COVID-19: Supporting Parents, Adolescents and Children during Epidemics (Co-SPACE)

Abstract

COVID-19 has caused major disruptions to families' lives, through social distancing, school closures and lock-down. This is also a rapidly changing situation where different pressures will arise for children, young people and their families over time. While research has provided valuable information about how parents can support their children's mental health in general, we know little about what is likely to be effective and what families need at different stages of the COVID-19 crisis. The Co-SPACE project is tracking the mental health of children and young people aged 2-16 years throughout the pandemic. This is through an online survey completed monthly by parents/carers and young people (if aged 11-16 years) and in-depth interviews with parents/carers, young people and other people who may be involved in supporting them, such as teachers. We will specifically track young people from vulnerable groups, such as those from low income households, with pre-existing mental/physical health difficulties and looked after children. The findings will help identify what protects children and young people from deteriorating mental health over time, and at particular stress points, so that we can work out what advice, support and help parents would find most useful and inform policy and public health efforts to protect mental health as the pandemic progresses.

Description Between March, 2020, and March, 2021, in the Co-SPACE sample, we have found:
- Clear increases in parent-reported symptoms of behavioural and attentional difficulties at times of peak restrictions, when most children were not physically attending school. These symptoms increased throughout the first national lockdown (March-June, 2020), decreased and stabilised as restrictions eased, increased again from January, 2021, when a further national lockdown was introduced, and then decreased again when most children returned to school in March, 2021.
- Average adolescent self-reported SDQ symptoms across all examined domains (behavioural, attentional, and emotional), and parent-reported emotional symptoms, were fairly stable over time; however, the highest levels of both parent-reported and adolescent-reported symptoms were when high levels of restrictions were in place and schools were closed to most children.
- We have seen greater changes in parent-reported mental health symptoms (in line with pandemic-related restrictions) among pre-adolescent children (aged 4-10 years), but a more stable pattern among adolescents (11-16 years).
- Stress has also been particularly high among parents of pre-adolescents when restrictions have been high. Notably, but unsurprisingly, parental stress was particularly high among single-adult households, among low-income families, and where children had special educational needs or neurodevelopmental disorders.
- Other clear patterns include the very high levels of parent-reported mental health symptoms among children and young people with special educational needs or neurodevelopmental disorders and those living on low family incomes throughout the pandemic. Although we saw overall improvements in parent-reported mental health symptoms in March, 2021, as restrictions eased, parents of children with special educational needs or neurodevelopmental disorders and in low-income families continued to report high levels of symptoms.

As a result of the work we are able to build an understanding of the psychological effects of the COVID-19 pandemic on children and young people in a way that can directly inform policy and practice.
Exploitation Route The materials used in the study have been made available to other researchers to be able to undertake complimentary research and parallel studies in the UK and other countries, using overlapping measures, in order to be able to make comparisons between datasets. Researchers will benefit from information around what protects children and young people from deteriorating mental health (over time, and at particular stress points) and how this varies according to child, family and environmental characteristics, in order to inform resources and support for families and policy and guidance in relation to supporting young people and families (e.g., around schools re-opening). The protocol is published on the Open Science Framework, the results of the study in peer-reviewed scientific journal and the data is currently being made available through the UK Data Service. Regular summaries of key findings have been and will continue to be published throughout the study (https://cospaceoxford.org/findings/) and sent directly to key stakeholders in a format that is accessible to a wide audience.

Description Findings from the Co-SPACE have enabled us to build an understanding of the psychological effects of the COVID-19 pandemic on children and young people by collecting data in a way that can directly inform policy and practice. We have conducted preliminary analyses of the findings, presented through 11 reports on the Co-SPACE website (https://cospaceoxford.org/findings/). We have also sent them directly to policy makers, professionals and organisations supporting children and young people during the pandemic. Data snapshots have been provided to professionals and policy makers through eight supplementary reports in response to specific questions at particular stress points (e.g., schools reopening) to rapidly inform resources and support for families. We have presented findings to academic audiences and to members of the public through webinars (e.g., Facebook Live event). Our findings were referenced in a House of Commons Early Day motion tabled by Dr Lisa Cameron to investigate the effect of the Covid-19 outbreak on children's mental health and presented as evidence to a Government all party parliamentary group. Findings have been included in Department for Education State of the Nation 'Children and Young People's Wellbeing' reports. We have also made our findings available through local and national press, radio (e.g., BBC Radio 4, Radio 2; Five Live; local radio stations) and television (e.g., Newsnight; ITV; BBC South).

PI Contribution We are working with Leaders Unlocked to design and facilitate a process for engaging young people in the UK and Japan as peer researchers and to co-develop guidance for policy makers and practitioners in Japan and the UK. Together, Leaders Unlocked and the University of Oxford Co-SPACE team will conduct a series of online international focus groups and events with young people (aged 11-16 years) and parents/carers. This work will be used to inform and enable a similar process to be implemented by collaborators at the National Center for Child Health and Development in Japan.
Collaborator Contribution Leaders Unlocked train and support young people in the UK to enable them to have a voice on the issue of COVID-19 mental health impact and to work with researchers, policy makers and practitioners to influence decision making. Results and Impact In connection to the further funding received from the Lorentz Centre, we held an international in-person meeting on child, adolescent, parent mental health research during the pandemic (December 2023). The workshop was attended by international collaborators from around the world, including from Australia, Denmark, Iran, Ireland, Japan, Malaysia, Netherlands, and Turkey, as well as Dutch young people and clinicians.

During the workshop, we discussed our global findings and experiences, such as different approaches to school closures and potential links to post-pandemic school absenteeism, particularly vulnerable groups, modifiable protective factors, and similarities between pandemic experiences and other public health crises. We also planned how the international Co-SPACE consortium data and insights can be used to inform future policy and strategies that prioritise young people's mental health before, during and after public health crises.
