Abstract
Many different things affect how healthy we are throughout our lives, and how long we can expect to live. These things include our genes, our behaviour and, most importantly, the social, political and environmental conditions in which we are born, grow up, live, work and age. These conditions are not the same for everyone and there are big differences in health between people who are well off and those who are not, and on average the richest live many more years in good health than the poorest.
Our programme seeks to understand what policy makers can do to help people live healthier for longer, and how we can address the unfair gap between rich and poor. Systems science is a collection of methods and approaches for understanding complex relationships between things and how they develop over time. It is often used in the natural sciences such as for understanding eco systems or the weather, but we believe it can also help us understand how social, environmental and political conditions (for example economic recessions, climate change) influence health and wellbeing.
Our programme will seek to develop a group of researchers with the necessary topic and methods skills and then work with research partners and policy makers to address one of the key challenges of our times – how we can live healthier for longer.

Technical Summary
In their Industrial Grand Challenges, the UK Government has set the twin targets of increasing healthy life expectancy by five years by 2035 whilst narrowing the gap between rich and poor. Healthy life expectancy, including inequalities in it, are considered in the Scottish National Performance Framework, the National Indicators for Wales and the Northern Ireland Outcomes Delivery Plan.
The aim of this programme is to provide underpinning research that can support decisions around which combination of policies and interventions, over which timescales, and implemented with which intensity, are most suitable to bring about the whole-systems change required. It also seeks to understand how to lock in gains over the longer term. Our focus will be on identifying policy and intervention options with the capacity to multi-solve, that is to exploit the interdependencies and synergistic dynamics between multiple government priorities, such as health and wellbeing, economic prosperity, strong communities, housing, and climate resilience.
The programme will carry out interdisciplinary, collaborative research and methods advancement to develop and apply soft systems approaches and simulation modelling. It will seek to build new capacity in soft systems methods and system dynamics, and draw on existing Unit/University strengths in data sciences/big data, health inequalities, microsimulation and agent based modelling.
The objectives are:

Throughout the launch of the product guide, a new exchange with the EU-based policy think tank Population Europe has been established. PI Contribution | This was a SIPHER-aligned proposal to the Wellcome Trust for their commission on workplace mental health (total amount requested £44,164.65 exclusive of VAT - note this was a commission for a contract not a grant). The proposal built on the existing collaborations developed as part of SIPHER and was focused on a SIPHER-relevant topic, specifically minimum wage increases and mental health outcomes. The aim was to review the evidence for the impact of minimum wage interventions on mental health outcomes and wellbeing measures. It brought together SIPHER's expertise in evidence synthesis, participatory research and policy analysis and applied these to a topic of direct relevance to SIPHER's focus on the economy and health. The proposal was led by SIPHER Research Associate (Macintyre) and involved collaboration with SIPHER colleagues (Chambers, Stewart, Smith) and SIPHER collaborators (Fearfull) as team members and SIPHER PI (Meier) on the expert advisory group, and extended this collaboration to include new collaborators in Meier's home institution (Thomson, Katikireddi). PI Contribution | This was a SIPHER-aligned proposal to the Wellcome Trust for their commission on workplace mental health (total amount requested £44,164.65 exclusive of VAT - note this was a commission for a contract not a grant). The proposal built on the existing collaborations developed as part of SIPHER and was focused on a SIPHER-relevant topic, specifically minimum wage increases and mental health outcomes. The aim was to review the evidence for the impact of minimum wage interventions on mental health outcomes and wellbeing measures. It brought together SIPHER's expertise in evidence synthesis, participatory research and policy analysis and applied these to a topic of direct relevance to SIPHER's focus on the economy and health. The proposal was led by SIPHER Research Associate (Macintyre) and involved collaboration with SIPHER colleagues (Chambers, Stewart, Smith) and SIPHER collaborators (Fearfull) as team members and SIPHER PI (Meier) on the expert advisory group, and extended this collaboration to include new collaborators in Meier's home institution (Thomson, Katikireddi). PI Contribution | SIPHER is working with Clackmannanshire (Clacks) Council as a case study for the Scottish Government. The collaboration began in summer 2020 and to date our work together has focused on understanding how we can support each other in the inclusive growth agenda. In November 2020 we co-hosted a systems mapping workshop with Clacks Council to map the inclusive growth/economy systems. We anticipate that this will be just the first step in our collaborative relationship. |
Collaborator Contribution | Clacks Council are facilitating the use of their local area as a case study for SIPHER and actively participated in the design and delivery of the participatory systems mapping workshop in November 2020. In December 2020 the results were presented to, and very well received by, local politicians including the Leader of the Labour Party at Clackmannanshire Council in an All Party Parliamentary Briefing on the Wellbeing Economy and Community Wealth Building. Clacks Council are now using the map to inform conversations around financial planning in Clacks for 2021 and beyond. |
Impact | This is a multidisciplinary, multi-sector collaboration between Clacks Council, the Scottish Government and SIPHER researchers (including public health, engineers, social policy researchers). The participatory systems map is now close to completion and is being using in ongoing conversations to inform systems thinking in Clacks Council and financial planning for 2021 onwards. |

The work consisted of two parts: (1) an analysis of routinely collected administrative data including hospital spells data and data from the National Drug Treatment Monitoring System, (2) an analysis of a novel survey-based full-scale synthetic population data set. For this collaboration, the SIPHER team has provided (2) - the underlying synthetic population data set. In addition, the team has supported the development of statistical code for performance-optimised statistical analysis, and advised on the usage of the data, and provided guidance on the interpretation of findings obtained from the data set. |

Across the two-week conference more than 2500 S3 pupils participated in Girls@COP26 - The Solutions are Feminist, at Glasgow Caledonian University. Schools joined forces to discuss the global issues around environment and gender - UN Sustainable Development Goal 5 - alongside other female related issues including health and challenges that can affect women and girls' bodies. The event has since won the Sister Cities International award and there will be a public exhibition launched in October with photos featuring Professor Meier and the other speakers. |

The Group is chaired by Lord Kerslake and met for the first time in October 2020. The discussion revolved around SIPHER's inclusive economy research plans and presentations sparked interesting questions. Many participants, who are senior representative from their organisations (e.g. JRF, Northern Powerhouse Partnership, CIPFA), offered to link up separately outside of the biannual Advisory Group meeting to support SIPHER's development. The group had now met 3 times and is due to meet again in April 2022. |

Results and Impact | Researchers of the SIPHER consortium have developed a survey-based, full-scale synthetic population data set. This novel large-scale data set represents over 50 million individuals in Scotland, England, and Wales. The data set is created by combining aggregate-level population statistics with individual-level information from the UK Household Longitudinal Study (Understanding Society). Representative at a granular geographic resolution, the data provides new opportunities to quickly assess needs or derive small-area indicators using data on individuals. In more advanced applications, the data can inform parameters of agent-based models or serve as an input data source in microsimulations. On December 12th, members of the SIPHER consortium (Andreas Hoehn & Kashif Zia) as well as an embedded researcher at Public Health Scotland (Emma Comrie) delivered a specifically tailored introduction for researchers at the University of Glasgow and analysts at the Glasgow City Region. Overall, the course had 11 participants, connecting researchers at the SPHSU with researchers at the Urban Data Centre, the Glasgow City Region and Public Health Scotland. All course materials (data, code, slides) were shared with all participants enabling an independent use beyond this event. Since the course, an ongoing exchange as evolved with course participants. |

Specific changes to the bid in response to this input included incorporating more attention paid to people with different abilities/needs within the community (e.g., people with mobility issues), and incorporating potential impacts on mental health (e.g., via 'community spirit'). Professor Meier participated in a roundtable discussion with the Minister for Green Skills, Circular Economy and Biodiversity, Lorna Slater MSP and Director General Economy, Louise Macdonald, to seek discuss how expert stakeholder can work together with the Scottish government to deliver on this vision of a wellbeing economy. OHID have provided us with a letter of support which states that "the SIPHER Consortium's work on identifying and mapping the complex relationships between health, health inequalities and their social determinants is of enormous interest to OHID and the local and regional public health teams we work with, particularly given our focus on improving the health outcomes of policies across government." |

The Group is chaired by Professor Arpana Verma at the University of Manchester and met for the first time in November 2020. Many participants, who are senior researchers and policy influencers around the world, offered to link up separately outside of the meeting to support SIPHER's development. |

The workshops were open to all staff & students and held in hybrid format at the Unit and on Zoom. No prior knowledge was needed for any of the workshop topics. Participants got the opportunity to explore systems methods through various interactive exercises and gain theoretical knowledge of their relevance to public health issues. Each one of the four 2-hour workshop provided introductions to theoretical and practical examples of complexity science methods. This week's topic was Agent-based Modelling presented by Cristina Chueca Del Cerro. The workshop introduced participants, with little to no prior knowledge of complexity sciences and computer simulations, to agent-based modelling. After this workshop, participants were familiar with this method and have some examples to work with in NetLogo.? Overall, about 10-15 individuals (PhD students, Masters students, Postdocs, Mid-level to senior researchers) were reached, which contributed to the visibility and discussion about how to best include systems methods in public health research from a variety of perspectives.? |

Results and Impact | I coordinated and organized the delivery of a workshop series on "Systems Science in Public Health Workshop Series" covering essential methods and their application to public health issues. The workshops were delivered by PGRs of the systems science, complexity and relationship team. The workshops ran every Wednesday (10 am - 12pm) between the 17th Nov to 14th Dec. The workshops were open to all staff & students and held in hybrid format at the Unit and on Zoom. No prior knowledge was needed for any of the workshop topics. Participants got the opportunity to explore systems methods through various interactive exercises and gain theoretical knowledge of their relevance to public health issues. Each one of the four 2-hour workshop provided introductions to theoretical and practical examples of complexity science methods. This session will present the basic concepts of system dynamics, showcase examples to provide a sense of what the models look like and what they can do. The session will be delivered online and include some individual activities and small group discussions. At the end of the session, participants should have a good understanding of the types of questions these methods are suited to answer. Overall, about 10-15 individuals (PhD students, Masters students, Postdocs, Mid-level to senior researchers) were reached, which contributed to the visibility and discussion about how to best include systems methods in public health research from a variety of perspectives. |

Results and Impact | Professor Petra Meier was invited to join the UKRI Population Health Improvement subgroup. The group was formed to support the Health Ageing and Wellbeing (HAW) agenda being developed by UKRI, to address its ambition of improving the health and wellbeing of the population. Within the HAW agenda, a theme was developed on Population Health Improvement with a "task and finish" group, to scope out existing and/or future research activities that would contribute to population health improvement, encompassing health equity, population health intervention development and prevention. The theme places emphasis on systems interventions at the population level. This has now led to the establishment of a new funding scheme - Population Health Improvement Clusters. |

Results and Impact | Dr Jonathan Olsen, Prof Rich Mitchell and Dr Jonny Stokes led a Westminster lead drop-in session for MPs entitled 'Creating Healthier Places: A Place-Based Approach to Research & Partnership'. The drop is session took place on Tuesday 27th February 2024 and was attended by approximately 15 MPs from across the UK. The Places and Health team disseminated their research about 20-minute neighbourhoods and aspects of place that can benefit or harm health and health inequalities. The team created a 4-page evidence summary document which was provided to MPs. |
