Building the Barricades (ES/S000720/1) Follow-On: mobilising research on mental health and substance use in Complexo da Maré, Rio de Janeiro

Abstract

This proposal arises from a unique opportunity to bring the research findings from Building the Barricades (ES/S000720/1) to new audiences. In the absence of medical interventions and social assistance in Maré, the studies undertaken in the original research have drawn attention to the importance of active participation in art and cultural activities for individuals' recovery from mental health disorders as well as to build mental health resilience and wellbeing. The follow-on proposal will use arts-based methodologies (live performance, installation and visual arts) to extend the impact of the research, whilst simultaneously contributing to increased well-being amongst the citizens of the Complexo da Maré, Rio de Janeiro.

The project's existing research has already engaged the international academic community through an ongoing series of publications, research seminars and conferences. The focus will now be on engaging general audiences as well as key stakeholders committed to, and responsible for, the delivery of mental health and well-being services in Rio de Janeiro and Latin America. The project will establish a Mental Health Committee in Rio de Janeiro, made up of 15 representatives from civil society organisations and government agencies delivering health care provision in Maré, who will develop recommendations based on the original research findings and advocate for health policy reform to improve provision in Maré. In collaboration with FIOCRUZ, we will host three roundtable debates, further engaging civil society organisations as well as city, state and federal agencies addressing the mental health consequences for residents of territories like Maré with the research. The research team will also host a knowledge exchange visit, sharing methodologies and the research findings with civil society organisations working in fragile territories in Bogotá, Lima and Buenos Aires as part of a regional network on arts and mental health established by HERITAGE and PRIEBE (Co-I).

This follow-on proposal will activate a range of voices in the interpretation and dissemination of the research. Working with the civil society and research organisation Maré Development Networks (MDN), the proposal will ensure that the research findings continue to register with the community in Complexo da Maré, a territory of 16 favelas in Rio de Janeiro which is home to over 140,000 residents. Over the past 18 months, the combined British and Brazilian research team have built an inter-disciplinary analytical and methodological framework that bridges geographical, social and disciplinary borders. One strand of the original research has used arts-based practices to produce narratives and images that challenge stigma and exclusion associated with Mental, Neurological and Substance Use (MNS) disorders. These practices included workshops and events (live and online) with residents from Maré and poets, musicians, photographers and visual artists. Alongside the research, members of the team also established a choir for those who frequent the open-use crack sites. The success of these creative initiatives has opened up unexpected opportunities to explore integrated ways to produce and disseminate learning between the various agents involved in this multidisciplinary research and a diverse range of new audiences beyond academia. Multiplicidade, one of Rio's leading international arts festivals, has invited HERITAGE (PI) to create a performance/installation based on the research findings at the Biblioteca Parque Estadual in October 2021. Provisionally entitled 'Mais Becos', this performance/installation will be directed by a leading Brazilian contemporary musician working with popular art-forms, music and photography, and engage 31 community-based arts initiatives mapped during the original research, including the choir, and over 1,000 residents from Maré.

Description On My Feet presents the rapper and poet Mc Martina and musician Jonathan Panta filmed in the largest complex of favelas in Rio de Janeiro - Maré - in 2021. The video clip was screened during the first mental health awareness week in Mare. The video was inspired by an original song composed by them for the BECOS podcast. Description Maré Mental Health Week - Rema Maré I
Based on the findings of the Building the Barricades: the Mental Health Impact of Violence in Rio's Largest Favela research, People's Palace Projects, in partnership with Redes da Maré, launched the first mental health awareness campaign in the Maré complex of 16 favelas in February 2021: Rema Maré. For a week, various cultural activities, debates, and interventions took place within the communities, fostering dialogues on mental health.
The program included a performance based on the Becos podcast, the creation of a tile mural, and the display of banners and posters across Maré's favelas, featuring excerpts from Becos artists' poems and data from the Building Bridges research. Additionally, the event featured video projections, discussion circles, outdoor cinema for children, art workshops, and the distribution of booklets containing information about the available mental health services in the area. All activities were centered on mental health, community, and culture.

Audio Drama/ Alleys podcast (Becos)
Over the first five months of the Covid-19 pandemic, six poets from Maré took part in online creative writing workshops led by People's Palace Projects' artistic director, professor Paul Heritage (Drama, QMUL) and Welsh theatre director Catherine Paskell (Dirty Protest).

Collaboratively, the young artists produced Becos, an audio drama in four acts, which is now available wherever you get your podcasts. The story is about joy, opportunities, violence, racism and injustice, and the resilience built by poor Brazilian communities every day. With poetry and music, the six young artists from one of the largest favelas in Rio de Janeiro, guide us through the alleyways of Maré.

Maré from Home (A Maré de Casa)
During the lockdown, residents of Maré were encouraged to document what they saw through their windows and entered their photos and text in a virtual competition. The finalists were selected for the digital arts exhibition A Maré de Casa curated by visual artist Tatiana Altberg and researcher Raquel Tamaio. On the same site, six young photographers also documented during four months their daily lives via written and photo documentation.

Satélite
Musician Rafael Rocha, who created the Becos soundtrack, also produced an album inspired by the audio drama's creative process, titled Satelite. Its 11 original tracks explore favela sounds and feature the poems and the manifesto written for Becos.

Second Mental Health Week - Rema Maré II
The second edition of Maré's Mental Health Week took place in November 2022, expanding and strengthening dialogues on mental health, community, and culture. The event brought together professionals, public service providers, and residents through a partnership between People's Palace Projects, Redes da Maré, and the Tide Setubal Foundation. That year, Rema Maré paid tribute to the legacy of Dr. Nise da Silveira and nurse and social worker D. Ivone Lara-key figures in the critique of institutional psychiatry in Brazil, who championed care in freedom and the use of art as a therapeutic tool.
Over six days, the program featured training sessions, cultural activities, discussion circles, roundtables, and workshops. Many of these activities were proposed and led by civil society groups selected through an open call.
Building on the work from the first edition, Rema Maré II launched the guide Psychological First Aid - Helping Those Who Help, Caring for Caregivers. This material deepened discussions previously introduced, offering new perspectives on mental health in the community, emphasizing Psychological First Aid, self-care, and mental health support for caregivers and helpers. Rather than focusing on diagnosis, the guide promoted a more compassionate and communicative approach.
As a follow-up to this material, two training sessions were held for professionals from Redes da Maré and workshop facilitators. The goal was to equip mediators, artists, and other participants with the tools to provide Psychological First Aid, offering compassionate support and guidance in mental health care. This experience became a Mental Health Tool Kit delivered for more than 150 people.
Exploitation Route The outcomes of this funding can be taken forward and put to use by others in several meaningful ways:
Scaling and Replicating Mental Health Initiatives
• The success of Rema Maré as an annual mental health awareness event in favelas demonstrates a replicable model that could be adopted in other communities facing similar challenges.
• The Mental Health Toolkit and training sessions developed through these projects provide practical resources that can be expanded and adapted for different regions and audiences.
Strengthening Community-Led Research and Capacity Building
• The involvement of community researchers in projects like Building the Barricades FoF and the Manguinhos project has created a framework for knowledge exchange between academia and local communities, empowering residents to take an active role in shaping mental health initiatives.
• Future projects can build upon these methodologies to train more community leaders and facilitators, ensuring sustainable, locally driven interventions.
Influencing Public Policy and Service Provision
• The engagement with public service providers, social organizations, and health professionals has strengthened discussions on mental health care models in underserved areas.
• The insights gained from these projects can inform policies on mental health, particularly in the context of socially vulnerable communities, leading to better integrated services and support systems.
Expanding Partnerships and International Collaboration
• The partnerships between People's Palace Projects, Redes da Maré, FIOCRUZ, Tufts University, and other institutions have created a strong foundation for further research and cross-sector collaborations.
• These networks can facilitate future joint initiatives, funding applications, and knowledge-sharing, helping to scale the impact beyond Brazil.

By continuing to share resources, expand training, and adapt these methodologies for different contexts, these outcomes can empower more communities, influence systemic change, and ensure long-term improvements in mental health awareness and support.

Description As part of this research project, we have worked together with the NGO Redes da Maré to create the first Mental Health Awareness week in Maré, a complex of 16 favelas in Rio de Janeiro, Brazil. After the completion of the research project and its funding, Redes da Maré went on to sustain the initiative for a second year and produced the 2nd Mental Health Awareness week in Maré during 2022, which consisted of a series of workshops led by mental health professionals, arts practitioners, social services professionals and others to discuss the mental health of residents and alternatives sources of support. Discussions and repercussions from the first mental health awareness week pointed towards a need to provide appropriate support to professionals working in community-based organisations who might face distressful situations with users, be it episodes of severe stress, mental health crisis, trauma, etc. To address this, we have developed a Mental Health First Aid toolkit and training, which was first piloted during the II Mental Health Awareness Week of Maré, and offered to Redes da Maré's staff and facilitators of the other workshops part of the programme. As a result, People's Palace Projects is already in talks with other community-based organisations to expand the training to other favelas in Rio de Janeiro. Continuing the dialogue on mental health in Maré, the third edition of the Mental Health Week, Rema Maré III, took place in December 2024, furthering discussions on well-being and care in favela territories. The event was collaboratively designed with input from participants of Espaço Normal-a community reference center on drug-related issues in Maré-and local health professionals. Organized in partnership between Redes da Maré and People's Palace Projects, it was funded by the Research Support Foundation of the State of Rio de Janeiro (FAPERJ).

Title Building the Barricades: Three Interdisciplinary Studies on Mental and Substance Use Disorders in the Context of Armed Violence in Brazil, 2018-2021 
Description The data consists of one spreadsheet including data from study 1, one spreadsheet with data from study 2, two files including the sampling description (in Portuguese and English), one file with survey guidelines and one file with the description of the data calibration. Study 1 - Maré Domestic Survey: an investigation of the mental health and wellbeing of Maré residents with a focus on their mental health, patterns of drug use (legal and illegal), family and educational background, income generation and access to social, health and drug treatment programmes. Study one included a domestic survey of 1,211 Maré residents from each of the 16 communities. Study 2 - Profiles of Regular Drug Users of Maré: A quantitative survey of 200 regular drug users living on or at risk of living on the streets within and on the borders of the Mare´ communities. The survey investigated the existing mental health of the respondents, their knowledge and perception about MNS disorders, their access to mental health services, the possibilities of self- and community-based care, and the existence of informal care networks.

Impact The research team surveyed 1,400 Maré residents over the age of 18, including 200 active drug users living on the streets or at risk of homelessness. They conducted in-depth face-to-face interviews and organised focus groups, mapping local health services, social services and cultural activities that were recognised by residents as support networks that could improve their well-being. Between June to October 2020, the research team also conducted three sub-studies, across social sciences, health and culture, to better understand the impact of the Covid-19 pandemic on the residents' mental health, well-being, available cultural activities and income. Results and Impact From May to July 2023, People's Palace Projects (PPP) conducted six "Mental Health First Aid - Helping the Helpers, Caring for the Carers" training sessions in two Brazilian states: Rio de Janeiro and Goiás. Four training sessions were exclusively for one organisation each, and two were open to the communities. The main organisations and community mediators were supported by PPP to publicise the training to their audiences.

The six trainings in Brazil were carried out with the participation of the local communities. During this process, the methodology used for the training was adapted and improved with the support of local mental health professionals who work within the communities and have extensive experience with and knowledge of the target audiences. They worked with a PPP researcher to prepare the material (presentation) and methodology for each training session, and also co-facilitated them. The printed version of the booklet was distributed during the training sessions and was the starting point for adapting the training for each audience and context.

Results and Impact People's Palace Projects' (PPP/QMUL) recent research projects have demonstrated how artistic activities are seen by young people as spaces where they can freely express feelings, thoughts, and their true identities. In many circumstances, these expressions can be related to past and present distressful experiences, as it was the case during the COVID-19 pandemic. Recognizing the responsibility for practitioners, artists, facilitators, and professionals in the voluntary and community sectors, PPP has partnered up with international mental health researchers and psychiatrists to develop a Mental Health First Aid toolkit aimed at professionals based at arts and cultural organizations who work with people subject to vulnerable conditions and events that can cause mental distress.

Through the toolkit, we seek to provide organizations with the knowledge and tools to respond to mental health situations that can happen to non-health professionals and that require their attention and care. The Mental Health Toolkit is composed by a booklet and a training named "Mental Health First Aid - Helping the helpers, caring for the carers". We devised both components so they can be discussed with interested partners and, in collaboration, adapted for different audiences and realities.

In 2021, PPP launched a Mental Health Guide with basic information to start a conversation about Mental Health with Maré Inhabitants for Maré's First Mental Health Week in 2021: a whole week focused on mental health awareness in one of Rio's biggest favelas, the Maré Complex, produced by PPP in partnership with Redes da Maré. For the 2nd edition of Rema Maré, we piloted the training and booklet part of the Mental Health Toolkit, deepening the questions that were first addressed in 2021. I proposes new ways of thinking about mental health based on a rationale for Mental Health First Aid, self-care and mental health, together with the carers and support workers in the community. The booklet distances itself from the concept of diagnosis and moves closer to building communication that is caring and welcoming. It contains simple information and guidance so that people without professional training in mental health can give mental health first aid to people that need mental health care, and guide them towards specialist services. Training "Psychological First Aid - Helping the helpers, caring for the carers" - The training presents the contents of the booklet through dialogue, valuing the exchange of knowledge and experiences between participants. This experience, intended to qualify the audience to deliver Psychological First Aid, welcoming people with affection and directing care towards mental. The structure of the training is straightforward and easily scalable and is also capable of being adapted to reflect the specificities of each territory. More than a lecture, it is as safe space to talk about mental health in a sincere and shared way, to teach and learn together.

Results and Impact 1851 recipients received the newsletter featuring the poster for the new documentary 'A Favela Thing', one of the developments of the Cultural Strategies in Manguinhos project. It also included the videos for 'Já de Pé' and 'Senhora Ansiedade', both of which were part of a research project on the impact of violence in favelas on the mental health of residents. The impact of this was raising awareness about the issues tackled in the documentary.

Results and Impact Continuing the dialogue on mental health in Maré, the third edition of the Mental Health Week, Rema Maré III, took place in December 2024, furthering discussions on well-being and care in favela territories. The event was collaboratively designed with input from participants of Espaço Normal-a community reference center on drug-related issues in Maré-and local health professionals. Organized in partnership between Redes da Maré and People's Palace Projects, it was funded by the Research Support Foundation of the State of Rio de Janeiro (FAPERJ).

Leading up to the main event, Rema Maré III hosted preparatory activities, including two training sessions on Psychological First Aid - Helping Those Who Help, Caring for Caregivers, aimed at professionals from Redes da Maré, as well as a film screening and debate on Lima Barreto for visitors of Espaço Normal.

Brazil has a significant legacy of mental health care through art, pioneered by Nise da Silveira and continued by many other professionals. Rema Maré III builds upon this tradition, strengthening mental health discussions and fostering community-based care in favelas.

Results and Impact The Workshop Cultural strategies in Manguinhos: mental health care and the leadership of favela residents was based on the research project "Cultural strategies as alternatives for social inclusion of vulnerable populations in the field of public policies on mental disorders and drugs: a case study in the community of Manguinhos" developed by LAPS-Fiocruz, FGV and Queen Mary University of London. The workshop was held by four artists and cultural activists from the Manguinhos favela complex. Franciele Campos (Women of the Wind), Graciara Silva (Stars of Mandela), Bruno Martins de Souza (Manguinhos Creates) and Luis Cassiano Silva (Green Roof Favela) spoke about their work, the political and social role of these activities and cultural groups, and how they impact the lives of their beneficiaries and the community in general. Based on their history and activities, they debated cultural and mental health production in favela territories in general.
