muscle resilience across the life course: from cells to society) network. Over the course of the two year project, they aim to build new collaborations and hypotheses for the future of research in this area and develop a roadmap to interventions that can achieve five more years of independent living. |

Results and Impact | Keith Godfrey wrote a blog for the March 2023 edition of Medically Speaking,entitled "A leading role for Southampton in improving population health and resilience to tackle to cost-of-living crisis. He outlined four research and knowledge exchange activities where the Faculty is playing a leading role in regional and national initiatives to fundamentallly improve population health and resilience. Regionally, the UKRI Arts and Humanities Research Council (AHRC) funded "Pathways to Health Through Cultures of Neighbourhoods". Then the first of three national initiaives relates to the UKRI National Ageing Networks where Southampton leads the MyAge "Muscle resilience across the lifecourse: from cells to society" national network. The second national initiative relates to involvement in setting a new UK Strategy for Clinical and Applied Diaetes Resarch. The third national initiative is a precenception care and health. Keith's work with the Children's Alliance charity and the All Parliamentary Group on a Fit and Healthy Childhood which has led to the recent tabling of an Early Day Motion in the House of Commons calling for a national preconception care strategy. |
